Objective:To explore the correlation of nurses ’ organizational commitment and their professional identity status.Methods:Using Chinese employee organizational commitment scale and nurses professional identity evaluation scale questionnaire survey was conducted in 400 cases of clinical nurses , nurse compare different characteristics of organi-zational commitment and professional identity level , and USES the Pearson correlation analysis to investigate the correla-tion of both.ResultsNurses organizational commitment and professional identity total score , respectively (2.16-0.45) and (3.49 +0.84);Different cultural degree , job title, working years and the way of hiring nurses ’ professional identi-ty score comparison, the difference was statistically significant (P <0.05);Different ways of title, working years, and hire nurses’ organizational commitment score comparison , the difference was statistically significant (P <0.05);Pear-son correlation analysis showed that organizational commitment and the scores of each dimension career cognitive apprais -al, professional social skills , professional setback coping , social support and total scores of professional identity were sig -nificantly positive correlation (P <0.05).Conclusions:The nurse was significantly positively related to organizational commitment and their professional identity , and both have big room to improve .

Objective To study the mental status of dento-maxillofacial deformities patients for orthognathic-orthodontic treatment,assess the testing results,and provide reference for doctors and nurses to evaluate mental features of patients.Methods 79 patients from a three-level and first-class hospital in Shanghai were collected from February 2011 to January 2012,who were to undergo orthognathic-orthodontic treatment.All the subjects filled in MMPI questionnaire before the operation.Results Scores of F and Pa subscales in male were significantly lower than the national norm.Scores of K and Mf subscales in female were significantly higher than the national norm.Scores of L,F,Hs,D,Pd,Pa,Pt,Sc,Si subscales were significantly lower than the national norm.Scores of F,Pa,Sc,Ma subscales in male were significantly higher than female.Scores of Mf subscales in female were significantly higher than male.Conclusions The whole personality of dento-maxillofacial deformities patients are normal.However,individualized personality deviation and psychological problems exist,including personality characteristics among different genders.

Objective To reduce the turnaround time for laboratory diagnosis of bacteremia, the feasibility of rapid identification and susceptibility testing using samples taken directly from positive blood culture bottles was evaluated. Methods The growth of microorganisms in blood culture bottles was screened by the BACTEC 9000 blood culture system. 65 positive blood culture bottles containing gram-negative bacteria were adopted to test. Culture fluid was injected into BD SST vacutainer and centrifuged to pellet blood cells. After collecting required McFarland units, they were cultured on Phoenix 100 NMIC/ID-4(identification-gram-negative bacteria and susceptibility testing) cards using 0.25 McF and 0.5 McF methods respectively. They were also evaluated by the standard method, involving subculture tests from positive blood culture bottles. Results 63 of 65 gram-negative bacteria (96. 9% ) were correctly identified with 0. 25 McF method. 59 of 65 gram-negative bacteria(90.8% ) were correctly identified with 0.5 McF method. For antimicrobial susceptibility testing, the 0.25 McF direct method had an agreement rate more than 94% , the 0.5 McF method was more than 85.7% and direct blood sample KB method was more than 93.8% compared to the standard method. But the overall minor error rate in susceptibility testing of direct blood sample KB method is higher than other methods. Conclusion Applying 0. 25 McF and 0. 5 McF rapid identification and susceptibility test was practical. During to possessing more prominent advantages, laboratory put the 0. 25 McF direct method into practice had a timely, remarkable significance.

OBJECTIVETo study the clinical effect of arthroscopic excision of popliteal cysts through a combined anterior and posterior approach.

METHODSFrom January 2010 to December 2012,20 patients with popliteal cysts were treated with arthroscopic excision. There were 14 males and 6 females, with an average age of 49.5 years old, ranging from 45 to 65 years old. The lump has been found for 4 to 18 months,with a mean time of 12 months. Their mean sagittal diameter was 4.5 cm (ranged from 3 to 7 cm). There were 12 popliteal cysts in the left and 8 popliteal cysts in the right. The main clinical manifestation included lump at popliteal fossa, swelling and pain at knee joint and some extent of dysfunction. All diagnoses were determined according to MRI, which clearly showed the communication of cyst and articular cavity. The cyst was removed under arthroscopy, through the posterior approach and then the intra-articular lesion was treated via the anterior approach. According to Rauschning and Lindgren classification, 2 patients were grade I, 6 patients were grade II, and 12 patients were grade III. The guidelines of Rauschning and Lindgren were used to evaluate the clinical effects.

