Public hospital reform is the key to medical reform in China, and it is found that there is an obvious effect boundary between the zero margin drug profit and adjustment of medical service price reform, after looking back to progresses in these years.Mainly because it is not reasonable to cut off and change the internal motivation of supplier-induced demand, by the above two polices, It is of great concern to arouse medical staff''s enthusiasm about public hospital reform, by adjusting the remuneration system of the public hospital, which is also the most deep-seated mechanism problem of Public hospital reform.Starting from the overall policy framework of Public hospital reform in Sanming city, this study illuminates the inherent logic and margin of effect between the policies of reform of physicians'' remuneration system. From the analysis, it has been realized that physicians'' compensation system is the logic foundation of deep-rooted institutional problem. The present study draws a conclusion that it is not realistic to make great achievement by one or more policies.Meanwhile, it is rational to reach consensus on key links and progress of reform, by the deep understanding and analysis of public policy integrality.

The main objective of today’s graduate education reform is to improve the quality of graduate students. Curriculum development, as an essential part of graduate education, is pivotal to this goal. While classroom teaching innovation is placed in the front of curriculum development, it has become an important issue for graduate education study to practice an alternative capability centered way other than the current knowledge centered classroom teaching. We offered the course called "literature presentation and discussion" to the class 2006 graduate students at Chinese Academy of Medical Sciences. This article briefed and discussed the purpose, practice of this course, feedbacks from students and problems and solutions.

Objective: To observe the effect of elk antlers ethanolic fluidextract on behavior and immune function of aging model mice. Methods: ICR mice were randomly divided into 4 groups: Normal control group (NG), model control group (MG), low-dose drug treatment group (LG,2g/kg) and high-dose drug treatment group (HG,4g/kg). Mice were given subcutaneous injection of D-galactose (D-gal) at 120 mg/kg to induce subacute aging model. At the same time LG and HG were respectively administrated corresponding concentration of elk antlers ethanolic extract. Six weeks later, the learning and memory test was run by Y-maze. Then single agar immunodiffusion method was used to detect IgG level in serum, and spleen lymphocyte transforming stimulation index (SI) was determined by methyl thiazolyl tetrazolium(MTT) method as well as the concentration of Interleukin-2(IL-2) and interferon-gama(IFN-?) in serum were determined by ELISA method. Results: The memory and immune function of aging mice declined signif icantly. Elk antlers ethanolic fluidextract could obviously improve memory ability, increase the IgG level, raise the lymphocyte transforming SI, and elevate the concentration of IL-2 and IFN-? in model mice induced by D-gal. Conclusions: Elk antlers ethanolic extract could improve behavior and immune function of aging mice to delay aging process.

Objective To evaluate the effect of the transcranial magnetic stimulation on upper-extremity function rehabilitation and changes in the excitability of cerebral cortex, and to evaluate from the viewpoint of electro-physiology the prognosis so as to guide the rehabilitation treatment of patients after stroke. Methods Forty-six patients in the early stage after a stroke were given TMS examinations of the ipsilateral brain region. Those with the motor evoked potentials (MEPs) amplitudes lower than 50 μV were classified into a motion-induced experimental negative group (the negative group),whiie those whose MEP amplitude reached 50 μV or more were classified as movement-induced positive (the positive group). Both groups were given the same treatment. Before and after 2,4 and 8 weeks of treatment the Fugl-Meyer movement function rating scale was used to assess their bilateral upper limb movement function.TMS technology was used to detect any change in the resting motor threshold (RMT) and the amplitude (Amp) of MEPs in the motor cortex. The incubation period of the cortex (CL) and the central motor conduction time (CMCT) in the contralateral motor cerebral cortex were also observed. Results After 4 weeks of treatment, the average score of the positive group on Fugl-Meyer upper movement function rating scale reached (54.99±2.76),significantly higher than before treatment and significantly higher than the negative group's average (P<0.05).After 8 weeks of treatment, the average score in the positive group had increased further to 73.11±2.98,still significantly higher than that of the negative group (P<0.01).After treatment,RMT decreased progressively in both groups, but that of the negative group dropped from (98.35±10.12) to (30.35±7.31) (9<0.01),with significantly greater decline in amplitude and rate than that of the positive group (P<0.05).After treatment, the Amp of both groups showed a gradual increasing trend.Amp increased earlier in the positive group, but there was no significant difference in the extent of the increase between the two groups (P>0.05).After the treatment the CL and CMCT had shortened significantly in the negative group compared to before the treatment (P<0.05),while there was no significant change in CL and CMCT after the treatment (P>0.05).Conclusions The excitability of the contralateral motor cortex changes after a stroke.TMS can be used to characterize the MEP to monitor and predict recovery. This should help clinicians prepare more scientific rehabilitation plans.
