Chinese Acupuncture & Moxibustion 2014;34(9):846-850
OBJECTIVETo verify the regulatory effects of acupuncture on exercise tolerance in patients with chronic obstructive pulmonary disease (COPD) at stable phase.
METHODSThirty cases of COPD were randomly divided into a treatment group (16 cases) and a placebo group (14 cases). Based on specified aerobic exercise, acupuncture was applied in the treatment group and placebo acupuncture was used in the placebo group. The acupoints included Danzhong (CV 17), Rugen (ST 18), Guanyuan (CV 4), Zhongwan (CV 12), Tianshu (ST 25) and so on. The needle did not penetrate into the skin for the placebo group. The treatment was required for 2 to 3 times per week for totally 5 weeks. The indices of exercise tolerance, including 6-min walking distance (6-MWD), exercise time, maximum oxygen uptake (VO2max) forced expiratory volume in 1 second (FEV1)/forced vital capacity (FVC), maximum ventilatory volume (MVV), St. George respiratory questionnaire (SGRQ) were observed in two groups before and after treatment.
RESULTS(1) Exercise tolerance: the differences of 6-MWD and exercise time were statistically significant between groups, which were more superior in the treatment group (both P<0.01); the VO2max was significantly increased after treatment in the treatment group (P<0.05), but there was no difference between two groups (P>0.05). (2) Pulmonary ventilation function: the differences of FEV1%, FEV1/FVC and MVV% were statistically significant between groups, which were more superior in the treatment group (P<0.05, P<0.01); (3) SGRQ: the SGRQ was significantly improved after treatment in the treatment group (P<0.05), but there was no difference between two groups (P>0.05).
CONCLUSIONThe acupuncture could improve the exercise tolerance in patients with chronic obstructive pulmonary disease at stable phase, and shorten the onset time of aerobic exercise. Besides, acupuncture combined with aerobic exercise could effectively improve the pulmonary function.

Objective To assess the application value of disposable syringe of low resistance (SLR) in epidural puncture. Methods One hundred and thirty-two patients scheduled to undergoing selective operation under epidural or combined epidural-spinal anesthesia were divided into glass syringe (GS) group and SLR group with 66 cases each by random digits table method.GS was applied in local anesthesia and epidural puncture with loss of resistance in GS group. While SLR was used in SLR group. The defect of the syringes' appearance was evaluated. The fastness between each sub-unit during the procedure was observed. The rupture, dropping, leakage, air leakage and blockage were also estimated. The heart rate, blood pressure, oxygen saturation and other adverse events were also observed. Results In SLR group, syringe excepted 1 case of slight resistance but did not affect the use,the rest of the components connected firmly, surface without defects, clear scale uniform color, not observed rupture, air leakage, leakage and blockage phenomenon, pump liquid and air resistance was small. In GS group, syringe surface without defects, clear scale uniform color,3 cases had loose connection,1 case with rupture when using,5 cases with air leakage and leakage,3 cases of pumping liquid or dry air resistance to 0.9% sodium chloride and moist after use. There was significant difference in appearance and usage between two groups (x2 =10.324 5,P =0.001 3).There was no significant difference in a puncture success, first three puncture success and inject air negative pressure sensitivity between two groups (P > 0.05).Conclusion SLR is worth to be applied in epidural puncture.

Objective To explore the determinants of Alzheimer' s disease (AD) patients with depression. Methods The degree of depression on AD patients was assessed by the geriatric depression scale.Improved cumulative logistic regression (ICLR) was used to analyze the determinants of AD patients with depression. Results 196 AD patients were investigated.Among the 196 AD patients, there were 60(30.6% ) males and 136(69.4% ) females,at 58-89 years of age (72.3 ± 6.0).Physical activity, diabetes,MoCA,hearing, economic sources and alcohol were related to the degree of depression of AD patients (P<0.10 ).The difference between “normal” and “mild depression” was smaller than difference between “mild depression” and “severe depression”.Conclusion AD patients with mild depression were the target population for prevention and they were influenced by several factors listed above.

To explore the influence of matrix materials in complicate ingredients on traditional Chinese medicine and investigate the excipients selection model based on balanced release characteristics of multicomponents, the influence of HPMC (K4M, K15M, K100M) and Carbomer (934P, 971P, 974P) was illustrated by testing in vitro release of ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in Panax notoginseng saponins (model drug, PNS). According to in vitro release results of PNS matrix tablets in water and artificial intestinal juice, the release curves were analyzed with Peppas equation and simulating factor (f). Significant differences in k value and n value among ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 existed in various formulations. The release behaviors from various excipients could be described with Non-Fickian transport or super Case II transport pattern. The f2 values for ginsenoside-Rg1, ginsenoside-Rb1 and notoginsenoside-R1 in 971P matrix tablet containing 30% Carbomer 971P were 74.91, 53.45, 57.89 in water and 79.35, 55.51, 51.89 in artificial intestinal juice, respectively. The release profiles fit for the regulation of FDA. The result revealed that the balanced release rates of Rg1, Rb1 and R1 in 971P matrix tablet were obtained.

Chinese Journal of Gastrointestinal Surgery 2008;11(4):331-334
OBJECTIVETo evaluate the efficacy of endoscopic self-expanded metal stent for the treatment of gastric outlet obstruction.
METHODSFifty-six cases of gastric outlet obstruction, collected from March 1999 to January 2008, were treated by endoscopic self-expanded metal stents. The success rate, complication rate and survival were evaluated.
RESULTSAmong the 56 cases, the male to female ratio was 33 to 23, and the average age was [65+/-14 (24-86)] years. Seventeen cases were gastric cancer, 4 duodenal carcinoma, 7 peri-ampulla carcinoma, 13 metastasis cancer and 15 presented recurrence after operations. The success rate of stent implantation was 98.2% (55/56). Forty-nine cases (87.5%) could eat liquid food 1-3 days after operations and eat semifluid food 4-9 days later. Forty-two cases (76.4%)could eat normal food after 10-16 days. The main complications included 14.5% of cases with pain, 1.8% with stent shift, 25.5% with bleeding and no one with perforation. Until January 20, 2008, 53 cases (96.4%) had been followed up. The mean survival time was [139+/-15 (14-332)] days, and the median survival time was 135 days. The re-stenosis rate was 5.7%, including 3 cases, happened in 143, 158 and 190 days after the operations respectively, and was successfully treated by second stent implantation.
CONCLUSIONEndoscopic implantation of self-expanded metal stent is a micro-invasive, safe, effective method for the treatment of gastric outlet obstruction.
