OBJECTIVE:To provide reference for rational use of antibiotics in terminal stage patients with malignant tumor. METHODS:The inpatients with malignant tumor who died in the department of medical oncology of our hospital from Mar. 2013 to Mar. 2016 were analyzed retrospectively in respects of general situation,infection situation and antibiotics use. RESULTS:Among 149 inpatients,a total of 129 infection cases were found in 113 patients(75.8%). The most frequent sites of infection were lung(65.9%),followed by digestive tract/abdomen(13.2%),and skin/wound(6.2%). 100 patients (67.1%)received antibiot-ics,61.1% of which were empiric treatment. β-lactam/β-lactam lactamase inhibitor(41.8%),fluoroquinolone(21.7%)and cepha-losporin(16.9%)were the top 3 frequently prescribed antibiotics. The effective rate of antibiotics treatment was as low as 15.8%, which was believed to be linked with the survival duration since infection occurred (P<0.001),the Karnofsky performance scale (KPS)score when infection occurred(P<0.001)and the duration of antibiotics treatment(P=0.025). CONCLUSIONS:Terminal stage patients with malignant tumor are vulnerable to infections,especially to pulmonary infection. The empirical broad-spectrum an-tibiotics are widely used in terminal patients with malignant tumor,but the effectiveness rate of antibiotic treatment is in low level. For those terminal stage patients with malignant tumor and with KPS score<60 points,when futile antibiotics treatment last for more than 7 days,timely termination of antibiotics treatment is a better choice.

The solid-phase method to synthesis HCV polypetides was used.The connecting rate and the polypeptides' productivity and purity were compared when Wang resin and 2-chlorotrity choloride resin acting as vectors.By the identification of mass spectrum,the products' molecular weight was analysed.By ninhydrin method we compared to the connecting rate between two resins and amino acid.By RP-HPLC we carried out the analysis and purity of hetero peptides.The result showed the first amino acid's connecting rate along with the purity and productivity which we need when 2-chlorotrityl choloride resin acted as vector was notably higher than those when Wang resin acted as vector.So 2-chlorotrityl choloride as vector is used to synthesize HCV polypeptides,which adapts to the demand of big scale's industrialization production.

Objective To compare the effects of electroacupuncture at points Shangjuxu(ST37), Zusanli(ST36), Xiajuxu(ST39) and Yanglingquan(GB34) on colonic expressions of interleukin-1b (IL-1b) and nicotinic acetylcholine receptor a7 mRNA (nAchRa7mRNA) in ulcerative colitis rats and investigate if large intestine lower He-Sea point Shangjuxu has relative specificity to fu organ diseases. Method Seventy healthy SD rats were randomized into blank, model, Shangjuxu, Zusanli, Xiajuxu, Yanglingquan and Chengjin groups, 10 rats, half male and half female, each. A rat model of ulcerative colitis was made by induction of 2-4-6 three nitrobenzene sulfonic acid/ethanol solution enema in every group except the blank group. After successful model making and ten days of treatment, rat colonic mucosal ulcers and inflammation were observed macroscopically, colonic IL-1bcontent was measured by ELISA and the expression of nAchRa7mRNA was determined by RT-PCR. Result Compared with the model group, colonic lesions were reduced in varying degrees, colonic IL-1b content was significantly lower and the expression of nAchRa7mRNA was higher in every acupoint group (P<0.05, P<0.01);the colonic ulcer score was lower in the Shangjuxu and Zusanli groups (P<0.05, P<0.01). Compared with the Shangjuxu group, colonic expression of nAchRa7mRNA was lower in the other four acupoint groups (P<0.01); colonic mucosal ulcers and inflammatory lesions were more severe and the colonic ulcer score and the IL-1bcontent were higher in the Xiajuxu, Yanglingquan and Chengjin groups (P<0.05, P<0.01). Conclusion The mechanism of electroacupuncture treatment for ulcerative colitis may be that it regulates abnormal immunologic function by modulating IL-1b and nAchRa7mRNA and reduces mucosal lesions. The overall therapeutic effect of Shangjuxu is better than those of Zusanli, Xiajuxu, Yanglingquan and Chengjin, indicating that Shangjuxu has relative specificity to fu organ large intestine.

Saccharmyces cerevisiae 220 has certain resistance to Pb2+ was found through the test of it's cultured on media containing lead cation. Sacchannyces cerevisiae 220 cultured was put into the media cotaining lead cation and it's biosorp-tion to lead cation was studied. The results show this strain has maximum biosorption ratio (96.6%) to 6mg/L lead cation, biosorption-equilibrilium period is 25-30min, biosorption kinitics equation is q-26.318C/ (1 + 0.437C) . Acetic acid is benefit to de-biosorption, glucose and KH2PO4 can improve biosorption ratio to lead cation.

