OBJECTIVE: To analyze the characteristics of adverse drug reaction (ADR) induced by Levofloxacin (LVFX) injection and to promote rational use of drugs in the clinic. METHODS: 137 LVFX injection-induced ADR cases reported in our hospital during 2006～2009 were collected and analyzed statistically with Excel in respect of age, gender, medication and clinical manifestation, etc. RESULTS: ADR cases occurred nearly in all ages of people, and 32.85% were appeared in people over 60 years. Main clinical manifestations were lesion of skin and its appedants (62.04%) and gastrointestinal injury (16.06%). Most of them were mild and medium ADR. CONCLUSION: Great importance should be attached to the rational use of LVFX injection and ADR monitoring to guarantee the safety of patients.

Objective To investigate the changes of ultrastructure and Bax,Bcl-2 expression of cerebral cortex in lymphostatic encephalopathy(LE).Methods LE models were established by occluding and removing both the shallow and deep cervical lymph nodes in rats.Ultrastructural changes of the cerebral cortex were observed at 1 d,2 d,3 d,5 d,7 d and 14 d after operation.Expression of Bax and Bcl-2 were examined by Western blot.Results Brain edema appeared under electron microscope in LE model group.The perivascular spaces in the cortex were enlarged and full of fluid,the dropsical nerve fibers had swelling and fragmented myelin sheath.Some neurons showed apoptotic conformation and some had characters of necrosis.The changes mentioned above were the most serious at day 5.The protein expression of Bax,Bcl-2 and the ratios of Bax/Bcl-2 in LE 3 d,5 d,7 d groups were significantly higher than those in normal control group(all P

Objective:To report and analyze a rare family of sponduloepiphyseal dysplasia congenital(SEDC) in order to supply more resources for genetic bone disease. Methods:Investigation and analysis was performed on a four generation's family of SEDC.Clinic characteristics including X-ray image and chromosome analysis were evaluated.Results:Nine persons suffered from SEDC in this four(generation's) family.The patients presented with same clinical characteristics.The main bone damages affected vertebrae,articulatio coxae,caput femoris and neck. Conclusion:The mode of inheritance of SEDC may be autosomal dominant inheritance.Gene defect during embryonic period may interfere the growth of osteoepiphysis.Further molecular pathologic studies were needed to find the evidence of genetic prognostication of SEDC.

Objective To investigate the expression of EphrinB2 and microvessel density(MVD) in colorectal carcinoma and to analyze the relationship between the expression of EphrinB2 and the biological characteristics of colorectal carcinoma. Methods EphrinB2 and MVD of 63 colorectal carcinoma diagnosed by pathology were examined by immunohistochemistry, and 25 specimens from normal colorectal tissue were examined as controls. Results The expression of EphrinB2 protein was 82.0 % (51/63) in colorectal carcinoma and 32.0 % (8/25) in the normal colorectal tissues, the difference was significant (P <0.05). Meanwhile, MVD was significantly higher in EphrinB2 positive group (39.39±8.97) than that in negative group (33.00 ±10.19) (P<0.05). The expression of EphrinB2 protein had significant positive correlation with differentiation grade, infiltration, lymph node metastases and clinical stage (P <0.05), and had no relation to age, sex, tumor size and location(P >0.05). Conclusion The EphrinB2 protein expression increase may be involved in development of colorectal carcinoma and play important roles in the tumorigenesis and progression of colorectal carcinoma.

Objective To retrieve and analyze the available evidence on prevention of needlestick injuries among healthcare workers and summarize the best available evidence.Methods We searched the BMJ best practice,Uptodata,Cochrane Library(2014),Joanna Briggs Institute Library,Registered Nurses' Association of Ontario,Scottish Intercollegiate Guidelines Network,National Guideline Clearinghouse,and complementally searched the PubMed,EM-base,CNKI and Centers for Disease Control to collect literatures including guideline,evidence summary,best practice information sheet,recommended practice and systematic review.Results Nine references were selected including a clinical decision support system,3 evidence summaries,2 recommended practices and 3 systematic review.Finally,7 items of best evidence were summarized,including:wearing double gloves,never recapping needles,using blunt-tip suture,using a neutral zone for passing sharp device,adopting the safety features of sharps,using sharp disposal containers,and continuous education for needlestick injuries.Conclusion Healthcare workers should regulate their behaviors of using sharp instruments during clinical practice and guarantee personal safety by consulting high level evidence.

