1.Three-stage swallowing rehabilitation for stroke patients with dysphagia

Objective To explore the effects of three- stage swallowing rehabilitation on the swallowing a-bility of stroke patients with dysphagia. Methods 60 stroke patients were divided into primary cerebral infarction and primary cerebral hemorrhage groups, then further divided into treated and control groups randomly. All groups were given the same routine internal medicine treatment. Patients in the treated group were given three stage swallo-wing rehabilitation training additionally. All patients were assessed using Caiteng's Grading Method at the outset and at the end of the 2nd week, the Ist month and the 2nd month. Results Swallowing function scores in the treated groups were higher than those in the control groups at every stage (P≤0.05). The treated groups' scores also im-proved more quickly than those in the control groups. Conclusion Three stage swallowing rehabilitation can signifi-cantly improve stroke patients' swallowing function.
2.Apoptosis of hypertrophic scar fibroblasts induced by HMME-PDT

Objective To investigate the apoptotic effects of hypertrophic scar fibroblast (HSF) induced by HMME-PDT.Methods Fibroblasts were cultured from nontreated hypertrophic scars,and cells at passages 4-6 were used for the experiments (photosensitizer dose 4 μg/ml,λ630 nm,pow er density 10 mw/cm2,energy fluence 2.5 J/cm2).Morphological and biochemical changes in fibroblasts were assessed by Hoechst 33258 staining and fluorescence microscopy.The rate of apoptotic or necrotic cells was detected by flow cytometry (FCM) through double staining of Annexin V -FITC and popodium iodide (PI),respectively.Results Marked morphological features of cell apoptosis were viewed under the fluorescent microscope through Hoechst 33258 staining.The analysis of FCM indica ted that the apoptotic rate was significantly increased after HMME PDT [(34.82 ± I.42) % vs (3.12±0.28) %,P<0.05],and apoptotic rate was higher than necrosis rate [(14.65±1.02) % vs (34.82±1.42) %,P<0.05].Conclusions Low level exposure to 630 nm PDT mediated by HMME appears to induce fibroblast apoptosis.
3.Effects of caspase 3 on hypertrophic scar fibroblasts in hematoporphyrin monomerthyl etherphotodynamic therapy (HMME-PDT)

ObjectiveTo investigate the role of caspase 3 in HMME-induced apoptosis in hypertrophic scar fibroblasts (HSFs).MethodsFibroblasts were obtained from 10 patients with untreated hypertrophic scar,and subjected to a primary culture.After 4 to 6 passages of culture,the HSFs were divided into 3 groups to remain untreated(control group),be treated with HMME followed by photodynamic therapy (HMME-PDT group),or the combination of HMME and Z-DEVD-FMK followed by photodynamic therapy (caspase 3 inhibitor group).At 12 hours after the therapy,HSFs were collected and immunofluorescence microscopy was used to observe the fluorescence intensity of caspase 3 after staining with fluorescein isocyanate (FITC) and popodium iodide (PI),flow cytometry was performed to determine the percentage of caspase 3-positive HSFs and apoptosis rate in HSFs after single staining with FITC and PI respectively.Results The fluorescence intensity of caspase 3 was weak in the control group and caspase 3 inhibitor group,but was strong in the HMME-PDT group.An increased percentage of caspase 3-positive HSFs was noted in the HMMEPDT group compared with the control group and caspase 3 inhibitor group(30.86% ± 1.21% vs.3.12% ±0.28% and 2.46% ± 0.18%,t =19.92,21.76,both P < 0.05).The apoptosis rate in HSFs was significantly higher in the HMME-PDT group and caspase 3 inhibitor group than in the control group(30.54% ± 3.78% and 10.46% ± 2.15% vs.2.45% ± 0.22%,t =35.90,27.97,both P< 0.05),and higher in the HMME-PDT group than in the caspase 3 inhibitor group.ConclusionsThe apoptosis in HSFs induced by HMME-PDT is closely related to the activation of caspase 3,while caspase 3 seems to be dispensable for the apoptosis.
4.The influence of blood pressure on prognosis during speech therapy after stroke

ObjectiveTo investigate the improvement of speech in patients with increased blood pressure(BP) and stable blood pressure during speech therapy. MethodsAfter monitoring blood pressure with dynamic blood-pressure meter during speech therapy, patients were divided into increased BP group and stable BP group. Patients received two-month speech therapy, then their score changes in ABC examination of pre-treatment and post-treatment were compared. ResultsIn oral expression, score changes in increased BP group were significantly different from those in stable BP group (P<0.05). In listening comprehension, score changes had no significant difference between two groups(P>0.05). Conclusions Patients in increased blood pressure group progressed obviously in oral expression.
5.Study on pulmonary function changes in infants with human bocavirus or mycoplasma pneumonia

