1.Practice and Exploration of Medical Course Construction for Community Pharmaceutical Service Talents Cultivation in Our College

OBJECTIVE:To explore medical course construction for community pharmaceutical service talents cultivation. METHODS:According to the requirements for community pharmaceutical service talents cultivation in vocational colleges, Yancheng Health Vocational & Technical College strengthened the medical course construction by optimizing the curriculum system structure,selecting the teaching contents,reinforcing the students’vocational ability education and infiltrating humanistic educa-tion. RESULTS:In terms of medical curriculum structure,the college integrated the basic medical and clinical disciplines respec-tively into Essentials of Basic Medicine and Essentials of Clinical Medicine. In terms of selection the teaching contents,the college tried to weaken medical discipline consciousness and to follow the logical relationship among medical disciplines,not only pursuing the integrity and systematization of the knowledge from different subjects. Besides,the college added teaching contents,strength-ened the cultivation of vocational skills,and increased the practical teaching contents as well as infiltrates the humanistic education into the practical teaching. CONCLUSIONS:Strengthening medical course construction makes it better for medical course structure and teaching contents to achieve the requirements of the pharmaceutical service talent cultivation goals of“good command of medi-cine and pharmacy”.
2.Effect of Atorvastatin on the level of serum high-sensitivity C-reactive protein and nitric oxide in patients with acute cerebral infarction

Objective To study the effect of Atorvastatin on the level of serum high-sensitivity C-reactive protein(hs-CRP) and nitric oxide(NO) in patients with acute cerebral infartion(ACI). Methods 60 patients with ACI were randomly divided into the Atorvastatin therapy group (n=30) and the conventional therapy group (n=30). At the basic of conventional therapy, the Atorvastatin therapy group was treated with Atorvastatin 20 mg/d. Both groups were treated for 14 consecutive days. The levels of serum hs-CRP and NO were measured and the scores of neurological deficit (NDS) were evaluated before and after treatment. Public hospital reform is the key to medical reform in China, and it is found that there is an obvious effect boundary between the zero margin drug profit and adjustment of medical service price reform, after looking back to progresses in these years.Mainly because it is not reasonable to cut off and change the internal motivation of supplier-induced demand, by the above two polices, It is of great concern to arouse medical staff''s enthusiasm about public hospital reform, by adjusting the remuneration system of the public hospital, which is also the most deep-seated mechanism problem of Public hospital reform.Starting from the overall policy framework of Public hospital reform in Sanming city, this study illuminates the inherent logic and margin of effect between the policies of reform of physicians'' remuneration system.From the analysis, it has been realized that physicians'' compensation system is the logic foundation of deep-rooted institutional problem.The present study draws a conclusion that it is not realistic to make great achievement by one or more policies.Meanwhile, it is rational to reach consensus on key links and progress of reform, by the deep understanding and analysis of public policy integrality.
4.Experience and exploration on the implementation of humanity education for student nurses at our college

5.The observation on apoptosis from vitrectomy specimens of proliferative vitreoretinopathy

urpose To investigate apoptosis in vitrectomy specimens of proliferative vitreoretinopathy. Methods Vitrectomy specimens from 60 cases of different classes of proliferative vitreoretinopathy were studied by TdTmediated dUTP nick end labelling(TUNEL)method. Results The characteristic change of apoptosis was observed in all vitrectomy specimens.The amount of apoptotic nonpigmentary cell is gradually decreasing along with the development of proliferative vitreoretinopathy,and apoptotic pigmentary cells are observed. Conclusion There are different kinds of apoptosis cell in vitrectomy specimens of proliferative vitreoretinopathy.It is suggested that apoptosis might be one of the important mechanisms of regulating the degree in proliferative vitreoretinopathy.
6.Strengthen the scientific quality of graduate students by literature presentation and discussion course

The main objective of today’s graduate education reform is to improve the quality of graduate students. Curriculum development, as an essential part of graduate education, is pivotal to this goal. While classroom teaching innovation is placed in the front of curriculum development, it has become an important issue for graduate education study to practice an alternative capability centered way other than the current knowledge centered classroom teaching. We offered the course called "literature presentation and discussion" to the class 2006 graduate students at Chinese Academy of Medical Sciences. This article briefed and discussed the purpose,practice of this course,feedbacks from students and problems and solutions.
7.New serum markers:changing the status in quo of the diagnosis and treatment of hepatocellular carcinoma