Objective To assess the efficacy of the laryngeal mask airway i-gel (LMA i-gel) in patients umdergoing laparoscopic cholecystectomy. Methods One hundred and twenty ASA Ⅰ or Ⅱ patients of both sexes,aged 34-62 yr, weighing 45-90 kg, undergoing elective cholecystectomy using fiberoptic laparoscope, were randomized into 2 groups (n =60 each): LMA i-gel group (group I) and LMA ProSeal group (group P). After induction of anesthesia with sufentanil 0.2 μg/kg, propofol 2.5 mg/kg and vecuronium 0.1 mg/kg, LMA i-gel and LMA ProSeal were inserted in group I and P respectively. A gastric tube was inserted through the drain tube of the LMAs. The number of attempts, success rate, success rate of gastric tube placement, airway seal pressure, SpO2 ,PETCO2, peak airway pressure and complications (hypoxemia, nausea and vomiting, choking hoarseness, sore throat and regurgitation of gastric contents) were recorded. The fiberoptic laryngoscope scores were assessed after sccessful LMA placement. The anesthesia time, duration of surgery, extubation time and emergence time were also recorded. Results There was no significant difference in the anesthesia time, duration of surgery, extubation time and emergence time between the two groups ( P ＞ 0.05). The SpO2, PETCO2 and peak airway pressure were within the normal range during operation in both groups. The first attempt success rate of LMA placement and fiberoptic bronchoscopic scores were significantly higher, and the LMA placement time was significantly shorter in group Ⅰ than in group P (P ＜ 0.05). The success rate of LMA and gastric tube placement was 100% in both groups. The incidence of sore throat was significantly lower in group I than in group P ( P ＜ 0.05). Conclusion LMA i-gel can provide adequate ventilation during operation with less complications and can be used effectively for cholecystic laparoscopic surgery.

Objective To evaluate the expression and clinical significance of matrix metalloproteinase 14(MMP-14) in osteoar-thritis(OA) patients in synovium and synovial fluid of knee joint .Methods Semi-quantitative RT-PCR were used to detect the ex-pression of MMP-14 mRNA in 32 case of OA (experimental group)and 26 non-OA patients(control group)synovium ;protein level expression of MMP-14 in synovial fluid were analyzed by ELISA .Results The expression of MMP-14 mRNA in experimental group synovium of knee joint was significantly increased compared with control group (P<0 .05);but the protein level of MMP-14 was low in experimental group synovial fluid compared with control group (P<0 .05) .Conclusion The expression of MMP-14 is increased in OA patients synovium and maybe promote the progress of OA directly ;the protein level of MMP-14 is decreased in OA synovial fluid ,indicated the function of MMP-14 in OA synoviocyte indirectly .

Objective To investigate the expression and significance of MMP-3 in knee joint synovium of different stage in osteoarthritis (OA) patients. Methods MMP-3 protein were detected by immunohistochemical method in knee synovial tissues from 90 OA patients (the OA group) and OA group was divided into 3 subgroups according to Kellgren and LawrenceX-ray diagnosing standards: the gradeⅠ(n = 30), gradeⅡ (n = 30) and grade Ⅲ subgroups (n = 30). 30 patients were enrolled as control group. Immunohistochemistry was used to detect the expression of MMP-3 protein in the tissue. Results The expression level of MMP-3 protein in OA group was significantly higher that in the normal synovium (P < 0.05). There existed significant difference in the expression of MMP-3 protein between the grade Ⅲ subgroup and the gradeⅠ, or Ⅱsubgroups (P < 0.05). The expression of MMP-3 protein related positively to the severity of OA (r = 0.912, P < 0.05). Conclusion The expression of MMP-3 protein related closely to the pathogenic mechanism of OA. It may serve as an important indicator of early diagnosis and the activity of OA.

Objective To approach the design and clinical application of large anterolateral thigh flap and its effect in wound repair.Methods The flaps were designed according to the anatomical features of perforating branches in the anterolateral thigh flaps.When a flap was chipped,a thick branch or a terminal branch of original vessel was reserved,another suitable perforating branch was selected in the proximal or distal end of the flap,and then the two vessels were anastomosed to enlarge the range of blood supply.If the vessel pedicle of a flap was a musculocutaneous perforating branch,the perforating branch of anastomosis was cut at out-point of muscle.If the vessel pedicle of a flap was a intermusclar branch or a direct skin artery,the perforating branch of anastomosis was cut widely.From May 2006 to May 2012,the technique was applied in 28 patients with large skin defect of limbs.The diameters of perforating branches obtained at out-point of muscles were measured during surgery.The survival of flaps was observed after surgery and complications in donor sites were checked during follow-ups.Results There were 18 flaps whose vessel pedicle were musculocutaneous perforating branches.The branches were cut at outpoint of muscles.The diameters of these vessels were measured during surgery.They ranged from 1.3 mm to 1.8 mm with an average of 1.45 mm.All of the vessels could be anastomosed.All 28 flaps survived.All flaps survived.The areas of the flaps ranged from 22 cm × 15 cm to 42 cm × 14 cm.Artery crisis happened in 2 flaps whose vessel pedicle were musculocutaneous perforating branches.The second look operation found that the areas of artery anastomosis of perforating branches and vessel pedicles were compressed by hematoma and thrombus formed.The 2 flaps survived after the hematoma was cleared away and the vessels were reanastomosed.There were no infections.Both the donor and recipient site healed by first intention with no necrosis of flap margin.All 28 patients were followed up by 4-13 months with an average of 8 months.There were no apparent collapse deformities,muscle necrosis,declines of muscle strength and muscle hernia in the donor sites.The appearance of flaps was flat,the color was close to normal and the quality was fine.Conclusion It is a safe and effective method to repair wound surface by large anaterolateral thigh flap obtained by the modus operandi of perforating branch anastomosis.
