1.Percutanenous intrapulmonary injection of IL-2 plus radiotherapy for luug cancer

Objective To evaluate the effects of percutanenous intrapulmonary injecti0n 0f IL-2 plus radiotherapy for lung cancer. Methods Twenty-two patients with inoperable non small cell lung cancer received percutanenous intrapulmonary injection of 100U ? 6 in 6 weeks; and radiation dose was 60 Gy/6wks. Results The total effective response rate (CR+PR) was 95. 4 % with the complete response rate (CR) of 40. 9 %. Couclusion Percutanenous intrapulmomary injection of IL-2 plus radiotherapy could improve the control rate of the patients with lung cancer.
2.Experience in the teaching of child neurologic training doctors

The training doctors achieved good results in the aspects of medical skills, research ability and comprehensive professional quality.
3.Expression and its clinical significance of Oct4 gene in prostate cancer

Objective To study the expression and its clinical significance of Oct4 in prostate cancer.Methods The Oct4 mRNA expression levels in 2 normal prostate (NP) samples and 8 prostate cancer (Pca) samples were determined by reverse transcription polymerase chain reaction (RT-PCR) analysis.The Oct4 protein expression levels in 10 NP samples,10 benign prostate hyperplasia (BPH) samples and 45 Pca samples were detected by immunohisochemical analysis.Results The relative expression levels of Oct4 mRNA were 0.91,0.76,0.66,0.63,0.55,0.50,0.48,0.42 in 8 Pca samples and 0 in 2 NP samples,respectively.The positive rates of Oct4 protein expression was higher in Pca tissues than in NP and BPH tissues (91.1% vs.0.0%,91.1% vs.0.0%,both P<0.05).The level of Oct4 protein was increased with higher hostologic grade and clinical stage of prostate cancer.The strong positive rates of Oct4 expression were 72.7% and 30.0% in Pca tissues with and without lymph node metastasis respectively,and there was a significant difference in the strong positive rate between them (P<0.05).Conclusions The Oct4 expression is closely related to the differentiation and invasion of tumor.Oct4 might be an important index for the prognosis of prostate cancer.
4.Platelet transfusion refractoriness caused by GBA gene mutation in one patient with Gaucher disease

Objective To analyze the mutation characteristics of GBA gene in one patient with Gaucher disease and platelet transfusion refractoriness.Methods A female patient with anemia and thrombocytopenia showed platelet transfusion refractoriness,and then the proband and her family were performed bone marrow smear,β-glucocerebrosidase activity in leukocytes (dried blood spot assay),Bultrasonography and gene sequencing examination and pedigree investigation.Results Pedigree investigation showed that the heterozygous mutation of GBA gene existed in the father,mother,son,daughter and sister of the proband.Bone marrow cytomorphologic examination showed that Gaucher cells accounted for 6.0% in the female patient.The β-glucocerebrosidase activity in leukocytes was 3.78 nmol/(h · mg Pro).B-ultrasonography showed slightly splenomegaly.Gene sequencing found that the homozygous mutation of GBA gene,c.484A > G,existed in the female patient.Conclusion The patients with Gaucher disease may appear platelet transfusion refractoriness due to hypersplenism.The mutation of GBA gene is the main pathogenic factor of the family with Gaucher disease.
5.Influence of hyperthermo-radiotherapy on T helper subgroup for patients with esophageal carcinoma

Objective To evaluate the influence of hyperthermia on T helper subgroup (T help type1/T help type2,Th1/Th2) for patients with esophageal carcinoma. Methods 22 patients of esophageal carcinoma were randomly divided into two groups. Both group patients were treated by external radiation, the total dose was 64-66 Gy/30-33 fractions. Radiotherapy was the only therapy method in group A. Hypertbermia was used twice every week, with 8-10 fractions except for radiotherapy in group B. All patients were phlebetomized at the beginning and the end of radiotherapy for examining Th1/Th2 by flow cytometry. Results Th1 was significantly decreased in group A(t = 5.33, P < 0.01). All indexes in group B had no difference (t = - 1.41, P > 0.05). TheThl in group A was decreased much more than that in group B(t = 4.28, P < 0.05). Conclusion Hyperthermia could rectify the balance of Th1/Tb2.
6.Character of cardiac valvular calcification in uremia patients and its influence on cardiac structure and function

