3.Clinical Observation on Qufeng Qingre Weiling Decoction Combined with the Decreasing ;Amount of Loratadine in Treatment of Chronic Urticaria

Objective To observe the clinical efficacy of Qufeng Qingre Weiling Decoction combined with the decreasing amount of loratadine in treatment of chronic urticaria (CU). Methods Totally 108 CU patients were randomly divided into treatment group and control group, 54 cases in each group. The treatment group was given Qufeng Qingre Weiling Decoction combined with the decreasing amount of loratadine (10 mg, 1 time per day) according to symptom controlling situation, and control group with the decreasing amount of loratadine only. Both groups were treated for 4 treatment courses (2 weeks as a treatment course). Symptom score reduce index was used to calculate the effective rate after 8 weeks of treatment. And the taking antihistamine medicine score was recorded 4, 6, 8 weeks of treatment. The follow-up visits were made to observe long-term efficacy of the cured patients 1 month, 3 months and 6 months after discontinuing the medication. Results Totally 27 cases from the treatment group were cured, with a total short-term effective rate of 96.30%;13 cases from the control group were cured, with a total short-term effective rate of 65.38%, with statistical significance (P<0.05). The taking antihistamine medicine score in treatment group (0.553 ± 0.239) was lower than that in control group (0.721 ± 0.245), namely the symptom improvement in treatment group was better than that in control group by taking less antihistamine medicine. Long-term clinical efficacy showed that the treatment group was also distinctively better than the control group. Conclusion Qufeng Qingre Weiling Decoction combined with the decreasing amount of loratadine is more effective for CU, and with better short-term and long-term clinical efficacy than the control group.
4.Relationship between Helicobacter Pylori and Pathological Lesiens in 458 Bioptic Specimens from gastric Mucosa in children

Objectives To search for the relationship between HP and lesions of gastric mucosa of children.Methods Four hundred and fifty-eight bioptic specimens-from eastrie mueosa in children were strained with hematoxylin-eosin(HE)and Warthin-Starry for light microscopic study.Results HP was detected in 152 specimens. HP deteetable rate-in specimens with normal mucosa was 1. 1%, in compari-on with 42.3% in those with chronic superficial gastritis and 14. 3% in those with chronic atrophic gastrins .Specimens with chronic active gastritls, mostry manitesting local active lesions,had HP detectable rate of as high as 80.2%、69. 7% of the specimene with HP were found to have obvious lymph follicles reaction within lamina propria in their gastric mucosa.Conclusions There is a close relationship between HP infection and Pediatric chronic gastritis(especially chronic active gastritis). Histopathologically, HP-associated gastritis is characterized by chronic and local active inflamations, and tbe formation of lymph follicles within lamina propria.
5.An experimental study on cytotoxicity of monocytes/macrophages against Tca8113 cells in hypoxic microenvironment

Objective; To investigate the cytotoxicity of monocytes/macrophages against Tca8113 cells in hypoxic microenvironment in vitro. Methods; Peripheral blood mononuclear cells were extracted from healthy person^ blood and cultivated to transform into monocytes/macrophages. The monocytes/macrophages were cultured with Tca8113 cells in hypoxic microenvironment (O_2 < 1%) and in normal microenvironment (20% O_2). The cytotoxicity of monocytes/macrophages against Tca8113 cell was examined by MTT assay. Results;The cytotoxicity of monocyles/macrophages against Tca8113 cells in hypoxic microenvironment was lower than that in normal environment(P<0.05). Conclusion; In hypoxic microenvironment, the cytotoxicity of monocytes/macrophages against Tca8113 cells is decreased. Bui the mechanism of monocytes/macrophages' function in tumors need further research.

