The authors first expound the connotation and extension, characteristics and forms of governmental purchasing and the significance and necessity of centralized drug purchasing. Then they focus their discussion on the specific procedures of centralized drug purchasing. Finally they point out that it is imperative to seek unity of thinking, standardize operations, adopt institutionalized means of management, and make a really good job of centralized drug purchasing.

3.An inquiry into application of Law for Protection of Rights and Interests of Consumers to disposal of medical disputes

The article discusses whether the "Law for Protection of Rights and Interests of Consumers" is applicable to the disposal of medical disputes between doctors and Patienes. From the point of the Principle of exchang of commodities, the relation between doctors and patients is not one of commodity consumption on the basis of exchange at equal value. In terms of the nature of the medical and health cause, hospitals are not operators managing goods. In light of the inherent characteristics of medical service, medical and health work is a special trade of services. In the perspective of law, medical services are no ordinary acts of economically legal relation between doctors and patients,they are rather a special consuming act on the side of consumers and there is a special economically legal relation between doctors and patients. Accordingly, the "Law for Protection of Rights and Interests of Consumers" is not applicable to the diposal of disputes between doctors and patients.
4.Clinical observation of two treatments for vitreous hemorrhage

AIM: To investigate the indications and therapeutic effect of the conservative treatment and surgical treatment for vitreous hemorrhage.
METHODS: Hemorrhage reasons, degree of illness, and treatment and final results of vitreous hemorrhage were recorded from 2008 to 2013, and curative effect of different treatments was analyzed.
RESULTS:Selected 22 cases ( 24 eyes ) of conservative treatment, vision degree of 6 eyes ( 25%) had raised, 5 eyes ( 21%) occurred secondary retinal detachment without treatment, 3 eyes (12. 5%) suffered neovascular glaucoma were underwent operation, but the vision had lost completely. In 26 cases ( 31 eyes ) of control (operation) group, vision of 17 eyes (55%) had improved, 2 eyes (6. 5%) suffered the second operation, 2 eyes (6. 5%) suffered the third surgery, 3 eyes (10%) suffered eyeball atrophy. The number of ultimate vision improved with conservative treatment were significantly lower than the number of cases with surgical treatment (P<0. 05).
CONCLUSION: The surgical treatment is still visible vitreous hemorrhage treatment means of positive, but there will be eyeball atrophy and other serious complications, so we should choose a different methods in according to the different treatment objects and treatment time.
6.Implementation of fast tract surgery in patients undergoing liver resection

Fast tract surgery (FTS) has been implemented in different fields of surgery to attenuate the surgical stress response and accelerate recovery.Liver resection is the preferred treatment for a variety of primary and secondary liver tumors.However,liver resection is associated with severe stress response and higher rate of postoperative morbidity and mortality.Fast tract surgery has been reported to accelerate recovery following liver resection.In this review,we summarize the recent progress of fast tract surgery in liver resection.
7.Experimental study on the use of ~(103)Pd radioactive stent in the prevention of post-traumatic bile duct stricture in dogs

