1.Discussion of several legal issues concerning compensation for mental damage

Based on a study of 33 cases of medical tangle wherein demand for compensation for mental damage was met, the paper analyzes the current status of demand for compensation for mental damage, including the cause, extent and smount of money. It also discusses the double effect of mental compensation and points out that perfecting the establishment of medical laws and regulations is the only way which must be passed in solving issues of medica1 tangle concerning mental compensation.
2.Comparison of clinical value of two kinds of imunoassay method in the detection of related antibody of patients with HCV

Objective To compare the clinical value of two kinds of imunoassay method including CLIA and ELISA in the detection of related antibody of patients with HCV.Methods 60 patients with HCV were chosen as study subjects,and the related antibodies of patients were detected by CLIA and ELISA.The positive rates of anti HCV antibody,anti-3E antibody,anti-GP210 antibody,anti-SP100 antibody,anti-PML antibody and anti-AMA -M2 antibody detected by CLIA and ELISA were compared.Results The positive rates of anti-HCV antibody detected by CLIA and ELISA were 83.33% (50/60),98.33% (59/60),respectively.The anti-HCV antibody detected by CLIA was significantly higher than by ELISA (x2 =4.73,P =0.03).The positive rates of anti-3E antibody,anti-GP210 antibody,anti-SP100 antibody,anti-PML antibody and anti-AMA-M2 antibody detected by ELISA were 11.67 % (7/60),6.67 % (4/60),11.67 % (7/60),10.00 % (6/60),11.67 % (7/60),respectively,those detected by CLIA were 73.33 % (44/60),50.00% (30/60),38.33 % (23/60),58.33 % (35/60),80.00%(48/60),respectively.The positive rates of anti-3E antibody,anti-GP210 antibody,anti-SP1O0 antibody,anti PML antibody and anti-AMA-M2 antibody detected by CLIA were significantly higher than by ELISA(x2 =18.81,11.51,13.23,15.08,13.75,all P<0.05).Concelusion Compared with ELISA,CLIA in the detection of related antibodies of patients with HCV can efficiently higher the positive rate of anti-HCV and autoantibody.
3.Long-term follow-up of inferior vena cava filters in conjunction with thrombolysis and anti-coagulant therapy in prevention of pulmonary embolism in patients with deep vein thrombosis

Objective To analyze retrospectively long term follow up of inferior vena cava filters in conjunction with thrombolysis and anti coagulant therapy in prevention and treatment of pulmonary embolism (PE) in 24 patients with deep vein thrombosis (DVT) Methods This study included 13 males and 11 females with an average age of 52 4 years (14-86 years) Percutaneous implantation of inferior vena cava filters was performed via a femoral vein in 24 patients with acute or subacute DVT, of whom two were given conjunctive catheter based urokinase thrombolysis After the procedure, 20 patients underwent intravenous urokinase thrombolysis with subcutaneous low weight molecular heparin for 10 days and subsequent oral warfarin for six months Results All patients underwent a successful interventional procedure with an average 15 month (10-48 months) follow up One week after the procedure, relief of symptoms related to DVT was achieved in all the 24 patients Neither filter migration and thrombolic occlusion of filter nor PE and major hemorrhage were observed in this series Conclusion The use of inferior vena cava filters in conjunction with thrombolysis and anti coagulant therapy is a safe and effective treatment modality in patients with DVT, which can be used to prevent subsequent PE
4.Use and short-term outcomes of intra-aortic balloon counterpulsation in acute myocardial infarction

Objective To examine current use, associated complications, and short-term outcomes in patients with acute myocardial infarction (AMI) requiring intra-aortic balloon pump (IABP) counterpulsation. Methods A single-centre registry was conducted between October 2004 and July 2008. Data were collected from 121 consecutive AMI patients treated with IABP counterpulsation at the 28th Division of Beijing Anzhen Hospital. Results Among 806 patients with AMI, IABP counterpulsation was employed in 121 patients. Placement of an IABP was indicated for hemodynamic support during high-risk catheterization and/or angioplasty (66.9%) and cardiogenic shock (27.3%). Balloon insertions were successful in 98.3% of patients and duration of mean use was 34.2?16.2 hours. No major IABP complications occurred in all the patients. Diagnostic catheterization was performed in all the patients inserted with IABP, and 93.4% underwent coronary revascularization before hospital discharge. The total in-hospital mortality was 13.2% (34.4% in patients with cardiogenic shock) and varied markedly by indication and use of revascularization procedures. Conclusion IABP counterpulsation may be successfully employed for a wide variety of conditions in the AMI setting, providing significant hemodynamic support with rare major complications in a high-risk patient population. IABP counterpulsation may reduce mortality in patients with AMI undergoing reperfusion therapy.
5.Role of nociceptive arcuate nucleus neurons in chloroquine-induced pruritic behaviors in mice.

