1.Directional differentiation of bone marrow mesenchymal stem cells into oligodendrocytes induced by the combination of various neurotrophic factors

BACKGROUND: Myelination following axonal regeneration is a key factor affecting the recovery of spinal cord injury. Oligodendrocyte survival directly affects the myelination following axonal regeneration. OBJECTIVE: To investigate the feasibility of differentiation of rat bone marrow mesenchymal stem cells (BMSCs) into oligodendrocytes induced by neurotrophic factors. DESIGN, TIME AND SETTING: The cell molecular biology in vitro study was performed at the Laboratory of Department of Orthopaedics, Tongji Hospital from September 2006 to June 2007. MATERIALS: A total of 5 Sprague Dawley rats aged 2-4 weeks, of both gender were selected. Bilateral femur and tibia bone marrow was obtained to harvest BMSCs. METHODS: At passage 4, BMSCs were incubated in serum-free medium, supplemented with N2, 20 ng/mL basic fibroblast growth factor, 20 ng/mL epidermal growth factor for 48 hours, and incubated in medium containing 500 ng/mL insulin-like growth factor I and N2 for 3 days. MAIN OUTCOME MEASURES: Morphological changes were observed using an phase contrast microscope. Semiquantitative RT-PCR was utilized to detect specific marker mRNA expression of oligodendrocytes. Using neuron marker anti-microtubule-associated protein, astrocyte marker anti-glial fibrillary acidic protein, oligodendrocyte marker anti-galactocerebroside, anti-myelin basic protein antibody, immunocytochemical staining was performed to detect the positive rate of the differentiation of BMSCs into oligodendrocytes. RESULTS: Morphological changes in BMSCs during the differentiation into oligodendrocytes: After the induction, a majority of BMSCs presented the morphological characteristics of oligodendrocytes. Cytoplasm retraction towards nucleus, cell process extension towards outwards, and strong refraction were found. With the prolongation of time, several cell processes connected and formed a typical net-shape structure. Specific marker mRNA expression of oligodendrocytes: Following induction, specific strap of myelin basic protein mRNA and galactocerebroside mRNA could be detected. Positive rate of oligodendrocytes: During induction, the positive rates of galactocarebroside, myelin basic protein and microtubule-associated protein were 65%, 45% and 10%, respectively. CONCLUSION: The combination of epidermal growth factor, basic fibroblast growth factor and insulin-like growth factor can effectively promote the directional differentiation of BMSCs into oligodendrocytes.

Objective:To explore the mechanism of renal injury in severe acute pancreatitis(SAP).Methods:54 SD rats were randomly divided into sham operation group(n=24) and SAP group(n=30).SAP model was induced by injection of 5% sodium taurocholate solution into bilo-pancreatic duct.Rats from each group were killed and the serum and ascite were collected at 6h,12h,24h after operation.Tumor necrosis factor-?(TNF-?)level of serum and ascite were measured by enzyme linked immuno-sorbent assay (ELISA).The expressions of TNF-? mRNA were assessed by RT-PCR analysis.The serum and ascites that had been collected at 24h after development were injected into medium of NRK cell,then the levels of TNF-? secreted by NRK cell were measured after injection 6h,12h,24h,and the cell apoptosis were also evaluated after injection 24h.NRK cell apoptosis were evaluated by flow cytometry (FCM).Results:①The serum and ascites levels of TNF-? were signficantly elevated in SAP groups compared with sham operation groups at any time point(P
3.The comparisons of sleep patterns between depression and insomnia without depressive symptom

ObjectiveTo verify the homology hypothesis of depression and insomnia.Methods All-night physiological signals of electroencephalogram (EEG), electrosculogram (EOG) and electromyogram (EEG) of 30 depression cases and 30 insomnia cases were recorded and analyzed with polysomnogram.ResultsThe rapid eye movement (REM) sleep of depression patients was more active, and there was a significant difference in all REM indexes compared with insomnia cases (P<0.01). Characteristics of insomnia cases were poor sleep continuity, easy wake after sleep-onset, and depression of slow wave sleep and REM sleep. There was no slow wave sleep in some insomnia cases.Conclusion The sleep patterns of depression and insomnia are different, REM sleep is hyperactive in depression; non-REM sleep is disturbance in insomnia.
4.The effects of γ-IFN on the expression of HLA-G mRNA in primary cultured astrocytoma cells

Objective To study the effects of γ-IFN on the expression of HLA-G mRNA in primary cultured astrocytoma cells. Methods Different concentrations of γ-IFN were added to primary cultured cells, and HLA-G mRNA were detected by RT-PCR. Results After γ-IFN treatment, HLA-G mRNA can not be determined from HLA-G originally negative astrocytoma cells. The expression of HLA-G are up - regulated in all original HLA-G positive astrocytoma cells in a dose-dependent manner. Conclusion γ-IFN can increase the ex-pression of HLA-G gene in the primary cultured astrocytoma cells which HLA - G are originally positive.
5.Grading prevention strategy for airway burn during CO2 laser surgery of larynx

OBJECTIVE To explore t he significance of reducing the incidence rate of airway burn of CO2 laser laryngeal surgery by spreading the idea of grading prevention st rateg y. M ETHODS We retrospectively reviewed the data of laryngeal lesions treated by the endoscopic CO2 laser surgery from Feburary 1992 to December 2016. The period was divided into two stages, the first one is from Feburary 1992 to December 2008, and the second one is from January 2009 to December 2016. We will distinguish the difference of airway burn between the two stages. RESULTS Four cases of airway burn were found in the first stage including 3 cases of glottic cancer and 1 case of vocal cord polyp with Reink's edema. But no airway burn happened in the second stage. Two cases of endotracheal intubation cuff broken by laser were found promptly, but with FiO2 decreasing from 70% to 30%, wet gauze filling subglottic region and the endotracheal tube reserved, the laser surgery proceeded till airway burn happened. The endotracheal intubation cuffs of the remaining two cases kept intact intraoperatively, the FiO2 was 70% when airway burn happened. One case had the cervical tracheal stenosis with long-term tracheotomy, The other 3 cases had no postoperative complications. CONCLUSION The grading prevention strategy is helpful for reducing airway burn during the CO2 laser surgery of larynx.
6.Diagnosis and therapy of lingual ectopic thyroid

