Chinese Journal of Tissue Engineering Research 2014;(10):1554-1559
BACKGROUND:The umbilical cord mesenchymal stem cells possess multipotent differentiation capacity, but less research focus on its differentiation into fibroblasts.
OBJECTIVE:To investigate the capacity of human umbilical cord mesenchymal stem cells differentiating into fibroblasts.
METHODS:Using adherent method, human umbilical cord mesenchymal stem cells were isolated, and flow cytometric analysis of the surface antigen was performed. Passage 3 cells were selected for osteogenic and
adipogenic differentiation, and cells differentiated into fibroblasts under the induction of basic fibroblast growth factor. RESULTS AND CONCLUSION:Adherent stem cells were stably isolated from the umbilical cord. Human umbilical cord mesenchymal stem cells lowly expressed CD31, CD45, CD40, HLA-DR, but strongly expressed CD29, CD90, CD44, CD105. Oil red O staining showed red droplets were ful of the cytoplasm after adipogenic induction;alizarin red staining showed red calcium nodules after osteogenic induction. After induced by basic fibroblast growth factor, the type I col agen expression was significantly higher than that in the control group. These findings indicate that the adherent human umbilical cord mesenchymal stem cells are reliably isolated with high purity;basic fibroblast growth factor can induce differentiation of umbilical cord mesenchymal stem cells into fibroblasts.

ObjectiveTo investigate the clinical characteristics of patients with acute coronary syndrome suffering hemorrhage during hospitalization. MethodsThe clinical symptoms, diagnostic and therapeutic characteristics and in-hospital outcome of 3807 inpatients who were recruited into SINO-GRACE study in China due to acute coronary syndrome from March,2001 to December,2007 were collected.Statistical methods were adopted to compare the differences in clinical data between hemorrhage group and non-hemorrhage group.ResultsHemorrhage had happened in 57 out of 3807 inpatients with the incidence of 1.50%. Five patients, which accounted for 9.6%of the overall hemorrhage cases, were fatal hemorrhage.Nine patients were intracranial hemorrhage with the incidence of 0.24%. There were 155 deaths among the 3807 patients, with an overall mortality rate of 4.1%. The mortality of hemorrhage accounted for 3.2%in overall mortality. Patients with one of the following factors were more apt to hemorrhage:> 70 years old, previous hemorrhage history, renal failure history, heart failure history and clopidogrel and glycoprotein (GP)Ⅱ b/Ⅲ a receptor antagonist administration for coronary artery bypass grafting.Patients who developed hemorrhage might need prolonged hospitalization and were liable to develop heart-related adverse events, including re-infarction and sustained ventricular tachycardia/fibrillation after they were admitted in hospital over 24 hours. ConclusionPatients with acute coronary syndrome who underwent coronary artery bypass grafting, with advanced age, previous hemorrhage history, renal failure history, heart failure history or treated with clopidogrel and GPⅡ b/Ⅲ a receptor antagonist are more vulnerable to hemorrhage.

Objective To investigate the clinical outcome of dual?plane high tibial osteotomy to treat posterolateral corner injuries combined with varus and/or hyperextension deformity. Methods From October 2008 to March 2014, there were 12 patients (13 knees) undergoing high tibial osteotomy. All the patients were double or triple varus knee combined with posterior cruciate ligament and posterolateral corner (PLC) injuries. During the surgery, the weight bearing line of lower extremity was set to 62%position of the tibial plateau on the coronal plane. The tibial slope was increased to diminish the hyperextension on the sagit?tal plane. Full?length weight bearing X?ray was used to evaluate the position of weight bearing line, femoral?tibial angle and tibial slope pre? and post?operatively. The stress radiograph was used to evaluate the posterior and posterolateral stability. The Op?ti_knee gait analysis system was used to evaluate the varus of angle during weight bearing stage. Results The weight bearing line was corrected from 19.6%±19.1% preoperatively to 42.6%±17.9% postoperatively (t=-4.178, P=0.002). The femoral?tibial angle was 172.8°±4.2° preoperatively and improved to 178.1°±4.2° postoperatively (t=-4.520, P=0.001). The tibial slope was increased from 10.2° ± 5.3° preoperatively to 18.4° ± 6.3° postoperatively (t=-5.735, P=0.000). The tibial posterior translation was decreased from 11.4±5.3 mm preoperatively to 8.1±6.9 mm postoperatively (t=2.415, P=0.042) in 9 patients. The separa?tion of lateral compartment was decreased from 16.3 ± 6.5 mm preoperatively to 14.2 ± 4.9 mm postoperatively (t=3.194, P=0.019). Gait analysis was performed in 7 patients and showed that the varus was improved from 3.0° ± 2.6° preoperatively to-2.7°±2.5° postoperatively during weight bearing stage (t=-8.500,P=0.014). Eight patients had sufficient improvement in knee function so that a subsequent posterolateral corner reconstruction was not necessary. Conclusion The dual?plane medial open wedge high tibial osteotomy can improve the alignment of lower extremity and the stability of knee joint, and 67%patients do not need a staged PLC and posterior cruciate ligament reconstruction.