Objective:To evaluate the character of cardiac valvular calcification in uremia patients and its effect on cardiac structure and function.Methods:Thirty five cases with mitral and aortic valve calcification were selected from 326 uremia patients.Control group consisted of 33 uremia patients without valvular calcification.Their age,LVEDD,LAD,AOD,LVWT,LVEF and blood pressure matched with calcification group.The observing indexs included distribution of calcification,degree of regurgitation,LVEDD,LAD,AOD,LVWT and LVEF.The observing duration was one year.Results:The results showed that characters of valvular calcification with uremia were similar to elder valvular calcification.The degree of regurgitation was more greater in calcification group than in control group after one year.The LAD was greater in calcification group after one year.Conclusion:Cardiac valvular calcification of uremia may cause the increase of regurgitation and enlarge LAD. 7.Preliminary achievements of 4+4 program in medical education in Shanghai Jiaotong University School of Medicine

Objective To explore and evaluate the implementation process and prelimnary achievements of 4+4 medical education program in Shanghai Jaiotong University School of Medicine. Methods The data of student background,curriculum,exam grades and graduate employment of class 2002 and 2003 were collected and analysed. Results The data showed that this program has enrolled high quality students,teaching hours of key courses of medicine has been reduce to 70%,while the average grades of the comprehensive examination were higher than those of the 5 year students,and the employment prospects for 4+4 program has been optimistic. Conclusion The 4+4 program has achieved preliminary success,and the experiences is beneficial for the further reform of medical educational system.

9.Establishment of MDCK cell models expressing human MATE1 or co-expressing with human OCT1 or OCT2.

To establish single- and double-transfected transgenic cells stably expressing hMATE1, hMATE1 cDNA was cloned by RT-PCR from human cryopreserved kidney tissue, and subcloned into pcDNA3.1(+) plasmid by virtue of both HindIII and Kpn I restriction enzyme sites. Subsequently, the recombined pcDNA3.1(+)- hMATE1 plasmid was transfected into MDCK, MDCK-hOCT1 or MDCK-hOCT2 cells using Lipofectamine 2000 Reagent. After a 14-day-cultivation with hygromycin B at the concentration of 400 µg · mL(-1), all clones were screened with DAPI and MPP+ as substrates to identify the best candidate. The mRNA content of hMATE1, the cellular accumulation of metformin with or without cimetidine as inhibitor, or transportation of cimetidine was further valuated. The results showed that all of the three cell models over expressed hMATE1 mRNA. The cellular accumulation of metformin in MDCK-hMATE1 was 17.6 folds of the control cell, which was significantly inhibited by 100 µmol · L(-1) cimetidine. The transcellular transport parameter net efflux ratios of cimetidine across MDCK-hOCT1/hMATE1 and MDCK-hOCT2/hMATE1 monolayer were 17.5 and 3.65, respectively. In conclusion, cell models with good hMATE1 function have been established successfully, which can be applied to study the drug transport or drug-drug interaction involving hMATE1 alone or together with hOCT1/2 in vitro.

10.Preparation of baicalin colon-specific solid dispersion and evaluation on its in vitro release.

OBJECTIVETo prepare pH-dependent baicalin colon-specific solid dispersion, with the aim of colon-specific delivery and rapid drug release.
METHODBaicalin-eudragit S100 solid dispersion was prepared by using the solvent method. The microscopic structure and physicochemical properties were analyzed by using differential scanning calorimetry (DSC), scanning electron microscopy (SEM), X-ray powder diffraction (XRD) and infrared spectroscopy (IR). And its in vitro release was also investigated.
RESULTThe results of DSC and XRD analysis suggested that baicalin may be dispersed in solid dispersion in the amorphous state. IR results indicated a non-covalent bond effect may exist between baicalin and eudragit S100. The results of in vitro release determination showed that very few baicalins in pH 1.2 diluted hydrochloric acid solution for 2 h at the baicalin-eudragit S100 ratio of 1 : 6. The accumulated dissolution rate was less than 15% in pH 6.8 phosphate buffer solution for 4 h, but exceeding 90% in pH 7.6 phosphate buffer solution for 1 h.
CONCLUSIONThe prepared baicalin-eudragit S100 solid dispersion could achieve the objective of colon-specific delivery and rapid drug release, and helps increase the concentration of baicalin in colons.
