1.Effect of preoperative neoadjuvant chemotherapy on cervical carcinoma

Objective To study the value of neoadjuvant chemotherapy in the treatment of cervical carcinoma. MethodsA clinical prospective study was carried out from Jan.1997 to Dec.2003,118 patients with cervical carcinoma were given two cycles of combined chemotherapy before operation(neoadjuvant chemotherapy group,group A).102 patients accepted surgery alone in our hospital from Jan.1997 to Dec.2003 were chosen as the comparison group(group B).Survival rate and complications were observed and analysed. Results5 year survival rate in group A was significantly higher than that in group B(P<0.01),The 5 year survival rate was significantly higher in group A than in group B,despite of different pathological types,with or without radiochemotherapy(P<0.05).The recurrent rate was lower in group A than that in group B,which show significant difference in the 5 year recurrent rate(P<0.05). ConclusionNeoadjuvant chemotherapy is an effective method for the treatment of cervical carcinoma in early stage.Because of satisfactory short-term control rate,excision rate,long-term survival rate and low recurrent and metastatic rate,neoadjuvant chemotherapy is worth popularizing in clinical treatment.

4.Diagnosis and treatment of male pseudohermaphroditism (report of 21 cases)

Objective To report and summarize the improvement of diagnosis and treatment of male pseudohermaphroditism. Methods Among the 21 cases (mean age of 13 years,range from 6 to 31 years),19 were socially female sex and 2 were male sex.Visual clinical examination indicated abnormal external genitalia,lumps on both sides of inguina or labia majora pudendi.Genetic examination showed that the sex chromosome karyotypes of all cases were 46,XY.Before operation diagnoses were established in 20 cases by blood biochemistry,B-ultrasound and CT examination with misdiagnosis in only 1 case. Results All the 21 cases received operation.Female features were retained in 19 cases and male features in 2.Of them 15 cases were followed up for 3 to 8 years.Of the 15 cases 14 were treated by hormone replacement (oral stilbestrol)with obvious improvement of secondary sexual character.Of the 14 cases 11 had normal prepuberal physical development.The appearances of the external genitalia were satisfactory.One patient who maintained male sex had obvious growth of breasts during puberty. Conclusions Early diagnosis and treatment are very important to the therapeutic effects and the life quality of the patients.It is suggested that operation of maintaining female sex can be preferred as treatment of choice for male pseudohermaphroditism.
5. Chronopharmacokinetics of Amlodipine in Rats

OBJECTIVE: To study the influence of circadian dosing time on pharmacokinetics of amlodipine in rats and to assess possible pharmacokinetic interactions. METHODS: Twelve male SD rats were divided into two groups, and a single dose of amlodipine (1 mgkg-1) were given intragastrically at either 8 00 or 20 00, respectively. Plasma samples were collected at 0, 0.33, 0.67, 1, 2, 4, 8, 12 and 24 h after drug administration. A HPLC-MS-MS detection method were developed to determine amlodipine concentrations in plasma. RESULTS: Absorption of amlodipine in rat was very slowly with MRT0-t about 12 h. Dosing time schedule influnced the pharmacokinetics of amlodipine dramatically and showed higher plasma drug levels (P<0.01), larger AUC (P<0.01) and lower CL/F (P<0.01) when dosing at 20 00 compared with that at 8 00 group. The difference of drug use between ρmax, AUC0-t and MRT0-t was statistically significant (P<0.05).The ρmax is three times as large as the morning medicine, AUC is four times as large as the drug in the morning. In the evening, MRT is extended for five hours longer than in the moring. CONCLUSION: The amlodipine plasma drug concentrations are lower, and CL/F, Vd/F are larger when dosing at 8 00 than those at 20 00.The administration of amlodipine at different times of a day shows a certain effect on the pharmacokinetics of amlodipine.
6. Effects of combination of amlodipine and valsartan on in vivo pharmacokinetics of valsartan in rats

OBJECTIVE: To study the phamackinetics of amlodipine and valsartan for valsartan in rats, and to investigate the antihypertensive effect of combined treatment. METHODS Twelve male SD rats were divided into two groups. The valsartan (16 mg��g-1) and Exforge(containing valsartan 16 mg��g-1, amlodipine 1 mg��g-1) were given by using stomach tube respectively to rats. Blood samples were taken for detecting plasma concentration before dosing and at 0, 0.33, 0.67, 1, 2, 4, 8, 12 and 24 h after a single intrauenous administration. A HPLC method was used to assay the valsartan concentration in plasma. RESULTS The plasma concentration-time course showed one compartment model of valsartan plasma in rats, with the increased dose after 2 h, Cmax and AUC has a significant increase(P<0.05), while Vd and CL has a significant decrease(P<0.01). CONCLUSION Combination of valsartan and amlodipine could increase the plasma concentration and AUC, while decrease CLand Vd, higher than single antihypertensive effect.
7.MR diagnosis of Arnold-Chiari malformation type I(analysis of 164 cases)

Objective:To evaluate the diagnostic value of MR for ACM.Methods:164 patients,with obscure clinical diagnosis from Jan.1990 to Dec.1999,were examed by a 1.0T superconductive MR system.The diagnosis of ACM Type Ⅰ was established in all cases and their MR appearances were summarized and analysed. Results:In all cases,60%(99/164)patients had a variable degree of syringohydromyelia;71%(117/164)had cervicomedullary kinking;75%(123/164)were associated with other malformation;70%(115/164)were associated with craniocervical junction malformation.Conclusion:MR has been proved to be the best method in diagnosis of ACM,and it can be used to find out accurately the associated malformation,degree and range of syringohydromyelia.
