1.Microinvasive therapy for intraspinal tumors resection via Key-hole on mono-lamina

Objective To research the microinvasive therapy for intraspinal tumors resection and study the ways for spinal function protection. Methods Twenty-four cases were detected by MR. The intraspinal tumors located variously and the size of them ranging from 0.9 cm to 3.0 cm. All the tumors underwent resection via Key-hole on mono-lamina and the clinical data were analyzed. Results After tumor pathological examination,the cases of neurofibroma,neurinoma,and spinal meningioma were 9,12 and 3 cases. respectively. All the patient recovered and they walked within one week after operation. No recurrenced on MR examination after 3 months and no spinal tube destruction on CT examination after 6 months. Conclusion The microinvasive therapy via Key-hole on mono-lamina is benefic to the totally resection of intraspinal tumors and also benific to the spinal function protection.

Targeted therapy of anti-angiogensis strategy is the standard treatment for metastatic renal cell cancer.In recent years,a number of new generation of anti-angiogensis agents have been tested in clinical trials,some of which have achieved promising outcomes.Other pathway inhibitors such as inhibitors of mamma-lian target of rapamycin and fibroblast growth factor receptor pathway have made progresses in some extent.
3.Second line treatment of non-small cell lung cancer

The cytotoxic agents pemetrexed and docetaxel and the epidermal growth factor receptor (EGFR)tyrosine kinase inhibitors(TKIs)erlotinib and gefitinib are standard second-line therapies for non-small cell lung cancer. For patients without the EGFR mutation,more and more evidence has suggested the superiority of chemotherapy over targeted therapy. Adding targeted agents to standard second-line treatment is an trend of exploration,but without promising results nowadays. Crizotinib,targeting at anaplastic lymphoma kinase,has been shown excellent efficacy for second-line therapy in non-small cell lung cancer.
4.MRI Features and Its Clinical Significance of Bone Contusion in Knee

Objective To discuss the MRI features and its clinical significance of bone contusion in knee joint.Methods MR images of injuries of knee in 40 cases were retrospectively analyzed,the complications were reviewed simultaneously.Results 69 lesions of bone contusion in 40 cases were discovered,of them,Ⅰ type was in 38 and Ⅱ type was in 31, in company with fractures were 24 in 22 cases and ligament injury was in 33 cases.Conclusion The present of bone contusion is prompt to the bone injuries,MRI is of significant value in showing bone contusion and its complications in knee.
5.Effects of dexmedetomidine on activity of nuclear factor kappa B in neutrophil granulocytes during one-lung ventilation in patients undergoing pulmonary lobectomy

Objective To evaluate the effects of dexmedetomidine on the activity of nuclear factor kappa B (NF-κB) in neutrophil granulocytes during one-lung ventilation in the patients undergoing pulmonary lobectomy.Methods Thirty-eight ASA physical status Ⅰ or Ⅱ patients,aged 40-64 yr,weighing 50-75 kg,scheduled for elective pulmonary lobectomy,were randomized into 2 groups (n =19 each) using a random number table:control group (group C) and dexmedetomidine group (group D).In group D,dexmedetomidine 1.0 μg/kg was infused intravenously over 15 min before induction of anesthesia,followed by infusion at a rate of 0.5 μg· kg-1 · h-1 until 30 min before the end of operation.The equal volume of normal saline was given in group C.At 5 min after intubation,0,30 and 60 min of one-lung ventilation,0 and 30 min of two-lung ventilation,and 30 min after operation,blood samples were taken from the radial artery for blood gas analysis and for measurement of plasma concentrations of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6).The oxygenation index and respiratory index were calculated.The nuclear protein of neutrophil granulocytes was extracted for measurement of NF-κB DNA-binding activity.Results Compared with group C,the respiratory index,plasma concentrations of TNF-α and IL-6 and NF-κB DNA-binding activity were significantly decreased,and no significant difference was found in the oxygenation index in group D.Conclusion Dexmedetomidine can inhibit the activation of NF-κB in neutrophil granulocytes and is helpful in reducing the systemic inflammatory responses during one-lung ventilation in the patients undergoing pulmonary lobectomy.

AIM: To investigate the relationships between I?1 hs1,2 VNTR polymorphism and IgA nephropathy. METHODS: Four hundred and ninteen patients with IgA nephropathy and their first-degree relatives were recruited. Two hundred and one sex and age-matched normal Chinese Han volunteers were also recruited as controls. After extracting genomic DNA, the VNTR genotypes of I?1 hs1,2 region were determined by PCR and electrophoresis, and the results were analyzed by transmission disequilibrium test (TDT) and haplotype relative risk (HRR) in the families, and Chi-Square test in the case-control analysis. Objective To evaluate the diagnostic ability of fusion images of SWI and DWI in ischemic penumbra(IP)of patients with acute ischemic stroke(AIS).Methods 47 AIS patients were retrospective analyzed.Two neuroradiologists analyzed the fusion images of SWI and DWI respectively.SDM was defined as that the hypo-intensity signals of intramedullary or sulcal veins were dilated or increased on fusion images compared with contralateral areas.The results compared with the PDM,which was considered as the“gold standard”.Areas under the receiver operating characteristic curve(AUC)were used to assess the efficacy of SDM which determined by fusion images and compared to PDM.Chi-square test was used to analyze the consistency between the two neuroradiologists and also the consistency between SDM and PDM in IP assessment Results The fusion images of SWI and DWI had a high diagnostic efficacy compared to PDM, AUC of the two radiologists were 0.885,0.877,the diagnostic sensitivity were 84.2%,78.9% and specificity were 92.9%,96.4%respectively.There was also a high consistency in SDM assessment through fusion images between the two neuroradiologists (Kappa=0.908,P >0.05). Conclusion Fusion images of SWI and DWI have a high diagnostic efficacy in IP assessment,which may be considered as a simple approach for IP assessment in patients with AIS.
8.Relationship between hepatitis B virus infection and B-cell non-Hodgkin lymphoma

