1.Intradural metastasis of extramedullary space of medulloblastomas:MRI

Objective:To evaluate MRI manifesattions of intradural metastases of medulloblastomas.Methods:The clinical data and MRI of 6 cases with intradural metastasis of medulloblastomas were reviewed retrospectively.Results:21 masses were found in 6 cases,mostly in lumbar segment.Most of them were oblong or fusiform shagped.The size averaged 1.8 cm?2.3 cm.They had the similar signal intensity with those in brain,but hemorrhage,cyst and calcification were less common.Conclusion:MRI plays an importment role in diagnosis of intradural metastases of medulloblastomas. [
2.Effect of all-trans retinoic acid on expression ofα-SMA in rat model with pulmonary hypertension

Objectives To explore the possible mechanism of all-trans-retinoic acid (ATRA) therapy for pulmonary ar-terial hypertension by observing the effect of ATRA on the expression ofα-smooth muscle actin (α-SMA) in monocrotaline (MCT)-induced rats with pulmonary hypertension. Methods Thirty SD rats were randomly divided into control group, MCT-induced pulmonary hypertension group (model group) and ATRA treatment group (therapeutic group). MCT (60mg/kg) was injected to the rats in the model group and the therapeutic group to induce pulmonary arterial hypertension. And at the same day ATRA [30mg/(kg?d)] was given to rats in the therapeutic group and continually given for 28 days. The mean pulmonary arte-rial pressure and right ventricular hypertrophy index (RVHI), percentage of wall thickness (WT%) and percentage of wall area (WA%) of pulmonary arterioles were evaluated at day 28. The expression ofα-SMA mRNA and protein in the lung tissue were detected by Real-time PCR and Western blot. Results Mean pulmonary arterial pressure, RVHI, WT%and WA%were signiif-cantly higher in the model group than those in the therapeutic group (P<0.01) but these data in the therapeutic group were higher than that in the control group (P<0.01). The expression ofα-SMA mRNA and protein were higher in the model group than those in the control group and the therapeutic group (P<0.01), while there was no difference was found between the therapeutic group and the control group (P>0.05). Conclusions ATRA has therapeutic effect on pulmonary hypertension through down-regulation ofα-SMA expression.
3. Study on chemical constituents of Kochia scoparia

OBJECTIVE: To study the constituents in the fruits of Kochia scoparia. METHODS: Various column chromatographic techniques were used to isolate the constituents, and their structures were identified by ESI-MS, NMR spectral data and by comparison spectra with literature data. RESULTS: Sixteen compounds were isolated and elucidated as stearic acid (1), β-sitosterol (2), oleanolic acid (3), daucosterol(4), 5, 7, 4'-trihydroxy-6, 3'-dimethoxyflavone (5), 5, 7, 4'-trihydroxy-6-methoxyflavone (6), isorhamnetin (7), quercetin (8), isorhamnetin -3-O-β-D-glucopyranoside(9), rutin(10), oleanolic acid-3-O-β-D-glucuronopyranoside(11), oleanolic acid-3-O-β-D-xylopyranosyl(1 → 3)-β-D-(6-O-methyl)-glucuronopyranoside(12), oleanolic acid-3-O-β-D-xylopyranosyl(1 → 3)-β-D-glucuronopyranoside(13), 3-O-β-D-glucuronopyranosyl-oleanolic acid-28-O-β-D-glucopyranoside(14), 3-O-β-D-xylopyranosyl(1 → 3)-β-D-glucuronopyranosyl-oleanolic acid-28-O-β-D-glucopyranoside(15), 3-O-{[β-D-β-D-glucopyranosyl(1 → 2)]-[β-D-xylopyrano-syl(1 →3)]-)-β-D-glucuronopyranosyl oleanolic acid-28-O-β-D-glucopyranoside(16). CONCLUSION: Compounds 5-10 were isolated from Kochia scoparia. for the first time. Copyright 2012 by the Chinese Pharmaceutical Association.
4.Establishment and Application of Immunomagnetic Bead Negative Enrichment and Immunofluorescence Antibody Technique for Detection of CTCs in Peripheral Blood of Patients with Ovarian Cancer