OBJECTIVE To explore the strategy of diagnosis and therapy of lingual ectopic thyroid(LET). METHODS We retrospectively reviewed the clinical data of6 cases of LET, and figured out the clinical features, imaging characteristics, therapy and prognosis.RESULTS 6 lingual ectopic thyroids were diagnosed by the Tc-99m thyroid static imaging; 3 cases accepted surgery, one only took oral levothyroxine, and one selected watch and see. 3 cases of lingual ectopic thyroid accepted the ectopic thyroid transposition and took oral levothyroxine postoperatively. Pharynx foreign body sensation and sleep snoring disappeared one month later. The lingual ectopic thyroid carcinoma resection was performed by direct laryngoscope in one case which was lost to follow up.CONCLUSION The thyroid static imaging is specific to the diagnosis of the LET. The indications of the surgery were obvious symptoms such as airway obstruction, snoring and pharyngeal foreign body sensation, and mass hemorrhage, cystic degeneration and carcinogenesis. The principle of therapy is to retain thyroid function as far as possible and comprehensive use of surgery, thyroid hormone replacement and iodine radiotherapy to improve symptoms, prevention and treatment of hypothyroidism.
7.Relationship between the expression of high mobility group protein B1 and blood-borne metastasis in pancreatic carcinoma

Objective To investigate the correlation of the expression of high mobility group protein B1 (HMGB1) and blood-borne metastasis in patients with pancreatic carcinoma. Methods The serum HMGB1 levels were determined by Western blot analysis in 68 patients with pancreatic cancer, 18 patients with chronic pancreatitis and 21 healthy controls. Among the 68 patients with pancreatic cancer, the serum HMGB1 levels of 37 patients before and after operation were compared. The expression of HMGB1 and CD31 were detected by immunohistochemistry in 67 patients with pancreatic carcinoma. Results The serum HMGB1 levels in patients with pancreatic cancer was (119.7±54.5) ng/ml, which was significantly higher than (40.2±25.5) ng/ml in chronic pancreatitis (P<0.001) and (13.1±4.3) ng/ml in healthy control (P< 0.001). The serum HMGB1 levels in patients with pancreatic cancer before operation was (120.2±8.2) ng/ml, which was significantly higher than (69.3±5.1) ng/ml after operation (P <0.001). The expression rate of HMGB1 in pancreatic cancer tissues was 43.6%. The expression of HMGB1 were significantly related with tumor differentiation, TNM stage, blood-borne metastasis, and vascular density(P <0.01). The HMGB1 positive tumor cells were adjacent to the blood vessels with lumen, and the rate of HMGB1 expression in intravascular tumor cells was 71%. Conclusions The HMGB1 was highly expressed in pancreatic cancer tissues. HMGB1 expression positive pancreatic carcinoma cells were prone to invade blood vessels with lumen which may be related to blood-borne metastasis in pancreatic cancer.

The improvement of diagnostics teaching system,including the establishment of curriculum system and evaluation system,is the base of promoting clinical- medicine teaching.Our study showed that the theoretical knowledge and clinical skill of medical students could be improved by constructing clinical diagnostics curriculum system and improving organization management and assessment system,which could pave the way for the transition from medical students to clinicians.
9.Laryngeal cyst after partial laryngectomy

OBJECTIVE To study the incidence,pathogenesis,diagnosis and treatment of laryngeal cyst after partial laryngectomy.METHODS The clinical data of 228 cases of partial laryngectomy from January 2003 to June 2009 were retrospectively analyzed,and related literature was reviewed.RESULTS Two cases were found to have laryngeal cysts after partial laryngectomy during the post-operative follow-up.One occurred after 20 months,and the other occurred after 19 months.The cyst located on the tongue surface of the epiglottic was removed by CO2 laser under suspended laryngoscope,while the one located at the subglottic area was removed via cervical approach.So far,a total of 9 cases were reported.CONCLUSION Laryngeal cyst after partial laryngectomy is uncommon.That may be the mucosa planted during the operation.It could be misdiagnosed as recurrent laryngeal carcinoma.CT or MRI is helpful for diagnosis.Operation with CO2 laser under suspended laryngoscope or via cervical approach may be chosen.
10.Construction and application of an intelligent injury monitoring system in Yinzhou District

Injury monitoring is an important approach for injury control and is an important part of comprehensive disease monitoring. With the development of medical digitalization, an intelligent injury monitoring system has been created in Yinzhou District, Ningbo City, Zhejiang Province using artificial intelligence techniques based on Ningbo Municipal Health Information Platform and has been applied across the district since 2019. The manual card-reporting mode has been transformed to intelligent card-reporting mode in this system, which achieves functions of epidemiological analyses of the monitoring data, early warning of high-incidence injuries, classified management of injury and quality control of report cards. Nearly 300 thousand cards have been automatically reported since the system was online available since November 2022, and the epidemiological characteristics of injury were preliminarily understood, which provide data supports to early earning and interventions of injury. The intelligent injury monitoring system greatly improves the injury monitoring efficiency and card-reporting quality, and saves a large number of manpower and material resources, which provides a powerful technical support to widespread injury monitoring.
