Objective To evaluate the clinical effect of Liyanjiedu decoction for the prophylaxis and treatment of acute radiotherapy-induced oral mucositis of nasopharyngeal carcinoma(NPC)and the influence to the effect of the radiotherapy to NPC.Methods A total of 96 patients with NPC were randomly divided into treatment group and control group,48 cases for each group.The two groups were given radical radiotherapy.The treatment group was given Liyanjiedu decoction for buccal.The control group were given western medicine for mouthwash.The course of two groups was all from the beginning of radiotherapy to the end of it.Results The occurring time of radiotherapy-induced mucositis in treatment group and control group were(21?4.2)d and(16?3.6)d,and radiotherapeutic dose were(40.252?11.758)Gy and(29.625? 11.632)Gy,with significant difference between the tow groups(P
2.Endovascular treatment of intracranial and extracranial arterial stenosis using multiple stents placement

Objective To explore our clinical experience on treatment of intracranial and extracranial arterial stenosis using multiple stents placement. Methods 17 stents were deployed into 12 arteries in 8 patients with intracranial and extracranial arterial stenosis. We utilized self expanding stents in extracranial carotid arteries and balloon expanding stents for intracranial arteries or vertebral arteries. Results All procedures were performed successfully and all stenoses were dilated and improved. Short term follow up angiogram revealed only one vessel occlusion and no significant restenoses in other stented vessels together with no recurence of clinical symptoms. Conclusions Multiple stents placement is a valid alternative for the treatment of intracranial and extracranial arterial stenoses.

4.Adverse Drug Reactions in Our Hospital:Analysis of 258 Cases

OBJECTIVE:To investigate the characteristics and general pattern of the adverse drug reactions (ADR) occurred in our hospital. METHODS:A total of 258 ADR cases collected from June 2006 to June 2008 were analyzed statistically in respect of patients' age and sex,drug variety,clinical manifestations,routes of administraiton and dosage forms etc. RESULTS:Of the total 258 ADR cases,the majority(64.34%) were induced by antimicrobial drugs; 45.74% presented with lesions of skin and its appendants,and 77.91% were induced by intravenous way. CONCLUSION:To develop and strengthen ADR monitoring and reporting should be regarded as the key to enhance medication safety.

6.Clinical efficacy of acupuncture in treatment ofchronic urticaria and its effects on the content of IgE and the imbalance of Th1/Th2 cell function

Objective: To observe the clinical efficacy of acupuncture in treatment of chronic urticaria and the change in the content of serum immunoglobulin E (IgE), and to discuss the effect of acupuncture on the imbalance of T helper (Th)1/Th2 cell function via observing the changes in the contents of interferon-γ (IFN-γ) and interleukin-4 (IL-4). Methods: Ninety patients meeting the inclusion criteria of chronic urticaria were randomized into an acupuncture- medication group, an acupuncture group and a Western medication group by the random number table method. The acupuncture-medication group was intervened by acupuncture, cupping, collateral-pricking bloodletting and oral administration of cetirizine hydrochloride tablets; the acupuncture group was treated with acupuncture, cupping and collateral-pricking bloodletting; the Western medication group only received oral administration of cetirizine hydrochloride tablets. Before treatment and after 6-week treatment, the changes in the symptom scores and the contents of serum IgE, IFN-γ and IL-4 in the three groups were observed. Results: There were no significant differences in the total effective rate among the three groups (all P>0.05), but the cured and markedly effective rate was significantly higher in the acupuncture-medication group than that in the Western medication group (P<0.05). After treatment, the total symptom score decreased in the three groups (P<0.05), and the improvement of total symptom score in the acupuncture-medication group was more significant than that in the Western medication group (P<0.05). The component symptom scores all decreased after treatment in the three groups (all P<0.05); the improvements of the scores of itch intensity, and skin lesion size and number were more significant in the acupuncture-medication group than in the Western medication group (all P<0.05); the improvement of the skin lesion size score was more significant in the acupuncture group than in the Western medication group (P<0.01). The contents of IgE and IL-4 dropped (all P<0.05) and the content of IFN-γ increased (P<0.05) after treatment in the three groups; the post-treatment changes in the serum contents of IgE and IFN-γ were more significant in the acupuncture-medication group than in the Western medication group (both P<0.05). The incidence rate of adverse reactions was significantly lower in the acupuncture-medication group and acupuncture group than in the Western medication group (both P<0.05), and the relapse rate was significantly lower in the acupuncture-medication group than in the Western medication group (P<0.05). Conclusion: Combined acupuncture and medication can enhance the cured and markedly effective rate in treating chronic urticaria. Acupuncture is equivalent to cetirizine hydrochloride tablets comparing the clinical efficacy in treatment of chronic urticaria. Acupuncture plus medication and acupuncture alone both can effectively mitigate the clinical symptoms, with low incidence of adverse reactions. The relapse rate is low when using acupuncture together with medication. Acupuncture plus medication can work better in regulating the contents of IgE and IFN-γ and improving the imbalance of Th1/Th2 cell function.
7. Comparison of lung injuries induced by drowning with equal volume of seawater and freshwater in rabbits