Objective To establish a cell level-based negative enrichmen technique to detect circulating tumor cells (CTCs) in the peripheral blood of patients with epithelial ovarian cancer.Methods The colon cancer SKOV-3 cells were mixed with 2 ml whole blood from healthy donors at different ratio.Quantification of CTCs was performed using immunomagnetic bead based negative enrichment combined with immunofluorescence antibody method.The method was evaluated the recovery rate of target cells,Samples of 32 patients with ovarian cancer and 10 controls were assayed for CTCs detection by above method.Results ①The recovery rate was ranging from 64% ~80% by spiking varying numbers of SKOV-3 into 2 ml blood samples of healthy volunteers.Regression analysis of number of recovered SKOV-3 cells yielded a regression equation of Y=0.782X-1.408 and a coefficient of determination of R2 =0.998.②Did not detect CK8/18-+ circulating tumor cells in 10 controls,and CK8/18+ circulating tumor cells in 18 cases of 32 Patients with ovarian cancer.The positive rate of CK 8/18 + circulating tumor cells was significantly differences between the two groups (x2 =7.681,P<0.01).③The presence ofCTCs was significantly correlated with distant metastasis (x2 =5.776,P<0.05).Conclusion The method of immunomagnetic bead based negative enrichment combined with immunofluorescence antibody technique for CTCs detection in peripheral blood of patients with ovarian cancer has a clinical value of application and extension.
5.Correlations of 24 biochemical markers in seminal plasma with routine semen parameters.

OBJECTIVETo investigate the correlations of 24 biochemical markers in the seminal plasma with routine semen parameters.
METHODSAccording to the WHO5 standards, we analyzed the routine semen parameters of 66 subfertile men, including the semen volume, sperm concentration, total sperm count, sperm motility, and the percentage of progressively motile sperm (PR). Based on the calibration and quality control measures and using an automatic biochemistry analyzer or electrolyte analyzer, we detected 24 biochemical markers in the seminal plasma of the patients, including total protein (TP), albumin (Alb), globulin (Glb), uric acid (UA), alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (AKP), γ-glutamyltransferase (GGT), lactate dehydrogenase (LDH), creatine kinase (CK), alpha hydroxybutyrate dehydrogenase (αHBDH), adenosine deaminase (ADA), glucose (Glu), triglyeride (TG), total cholesterol (TC), urea nitrogen (UN), creatinine (Cr), high-sensitive C-reactive protein (hsCRP), K+, Na+, Cl- , Ca, Mg, and phosphorus (P). Then we analyzed the correlations of the 24 biochemical markers with routine semen parameters.
RESULTSThe levels of the TP, Alb, and Glb proteins in the seminal plasma were positively correlated with sperm concentration, so was that of Alb with the total sperm count, and the AST and LDH activities with sperm concentration and total sperm count. The AKP activity in the seminal plasma was correlated negatively with the semen volume, but positively with sperm motility. The αHBDH activity exhibited a positive correlation with both sperm concentration and total sperm count, with a coefficient of correlation (r) above 0.7. The UN level was correlated negatively with the semen volume, so was the Cr level with the semen volume, sperm concentration, and total sperm count, and the Glu level with sperm concentration and total sperm count. The TG level was correlated positively with the semen volume, but negatively with sperm motility. The levels of seminal plasma ALT, GGT, ADA, UA, TC, CK, and hsCRP showed no correlation with the above-mentioned semen parameters. None of the seminal plasma K+, Na+, Ca, Mg, and P levels was found correlated with semen parameters except the Cl- level, which was negatively correlated with the semen volume.
CONCLUSIONMany biochemical markers in the seminal plasma are closely related to routine semen parameters, indicating that these biochemical components may play roles in spermatogenesis, sperm maturation, and physiological metabolism.

