Chinese Journal of Thoracic and Cardiovascular Surgery 2003;0(02):-
Objective: To evaluate the clinical effect of the double-orifice technique in the treatment of the anterior leaflet prolapse of mitral valve. Methods: 35 patients with severe anterior leaflet prolapse of mitral valve underwent valve repair using the double-orifice technique. The condition of valve was assessed with echocardiography preoperatively, intraoperatively, and postoperatively. Results: The mean postoperative valve area was (4 1?1 8) cm 2 against the preoperative valve area of (8 3?1 6)cm 2. Mild regurgitation was found in 2 patients after operation. There were no hospital deaths. At the latest follow-up, the heart function was improved significantly in all patients (NYHA class I). Echo-Doppler assessment of valve showed stable valve function in all patients. There were no late stenosis of valve and no late death. Conclusion: The results of this study show that the double-orifice technique is a safe and effective method for treatment of anterior leaflet of mitral valve with low mortality and morbidity, and excellent early and mid-term results.

Modern Clinical Nursing 2013;(1):35-39
Objective To study the correlation between psychological state and coping styles of primary caregivers of stroke patients.Method 286 primary caregivers were screened out by the random sampling method for the investigation of their psychological states and coping styles using SCL-90 and the medical coping modes questionnaire(MCMQ).Results There were significant differences between primary caregivers and Chinese norm in terms of all factors(P<0.01).The major caregivers with higher education were scored significantly lower than those with less education on the factors of depression, anxiety, hostility, fear, psychoticism (P<0.01).There were significant different differences between primary caregivers and Chinese norm in coping style(P<0.01): the score on confrontation in the coping style lower than the norm while the score on avoidance in the coping style higher than in norm. The confrontation and avoidance in the primary caregivers were negatively correlated to the factors of SCL-90(P<0.05)and yielding in the coping style positively to the factors of SCL-90(P<0.05).Conclusions The primary caregivers of stroke patients are at the poor psychological states and they tend to take the passive and negative coping styles. The more they take active coping style, the better their mental states grow and vise versa. Therefore, the caregivers need professional training so that they can use the supports from their families and be encouraged to speak out their poor moods.Post-discharge visits to them, besides, are helpful for the improved psychological states for the improvement of their quality of life.

Objective To explore the preventive and therapeutic role of silencing type Ⅰ rat platelet-binding protein motifs depolymerization protein-like metalloproteinase 2(ADAMTS2)by siRNA on experimental liver fibrosis in vitro.By studying the mechanism of siRNA silencing of ADAMTS2,we also aim to evaluate the feasibility of ADAMTS2 as a target for anti-liver fibrosis therapy. Methods Three pairs of siRNAs targeting ADAMTS2 mRNA 2237,2597 and 690 targets were designed and synthesized by utilizing RNA design software. The most effective siRNA was chosen to transfect HSC-T6 cell line to test the tendency of hepatic stellate cell(HSC)activation and ex pression of ADAMTS2,COL1α1,COL(I),α-SMA,TGF-β1,MMP-2 and TIMP-3.These were quantified using real time-PCR,Western blotting, and MTT assays. Results Of the same dosage and time of injection,siRNA 2237 inhibited ADAMTS2 gene expression significantly more than other siRNAs.siRNA-ADAMTS2 2237 markedly inhibited ADAMTS2 gene and protein expression of HSCT6 with more than 80% efficiency.Conversely,siRNA-ADAMTS2 2237 markedly reduced the gene and protein expressions of COL(I),α-SMA and TGF-β1 on HSC-T6 and inhibited the proliferation of HSC.Conclusions siRNA-ADAMTS2 2237 could effectively knockdown the gene and protein expression of ADAMTS2 in HSC-T6 cell lines.Silencing ADAMTS2 by siRNA significantly inhibited the activation, proliferation of HSC and the gene and protein expressions of COL(I),α SMA,and TGF-β1,and it may have a potential anti-fibrotic effect.ADAMTS2 might be an efficient target for anti-fibrotic therapy.

Objective:To investigate the effectiveness of heart rate deceleration capacity (DC) measurement in predicting the car-diotoxicity of malignant tumor patients treated with epirubicin-based chemotherapy. Methods:The clinical medical records, including CK-MB and cTnI levels and dynamic electrocardiogram (ECG) parameters before and after each chemotherapy cycle, of 140 patients treated with epirubicin-based chemotherapy were analyzed. Patients were divided into the DC>4.5 ms group and the DC≤4.5 ms group based on the calculated DC values. The CK-MB and cTnI levels and the dynamic ECG parameters of the two groups were compared af-ter two and four cycles of chemotherapy. Results:Patients in the two groups exhibited no statistically significant difference in their rele-vant clinical and pathological data before receiving chemotherapy (P>0.05). However, after four cycles of chemotherapy, the DC≤4.5 ms group showed a significantly greater increase in serum CK-MB and cTnI concentrations over the pre-chemotherapy levels compared with the DC>4.5 ms group. After two and four cycles of chemotherapy, the DC≤4.5 ms group also exhibited a significantly greater in-crease in mean heart rate (beats/min) and supraventricular and ventricular arrhythmia counts (times/24 h) over the pre-chemotherapy values compared with the DC>4.5 ms group (P<0.05). After four cycles of chemotherapy, 23 cases showed abnormally elevated cTnI levels in the DC≤4.5 ms group. In this group, patients with elevated cTnI level exhibited no statistically significant difference in CK-MB and cTnI concentrations, mean heart rates, and supraventricular and ventricular arrhythmia counts compared with those with nor-mal cTnI level before chemotherapy (P>0.05). However, the DC values of patients with elevated cTnI were significantly lower than those with normal cTnI level (P<0.05). Conclusion:The risk of epirubicin-induced cardiotoxicity increased with decrease in DC value. The DC test was shown to be an effective predictor of the risk of epirubicin-induced cardiotoxicity.
