1.Application and Development of the Static Balance Test and Training Equipment in Clinical Rehabilitation (review)

The static balance test and training equipment has been used over ten years in our country. Because balance function of patients can be assessed objectively and quantitively and pertinent taining can be performed with balance equipment, this instrument has a abroad domestic application. The application of balance equipment in clinical rehabilitation is sumarized in this article.

Chronic infection of hepatitis B virus(HBV)presents one of the serious public health challenges worldwide.Current treatment of chronic hepatitis B(CHB)is limited,and is composed of interferon and nucleoside/nucleotide reverse transcriptase inhibitors(NRTI).Interferon is poorly tolerated and is only responsive in a small fraction of CHB patients and NRTIs often face the problem of emergence of drug resistance during long-term treatment.The current treatment of CHB earl be improved in several ways including genotyping mutations associated with drug resistance before treatment to guide the choice of NRTIs and suitable combinations among NRTIs and interferon.It is important to continue research in the identification of novel therapeutic targets in the life cycle of HBV or in the host immune system to stimulate the development of new antiviral agents and immunotherapies.Several antivirai agents targeting HBV entry,cecDNA,capsid formation,viral morphogenesis and virion secretion,as well as two therapeutic vaccines are currently being evaluated in preclinical studies or in clinical trials to assess their anti-HBV efficacy.
3.EFFECTS OF SOME SALT-TOLERANT FUNGI IN DISSOLBING PHOSPHATE ROCK UNDER DIFFERENT SALT AND ALKALINE

Four specific salt-tolerant fungi for dissolving phosphate rock (FM) w ere isolated from the coastal saline soils. It was found that the biomass and a b ility to release phosphate from the materials decreased with increasing of the N aCl content. During pH7.0~8.5, the biomass and ability of to release phospho rus from the materials of FM1 decreased sharply with pH rising, but the FM2 and FM4 decreased not considerably, optimal pH for FM3 was at pH8.5. At pH9.0, the bi oma ss and ability of to release phosphorus from the materials decreased quickly. F M2 was stable at alkaline and high salt content condition.
4.Clinical study of electroacupuncture plus stuck-needle lifting method for intractable facial paralysis

Objective:To observe the efficacy of using electroacupuncture(EA)plus stuck-needle lifting method to treat intractable facial paralysis based on the myofascial theory. Methods:Ninety patients with intractable facial paralysis were divided into a control group and an observation group using the random number table method,with 45 cases in each group.The control group was given conventional EA treatment,and the observation group received EA plus the stuck-needle lifting method based on the myofascial theory for 4 consecutive weeks,6 sessions each week.The electromyographic results,modified Portmann scale(MPS)score,facial nerve function index(FNFI),and total effective rate were compared. Results:There were no significant differences in the MPS and FNFI scores between the two groups before treatment(P>0.05).After treatment,the total effective rate and FNFI and MPS scores were notably higher in the observation group than in the control group(P<0.05).There were no significant differences in the electromyographic readings between the two groups before the intervention(P>0.05).After the intervention,the observation group had a shorter blink reflex R1 latency and a higher facial nerve compound muscle action potential compared with the control group,and the between-group differences were statistically significant(P<0.05). Conclusion:EA plus stuck-needle lifting method based on the myofascial theory can enhance treatment efficacy for intractable facial paralysis.

6.Effect of bundle intervention on ventilator-associated pneumonia in an inten-sive care unit

Objective To evaluate the effect of bundle intervention measures on preventing ventilator-associated pneumonia (VAP).Methods Patients who were admitted to an intensive care unit(ICU)of a hospital from January 2012 to December 2013 were monitored,patients from January to December 2012 were as control group,while from January to December 2013 were as intervention group (bundle intervention measures were implemented).Usage rate of ventilators and incidence of VAP between two groups were compared.Results A total of 4 560 patients were mo-nitored,2 608 in intervention group and 1 952 in control group.Usage rate of ventilators in intervention group was lower than control group (53.95% vs 61 .17%;χ2 =65.756,P <0.01).Incidence of VAP per 1 000 ventilator days in intervention group was lower than control group (13.00‰ vs 19.56‰;χ2 =4.649,P =0.031 ).Percentage of late-onset VAP per 1 000 ventilator days in tervention group was higher than control group(41 .82‰ vs 24.59‰). Conclusion Bundle intervention measures are helpful for reducing the incidence of VAP in ICU patients.
7.The 1∶1 case-control study on direct economic loss due to surgical site infec-tion