Despite its clinical importance, the underlying central mechanisms of pruritic behaviors are poorly understood. To investigate the role of nociceptive arcuate nucleus neurons in chloroquine-induced pruritic behaviors in mice, we tested the effect of arcuate nucleus neurons and interscapular brown adipose tissue (IBAT) on itch produced by intradermal injection of chloroquine in the nape of the neck. Our results provide several lines of evidence for an important role of nociceptive arcuate nucleus neurons in chloroquine-induced pruritic behavior: (1) Intradermal microinjection of chloroquine resulted in a dramatic increase in itch behaviors accompanied by the activation of c-Fos positive neurons in arcuate nucleus; (2) Microinjection of chloroquine significantly increased IBAT temperature in the mice. These findings suggested that chloroquine-induced pruritic behaviors were associated with the activity of nociceptive arcuate nucleus neurons.
6.Predictive value of 18F-FDG PET-CT for lymph node metastasis of peripheral lungadenocarcinoma

Objective Classification of non-small cell lung lymph (NSCLC) node (N) is one of the key factors influencing treatment, however, the cilinical noninvasive and invasive approaches to N classification have their limitations.This study aimed to investigate the risk factors of lymph node metastasis of peripheral lungadenocarcinoma by using CT and PET / CT scans.Methods Retrospective analysis had been done on a total of 248 patients who underwent surgical resection from February 2010 to November 2015 in our hospital.All of them underwent chest CT and 80 patients underwent PET/CT examination.Univariate analysis was applied in the relation of lymph node metastasis to gender, age, smoking situation, CEA, SUV, cancer size, pathological variants, and the degree of differentiation.Multivariable logistic regression analysiss were performed in the prediction of risk factors for lymph node metastasis.ResultsSeventy-four patients (29.8%) had regional lymph node metastases.Univariate analysis showed that lymph node metastasis was related to the serum CEA level, degree of differentiation, SUVmax, tumor size, lobulation/spiculation, pleural retraction, mediastinal or hilar lymphadenopathy (P<0.05).In the multivariable analysis of risk factors, including serum CEA, SUVmax and CT features, for predicting lymph node metastasis, the most important and significantly independent risk factors identified were SUVmax, CEA level, mediastinal or hilar lymphadenopathy, cavitation/bubble-likelucency and pleural retraction (P<0.05).Conclusion The lymph node metastasis is associated with SUVmax of primary tumor, serum CEA level, mediastinal or hilar lymphadenopathy, cavitation/bubble-likelucency and pleural retraction.The combination of radiographic features and serum CEA can help to predict more accurately the risk of lymph node metastasis in patients with peripheral lung adenocarcinoma.
7.Bacterial Resistance of 1850 Pathogen Strains

OBJECTIVE To acquaint pathogen distribution of infection and present drug-resistance conditions of the commonly encountered pathogens.METHODS The statistics of the antimicrobial susceptibility testing of all the clinical isolates from Jan to Nov 2006 in our hospital was condncted.RESULTS Totally 1850 bacteria strains were isolated.From them 620(33.51%) were Gram-positive bacteria,1230(66.49%) were Gram-negative bacteria,25.8% of which were Escherichia coli,8.1% of which were Staphylococcus epidermidis,7.5% were Klebsiella pneumonice and 7.2% were MSSA;Statistics of drug sensibility test suggested,Gram-positive cocci isolated from the clinical issue be highly sensitive to vancomycin,teicoplanin and nitrofurantoin.They were still sensitive to rifampicin.and lower sensitive to other antibiotics.Gram-negatives were high sensitive to imipenem.They were still sensitive to piperacillin/tazobactam and the third-generation cephalosporins.CONCLUSIONS The drug resistance of the isolates is severe.It is suggested that there be ungent need for surveillance of bacterial resistance and rational use of antimicrobial agents be emphasized during clinical therapy.
8.The impact of primary PCI in culprit Artery on epicardial blood flow of nonculprit artery in patients with anterior STEMI

