1.Combination therapy of DPP-4 inhibitor and metformin: Rationality and its evidence

Metformin improves glycemic control mainly due to improved insulin sensitivity.Dipeptidyl peptidase-4(DPP-4) inhibitor,which suppresses degradation of glucagon-like peptide-1 (GLP-1) and maintains the bioactivity of endogenous GLP-1,improves islet dysfunction.Moreover,recent studies suggested that metformin enhanced the biological effect of GLP-1 via different mechanisms and that DPP-4 inhibitor enhanced insulin sensitivity in type 2 diabetes.Therefore,the combination therapy of DPP-4 inhibitor and metformin could simultaneously ameliorate the two major defects of type 2 diabetes and obtain complementary and synergistic benefits for glycemic control.
2.Research of concentration in clinical service:a patient-disease-medical humanities perspective

Objective To investigate the concentration on clinical service of both medical workers and patients,for the purposes of better quality of care and concentration of medical workers,and better mental feelings of the patients′ and doctor-patient relationship.Methods 1 000 medical workers and patients were sampled from 8 hospitals of four regions in Jiangsu province by stratified randomization for questionnaire survey.The purpose is to study the clinical concentration of medical workers in terms of interpretation,listening,emotion and empathy,and compare the differences between doctors and patients regarding clinical concentration.Results 48.5% of the medical workers and 44.8% of the patients said that medical workers would interrupt the patients within 2 minutes.Time constraints of clinical consultation contribute to poor disease information collection,medical explanation and informing.The investigation found the difference between doctors and patients in clinical concentration is significant statistically(χ2=104.329,P=0.000),while medical workers were unaware of their lack of concentration to patients.18.2% of the patients complained that medical workers could hardly or refuse to understand their feelings.Conclusions Medical workers are advised to respect their patients by offering total concentration on diagnosis and treatment;to respect their career with sufficient concentration on diagnosis and treatment;and to respect medical science to offer medical humanistic care.
3.Maternal homocysteine folic acid, MTHFR gene polymorphism and congenital heart defects in offspring

Objective To explore the relationship between maternal homocysteine (HCY), folic acid, 5, 10 methylenetetrahydrofolate reductase (MTHFR) genotypes(677C→T) and occurrence of congenital heart disease (CHD) in offspring. Methods HPLC technique was used to measure the level of total plasma HCY in 32 mothers of CHD children and 23 mothers of normal children, radio immunoassay was used to measure folic acid, and MTHFR genotypes were detected by PCR RELP analysis. 4.Querying and Searching of XML Electronic Medical Records

Objective To implement XML-based electronic medical records' searching and querying using new technology of XML storing and indexing.Methods The information in electronic records was expressed with CDA XML standard,and XML Query and XML IR were used for querying of electronic records.Results Based on XML querying and indexing models,a new electronic medical record XML querying and searching system was built.Conclusion Querying and searching over XML database has been paid more attention to,with wide use of data adapting to HL7 CDA XML standard in electronic medical records.
5.Development of major of basic medical science and reflection on the talents cultivation

6.Study on cellular localization of VP3 and its apoptosis-inducing effect on breast cancer cells

Objective:To observe the localization of chicken infectious anemia virus VP3 gene in normal cells and breast cancer cells in different times and its apoptosis-inducing effect.Methods: The fundamental cloning method,inserting the VP3 gene of chicken anemia virus into the eukaryotic expression vector pEGFP-C1 was used.Then,the positive recombinant containing VP3 gene pEGFP-C1-VP3 was transfected into human breast cancer cell line MCF-7 and mouse fibroblasts L929 by FuGENE(R)6 transfection reagent in vitro respectively.After 24 hours,48 hours and 72 hours,fluorescence microscope was used to observe the distribution of VP3 in cells and the rate of apoptosis was studied on the treated MCF-7 cells by FCM(Flow Cytometry).Results: The recombinant plasmid pEGFP-C1-VP3 could be localized in the nuclei of breast cancer cells,which showed the typical nuclear changes in different stages of apoptosis.In L929 cells,pEGFP-C1-VP3 underwent a process of migration from the nucleus to the cytoplasm,which didn′t induce apoptosis of L929 cells.Conclusion: VP3 located in the nucleus of MCF-7 breast cancer cells,which can led to cancer cell death by inducing apoptosis,and the apoptosis rate was higher than the control group with time dependence.VP3 located in the cytoplasm of normal cells,and didn′t induce apoptosis.
7.Effects of atmospheric fine particulate matter on respiratory diseases and symptoms of community residents in Chun'an County

