1.Effects of curdione on cognitive function and expression of inflammatoryfactors in the hippocampus after partial hepatectomy in aged mice

Objective To explore the effects of curdione on cognitive function and expression of inflammatory factors in the hippocampus after partial hepatectomy in aged mice.Methods The animals were divided into control group, sham group, operation group, and high-, medium-and low-dose curdione groups.The model of partial hepatectomy in aged mice was established according to the method reported in literature.The levels of SOD, MDA, CAT and GSH-Px in the hippocampus were detected by enzyme-linked immunosorbent assay (ELISA), while the expressions of NF-κB, IL-1β, IL-6 and TNF-α were measured by Western blot.Results The escape latency was significantly increased (P<0.01), and the number of platform-site crossovers was significantly decreased in operation group (P<0.05).Compared with those in operation group, curdione significantly decreased escape latency and increased platform-site crossovers (P<0.05, P<0.05).The levels of SOD, CAT and GSH-Px in the hippocampus were significantly decreased (P<0.05) and the content of MDA was significantly increased after partial hepatectomy (P<0.01), and these were reversed by curdione (P<0.05, P<0.05, respectively).The expressions of NF-κB, IL-1β, IL-6 and TNF-α protein were significantly inhibited by curdione (P<0.05).Conclusion Curdione can significantly improve cognitive dysfunction after partial hepatectomy in aged mice and the mechanism may be related to its inhibition of oxidative stress, inhibition of NF-κB activation and reduction of inflammatory cytokines IL-1β, IL-6 and TNF-α in the hippocampus.
2.Effect of xingnaojing injection and angong niuhuang wan on neurological functional defect in acute stage of cerebral infarction

BACKGROUND: It is discovered in clinical practice that the principles of regaining consciousness, opening orifice combining with benefiting qi and nourishing yin for the treatment of both root causes and symptoms can improve remarkably neurological functional recovery in acute stage of cerebral infarction.OBJECTIVE: To observe the effects of xingnaojing injection and angong niuhuang wan on neurological functional disturbance in acute stage of cerebral infarction.DESIGN: Randomized controlled study based on patients.SETTING: Guangdong General Hospital of Chinese People' s Armed Police Force.PARTICIPANTS: Inpatients with cerebral infarction in Department of Neurology of Guangdong General Hospital, Chinese People' s Armed Police Force from September 1998 to September 2003.METHODS: Totally 120 cases in the acute stage of cerebral infarction were randomized into xingnaojing injection group(XNJ), angong niuhuang wan (ANW) group and danshen fen(DSF) group, 40 cases in each group. Fourteen days after treatment, the therapeutic effects and neurological functional improvement were observed.MEAN OUTCOME MEASURES: Comparison of the improvement in neurological function in groups was carried on in the experiment. RESULTS: The total effective rates were 95%, 90% and 65% in xingnaojing injection group, angong niuhuang wan group and danshen fen group successively. The paralysis of limb was recovered more markedly in xingnaojing injection group and angong niuhuang wan group compared with the control. The therapeutic effect was significant in xingnaojing injection group.CONCLUSION: Both xingnaojing injection group and angong niuhuang wan group treat cerebral infarction of acute stage with better therapeutic effects, and the former provides rather remarkable effect on the recovery of neurological functional defect.
3.Clinicopathological analysis of papillary renal cell carcinoma

Objective To investigate the histological features of papillary renal cell carcinoma, and to explore the relationship between nuclear grading and prognosis. Methods The pathological slides and clinical data of 13 cases of papillary renal cell carcinoma were reviewed. The pathological features were scrutinized and nuclear grading was conducted according to Fuhrman grading system. Immunohistochemical staining for CK7, EMA, and vimentin was also respectively done. The correlation between nuclear grade and prognosis was analysed. Results The age range of the patients was from 35 to 67 (average 53) years. 10 were male and 3 were female. Most of the tumors were located in the upper pole (5cases) or lower pole (5cases). Three were located in the middle part of the kidney, with some inclination to either pole. All the tumors were well circumscribed with a size ranged from 2cm to 6.3cm (average 3.5cm) in diameter. Hemorrage and cystic change were obviously seen in large tumors. Microscopically, the tumors were composed of papillary or tubular structures, with hemorrhage, foamy macrophages and cystic change as prominent features. 6 cases were multifocal. Fuhrman nuclear grade Ⅰ or Ⅱ was made in 6 cases, and Ⅲ or Ⅳ in 7 cases. All the tumors were positive for CK7, EMA and vimentin. The patients were followed up for 28 to 102 months (average 52 months). 3 patients died of tumor recurrence or metastasis after the operation at 36, 41 and 50 months, respectively, while others survived without tumor up to now. The three dead patients were graded Ⅲ or Ⅳ by Fuhrman nuclear grading system. Conclusion Papillary renal cell carcinoma was different from other renal carcinoma variants in that they, usually have a better prognosis.
4.The clinicopathological analysis of 16 cases of sarcomatoid renal cell carcinoma