RESULTSNo complications such as the injury of blood vessel and nerve, or incision infection occurred in all patients. All the patients were followed up, and the duration ranged from 8 to 24 months, with a mean time of 16 months. According to the criteria of Rauschning and Lindgren, there were 14 cases of grade 0, and 6 cases of grade I after operation, which was improved obviously compared with that pre-operation. No cyst reoccurred and the knee joint pain was relieved.

CONCLUSIONTreatment of popliteal cysts with arthroscopic excision through a combined anterior and posterior approach is effective to remove the cyst sac and treat intra-articular diseases simultaneously, resulting in the decrease recurrence rate of cyst.

Objective To investigate the significance of B7-CD28/CTLA-4 (CD152),co-stimulatory molecules of T lymphocytes,in the onset,development and prognosis of genital congdyloma acuminatum (CA) in females.Methods Flow cytometry was utilized to detect the expression levels of CD80/CD86 in peripheral blood lymphocytes and of CD28/CD152 (CTLA-4) in CD4~+/CD8~+ T lymphocytes from 30 CA patients (17 primary CA,13 recurrent CA,15 at recovery stage of CA) and 15 healthy volunteers as con- trols.Results No significant difference was found for the frequencies of CD80~+,CD86~+,CD4~+/CD28~+ and CD8~+/CD28~+ lymphocytes between the primary or recurrent group and the control group.The frequencies of CD8~+/CD28~+,CD4~+/CD28~+ and CD80~+ lymphocytes were significantly higher in the recovery group than those in the recurrent group (P0.05).Conclusions There is an abnormal expression of co-stimulatory molecules B7-CD28/CTLA-4 (CD152) in periphoral blood lym- phocytes,CD4~+ and CD8~+ T lymphocytes in female patients with genital CA,and the expression abnormaility is closely linked with different disease stages of CA.

OBJECTIVE To understand the distribution and epidemiology of ESBLs in ceftazidime or cefotaxime-(resistant) clinical Enterobacter cloacae isolates.METHODS Twenty seven ceftazidime or cefotaxime-resistant(nonrepetitive) E.cloacae were collected from 27 patients hospitalized at the Huashan Hospital,Shanghai.PCR and(sequencing) were performed to understand the distribution of ESBLs in E.cloacae;rep-PCR was(performed) to(understand) the epidemiology of ESBLs in E.cloacae.RESULTS CTX-M-3 like(ESBLs) were the most prevalent in our study(48%);this was the first report of VEB-1-like ESBLs in the member of(Enterobacteriaceae) in China,and the first report of the ESBLs VEB-1-like and CTX-M-3-like in an isolate simultaneously;the majority of(ESBLs) producers exhibited the same rep-PCR pattern,but harbored different ESBLs gene.(CONCLUSIONS) In our study,ESBLs have become prevalent in clinical E.cloacae isolates,and become an important factor of E.cloacae isolates resistant to extended-spectrum beta(-lactams).

Objective To construct the small interfering RNA (siRNA) targeting heparanase gene and its expressing vector,and to observe its interference effect on the expression of heparanase gene and inhibitory effect on the invasive potential of human malignant melanoma A375 cells.Methods Three siRNAs were designed.The recombinant plasmid pRNATU6.1/heparanase-siRNA was designed and constructed. A375 cells were cultured,and transfected with pRNATU6.1/heparanase-siRNA.The cells treated with lipo- fectamine or Opti-MEM served as the controls.Real-time fluorescence quantitative PCR and Western blot were performed to evaluate the expression of heparanase RNA and protein in these treated A375 cells.The in vitro invasive potential of treated A375 cells was assessed by Matrigel gel assay.Results The siRNA targeting heparanase gene was successfully cloned to the eukaryotic expressing vector pRNATU6.1.The expression levels of both heparanase RNA and protein decreased significantly in siRNA-transfected A375 cells than those in the control cells.The in vitro invasive potential of siRNA-transfected cells was also signifi- cantly inhibited as compared with that of the control cells (P