8.Thyroid-stimulating hormone decreases insulin receptor substrate 1 expression via stimulating TNF-αsecretion in 3T3-L1 adipocytes

3T3-L1 adipocytes transfected with TSH receptor (TSHR) shRNA were incubated with bovine TSH.The concentration of tumor necrosis factor (TNF)-α in culture medium was measured by enzyme linked immunosorbent asssy.Protein level of insulin receptor substrate 1 (IRS-1) was quantified by Western blotting.Tyrosine phosphorylation of IRS-1 was measured by immunoprecipitation.The results showed that 1 mIU/ml TSH significantly sitmulated TNF-α release in 3T3-L1 adipocytes [(341.85 ± 12.00 vs 522.67 ± 36.22) ng/L,P<0.01],along with the decreases in IRS-1 protein expression and its tyrosine phosphorylation (P< 0.01).These effects disappeared when TSHR expression was down-regulated with RNA interference in 3T3-L1 adipocytes.In addition,WP9QY,a TNF-α antagonist,blocked TSH-decreased IRS-1 expresssion.These results suggest that TSH downregulates IRS-1 protein expression and its tyrosine phosphorylation through stimulating production of TNF-α,and thus contributes to the development of insulin resistance.
9.Assessment of left ventricular global and regional systolic function after transcatheter aortic valve implantation by three-dimensional speckle tracking imaging

Objective To investigate the alternation of left ventricular systolic function in patients underwent transcatheter aortic valve implantation operation(TAVI) by three-dimensional speckle tracking technology (3D-STI).Methods Totally 20 patients with severe aortic stenosis were enrolled.All the subjects underwent successful TAVI operation.The real-time 3D full volume datasets on apical four-chamber view were acquired on before,7 days and 1 month after TAVI.Left ventricular global longitudinal strain(GLS),regional peak systolic longitudinal strain(LS),regional peak systolic circumferential strain(CS) and regional peak systolic radial strain(RS),were analysed using off-line TomTec software,the differences among the three groups were compared.Results Compared with the preoperation,aortic valve blood flow velocity (AV),mean aortic valve pressure gradient(mPAG) of 7 days after operation decreased significantly.Threedimensional left ventricular ejecation fraction(3D-LVEF) among the patients whose 3D-LVEF under 50%had a remarkable increase and whose 3D-LVEF exceed 50% before operation had no significant change,while 1 month after operation the 3D-LVEF had a significant improvement compared with the preoperational data regardless of 3D-LVEF under 50% or not.The GLS and LS of all segments of 7 days after TAVI were higher than pre-operation(all P <0.05),and it had a further improvement 1 month after TAVI.Conclusions LV systolic function had improvement early after TAVI.3D-STI is a new,convenient way to detect the global and regional left ventricular systolic function of TAVI patients.
10.Lymphomatoid papulosis,type C:1 case report and literature review

Background and purpose:In the primary cutaneous CD30 positive lymphoproliferative disorders, despite the common reports about the primary cutaneous anaplastic large cell lymphoma and lymphomatoid papulosis, such reports about the bordline disease-lymphomatoid papulosis (type C) were rare. This article was to introduce and discuss its clinical manifestations and morphological and immunophenotypic features. Methods:Based on the related literature, we reviewed the clinical,pathological and immunohistochemical features from one case presented in Huashan Hospital. Results:The patient, male, was 56 years old with systemic red papuloid skin lesions accompanied with pruritus, which were sporadic, chronic, recurrent and self-healing. The duration of the disease persisted more than 26 years. Recently, the multiple papuloid lesions developed to be agrias, ulcerations and black eschars. Histopathologically, besides the ulcerated epiderm exfoliation, there were medium and large atypical lymphocytes in? trating in dermis layer. These cells showed postive for CD30,TIA-1 and CD3, and negative for ALK and CD20. Conclusions:When diagnosing LyP (type C),a rare disease, we should differentiate it from the primary cutaneous anaplastic large cell lymphoma (C-ALCL) and systemic anaplastic large cell lymphoma involving the skin, and also combine the clinical and histopathological characteristics and immunophenotype to identify its defi nition and nature.