Objective:
To evaluate the impact of fine particulate matter ( PM2.5 ) on respiratory diseases and symptoms of community residents in Chun’an County, so as to provide the basis for air pollution treatment strategies.
Methods:
Using the cluster random sampling method, the permanent residents in Qiandaohu Town of Chun’an County were investigated from 2017 to 2018. The demographic information, respiratory diseases and symptoms were collected by using Surveillance Plan for the Impact of Air Pollution ( Haze ) on health ( 2016 Edition ). The air pollutants and meteorological data were collected through Qiandaohu station of Hangzhou Ecology and Environment Monitoring Center and Hangzhou Meteorological Information Center. The effects of PM2.5 on respiratory diseases and symptoms of residents were analyzed with generalized estimating equation.
Results:
Totally 1 181 people aged 6 months to 95 years were recruited, including 557 ( 47.16% ) males and 624 ( 52.84% ) females. Acute nasopharyngitis (common cold) and tracheitis/tonsillitis occurred most frequently, with 203 cases, accounting for 3.44%. The median of daily average concentration of PM2.5 was 24 μg/m3, with the standard exceeding rate of 2.80%. The results showed that PM2.5 increased the risk of acute nasopharyngitis ( common cold ), tracheitis/tonsillitis, cough, expectoration, runny nose, sore throat and nasal congestion ( lag 3 days, OR: 1.015-1.022, 95%CI: 1.001-1.037 ); the effect of PM2.5+PM10 ( OR: 1.020-1.040, 95%CI: 1.006-1.070 ) and PM2.5+O3 ( OR: 1.017-1.024, 95%CI: 1.005-1.035 ) was greater than that of PM2.5 alone on respiratory diseases and symptoms.
Conclusion
Atmospheric PM2.5 exposure in Chun’an County increases the risk of respiratory diseases and symptoms among community residents.
8.Distribution of calcitonin gene-related peptide immunoreactive nerve fibers in rat periodontal tissue

Objective:To investigate the distribution of calcitonin gene-related peptide immunoreactive(CGRP-IR) nerve fibers in periodontal tissue of rats, providing morphological data of CGRP in periodontitis. Methods:Five adult SD rats were sacrificed by transheart perfution fixation with 4% paraformaldehyde. The mandibles were removed and decalcified. Frozen sections were made and CGRP-IR nerve fibers in periodontal tissue were observed with immunohistochemical technique. Results: CGRP-IR nerve fibers were observed in gingiva, alveolar bone .periodontal membrane; many heavily stained CGRP-IR nerve fibers were found beneath junctional epithelium, epithelium of gingival sulcus and epithelium of gingival col; some CGRP-IR nerve fibers even penetrated into junctional epithelium and epithelium of gingival col. Conclusion; CGRP-IR nerve fibers extensively disdtribute in the periodontal tissue with the densest in the initial area of periodontitis, suggesting that CGRP may play a role in the genesis and development of periodontitis.
9.Advance in Mechanism of Alkane Uptake by Microorganism

Recent advance in researches of alkane uptake by microorganisms was reviewed. To improve efficiency of bioremediation, knowledge of mechanism of water-insoluble substrate, especially alkanes, uptake by microorganism is badly needed. Powered by novel ideas and advanced techniques, more findings and conclusions were reported. Effects of surfactants on uptake of alkane, adjustment on cell surface properties and transport of alkane into the cells were discussed in this review, at the same time, evidences from analyses of cell ultra-structure and researches on cell chemotaxis were cited. There are still some questions in this area should be taken into consideration.

In both type 1 and type 2 diabetes mellitus,β-cell mass (BCM) is lost.Various treatments are developed to restore or reconstruct BCM.The development of non-invasive methods to quantify BCM in vivo offers the potential for early detection of β-cell dysfunction prior to the clinical onset of diabetes.PET imaging with radioligands that directly target the pancreatic β-cells appears promising.The ability to determine the BCM has been investigated in several targets and their corresponding radiotracers,including radiolabeled receptor ligands,antibodies,metabolites and reporter genes.Therefore,we summarize the recent progress in radionuclide molecular imaging of pancreatic β-cells.