Objective To investigate the preventive effects of 103 Pd radioactive stent on stenosis after bile duct injury in dogs.Methods Twelve healthy dogs (15~20kg) were randomly divided into 103 Pd radioactive stent group (n=6) and control group (n=6). Immediately after balloon dilatation injury to the bile duct, the 103 Pd radioactive stent(experiment group) or the ordinary stent(control group) was positioned into the target segment. The dogs were killed one month later. The injured bile duct segments were dissected free from the dogs, and were examined radionucleonically, immunohistochemically and pathologically. Muscular proliferation area and lumen area were determined by computer assisted picture analysis system. Objective To predict and identify liner B-cell epitopes in the hemagglutinin ( HA) of human-infected avian-origin H7N9 influenza virus and analyze the specificity of H7 subtype.Methods Three serum samples collected at different times from the same patient who was confirmed to be infected with H7N9 influenza virus were provided by Shaoxing People’s Hospital, and one serum sample from healthy person was collected as the control.The extracellular region of HA protein was predicted by TMHMM Sever v.2.0.The potential B-cell epitopes were predicted by DNAStar Lasergene’ s Protean, BcePred and ABCpred tools, and the immunogenicity of the predicted B cell antigen epitopes was assessed by indirect enzyme-linked immunosordent assay ( ELISA ) .H7 subtype specificity was analyzed by comparing HA protein amino acid sequence with H7N9 and H1-H16 subtype influenza virus from Genbank using Clustal X 2.1 software, and Cn3D 4.3.1 software was used to detect the distribution and 3D structure of predicted epitopes on the HA protein of H7N9.Results The potential B-cell epitopes may be located in 172-183, 363-380, 452-472 and 491-506 of extracellular N-terminus of HA protein.ELISA showed that four predicted eptiopes specifically reacted with positive serums from patient.Multi-sequence alignment demonstrated that peptide 172-183 and 363-380 had higher H7 subtype specificity compared with amino acid sequences of other subtypes.Moreover, the predicted linear B-cell epitopes all located on the surface of HA protein according to the 3D structure analysis.Conclusion Four potential B-cell epitopes were identified, in which peptide 172-183 and 363-380 have higher H7 subtype specificity, and may be used in the design of epitope-based vaccines and diagnostics tests.
9.The effect of intracholangeal radiation on the expression of BCL-2 in dogs and its relationship to apoptosis of smooth muscle cells

Objective To study the effect of BCL-2 ?-radiation on BCL-2 gene in dogs, and its relationship and signifcane on apoptosis of proliferated smooth muscle cells of bile duct wall. Methods The ~(103)Pd (radioactivity) stent(experiment group) or ordinary stent(control group) was positioned into the target segment of bile duct. The injured bile duct segments were dissected free from the dogs, and BCL-2 gene in the (control) and r-radiation-induced apoptotic smooth mucle cells of bile duct wall was analysed by using (immuno-histochemical) technique. The number of apoptotic cells was counted, and size of lumen of bile duct in both groups was measured by a computerized imaging system.Results BCL-2 gene expression was weaker in the ~(103)Pd radioactive stent group than in the ordinary stent group. The group of dogs with low expression of BCL-2 genes showed marked apoptosis of proliferated smooth mucle cells of bile duct and there was no overt stenosis of extrahepatic bile ducts. The group that showed high expression of BCL-2 gene did not show marked apoptosisi of proliferated smooth muscle cells of bile duct, and there was marked stenosis of extrahepatic bile duct.Conclusions The expression level of BCL-2 in experimental dogs is related to the develoment of (cellular) apoptosis and to radiation sensitivity of the cells. ~(103)Pd radioactive stent can reduce the expression of BCL-2 gene, promote apoptosis of proliferated smooth muscle cells of bile duct, and suppress stricture (formation) of extrahepatic bile duct.
10.Study on induction of apoptosis of biliary duct cancer cells by ?-ray of the ~(103)Pd biliary stent

Objective To investigate the mechanism and role of ?-ray of 103Pd in the treatment of biliary duct cancer.Methods A series of biliary duct cancer cells were treated with different ?-ray dose,and MTT [3-(4,5-dimethy thiazol-2-yl)-2,5-diphenyl terazolium-bromide] technique was used to determine the inhibition rate of ?-ray of 103Pd on the biliary duct cancer cells;and electron micro-technique,DNA agarose gel electrophoresis and flow cytometry to evaluate the morphological characteristics and apoptosis rate of the biliary duct cancer cells were also used.Results The ?-ray radiation of 103Pd resulted in significant inhibition of the biliary duct cancer cells.The features of biliary duct cancer cells apoptosis(e,g:apoptic bodies,DNA ladders band hypodiploid DNA peak) could be seen in the group with lower dosage(5.333mci),and cell necrosis was seen in higher dosage(more than 6.645 mci).Conclusions The ?-ray radiation could induce apoptosis of the biliary duct cancer cells,but with dose dependence,and apoptosis can be an important mechanism for radiation treatment of biliary duct cancer.