Objective To evaluate the molluscicidal effect and the using value of qiangluocide powder by dusting in the fields. Methods The two different ways of dusting with qiangluocide and spraying with luomiecide for controlling snails were compared in the fields, and the cost-effectiveness was analysed. Results The corrected mortalities and the reduced rates for density of snails were 88.48%, 96.26%, 96.96%, 95.67%, and 87.58%, 95.40%, 96.14%, 96.67%, respectively, on the marshland in Longtan, Nanjing, 73. 65% , 98. 31% , 99. 22% , 99. 26% and 69. 30% , 98.14%, 98.84%, 98. 60% , respectively, on the marshland in Yizheng, Yangzhou, on 1 d, 3d, 7 d, 15 d after dusting with qiangliocide power; and those on 1d, 3d, 7d, 15d after the spraying with luomiecide were 69.15%, 88.01%, 86.46%, 85.56% and 75.27%, 84.99%, 87.50%, 87.96% on the marshland in Longtan, Nanjing, 18.70%, 63.11%, 1.25%, 12.60% and 34. 88% , 89. 02% , 45.37% , 46. 59% , respectively, on the marshland in Yizheng, Yangzhou. The st of dusting with qiangluocide powder was 0. 20 yuan/m2, and that of spraying with luomiecide as 0. 21 yuan/m2 for snail control in the fields. The costs of reduced one percentage for the cor rected mortalities and the density of snail were 6. 27-10. 07 yuan and 6. 21-10. 14 yuan, respectively, on 15 d after dusting with qiangluocide power and those on 15 d after spraying with luomiecide were 7. 36-83. 33 yuan and 7. 16-22. 54 yuan , respectively. Conclusion The dusting with qiangluocide powder is a promising new formulation, which is suitable for the areas without water resource and possessed of high effectiveness against snails. The method ofdusting with qian-gluocide powder for controlling snails is simple, convenient, cheap and easy to be carried out for the inspection and quality control.

Objective To explore the rela ti onship between the environmental temperature and the oxygen demand of [WT5” BX]Oncomelania snails, and the relationship between the hiber nation or aestivation and the oxygen demand of snails.Me thods The oxygen demand of Oncomelaniasnails between 0 ℃ and 40 ℃ was determined by increasing the temperature gradually in lab, the relationship between the environmental tempera ture and the oxygen demand, and the correlative relationship between the oxygen demand and the percentage of snails in hibernation or aestivation were analysed. Objective:To observe the influence of pressing force and time on the thermal effect of An-pressing manipulation.Methods:Eight healthy volunteers were recruited to receive An-pressing manipulation at Xinshu (BL 15) on the right side.The pressing force and time were both divided into five levels:the force described as extremely mild,mild,moderate,strong and extremely strong and time given by 2.5 min,5.0 min,7.5 min,10.0 min and 15.0 min.The real-time change in local acupoint temperature as well as the change during 1.0-15.0 min after the manipulation were observed.Results:Compared with the baseline data,the real-time changes in the temperature after An-pressing Xinshu (BL 15) on the right side with different levels of force (from mild to strong) were respectively (1.88t0.64) ℃,(2.05±0.68) ℃,(2.25±0.59) ℃,(2.35±0.61) ℃ and (2.32±0.69) ℃;the changes in 15.0 min after the manipulation were respectively (-0.11±0.11) ℃,(0.03±0.14) ℃,(0.59±0.58) ℃,(1.38±0.70) ℃ and (2.09±0.98) ℃.The real-time temperature changes after the manipulation for different durations (from short to long) were respectively (1.94±0.37) ℃,(2.33±0.29) ℃,(2.49±0.31) ℃,(2.51±0.39) ℃ and (2.41±0.55) ℃;the changes in 15.0 min after the manipulation were respectively (0.53±0.49) ℃,(0.33±0.30) ℃,(0.52±0.33) ℃,(0.55±0.38) ℃ and (0.76±0.36) ℃.Conclusion:The thermal effect presented an increasing tendency with the extension of pressing time,and the temperature reached the top at 7.5 min;the thermal effect showed an increasing tendency with the rise of pressing force,and the temperature reached the top upon a moderate level of force.The pressing time can produce a greater influence on the real-time temperature than the pressing force;the pressing force can produce a greater influence on maintaining the temperature than the pressing time.

Objective To establish and verify the prediction model for ischemic cardiovascular disease(ICVD)among the elderly population who were under the current health care programs. Methods Statistical analysis on data from physical examination, hospitalization of the past years, from questionnaire and telephone interview was carried out in May, 2003. Data was from was randomly selected to generate both module group and verification group. Baseline data was induced to make the verification group into regression model of module group and to generate the predictive value. Distinguished ability with area under ROC curve and the predictive veracity were verified through comparing the predictive incidence rate and actual incidence rate of every deciles group by Hosmer- Lemeshow test. Predictive veracity of the prediction model at population level was verified through comparing the predictive 6-year incidence rates of ICVD with actual 6-year accumulative incidence rates of ICVD with error rate calculated. Results The samples included 2271 males over the age of 65 with 1817 people for modeling population and 454 for verified population.All of the samples were stratified into two layers to establish hierarchical Cox proportional hazard regression model, including one advanced age group(greater than or equal to 75 years old), and another elderly group(less than 75 years old). Data from the statically analysis showed that the risk factors in aged group were age, systolic blood pressure, serum creatinine level, fasting blood glucose level, while protective factor was high density lipoprotein; in advanced age group, the risk factors were body weight index, systolic blood pressure, serum total cholesterol level, serum creatinine level, fasting blood glucose level, while protective factor was HDL-C. The area under the ROC curve (AUC)and 95%CI were 0.723 and 0.687-0.759 respectively. Discriminating power was good. All individual predictive ICVD cumulative incidence and actual incidence were analyzed using Hosmer-Lemshow test, x 2=1.43, P=0.786, showing that the predictive veracity was good.Conclusion The stratified Cox Hazards Regression model was used to establish prediction model of the aged male population under a certain health care program. The common prediction factor of the two age groups were: systolic blood pressure, serum creatinine level, asting blood glucose level and HDL-C. Thc area under the ROWC curve of the verification group was 0.723, showing that the distinguished ability was good and the predict ability at the individual level and at the group level were also satisfactory. It was feasible to using Cox Proportional Hazards Regression Model for predicting the population groups.