Objective To evaluate the direct economic loss due to surgical site infection(SSI)from a medical eco-nomics perspective,and provide basis for health administrative departments to make strategies.Methods SSI in pa-tients undergoing breast,stomach and colorectal operation between March 2008 and February 2010 were selected for 1∶1 case-control study,patients with SSI were as SSI group,without SSI during the same period were as control group,difference in medical expense and length of stay between two groups were compared,economic loss due to SSI was evaluated.Results A total of 1 523 operation cases were investigated,75(4.92%)developed SSI.69 effec-tive pairings were obtained.Wilcoxon signed rank test revealed that difference in medical expense between two groups was statistically significant(Z =6.586,P <0.001),the median of average medical expense of SSI group was 6 828.60 yuan higher than control group,all kinds of medical expense in SSI group were all higher than control group;the median length of stay of SSI group was 10 days longer than that of control group (Z = 5.939,P <0.001).Conclusion SSI can prolong hospitalization,increase medical expense,decrease bed turnover rate,and in-fluence economic income and medical quality.
8.Activities of daily living among disabled people demanding assistive devices in Jiading Township

Objective:
To investigate the activities of daily living and the use of assistive devices among disabled people demanding assistive devices in Jiading Township, Jiading District, Shanghai Municipality, so as to provide the evidence for the implementation of rehabilitation interventions and the selection of assistive devices for the disabled people living in the community.
Methods:
The disabled people demanding assistive devices were sampled from 17 communities in Jiading Township in January 2020. A face-to-face questionnaire survey was conducted to collect participants'age, gender, type of disability, provision and use of assistive devices. The activities of daily living were evaluated using the Activities of Daily Living Scale, and the difference in the frequency of assistive device use was compared among the disabled people with different activities of daily living.
Results:
A total of 377 questionnaires were allocated, and 328 valid questionnaires were recovered, with an effective recovery rate of 87.00%. The respondents included 187 men ( 57.01% ) and 141 women (42.99%), and had a mean age of ( 71.78±13.86 ) years. Among all respondents, limb disability was predominant (248 people, 75.61%), and 84.45% (277 people) were provided with assistive devices. Assessment of activities of daily living showed complete self-care in 85 respondents ( 25.91% ), functional decline in 39 respondents ( 11.89% ) and functional dysfunction in 204 respondents ( 62.20% ). The proportion of functional dysfunction was predominantly detected among disabled people at ages of 80 years and older ( 79.63% ) ( P<0.05 ). Of the disabled people with assistive devices, 139 people used the devices daily ( 50.18% ), and the proportion of daily use of assistive devices was significantly higher in people with functional dysfunctions than in those with complete self-care ( 59.33% vs. 29.41%; P<0.05 ). In addition, no need was identified as the predominant cause of non-provision of assistive devices ( 29 people, 56.86% ).
Conclusions
Functional dysfunction of daily living is highly prevalent among the disabled people demanding assistive devices in Jiading Township, in whom the elderly are predominant, and the proportion of assistive device use is high among people with functional dysfunctions. The provision of assistive devices recommends to be improved based on assessment of activities of daily living.
9.Content Validity of Gross Motor Function Measure for Cerebral Palsy: Based on International Classification of Functioning,Disability and Health Children and Youth Version

Objective To analyze the content validity of Gross Motor Function Measure (GMFM) for cerebral palsy. Methods Concept contained in each item of GMFM was determined by 3 rehabilitation physicians or therapists who evaluated with GMFM for more than 2 years. All the concepts extracted were discussed in a meeting. The terms finalized were linked to the categories of International Classification of Functioning, Disability and Health Child and Youth version (ICF-CY) by 2 rehabilitation physicians who are familiar with the terms of ICF-CY according to the linking rules after discussion. Third party would intervene informally if there was a disagreement. The distribution of the ICF-CY categories involved were analyzed. Results 88 items from the GMFM linked to 27 categorise of 3rd level within 12 categories of 2nd level, belong to b7 Neuromusculoskeletal and Movement-related Functions and d4 Activities. 86 items linked to d4x, and 47 of then linked to b7x too. 2 items linked to b7x only. 16 items linked to the categories added to ICF-CY 16 times, involved 3 3rd level categories in a 2nd level category. Conclusion GMFM mainly focused on the activities in the framework of ICF-CY, especially the activities of changing and maintaining functional as well as the body posture and walking or moving. For the body function, the GMFM mainly focus on the support from arms.
10.Expression of transforming growth factor-? and metalloproteinases in joint capsule of frozen shoulder

Objective To explore the role of transforming growth factor (TFG)-? and matrix metalloproteinases(MMPs)in the development of frozen shoulder. Methods Twenty-four patients who underwent shoulder arthroscopy were included,and were divided into frozen shoulder group ( n =12) and control group ( n =12; n =2 for shoulder instability,n =5 for rotator cuff tear and n =5 for subacromial impingement). Joint capsule tissues at the rotator cuff interval were obtained,and the expression of TGF-?,MMP-1,MMP-2,MMP-3,MMP-9 and MMP-12 mRNA and protein was detected by Real-time PCR and Western blotting,respectively.
