Beginning with the principle of justice, this article analyzed the reasons of medical disputes due to the lack of the principle of justice from four aspects including patients, social media, medical institutions, and medical staff. Corresponding countermeasures were put forward based on the causes of the disputes: fostering cor-rect consciousness of medical treatment, persisting on objective propaganda and creating a favorable environment, strengthening the construction of medical ethics and increasing health investment, strengthening the professional competency education for medical staff, and setting up medical dispute mediation organizations dominated by the third party.

Objective To investigate the changes of expression of neuronal nitric oxide synthase(nNOS) after sleep deprivation(SD) in rats,and to study the effects of Xingnaojing on them. Methods Rapid eye movement(REM) SD models(SD group,n=40) were established by the modified multiple platform methods.The rats in SD group were subjected to drug intervention group(n=20) and non-drug intervention group(n=20).Xingnaojing injection was administered to the drug intervention group.Rats in SD group were deprived of sleep for 1,3,5 and 7 d. The expression of nNOS in brain tissues was detected by immunohistochemistry.The rats without SD were severed as cage control group(n=10) and tank control group(n=10). Objective To know the current conditions of institution set-up,personnels allocated and equipment supplied in county-,city-and country-level in endemic disease prevention and control.Methods Questionnaires,regarding institution set-up,personnels allocated and equipment supplied,was performed among staffs from administrative apartment and professional apartment.Results There were 31 institutions for endemic disease prevention and control in province level of our country(except Hainan Province),with one institution in each province.There were 308 institutions for endemic disease prevention and control in city level,and 23 cities did not have one.There were 2340 institutions for endemic disease prevention and control in county level,none in 502 counties.There were 20 provinces which had the professional crew of the endemic disease administration,but other provinces did not.The proportion of high-grade professional staff of province,city,county level was 24.84% (310/1248),17.05%(481/2821),5.31%(598/11 256)respectively.The averaged size of wdrking place at province-,city-and county-level was 2603.9,324.6,1227 m2.The averaged area of laboratory of province,city,county level was 1093.8,147.4 and 46.6 m2 respectively.The average quantity of equipments valued over ten thousand yuan at province-,city-and county-level is 15.5,2.2,0.6 respectively.Conclusions After the innovation of organizations for endemic disease prevention and control,the number of personnels devoting to this work has been curtailed,working place and equipment is insufficient,which hinders the prevention and control.

Objective To demonstrate the origin of the right inferior phrenic artery(RIPA) in normal and hepatocellular carcinoma(HCC) patients and provide valuable anatomical information for angiographers before and after transcatheter arterial chemoembolization(TACE).Methods Four hundred and forty consecutive patients including 133 HCC cases who had biphase abdominal CT were assessed in this study.The routine abdominal enhanced CT scan(GE,LightSpeed16) was performed with 120 kV,200—240 mAs,10 mm collimation,1.375 pitch,and 10 mm reconstruction interval at 22—25 seconds for arterial phase triggered by timing bolus,60 seconds for portal venous phase after injection of 100 ml contrast material(300 mg I/ml) at a rate of 3.5 ml/s.Multiplanar reconstruction(MPR) and maximum intensity projection(MIP) images were generated using 1.25 mm images reconstructed with 1 mm interval in arterial phase and reviewed by two radiologists.An enhanced artery medial-posterior to the IVC,originated from aorta or its branches to the diaphragmatic dome was interpreted as the RIPA.Results The RIPA was showed in all(440 patients)(100%).Among 218(49.5%) RIPAs originated from the aorta,140 were from the right side of the aorta,22 from the left side of the aorta,56 from the anterior wall of the aorta,36 RIPAs had the same origin with the left inferior phrenic artery.Among 138(31.4%) RIPAs from the celiac artery,10 RIPAs had the same origin with the left gastric artery,and 33 RIPAs had the same origin with the left inferior phrenic artery.78(17.7%) were from the right renal artery,6(1.4%) were from the left gastric artery(the left gastric artery from aorta).The dilatation of the RIPA was demonstrated in 16 of(133 hepatocellular) carcinoma patients.Conclusion Multislice helical CT could demonstrate the origin of the RIPA in arterial phase and provide useful anatomical information for angiographer before and after TACE.

Objective To investigate the relationship between the color doppler sonography and contrast-enhanced echocardiography(CEE)in assessment of intrapulmonary shunting in patients with hepatopulmonary syndrome and HIF-1α,iNOS and portal hemodynamic.Methoods To examine the portal vein diameter and blood flow velocity by color Dop-pler,and intrapulmonary shunting by contrast-enhanced echocardiography(CEE).The expressions of HIF-1α and iNOS in serum were determined by ELISA in the normal,hepatocirrhosis amalgamating HPS and unamalgamating HPS separately.Comparison of HIF-1α and iNOS in plasma was carried out and relationship between the portal hemodynamic and the intra-pulmonary shunting was examined in three groups.Results The levels of HIF-1α and iNOS in hepatocirrhosis amalgamating HPS were higher than those in unamalgamating HPS,the diameter of portal vein dilated and the velocity of portal vein dropped,there was correlation(P＜0.05).There were positive correlation with intrapulmonary shunting.Conclusion The color doppler sonography and contrast-enhanced echocardiography (CEE)which reflected HIF-1α,iNOS and the hemodynamic played an role in developing of HPS,and was available to early examine change of intra-pulmonary vessel.

Objective To investigate the refraction change and nursing character in myopic amblyopia during the perceptual learning treatment. Methods Refraction of 54 children (98 eyes) with myopic amblyopes were selected,which were selected from the amblyopia clinic between 2007 and 2009, the spherical diopter, astigmatism and spherical equivalent were collected before and after perceptual learning,the refractive dynamic was compared according to the degree of amblyopia. Results The spherical average annual growth of myopic amblyopia during treatment was 0.52D, the growth of astigmatism was 0.03D;the spherical equivalent average annual growth was less than 0.50D accounted for 57.14%. The dynamic changes of spherical or SE of mild, moderate and severe amblyopia had significant difference, but the cylinder had not significant difference in change. Conclusions The treatment of perceptual learning in patients has little effect on the growth of refraction in myopic amblyopia, however, it is still necessary to pay close attention to refraction changes of in myopic amblyopia in the course of treatment.