Objective To study the pathological features and clinical significance of sarcomatoid change of renal cell carcinoma. Methods The specimens of 16 cases of sarcomatoid renal cell carcinoma admitted to our hospital were reviewed, and stress of observation was put on the the morphology and the number of the sarcomatoid components, and then their relation with clinical course, staging, and prognosis was retrospectively analyzed. Results Among the 16 cases of renal cell carcinoma with sarcomatoid transformation, 9 cases were conventional clear cell carcinoma, 3 cases were papillary renal cell carcinoma, 2 cases were chromophobic cell carcinoma, and 2 cases were collecting tubule carcinoma. In 9 cases the sarcomatoid elements were over 50% of total tumor area, while in the other 7 cases the sarcomatoid elements were less than 50%. The TNM stage among the 9 cases with more than 50% sarcomatoid elements were T2 (1/9), T3 and T4 (8/9). The TNM stage among the 7 with less than 50% sarcomatoid elements were T2 (4/7), T3 and T4 (3/7). The sarcomatoid area resembled malignant fibrous histiocytoma or fibrosarcoma. The average survival period of the patients was 19 months (8-154). 7.Detection of methicillin-resistant S . aureus from respiratory specimens of ICU patients by conventional sputum culture compared with rapid polymerase chain reaction assay

Objective To evaluate the performance of polymerase chain reaction (PCR)‐based rapid detection of methicillin‐resistant S .aureus (MRSA) from the respiratory specimens of ICU patients in comparison with conventional sputum culture . Methods We collected 3 respiratory specimens from each patient simultaneously for bacterial culture ,fungal culture and PCR‐based detection of MRSA .The performance of the established PCR method was compared with the results of sputum culture in terms of sensitivity ,specificity ,positive predictive value (PPV) and negative predictive value (NPV) .Results The sensitivity , specificity ,PPV and NPV of PCR assay were 88 .9% ,73 .6% ,36 .4% ,and 97 .5% ,respectively ,which were lower than the reported results of PCR assay with nasal swabs .Conclusions The utility of PCR assay for active screening of MRSA from respiratory specimens still needs further supportive data .However ,PCR assay is somewhat informative in excluding MRSA infection or colonization because of its high NPV .
8.Three managements of bladder tumors under cystoscopy

Objective To discuss three managements of bladder tumors under cystoscopy.Methods Of all the patients,1174 were injected compound aluminum sulphate in tumor pedicle,and 700 cases were treated with transurethral resection of bladder tumor (TURBT) and 34 cases were treated with holmium laser.Results The first management,without obturator nerve reflex and tumor recurrence in situ,was easy and economical but not fit for big tumors without pedicles.With the requirements for related techniques and equipments,TURBT might be accompanied by bleeding,obturator nerve reflex and tumor recurrence in situ.Of the 34 cases treated with holmium laser,there were 3 ones with ectopic recurrence and no one with recurrence in situ.With the requirement for holmium laser equipment,the third management was the most expensive one.Conclusion All of the three managements have their advantages and disadvantages.The doctor should select the right management or perform cross application according to the condition of the patient.
9.Related complications of implantable venous access port

Objective To explore the incidence and clinical treatment of related complications caused by implantable venous access port(IVAP) in patients with breast cancer during chemotherapy.Methods The data of 755 patients with breast cancer recieved chemotherapy by which caused some related complications in our hospital from January 2014 to March 2016 were retrospectively analyzed.Results 753 patients IVAPs were implanted succussfully.The total placement time of implantable venous access port was from 110 days to 940 days,with median placement 147.33 days.The related complications of IVAP were catheter malposition(0.79%,6/755),catheter-related thrombosis(27.81%,210/755),catheter fracture(0.13%,1/755),port exposure(0.93%,7/755) and IVAP-related bloodstream infection(0.13%,1/755).The IVAP-related complications and thrombosis rate were significant higher when IVAPs implanted in the left internal jugular veincompared with that in right internal jugular vein(34.88% vs.25.74%,33.10% vs.24.68%).Conclusion Application of IVAP in patients with breast cancer during chemotherapy is a safe and effective operation.The most common complication is asymptomatic mural thrombus formation around the catheter,which should be paid attention to.
10.Relationship between single nucleotide polymorphism-229C/T in P1 promoter of furin gene and functions of hepatocytes in patients with liver cirrhosis

AIM: To study the influences of P1 promoter activity of furin gene on the functions of hepatocytes in patients with liver cirrhosis.METHODS: The patients with liver cirrhosis of 180 cases were recruited.The single nucleotide polymorphism(SNP-229 C/T) in P1 promoter of furin gene was genotyped using competitively differentiated polymerase chain reaction.The relationships between the promoter activity based on genotyping and the serum levels of liver enzymes,total bilirubin,albumin and prothrombin were observed.RESULTS: The distribution frequencies of allele C and T were 75.3%(271/360) and 24.7%(89/360).Those of genotypes CC,CT and TT were 62.2%(112/180),26.1%(47/180) and 11.7%(21/180),respectively.The distribution frequencies of the genotypes were not related to the serum levels of major liver enzymes,albumin,total bilirubin and prothrombin,except for alkaline phosphatase and ?-glutamyl transferase.CONCLUSION: The activity of furin promoter exerts no effects on the main functions of hepatocytes,suggesting that furin may be a new therapeutic target for HBV infection.