Hepatocellular carcinoma(HCC)is one of the malignant tumors with the highest cause of death and increasing incidence worldwide.Accurate diagnosis in early stage is vital for the treatment of patients.Presently,routine screening strategies including ?-fetoprotein(AFP)and ultrasound every 6 months have been recommended for early detection in patients with liver cirrhosis to detect HCC at earlier stages.However,the sensitivity and specificity of AFP are far from satisfaction.With the development of recently techniques such as proteomics,it is possible for new HCC-specific markers being available in the near future.Golgi Protein 73(GP73)is most likely to be a promising serum marker.In a few available literatures,GP73 has sensitivity and specificity of 69% and 90%.And fucose-studded GP73 has sensitivity and specificity of 90% and 100%.Apart from GP73,lens culinaris agglutinin-reactive AFP,des-gamma carboxyprothrombin,?-L-fucosidase,glypican-3,hepatocyte growth factor,transforming growth factor-?1,vascular endothelial growth factor,and mucin 1 have been proposed as markers for HCC detection.Clinical trials are undergoing to estimate the value of the above markers,and may change the status in quo ofthe diagnosis of HCC.We have started a multi-centeral trial to investigate GP73 in HBV and HBV related HCC patients with positive preliminary results.
8.Improvement of preparing technique for compound glycyrrhiza oral solution

Objective To obtain a clear and qualified compound glycyrrhiza oral solution by using NaSO3 and EDTA as stabi-lizers and Tween80 as solubilizer so as to solve the problem of morphine content instability. Methods NaSO34g and EDTA 0.6 g as stabilizers,and Tween803 g as solubilizer were added in the traditional method. The pH of the solution was adjusted to 8.0. Then the solution was obtained and filled in the brown polyester bottle. Results The preparation was clear,qualified and the content of mor-phine was steady. Conclusion The improved method is feasible,simple,stabilized and suitable for manufacturing.
9.Exploration of Community Pharmaceutical Care Talents in Higher Vocational Colleges

OBJECTIVE:To explore the cultivation method for community pharmaceutical care talents in higher vocational colleg-es. METHODS:According to the talent training aims to meet the demand of community pharmaceutical service positions in higher vo-cational colleges,the teaching reform was carried out for talent cultivation mode,course structure system,training base construction and teaching methods. RESULTS:In terms of talent cultivation mode,the work-integrated learning mode was built;in terms of course structure system,“Medicine,Pharmacy,Humanities and Expansion”course module was constructed to further optimize it;in terms of training base construction,the capital investment was increased to improve the school training base,meanwhile,cooperation between schools and enterprises was carried out to strengthen the construction of out-of-school training bases;in terms of teaching methods, PBL,case teaching model and situational teaching method were implemented. CONCLUSIONS:The quality of pharmaceutical talents has been significantly improved by implementing reform of the talent cultivation mode,the course structure system and teaching meth-ods,together with constructing training bases.
10.Chylothorax in non-Hodgkin's lymphoma: a case report and review

Objective To explore the diagnosis and treatment of non traumatic chylothorax in non-Hodgkin's lymphoma (NHL) and to understand the differences in diagnosis and treatment between chylothorax and pseudochylothorax. Methods The patient aged 83 years was confrimed as chylothorax and NHL after lymph node biopsy.We reviewed literatures about chylothorax in NHL to analyse the possible mechanism,its diagnosis and treatment. Results The patient was sufferring from unilateral chylothorax diagnosed as true chylothorax by thoracentesis,and progressed to bilateral chylothorax after 1 year.PET/CT examination showed intrathoracic and right cervical lymph nodes enlargement and an increasing metabolic activity.Cervical lymph node biopsy revealed diffuse large B cell type non-Hodgkin's lymphoma.The patient refused any other treatment except the diet therapy and died after 19 months.We searched 19 cases with NHL chylothorax in associated literatures about the treatments including radiotherapy (6/6 improved),chemotherapy (6/11 improved),thoracic duct ligation (I/1 improved),thoracic duct ligation and drug pleurodesis(1/1 improved). Conclusions PET/CT is useful in finding the hidden clues of chylothorax in NHL.There is no standard mangement for NHL chylothorax and the treatment must be individualized. The overall prognosis of NHL chylothorax is similar to that of non-Hodgkin's lymphoma and the patient needs early diagnosis and general treatment in order to prolong the survival time.
