1.Osteopontin regulation by protein kinase B in HepG2 cells

Objective It has been unclear how osteopontin, one of metastasis-associated genes, could be regulated in HepG2 cells. The aim of this study was to investigate the effect of protein kinase B(Akt) on the expression of osteopontin in HepG2 cells, and to explore the relationship between Akt and osteopontin expression. Methods HepG2 cells were transfected with constitutively active Akt and dominant negative Akt by lipofectmine, and transfectants were confirmed using Western blot for Akt. Osteopontin expression was detected by Northern blot and Western blot. Results HepG2 cells were successfully transfected with Akt genes including constitutively active Akt and dominant negative Akt, and overexpression of exozegenes Akt could be detected in HepG2 cells by Western blot. Using Northern blot and Western blot, we found that Akt gene regulated osteopontin expression in RNA level and protein level. In serum-free condition, HepG2 cells constitutively expressed low levels of osteopontin. Transfection of constitutively active Akt increased osteopontin mRNA and protein expression. Transfection of dominant negative Akt decreased osteopontin expression.Conclusions The fact that osteopontin gene expression can be regulated by Akt indicates that osteopontin synthesis can be blocked by inactivation of Akt gene and that metastasis of liver cancer might be inhibited by this intervention.
2.Dilingdan Decoction prevents renal interstitial fibrosis in a rat model of unilateral ureteral obstruction

OBJECTIVE: To investigate the preventive and therapeutic effects and the possible mechanisms of Dilingdan Decoction (DLDD), a compound Chinese herbal medicine, on rats with renal tubulointerstitial fibrosis induced by unilateral ureteral obstruction (UUO). METHODS: Sixty SD rats were randomly divided into sham-operated group, untreated group, enalapril-treated group and DLDD-treated group. Blood urea nitrogen (BUN), serum creatinine (SCr), urine protein quantization in 24 hours and pathological changes of the obstructed kidney were observed. The expressions of transforming growth factor-beta1 (TGF-beta1), alpha-smooth muscle actin (alpha-SMA), fibronectin (FN) and laminin (LN) were detected by immunohistochemical method and colored-multimedia pathological image analysis system. RESULTS: Massive inflammatory infiltrates and collagen expression in renal interstitial in the untreated group were observed on the 7th day. Compared with the sham-operated group, percentages of area of TGF-beta1, alpha-SMA, FN and LN expressions in the untreated group were markedly increased (P<0.05), while the percentage of area of interstitial fibrosis was decreased in the DLDD-treated group as compared with the untreated group (P<0.05). On the 14th day, the percentages of area of TGF-beta1, alpha-SMA, FN and LN expressions were declined in two treated groups as compared with the untreated group (P<0.05), but had no statistical difference in biochemical indicators, including BUN, SCr and 24-hour urinary protein. On the 21st day, the level of SCr and the percentage of area of TGF-beta1 expression in the DLDD-treated group were lower than those of the enalapril-treated group (P<0.05). CONCLUSION: DLDD can reduce the excretion of urinary protein and the degree of interstitial fibrosis, and significantly inhibit the expressions of TGF-beta1, alpha-SMA, FN and LN. DLDD is superior to enalapril in protecting renal function after long-time application in rats.
3.Changes of serum fibroblast growth factor 23 in patients with end stage renal disease and its clinical significance

Objective:To observe the serum level of fibroblast growth factor-23(FGF-23) in patients with end stage renal disease (ESRD) and study its association with phosphorus and vitamin D metabolism.Methods: Serum FGF-23 level was determined by enzyme-linked immunosorbent assay (ELISA) in ESRD patients undergoing haemodialysis (HD, n=50) and peritoneal dialysis (PD, n=24) and in twenty healthy controls (control group,n=20).Serum level of 1,25-(OH) 2VitD was measured by enzyme immunoassay(EIA).Serum intact parathyroid hormone (PTH), creatinine, and calcium and phosphorus were also measured.Results: Serum FGF-23 was obviously higher in HD group([88.51?35.01] ng/L vs [11.76?3.63] ng/L)and PD group([87.85?33.65] ng/L vs [11.76?3.63] ng/L)than in control group. Moreover, the serum level of 1,25-(OH)2VitD was lower in HD and PD groups than in control group ([19.82?4.99] pmol/L vs [48.37?3.47] pmol/L; [24.31?7.11] pmol/L vs [48.37?3.47] pmol/L ), and the level of 1,25-(OH)2VitD was much lower in HD group than in PD group. Objective:To determine whether dexmedetomidine (Dex) improves oxygenation and lung mechanics in patients with moderate chronic obstructive pulmonary disease (COPD) during lung cancer surgery.Methods:Fifty-six patients with moderate COPD were randomly allocated to a control group and a Dex group (n=28 each).In the Dex group,dexmedetomidine was given as an initial loading dose at 1.0 μg/kg lasting for 10 min followed by a maintenance dose at 0.5 μg/(kg-h) during OLV while the control group was administered an equal volume of 0.9% saline accordingly.Results:Patients in the Dex group had a significantly higher oxygenation index (P<0.05) and higher dynamic lung compliance at Dex-30 and Dex-60 (P<0.05) compared with those in the control group.In the Dex group,oxygenation index in the postoperative period was significantly higher (P=0.025) and postoperative complications were lower than those in the control group.Conclusion:Dex administration may provide dinically relevant benefits by improving oxygenation index and lung mechanics,and reducing postoperative pulmonary complications in patients with moderate COPD underwent lung cancer surgery.
6.Effect of Xinnaoning tablet on blood stasis model in rats