Objective To analyze the compatibility of bone marrow mesenchymal stem cells (BMSCs) with amphiphilic peptide three-dimensional gel.Methods Three healthy 3-week old SD rats were taken for separating femur and tibia to obtain BMSCs,the BMSCs surface antigen was detected by flow cytometry;the 10 mg/mL RGD-cyclic amphiphilic peptide solution was added into the same volume of DMEM/F12 culture medium,after a few seconds,which was self assemble into three-dimensional gel.Three dimensional gel structure was observed by transmission electron microscope (TEM).1 × 106 cells/mL BMSCs suspension and RGDcyclic amphiphilic peptide were mixed to form a 3D culture system,1 × 106 cells/mL BMSCs suspension was mixed with polylysine to form a 2D culture system,the serum-free culture was conducted;the CCK-8 method was used to observe the cell growth situation,calcein acetoxy methyl ester/propidium iodide (PI) double standard staining was performed.The effect of RGD-cyclic amphiphilic peptide on the proliferation of BMSCs was observed by fluorescence microscopy.Results The separated and cultured BMSCs highly expressed CD29 and CD90,but lowly expressed or did not express CD34 and CD45;TEM showed that the gel was composed of multiple empty nanofibers with the nanofiber diameter of 2-5 nm and length of 100-1 000 nm;the molecular weight of synthetic peptides detected by mass spectrometry (MS) was 1 256.37,which was consistent with the theoretical value;the HPLC analysis showed that RGD-amphiphilic peptide purity was 95.88 ％;the calcein acetoxyl methyl ester/PI double staining showed that in the 3D culture system,a few BMSCs died after 30 min and the cells began to proliferate after 12 h,the proliferation was more active than that of 2D culture,and the difference was statistically significant (P＜0.05);CCK-8 cell count showed that the proliferation activity of 3D culture system was higher than that of 2D culture system,and the difference was statistically significant (P＜0.05).Conclusion RGD amphiphilic peptide has a good biocompatibility with BMSCs,and may become the tissue engineering scaffold material.

Objective To investigate the current situation of nurses' caring behavior and analyze its influencing factors,in order to provide basis for the practice of caring behavior.Methods Caring nurse-patient interaction scale-short scale ( CNPI-23 ) was used to investigate 272 chnical nurses' caring behavior from 3 aspects of importance,competence and feasibility.The score of each item for each aspect was 5 to 15,and total score of the scale was 69 to 345.The Cronbach' s α of the scale was 0.97,and of the 3 aspects was 0.92 to 0.95.The exploratory and confirmative factor analysis showed that CNPI-23 had good construct validity.Results 272 questionnaires were given out,267 were called back and 260 were effective,with the effective rate of 95.6％.The score of each item ranged from 11.67 to 14.23,of which the highest 5 items were mainly about operative caring behavior,and the lowest 5 items were mainly about expressive caring behavior.The age and working life of nurses were positively correlated with the score of competency aspect ( r =0.35,P ＜ 0.05 )and clinical caring dimension within feasibility aspect ( r =0.40,P ＜ 0.05 ).Nurses' age was also positively correlated with the score of humanistic care within competency aspect ( r =0.13,P ＜ 0.05 ).The higher nurses' professional title was,the higher their scores in competency aspect ( x2 =22.00,P ＜ 0.05 ) and feasibility aspect ( x2 =10.71,P ＜ 0.05 ) were.Conclusions Rich life and work experience and the improvement of competency can promote nurses' caring behavior.

Objective To observe the changes of multi-noninvasive indexes including endothelial function, arterial flexibility, carotid intima-media thickness (IMT) and serum inflammatory cytokines in patients with mild coronary stenosis. Methods One hundred and five patients were divided into three groups according to the result of coronary angiography: coronary heart disease (stenosis ≥ 50% in at least one coronary segment), mild coronary stenosis (stenosis < 50% in at least one coronary segment) and control group (normal coronary). Brachial-ankle pulse wave velocity (baPWV) and ankle-brachial index (ABI), reflecting arterial flexibility and the lower extremity vascular disease respectively, were measured by a Colin system, carotid artery IMT was detected echocardiographically. Serum levels of NO, vWF, hs-CRP were measured before coronary angiography in all patients. Results baPWV [(1482±155) cm/s vs. (1249±158)cm/s] and carotid IMT [(0.88±0.18)mm vs. (0.72±0.20) mm] were significantly higher while serum levels of NO [(64±17) μmol/L vs. (83±17) μmol/L] was significantly lower in mild coronary stenosis group than those in control group (all P<0.05). vWF, ABI and hs-CRP were similar between the two groups (all P> 0.05). Logistic regression analysis showed that NO, baPWV, smoking are independent predicting factors for mild coronary stenosis (all P < 0.05) . Conclusions Endothelial dysfunction, reduction of the arterial flexibility as well as increased serum inflammation were associated with mild coronary stenosis.
