1.Determination of the Propofol Concentration of Plasma in Children Undergoing Cardiopulmonary Bypass by RP-HPLC

Objective To establish a HPLC method to detect the propofol concentration of plasma in children undergoing cardiopulmonary bypass. Methods RP-HPLC with fluorescence detector was used in the study. Plasma samples were precipitated by the thymol-methanol solution, and centrifugated. 20?l of supernatant fluid was injected for analysis and separated by Dikma Diamonsil C18 column. The mobile phase was composed of acetonitril:water =56:44(V/V), flo rate was 1.0ml/min. Fluorescence detective wave lengths were ?ex= 276nm, ?em=312nm. HPLC machine was Agilent1100 series. Main column was Diamonsil C18(150?4.6nm,5um),guard column was Phenomenex Security(4mm?3mm). Results There was a good linear relationship within the range of 0.02~8.0mg/L(r=0.9993, n =9),the limit of detection was 5?g/L,average recovery was 96.99%,intra RSD(relative standard deviation )was between0.810% and 5.73%,inter RSD was between 0.932% and 7.337%. Conclusions The method is sensitive, simple, rapid, good linear relationship and precise. The experiment can be used for determination of propofol concentration in child undergoing cardiopulmonary bypass.
2.Numerical simulation research of the triangular cross-section structure flow resistance in endovascular stent

To provide a reference basis for the structural design and optimization of endovascular stent by analyzing the flow resistance of the new stent with triangle cross-section using numerical simulation method, and finding out the stent structure which will influence hemodynamic parameters. Four kinds of models of the triangle cross-section bare stent were constructed in infinite flow field using solidworks software. Numerical simulations of the four models were performed respectively using ANSYS finite element software. Steady flows and transient flows in these models were studied. Hemodynamics data in the four models were collected, such as the flow patterns, the distribution of pressure and the flow resistance of the new stents. The stent with triangular wire cross-section can be applied to cover aneurysm cavity because that resistance of blood inflow to aneurysm was large and outflow from aneurysm was small. Thus, it was easy to outflow than inflow. Thereby blood perfusion and flow of aneurysm cavity was restrained, and the pressure of aneurysm cavity was reduced, which plays a certain effect on the treatment of aneurysm. This result provides some instructions for the design of stent structure.
3.Approach to the patients of diabetic foot disease with refractory ulcers - Report of 4 typical cases with therapeutic experiences

The professional care by multi-disciplinary team and priority of prevention should be carried out in the treatment of diabetic foot disease to reduce diabetic amputation.This article describes the professional experience in the treatment of four complicated cases with diabetic foot disease and emphasizes the importance of the co-operation among different specialists,including diabetologists and wound,vascular,orthopedic surgeons,etc.as well as of varied therapies applied in staged management of the diabetic foot care,by treating these patients with diabetic foot disease as early as possible.
4.Serum uric acid levels predict the risk of developing prediabetes in an adult population

Objective To evaluate whether the serum uric acid (SUA) levels are related to development of prediabetes. Methods This was a 6-year cohort study, subjects were recruited from Tianjin Medical University General Hospital Health Management Center. A prospective assessment (n=30 910) was performed. Subjects without a history of prediabetes were followed up for 6 years (with a median follow-up of 2.7 person-years). All relevant variables including SUA concentrations, and fasting plasma glucose were assessed at baseline and yearly during the follow-up. Prediabetes was defined in accordance with the American Diabetes Association criteria of 2014. Multivariate-adjusted Cox proportional hazards regression models were fitted to assess relationships between the quintiles of baseline SUA and the incidence of prediabetes. Results The incidence of prediabetes was 55 per 1 000 person-years. In comparison with subjects in the lowest quintile, the odds ratios and hazard ratio (95%confidence interval) of higher quintile of SUA were 1.04 (0.93, 1.15), 1.07 (0.95, 1.19), 1.13 (1.01, 1.28) and 1.15 (1.02, 1.30) (P for trend=0.01), respectively, after adjusting for potential confounders. Conclusion SUA levels predicted an increased risk of prediabetes in adults.
5.Application of arginase-Ⅰ and glypican-3 combined examination in differential diagnosis of hepatocellular carcinoma