Objective To evaluate the association between B-cell non-Hodgkin lymphoma (NHL) and hepatitis B virus (HBV).Methods The positive rates of HBV markers in 284 patients of B-cell NHL who were admitted to our department between January 2003 and December 2009 were investigated.The positive rates of HBV markers in colorectal cancer patients were used as controls.Results The HBsAg-positive rates of patients aged 18~ 39 and stage m/Ⅳ patients were 42.6 % (26/61) and 37.0 % (50/135),which was higher than other groups.The x2 value and P value were 7.573 and 6.874,0.023 and 0.009,respectively.Compared with the control group, the B-cell NHL had significantly higher prevalence of positive HBsAg and positive HBeAg (29.6 % vs 14.5 %,6.7 % vs 0.8 % ).The Wald values were 25.174 and 20.496,respectively.Both of the P value were <0.001 and lower prevalence of positive anti-HBs (45.4 % vs 58.0 %,Wald =11.062,P =0.001).The coexpression of HBsAg, HBeAg and anti-HBc was higher in the B-NHL group than in the control group (6.0 % vs 0.8 %,x2 =31.619,P <0.001).Similarly,the coexpression of HBsAg,anti-HBe,and anti-HBc was higher in the B-NHL group (16.2 % vs 11.5 %,x2 =4.542,P =0.033).Significantly higher rate of positive anti-HBc and negative anti-HBs was observed in the B-NHL group (37.0 % vs 24.5 %,Wald =17.708,P < 0.001),whereas the same group showed a lower rate of negative anti-HBs compared with the control group (20.8 % vs 27.8 %, Wald =5.646, P =0.017).Conclusion This finding of a positive association between HBV infection and B-NHL suggests that HBV may play an etiologic role in the induction of B-NHL.
9.Expression of c-met, e2f-1 and Ki-67 in tissues of gastric cancer

Objective To detect the expression of c-met (hepatocyte growth factor receptor), e2f-1 (transcription factor) and Ki-67 in tissues of gastric cancer, explore the relationship among them, and investigate their correlationship with clinicopathological characteristics and prognosis. Methods The tissue samples of gastric cancer from 86 patients with radical resection were subjected to immunohistochemical staining, and the expression of c-met, e2f-1 and Ki-67 was detected. The relationship between the clinicopathological characteristics and the expression of c-met, e2f-1 and Ki-67, and that among the expression of c-met, e2f-1 and Ki-67 were explored by univariate and multivariate data analysis. And the relationship between prognosis and expression of c-met, e2f-1 and Ki-67 was analysed by Log rank test. Results c-met, e2f-1 and Ki-67 were widely expressed in tissues of gastric cancer. The higher expression of c-met was associated with higher expression of Ki-67, shorter survival time, upper tumor location, higher lymph node metastasis rate, higher N stage and TNM stage (P<0.05), and the lower expression of e2f-1 was associated with larger tumor diameter, higher TNM stage, deeper invasion, higher lymph node metastasis rate and higher N stage (P<0.05). The multivariate analysis revealed that depth of invasion, N stage, TNM stage and expression of Ki-67 were independent factors for positive expression of c-met, and age and survival time were independent factors for positive expression of e2f-1. Log rank test demonstrated that the factors related to survival included expression of c-met, e2f-1 and Ki-67, N stage, tumor diameter, tumor location, lymph node metastasis rate, depth of invasion and TNM stage. Conclusion c-met is an indicator for malignant behavior and poorer prognosis of gastric cancer. There is a higher expression of e2f-1 in early gastric cancer, while advanced gastric cancer may be associated with a lower expression of e2f-1.
10.Phase Ⅱ trial of second-line chemotherapy with docetaxel and capecitabine in advanced esophageal squamous cell carcinoma

Objective To evaluate the effects of antitumor,toxicity and survival of second-line chemotherapy with docetaxel and capecitabine in patients with advanced esophageal squamous cell carcinoma.Methods Thirty eligible patients with measurable lesions received 1-hour intravenous treatment of docetaxel (60 mg/m2 on day 1) plus oral capecitabine (825 mg/m2 twice daily on days 1-14) every 3 weeks for up to 6 cycles.Results Patients received a median of two cycles of treatment (range 2-6).The median follow-up interview was 15.4 months (3.0-31.5 months).Intent-to-treat efficacy analysis demonstrated an overall response rate of 23.3 % (0 complete and 7 partial) and stability of 43.3 % (13 cases).The median time to progression was 3.0 months (95 % CI 1.929-4.071).The median survival was 8.3 months (95 % CI6.848-9.752).Severe adverse events (grade 3/4) reported were neutropenia (10 cases),anaemia (5 cases),thrombocytopenia (3 cases),hand-foot syndrome (4 cases),and fatigue (3 cases).Conclusion Docetaxel plus capecitabine have a manageable adverse event profile and promising activity in advance esophageal squamous cell carcinoma as a second-line treatment.