ObjectiveTo study the impact of primary PCI in culprit artery on epicardial blood flow of nonculprit artery in patients with STEMI. MethodsEnrolled 117 patients with anterior wall STEMI were treated with primary PCI in the culprit artery, left anterior descending artery (LAD, as study group.Another 100 patients with normal coronary artery evidenced by angiography were enrolled as control group.The differences in CTFC (corrected TIMI frame count measured by using digital subtraction arteriography,TIMI =thrombolysis in myocardial infarction) and MBG (myocardium blood flow perfusion grading)between pre and post primary PCI in both culprit artery and nonculprit artery ( left circumflex artery, LCX),and CTFC and MBG were also detected in the subjects of control group. Blood samples were collected and the levels of CRP (C-reactive protein) were assayed. Clinical and angiographic features were analyzed.ResultsThe CTFC of nonculprit artery (LCX) and the level of MBG in patients with anterior wall STEMI were different from the level of MBG and CTFC in control group ( P<0. 05) before primary PCI. The level of MBG and CTFC in nonculprit artery (LCX) were improved (P < 0. 05 ) after primary PCI, but they did not resume to normal level. Patients without reflow in culprit artery had higher incidence of no reflow in nonculprit artery than patients with re-flow (78% vs. 8%, P < 0. 01 ), and the level of CRP in patients without reflow in nonculprit artery were higher than those in patients with re-flow ( P < 0. 05). Conclusions The perfusion of nonculprit artery may be impaired in patients with STEMI. Although the perfusion of nonculprit artery may be improved after primary PCI in culprit artery, but it was still lower than those in the control group, and inflammation mechanism might contribute to it.

Objective:To detect the expression of microRNA-218 (miR-218) in human acute lymphocyte leukemia (ALL) T lymphocytes ( CCRF-CEM) ,explore its effects on the biological features of CCRF-CEM cells and the expression of its target gene c-kit, so as to provide new insights for leukemia treatment.Methods: Using the quantitative real-time polymerase chain reaction ( qRT-PCR) ,we detected the expression of miR-218 in the normal peripheral blood T lymphocytes and CCRF-CEM cells.Forty-eight hours after the miR-218 mimic was transfected into the CCRF-CEM cells,the expression of miR-218 in the CCRF-CEM cells was detected by qRT-PCR.The effect of miR-218 on the CCRF-CEM cell viability was detected using MTT.The effect of miR-218 on the proliferation and apoptosis of CCRF-CEM cell was analyzed using flow cytometry.c-kit gene was identified to be a target gene of miR-218 by luciferase reporter enzyme system,and the effect of miR-218 on the expression of KIT protein in cells were determined using Western blot.Results:As shown by qRT-PCR,compared with that in the normal peripheral blood T lymphocytes,the expressions of miR-218 in ALL T lymphocytes cell lines were significantly decreased ( P<0.01 ) .Compared with the control group, the expression of miR-218 increase significantly in CCRF-CEM cells transfected with miR-218 mimic for 48 hours ( P<0.01).MTT showed that the cell viability decreased significantly after the over-expression of miR-218 in the CCRF-CEM cells ( P<0.05 ) .Flow cytometry showed that the S-phase fraction significantly declined after the over-expression of miR-218 ( P<0.01 ) , and meanwhile the apoptosis of cells also significantly increased (P<0.01).Luciferase reporter gene assay showed that,compared with the control group,the relative luciferase activity significantly declined in the miR-218 mimic transfection group (P<0.01).Compared with the control group,the expression of KIT protein in the CCRF-CEM cells transfected with miR-218 mimic for 48 hours significantly decreased ( P<0.01).Conclusion:The expression of miR-218 decreases in ALL T lymphocytes cell lines.MiR-218 can negatively regulate the expression of KIT protein,inhibit the proliferation and increase the apoptosis of CCRF-CEM cells.Treatment based on the enhanced expression of miR-218 may be a promising strategy for leukemia.