Objective To explore the value of arginase-1(Arg-1) and glypican-3 (GPC-3) combined examination in the differential diagnosis of hepatocellular carcinoma (HCC),metastatic carcinoma (MC) of liver and intrahepatic cholangiocarcinoma (ICC).Methods From January 2005 to December 2011,a total of 54 patients with HCC were selected,including 10 cases with high differentiation,25 cases with moderate differentiation and 19 cases with poor differentiation.At the same time,25 patients with MC of liver and 20 patients with ICC were selected.A total of 31 normal liver specimens were set as control.The expressions of Arg 1 and GPC-3 in the above tissues were detected by immunohistochemistry method.The sensitivity and specificity of the examination in the diagnosis of HCC were analyzed.Chi-square test was performed for count data analysis.Results The positive expression rate of Arg-1 in HCC,MC of liver,ICC and normal liver tissues was 87.0% (47/54),4.0% (1/25),5.0% (1/20) and 100.0% (31/31),respectively.The Arg 1 positive expression rate in HCC tissues was higher than that in other tumor tissues of non-HCC,and the difference was statistically significant (x2 =66.98,P<0.05).The positive expression rate of GPC-3 in HCC,MC of liver,ICC and normal liver tissues was 70.4% (38/54),12.0% (3/25),5.0% (1/20) and 0 (0/31),respectively.The GPC-3 positive expression rate in HCC tissues was higher than that in other tumor tissues of non-HCC and the difference was statistically significant (x2=37.98,P<0.05).The sensitivity and specificity of Arg-1 or GPC-3 positive in HCC diagnosis was 92.6% (50/54) and 86.7% (39/45).The sensitivity and specificity of both Arg 1 and GPC-3 positive in HCCdiagnosis was 64.8% (35/54) and 100.0% (45/45).Conclusion Arginase-1 and glypican-3 combined examination has an important value in HCC diagnosis and differential diagnosis.
6.Complications following paclitaxel-eluting stent implantation:6-month follow-up

BACKGROUND:The safety and efficacy of paclitaxel-eluting stents (Taxus DES) has been proved by international researchers in clinic investigations.Based on further inclusion criteria,the incidence of in-stent restenosis is still lower than that of bare-metal stent.OBJECTIVE:TO observe restenosis of Taxus DES and the effect on local vessels through applying the follow up of coronary angiography and to investigate the biocompatibility of stent and host.DESIGN:Following-up observation.SETTING:Department of Cardiology,General Hospital of Chinese PLA.PARTICIPANTS:A total of 297 patients who had undergone coronary Taxus DES implantation for coronary artery disease were selected from Cardiovascular Department of General Hospital of Chinese PLA from May 2003 to May 2005.There were 265 males and 32 females and their ages ranged from 36 to 76 years.All patients provided informed consent,and the experiment had got confirmed consent from local ethic committee.METHODS:All patients were implanted Taxus DES and received the follow up within 6 and 12 months.In addition,at 6 months after operation,coronary angiography was used to measure the reference vessel diameter (RVD) and the minimal lumen diameter(MLD),calculate diameter restenosis rate and observe late loss.MAIN OUTCOME MEASURES:Coronary angiography at 6 months after Taxus DES implantation and biocompatibility of stent and host.RESULTS:①Quantitative angiographic analysis(QCA):Angiographic follow-up showed that the late loss of in-stent was significantly higher than that of pro-in-lesion and dis-in-lesion(P<0.05).②Coronary angiography in-stent restenosis:In 134 angiographic follow-up patients,a total of 14 patients experienced restenosis,and the incidence was 10.4%(14/134).The patterns of restenosis were diffuse in-stent in 7 patients and the rate of revascularization was 6.7%.③Stent aneurysm:Angiographic evidence of aneurysm was observed in one patient among follow-up cases,and the rate of which was 0.75% (1/134).④Clinic follow-up major adverse cardiac events(MACE):Among 297 patients,one patient was attracted sudden death 5 months after intervention (0.34%; 1/297),and one patient was suffered subacute thrombosis 5 days after stent implant (0.34%;1/297),and late thrombosis occurred in 2 patients.The general rate of MACE was 1.35%.CONCLUSION:①The late loss of Taxus DES mainly takes place in in-stent.The patterns of restenosis of Taxus DES are in majority of diffuse in-stent,and the incidence of MACE is low.②Taxus DES possibly results aneurysm in local vessels.The follow up indicates that Taxus DES has a good biocompatibility to patients.
7.Levels of Th1/Th2 type cytokines in TNBS and OXZ colitis without or with Trichinella spiralis infection

Objective To study the levels of Th1 and Th2 type cytokines in 3-nitrobenzene sulfonate(TNBS) and oxazolone(OXZ) colitis without or with Trichinella spiralis( T. spiralis) infection. Methods Female BALB/c mice were randomly divided into 4 groups: 50% Ethanol, T. spiralis only, TNBS or OXZ,T. spiralis +TNBS or OXZ(at least 6 in each group when mice were killed). The levels of IFN-γ, IL-12,IL-4, IL-10 in colon in mice with colitis induced by TNBS or OXZ without or with T. spiralis infection 3 d and 7 d post-induction were assessed using ELISA method. Results Colonic protein levels of IFN-γand IL-12 were significantly increased 3 d and 7 d after intra-colonic injection of TNBS( P <0.05 ). Concurrent infection with T. spiralis prevented this rise in IFN-γand IL-12 secretion and tended to induce a rise in colonic IL-4 and IL-10 content(P <0. 05). The levels of IFN-γ, IL-4 and IL-10 protein in colitic mice colon with prior nematode infection on days 3 and 7 post-induction of colitis were significantly higher than that seen in colitic mice without prior nematode infection ( P < 0.05 ). Conclusion T. spiralis infection significantly attenuates TNBS-induced colitis in mice. The local immunologic mechanism is that T. spiralis can down-regulate strongly Th1-type immune response of colitis and up-regulate Th2 response, Tr1-cytokines. According to the change of Th1/Th2 cytokines, prior T. spiralis infection doesn't reduce the severity of OXZ-induced colitis, but without aggravating colitis. The exactly immunologic mechanism deserve to be explored deeply.
8.An in vitro experimental study and clinical applications of MR angiography with low-dose contrast agent of lower limb arteries at 3.0 T