Objective: To compare the lung injuries caused by drowning with equal volume of seawater and freshwater in rabbits. Methods: Twenty-four healthy New Zealand rabbits were randomly allocated to three groups (n = 8): control group(CG), seawater-drowning group (infused with seawater, 2 ml/kg) and freshwater-drowning group (infused with freshwater, 2 ml/kg). All rabbits were observed for 3 hours for breathing frequency, heart rate, mean arterial pressure (MAP), blood gas analysis and levels of TNF-α and IL-6. Pulmonary histopathology, ratio of wet/dry lung weight and permeation index of pulmonary capillary were recorded after the animals were sacrificed at the end of the experiment. Results: After infusion of seawater or freshwater, the respiration rates, serum levels of TNF-α and IL-6 were significantly increased (P<0.01), and the MAP was decreased (P<0.01), leading to hypoxemia and hypocapnia. Lung injuries were noticed in both seawater-drowning group and freshwater-drowning groups, with those in the former more persistent. The wet/dry ratio and permeation index of lung capillary were significantly increased after infusion in the seawater group compared with those in the other two groups (P<0.05). The cell number in the bronchoalveolar lavage fluid was significantly larger in the seawater-drowning group compared with that in the control group (P<0.01), and the lung injury was more severe than that in the freshwater group. Conclusion: Seawater infusion can lead to more serious and persistent lung injury in rabbits compared with equal volume of freshwater.
8.Treatment and correlation factors of aqueous misdirection during surgery on glaucoma patients

Objective To investigate the emergency treatment and correlation factors of aqueous misdirection during glaucoma and cataract surgery in glaucoma patients.Design Retrospective case series.Participants 126 cases(132 eyes)of angle-closure glaucoma patients with aqueous misdirection during surgery,100 cases(100 eyes)of angle-closure glaucoma patients without aqueous misdirection during surgery and 100 cases(100 eyes)of senile cataract patients.Methods We performed pars plana vitreous water-bag aspiration or anterior vitrectomy on glaucoma patients with aqueous misdirection during surgery.All the factors in observation group and control groups were compared and analyzed by logistic regression.Main Outcome Measures Preoperative highest intraocular pressure(HIOP),intraocular pressure after maximum dose(IOP),anterior chamber depth(ACD),lens thickness(LENS),axial length (AXL),mean keratometry(K),intraocular lens diopter within-0.5D corrected by SRK-T formula(IOLD).Results Among all the 132 angle-closure glaucoma eyes whose posterior chamber pressures were increased during surgery,we performed pars plana vitreous water-bag aspiration in 123 eyes(93.18%),and anterior vitrectomy in 9 eyes(6.8%).Postoperative complications included shallow anterior chamber in 20 eyes(7 eyes with malignant glaucoma,9 eyes with choroidal detachment,4 eyes with delayed local suprachorodal hemorrhage),and vitreous local hemorrhage in 1 eye.Means of all the observation indexes on three groups showed significant difference(all P

Objective To evaluate the clinical value of serum MUC1(sMUC1)in the patients with multiple myeloma.Methods The enzyme linked immunosorbent assay(ELISA)was used to compare the sMUC1 levels in the 12 cases of newly diagnosed multiple myeloma.15 cases of post-chemotherapy and 46 healthy donors.Results The mean concentration of sMUC1 in the newly diagnosed patients with multiple myeloma was 33.44 U/ml(10.86~88.80 U/ml).In the patients of post-chemotherapy the mean concentration was 11.6U/ml(3.92~22.22 U/ml),and in the healthy donors the concentration was 12.81 U/ml(3.84~30.45 U/ml).The level of sMUC1 in the new diagnosed patients was significandy higher than that in the patients of post-chemotherapy(P=0.001)and healthy donors(P=0.000).There was no significant difference between post-chemotherapy and healthy donors(P=0.461).Conclusion sMUC1 may be one of the tumor markers for the diagnosis of multiple myeloma.sMUC1 could also be used as one of the indicators of prognosis and the evaluation of the chemotherapy efficacy.sMUC1 might be a potential target for the immunotherapy to the patients with multiple myeloma.
10.Using hierarchical teaching to improve geriatrics education quality

In order to improve the Geriatrics education quality,Hierarchical teaching was applied in the process of Geriatrics education.On the one hand,the overall thinking,the international perspective,the scientific spirit and humanistic care concepts were cultivated.On the other hand,the distinction between the undergraduate,graduate,postgraduate students and individual differences were paid attention to and hierarchical teaching was given to them respectively.Setting different teaching goals,using different teaching methods and evaluation methods is used to improve the teaching pertinence.With the application of the traditional teaching method and hierarchical teaching,as far as possible to teach students in accordance with their aptitude,improve the pertinence of teaching,arouse students' learning potential,thus improving the quality of teaching,satisfactory results was obtained.
