Objective To reduce the turnaround time for laboratory diagnosis of bacteremia, the feasibility of rapid identification and susceptibility testing using samples taken directly from positive blood culture bottles was evaluated. Methods The growth of microorganisms in blood culture bottles was screened by the BACTEC 9000 blood culture system. 65 positive blood culture bottles containing gram-negative bacteria were adopted to test. Culture fluid was injected into BD SST vacutainer and centrifuged to pellet blood cells. After collecting required McFarland units, they were cultured on Phoenix 100 NMIC/ID-4(identification-gram-negative bacteria and susceptibility testing) cards using 0.25 McF and 0.5 McF methods respectively. They were also evaluated by the standard method, involving subculture tests from positive blood culture bottles. Results 63 of 65 gram-negative bacteria (96. 9% ) were correctly identified with 0. 25 McF method. 59 of 65 gram-negative bacteria(90.8% ) were correctly identified with 0.5 McF method. For antimicrobial susceptibility testing, the 0.25 McF direct method had an agreement rate more than 94% , the 0.5 McF method was more than 85.7% and direct blood sample KB method was more than 93.8% compared to the standard method. But the overall minor error rate in susceptibility testing of direct blood sample KB method is higher than other methods. Conclusion Applying 0. 25 McF and 0. 5 McF rapid identification and susceptibility test was practical. During to possessing more prominent advantages, laboratory put the 0. 25 McF direct method into practice had a timely, remarkable significance.

OBJECTIVETo study the clinical effect of arthroscopic excision of popliteal cysts through a combined anterior and posterior approach.
METHODSFrom January 2010 to December 2012,20 patients with popliteal cysts were treated with arthroscopic excision. There were 14 males and 6 females, with an average age of 49.5 years old, ranging from 45 to 65 years old. The lump has been found for 4 to 18 months, with a mean time of 12 months. Their mean sagittal diameter was 4.5 cm (ranged from 3 to 7 cm). There were 12 popliteal cysts in the left and 8 popliteal cysts in the right. The main clinical manifestation included lump at popliteal fossa, swelling and pain at knee joint and some extent of dysfunction. All diagnoses were determined according to MRI, which clearly showed the communication of cyst and articular cavity. The cyst was removed under arthroscopy, through the posterior approach and then the intra-articular lesion was treated via the anterior approach. According to Rauschning and Lindgren classification, 2 patients were grade I, 6 patients were grade II, and 12 patients were grade III. The guidelines of Rauschning and Lindgren were used to evaluate the clinical effects.
RESULTSNo complications such as the injury of blood vessel and nerve, or incision infection occurred in all patients. All the patients were followed up, and the duration ranged from 8 to 24 months, with a mean time of 16 months. According to the criteria of Rauschning and Lindgren, there were 14 cases of grade 0, and 6 cases of grade I after operation, which was improved obviously compared with that pre-operation. No cyst reoccurred and the knee joint pain was relieved.
CONCLUSIONTreatment of popliteal cysts with arthroscopic excision through a combined anterior and posterior approach is effective to remove the cyst sac and treat intra-articular diseases simultaneously, resulting in the decrease recurrence rate of cyst.

This study was aimed to investigate the expressions of vascular endothelial growth factor (VEGF) and cyclooxygenase-2 (COX-2) in patients with multiple myeloma (MM) and its clinical significance. Expression of VEGF was detected by enzyme linked immunosorbent assay (ELISA) and the level of COX-2 was detected by Western blot. The results showed that the serum VEGF level of multiple myeloma patients (365.34 +/- 65.63 pg/ml) was higher than that in the normal persons (122.52 +/- 39.29 pg/ml) (p < 0.05); the serum VEGF level of patients at advanced stage (395.07 +/- 54.90) pg/ml was higher than those at stable stage (300.33 +/- 44.22) pg/ml (p < 0.05). The serum Cox-2 positive rate in the patients (31%) was higher than that in normal persons (0%) (p < 0.01); the serum Cox-2 positive rate in the patients at advanced stage (50%) was higher than those at stable stage (21%) (p < 0.01). It is concluded that VEGF and COX-2 may play an important role in the pathogenesis and development of multiple myeloma, they can be used to evaluate the status of patients with MM.

Objective:To examine the effects of tranexamic acid on epidermal melanocytes of mice after ultraviolet (UV) B ear irradiation. Methods:Twenty four female C57 mice, about 20 to 24 g on average, were randomly divided into normal saline group (NS), tranexamic acid group (TA), UVB/normal saline group (UVB/NS), and UVB/tranexamic acid group (UVB/TA). The UVB-irradiated group was locally irradiated to the ear with UVB at a dose of 250 mJ/cm 2 per day for 3 days. The UVB/TA group and TA group were orally administered tranexamic acid (750 mg/kg/d) at 30 min before UVB exposure; at the same time, the UVB/NS group and NS group were given to equal amount of normal saline by gavage. Cell counter and the morphological changes were accessed by using optical microscope in combination with Dopa staining. Quantitative real-time PCR (qRT-PCR) was used to detect the mRNA expression of tyrosinase (TYR), tyrosinase-related protein-1 (TYRP1), tyrosinase-related protein-2 (TYRP2) and microphtalmia transcription factor (MITF) of the ear. Results:According to the DOPA staining, there was no difference between the NS group and TA group. As compared with the NS group, the number of Dopa-positive melanocytes increased remarkably in the UVB/NS group ( t=6.653, P<0.01), the dendritics of these cells were numerous and prolonged ( t=6.364, 3.844, P<0.01). In comparison with the UVB/NS group, the expression of Dopa-positive melanocytes was significantly decreased in the UVB/TA group ( t=3.649, P<0.05), the number and length of dendrites were also alleviated with significant difference ( t=4.146, 2.197, P<0.05). The relative expression level of TYR, TYRP1, TYRP2, MITF in the skin of the ear was calculated via 2-△△CT method. The expression levels of target gene in the UVB/NS group were increased and had significant difference from that in NS group ( t=14.030, 5.427, 9.800, 5.891, P<0.05). While the mRNA expression levels of TYR, TYRP1, TYRP2 and MITF in the UVB/TA group were lower than those in the UVB/NS group, with statistically significant difference ( t=2.078, 1.905, 3.138, 1.732 P<0.05). Conclusions:Tranexamic acid suppresses UVB ear irradiation-induced epidermal melanocyte activation of mice by decreasing the mRNA expression levels of TYR, TYRP1, TYRP2 and MITF.