Objective To evaluate the feasibility and diagnostic accuracy of CE-MRA with low dose contrast agent by comparison with DSA in diabetic patients with peripheral arterial diseases. Methods ( 1 )Study in vitro: test tubes containing Gd-DTPA of different concentrations were scanned, and the relationship between signal intensities and concentrations of GD-DTPA was analyzed. DSA and CE-MRA with selected concentrations of Gd-DTPA were performed on stenotic vascular models to estimate the proper low dose of GD-DTPA for clinical applications. (2) Clinical applications: 78 diabetic patients with peripheral arterial diseases were scanned from the abdomen and pelvis station to the calf-foot station in a 3 T MR system with standard bolus chase 3D CE-MRA sequence after injection of 13 ml GD-DTPA . The image quality,diagnostic rate of stenosis of arteries in calf and degree of venous contamination were evaluated with Fisher's exact test. DSA images of 220 vascular segments in 22 patients ( 10 segments per patient) were acquired as the gold standard and compared with CE-MRA by using Kappa test. Results The MR signal intensities were proportional to the concentrations of contrast agent in present study, and all stenotic segments of vascular model were displayed by CE-MRA with GD-DTPA at lower concentration of 1.5 mmol/L. As for MRA images of 78 diabetic patients with low dose Gd-DTPA, about 97.4% (76/78) showed diagnostic image quality for pelvic and thigh stations. But the MRA images of lower extremities were interfered by the venous contamination significantly (P < 0.01 ). Compared with DSA for 22 patients, the diagnostic sensitivity, specificity and agreement coefficient (Kappa value) of MRA were 96. 0% ( 168/175), 73.3%(33/45), and 0.72 (P<0.01), respectively. Conclusion Using 3.0 T MR scanner, high quality CE-MRA of lower limb arteries can be obtained for clinical applications with contrast agent dose as low as 13 ml,which has comparable diagnostic sensitivity and specificity with DSA. But the limitation of venous contamination in MRA image should be resolved in further studies.
9.Colorectal polyp segmentation algorithm integrating Transformer and convolution

In response to the challenges of varied sizes and diverse shapes of colorectal polyps,especially with blurred boundaries that often complicates localization and smaller polyps being particularly prone to oversight,a colorectal polyp segmentation algorithm integrating Transformer and convolution is proposed.Transformer is employed to extract global features from images for ensuring the network's capability for global modeling and improving the localization capability for both main polyp regions and vague boundaries.Subsequently,convolution is introduced to augment the network's ability to process polyp details,refining boundary segmentation and enhancing the capture capability for small-sized polyps.Finally,a deep fusion of the features extracted by Transformer and convolution is carried out to realize feature complementarity.The experimental evaluation using CVC-ClinicDB and Kvasir-SEG datasets show that the algorithm has similarity coefficients of 95.4%and 93.2%,and mean intersection over union of 91.3%and 88.6%,respectively.Further tests on the generalization capability of the algorithm are conducted on CVC-ColonDB,CVC-T,and ETIS datasets,in which similarity coefficients of 81.3%,90.9%and 80.1%are obtained.The results indicate a notable improvement in the accuracy of polyp segmentation achieved by the proposed algorithm.
10.Infection of Oligochaetes, Limnodrilus hoffmeisteri (Annelida: Oligochaeta), in the Nasal Cavity of a Chinese Man.

The infection by Limnodrilus hoffmeisteri Claparède, 1862 (Oligochaeta: Tubificinae) in humans is relatively uncommon. The present report is to describe an incidental human infection with oligochaetes in the nasal cavity of a Chinese man, a 25-year-old man residing in Zhangjiakou city, Hebei province, China presenting with nose bleed, severe itching, continuous sneezing, and rhinorrhea. A lot of oligochaete worms were found in the nasal discharge of the patient. The detected worms were identified as Limnodrilus hoffmeisteri (Annelida: Oligochaeta) based on morphological and molecular characteristics. This incidental L. hoffmeisteri nasal infection is the first case in China and indicates that oligochaete worms can be encountered in humans.