6.Primary Carcinoma of the Duodenum:CT Findings

Objective To evaluate the role of CT in the diagnosis of the primary carcinoma of the duodenum.Methods CT findings of 15 patients with primary carcinoma of the duodenum(12 adenocarcinoma,3 carcinoid)underwent surgical treatment or biopsy were reviewed.Results 9/12 of adenocarcinomas were shown as local soft tissues masses;6/12 of adenocarcinomas manifested as ductal wall irregular or circular thickening;3/12 of adenocarcinomas were shown as local soft tissues masses accompanied with deformation and constriction of duodenum.Cholangiectasis,metastasis of retroperitoneal lymph nodes and liver were also founded.CT cannot difference adenocarcinoma with carcinoid.After enhancement,the masses were manifested as mild or intermediate enhancement in the early stage and they did not show obviously enhancement in the delay or portal stage.Conclusion CT play an important role in both the localization and qualitative diagnosis of the primary carcinoma of duodenum,but it is difficult to difference adenocarcinoma with carcinoid.
9.Effections of different ways of mechanical ventilation with pulmonary surfactant on respiratory mechanics of patients with neonatal acute respiratory distress syndrome

Objective To investigate the respiratory mechanics and treatment outcomes of different types of mechanical ventilation for patients with neonatal acute respiratory distress syndrome(ARDS):the high frequency oscillation ventilation + pulmonary surfactant (HFOV+PS),conventional mechanical ventilation + pulmonary surfactant(CMV+PS),conventional mechanical ventilation(CMV). Methods Seventy-five cases with neonatal ARDS,25 cases in the HFOV+PS group,30 cases in the CMV+PS group,20 cases in the CMV group. Patients in the former two groups received 70 mg/kg PS at a time. PaO2,PaCO2,PaO2/FiO2,oxygenation index(OI)and respiratory index(RI)were detected at 0 h,12 h,24 h,48 h,72 h post-mechanical ventilation. Results At 12, 24,and 48 hours post-mechanical ventilation,patients in the HFOV+PS group had a significantly higher level of PaO2 and a significantly lower level of PaCO2 than those of patients in the CMV+PS group and the CMV group(P<0.05). At 12,24,48,and 72 hours post-mechanical ventilation,patients in the HFOV+PS group had a significantly higher level of PaO2/FiO2 and significantly lower level of OI and RI than those of patients in the CMV+PS group and the CMV group (P < 0.05,respectively). Patients in the HFOV+PS group also had significantly shorter durations of mechanical ventilation and oxygen usage than those of patients in the CMV+PS group and the CMV group (P < 0.05). No significant differences were observed in Gas leak,the incidence of intracerebral haemorrhage and cure rate among the three groups. Conclusions Application of HFOV with PS therapy for patients with neonatal ARDS can timely improve the oxygenation,shorten the time of mechanical ventilation and the usage of oxygen,without increasing complications.
10.Clinical Significance of Circulating Tumor Cells Detection from Peripheral Blood of Patients with Epithelial Ovarian Cancer

Objective To estimate the diagnostic vaule of circulating tumor cells (CTCs) detection for epithelial ovarian cancer,and investigate the relationship between the presence of CTCs and the clinic pathologic characteristics of epithelial ovarian cancer patients.Methods Quantification of CTCs were performed using immunomagnetic bead based negative enrichment combined with flow cytometry in 65 patients with epithelial ovarian cancer,21 patients with benign ovarian diseases,and 10 healthy subjects.The differences among groups were analyzed by the Kruskal-Wallis H test (multi group comparison) and the Mann-Whitney U test (two group comparison),and the chi-square test was used in the positive rate comparison,the receiver operating characteristic (ROC) curve was established.The relationship between CTCs and clinic pathologic characteristics of epithelial ovarian cancer patients were analyzed.Results The receiver operating characteristic analysis showed the cut off value was 4.5 (> 4),the AUC was 0.806,and sensitivity,specificity and positive value (PPV) were 55.4 %,96.8 % and 97.3 % respectively,in detecting patients with ovarian carcinoma malignancy.Quantification of CTCs in epithelial ovarian cancer was correlated with FIGO stages or distance metastasis (all P<0.05),but not with patient age,histological types,pathologic differentiation,amount of ascites,tumor size and lymph node metastasis (all P>0.05).Conclusion The detection of peripheral CTCs has a certain diagnosis value in epithelial ovarian cancer,especially with related to FIGO stages and distant metastasis.