Objective To discuss the pulmonary function change in infants with human bocavirus (HBoV) pneumonia or mycoplasma pneumonia (MP) and its clinical signiifcance. Methods One hundred and forty infants under 3 years old who were admitted due to pulmonary infection were recruited from January, 2013 to October, 2013. Among them, HBoV-DNA was detected in 64 cases, and MP-DNA was detected in 76 cases. Thirty eight normal age-matched infants were selected as controls. The shape of tidal breathing lfow-volume loops (TBFVLs) and change of every index were measured with a Pulmonary Testing System (Jaeger MasterScope). Results The ratio of time to peak tidal expiratory lfow (PTEF) to total expiratory time (TPTEF/TE), the ratio of VPTEF to expiratory volume (VPTEF/VE), tidal expiratory lfow at 25%of the remaining tidal volume (TEF25%) and the ratio of TEF25%to PTEF (25/PF) were signiifcantly decreased in infants with HBoV and MP infection as compared with healthy infants (P<0.05). However, there was no signiifcant difference of the above indices between infants with HBoV and MP infection (P>0.05). The shape of TBFVLs in infants with HBoV and MP pneumonia was changed and characterized by left-shifted PTEF and trough-like concave in descending limb. The PTEF was decreased in infants with pulmonary infection. Conclusions HBoV or MP infection results in impaired pulmonary function with manifestations of obstruction in small airway. The shape of TBFVLs in infants with HBoV and MP pneumonia is characterized by left-shifted PTEF and trough-like concave in descending limb.

Objective To investigate the management and control of hypertension in patients with chronic kidney disease(CKD) and its associated factors.Methods Data of 726in-patients with CKD and hypertension who hospitalized in our hospital from March 2009 to April 2010 were studied.Results 91.74% of patients was treated with antihypertensive medications,and 21.21%, 22.59%, 19.56%, 28.37% of patients received 1, 2, 3, ≥4 antihypertensive drugs,respectively.42.4% of patients with CKD and hypertension could be controlled up to the standard,and the mean blood pressure was(137.86±20.75)/(76.30±11.35) mm Hg.There was significant difference among stage 1 plus 2, 3, 4 plus 5 (non-dialysis), 5 (dialysis) kidney diseases, with the hypertension control rate being 50.8%, 46.7%, 42.0%, 33.5%, respectively.The hypertension control rate of non-dialysis patients was significantly higher than that of dialysis (44.9% vs 33.5%,P<0.05).There was no significant difference between blood dialysis group and peritoneal dialysis group(32.3% vs 38.7%, P>0.05).Multivariate Logistic regression analysis showed that female (OR=1.787, 95%CI 1.045-3.056)and ACEI application (OR=4.378, 95%CI1.830-10.472) were positively associated with hypertension control.Whereas, diabetes (OR=0.415, 95%CI 0.188-0.919)and pulse pressure (OR =0.847, 95% CI 0.811-0.885) were associated with inadequate blood pressure control.ConclusionsDespite almost universal hypertension treatment is used in patients with CKD and high blood pressure, the hypertension control rate is still suboptimal.Female and ACEI are positively associated with adequate hypertension control, whereas diabetes and pulse pressure are negatively associated with the standard.
8.Effect of calcium antagonist verapamil on tolbutamide-induced insulin release from islet?-cells of rats

Objective To investigate the effect of calcium antagonist verapamil on the function of rat?- cells and tolbutamide (D860)-induced insulin release.Methods Insulin released from isolated islets were measured in control,verapamil,D860,and verapamil+D860 groups.Furthermore,intravenous glucose tolerance test (IVGTT) was conducted in acute experiments treated with verapamil and D860 respectively to assess?-cell function in rats with the same allocation as in vitro.Another IVGTT was performed in the end of 4 weeks' treatment.The insulin contents in pancreas were assayed and pancreas islets morphology were observed with immunohistochemistry.Results Verapamil could inhibit insulin release from isolated islets.Verapamil group was [(1.244?0.082)ng?ml~(-1)?islet~(-1)]and control group (2.623?0.226) ng?ml~(-1)?islet~(-1)(P0.05).Also,similar results were obtained in normal rats during acute experiments and verapamil reduce the hypoglycemic effect promoted by D860. However,above results were not observed in the end of 4 weeks experiments,and no difference for insulin content and morphological change in islets was found among four groups.Conclusion Treatment of verapamil chronically does not impair islet function and interfere with the hypoglycemic effect of D860 in rats .

10.Preparation and evaluation of novel mesoporous molecular sieve of baicalin surface molecularly imprinted polymers.

Taking mesoporous molecular sieve MCM-41 as a substrate, baicalin (BA) as template, acrylamide (AM) as the functional monomer, ethylene glycol dimethacrylate (EGDMA) as a cross-linking agent, ethanol as solvent, under thermal polymerization initiator of azobis isobutyronitrilo (AIBN) , a kind of selective recognition of baicalin surface molecularly imprinted polymer was synthesized. The surface morphologies and characteristics of the MIPs were characterized by infrared spectroscopy (IR) and transmission electron microscope (TEM). The adsorption properties of polymer microsphere for the template were tested by the dynamic adsorption equilibrium experiments and static adsorption equilibrium experiments. The experiment showed that the imprinting process was successfully and the well-ordered one-dimensional pore structure of MCM-41 was still preserved. Furthermore, molecularly imprinted polymers had higher selective ability for BA, then provided a new method for the efficient separation and enrichment of baicalin active ingredients from medicinal plants Scutellaria baicalensis.