Objective To study the effect of Xinnaoning tablets on blood stasis rat model which was caused to dexamethasone sodium phosphate injection with symptom score , K-value of blood, modified index, electrophoresis index.Methods 60 rats, were di-vided into 6 groups stochastically , and 1 group for vacuity contrast group , the other 5 groups for blood stasis model , the 5 groups were model group , Naoan tablets group , high dose , medium dose and low dose Xinnaoning tablets groups .Symptom score were evaluated , K-value of blood , Modified index , Electrophoresis index were measured .Results The Model was constructed successfully , model group and administrate groups had significant blood symptoms (P<0.01).The effect of high dose Xinnaoning tablets group to blood stasis rat model which was caused to dexamethasone sodium phosphate injection was best .High dose and medium dose Xinnaoning tab-lets groups could induce K-value significantly (P<0.01).Xinnaoning tablets groups could improve modified index and electrophore-sis index significantly (P<0.01), and the effect of high dose and medium dose Xinnaoning tablets groups to blood stasis rat model which was caused to dexamethasone sodium phosphate injection were better .Conclusion Xinnaoning tablets had good improvement effect on blood stasis .
7.Mutational analysis of MECP2 gene in Rett syndrome.

OBJECTIVETo investigate mutations of MECP2 gene in classical sporadic Rett syndrome (RTT) patients in China.
METHODSPolymerase chain reaction, single strand conformation polymorphism, cloning and direct sequencing were employed to analyse the three exons of MECP2 gene in 26 RTT patients and their parents, and in 2 sisters of 2 of the RTT patients.
RESULTSNine different mutations in exon 3 were identified in 14 of the 26 patients with RTT, including 3 missense mutations, 3 nonsense mutations, and 3 frame-shift mutations (2 deletion mutations and 1 insert mutation); 2 of these were novel. A missense variant was also identified, which was carried by unaffected father and affected daughter.
CONCLUSIONMutations in MECP2 gene were found over 50% of patients with RTT in China.

8.Tumor lysis syndrome in multiple myeloma treated by daratumumab combined with chemotherapy: report of two cases and review of literature

Objective:To investigate the clinical safety of daratumumab (DARA) in patients with relapsed/refractory multiple myeloma (MM).Methods:The diagnosis and treatment process of 2 cases of relapsed/refractory MM treated in Huadong Hospital of Fudan University in 2020 were retrospectively analyzed, the related laboratory indicators indexes after using DARA were analyzed, and the related literature was reviewed.Results:After using DARA, the 2 patients rapidly developed tumor lysis syndrome (TLS) such as high potassium, high phosphorus, high uric acid, low calcium and kidney damage, which eventually led to the death of patients.Conclusions:Patients with high tumor load treated by DARA are prone to TLS, which is often life-threatening. Clinicians should be alert to the occurrence of such events.
9.Clinical efficacy and prognosis analysis of 76 elderly cases with diffuse large B-cell lymphoma

Objective:To investigate the clinical efficacy of elderly patients with diffuse large B-cell lymphoma (DLBCL) and the influencing factors of prognosis.Methods:The clinical data of 76 elderly (≥60 years old) patients with DLBCL admitted to Huadong Hospital Affiliated to Fudan University between January 2015 and December 2019 were retrospectively analyzed. The R-CHOP regimen was the preferred treatment for 54 patients, while the remaining patients received R-miniCHOP, CHOP or other regimens or supportive treatments due to age, physical condition, economic factors, etc., which were not included in the efficacy analysis. Kaplan-Meier method was used to analyze the survival status of patients. Multivariate Cox proportional risk model was used to analyze the prognostic factors.Results:Among the 54 patients who preferred R-CHOP regimen for treatment, 26 cases (48.1%) achieved complete remission and 14 cases (25.9%) achieved partial remission, and the total effective rate was 74.1% (40/54); Among them, the total effective rate of 37 cases aged 60-69 years was 70.3% (26/37), and the total effective rate of 17 cases aged 70-79 years was 82.4% (14/17); there was no statistically significant difference in the total effective rate between the two groups ( χ2 = 3.01, P = 0.390). All 76 patients were followed up for 1-60 months. As of the last follow-up, 49 patients (64.5%) died, with the median overall survival (OS) time of 16 months and 5-year OS rate of 35.5%. Kaplan-Meier method showed that age ≥ 70 years old at initial diagnosis, Eastern Cooperative Oncology Group (ECOG) score ≥ 2 points, presence of B symptoms, international prognosis index (IPI) score >3 points, elevated lactate dehydrogenase, immunohistochemistry positive for bcl-2, and non-germinal center type were associated with poor OS (all P < 0.05). Multivariate Cox analysis showed that age ≥ 70 years old at initial diagnosis, presence of B symptoms, positive expression of bcl-2, non-germinal center type were independent risk factors for OS (all P < 0.05). Conclusions:Elderly DLBCL patients have poor survival. Old age at initial diagnosis, B symptoms, bcl-2 positive, and non-germinal center type are independent risk factors of prognosis.

Objective To comprehend neonatal fathers′views on breastfeeding and their experience engaging in and supporting breastfeeding,so as to provide theoretical basis for fathers′playing better roles in breastfeeding and promoting breastfeeding.Methods Using phenomenology method,in-depth interviews were conducted to 1 2 neonatal fathers about their experience in their wives′breastfeeding period.Using content analysis method,data were settled,concluded and analyzed.Results In neonatal fathers′experience in breastfeeding period,five themes were extracted,including fuzzy understanding of breastfeeding,tortuous breastfeeding experience,father′s important role in breastfeeding,ways of fathers involving in breastfeeding and professional support fathers required.Conclusions Health professionals in Obstetrical Departments should attach more importance to fathers′role in breastfeeding,give them effective help and professional guidance,so as to strengthen their participation and support in breastfeeding.
