2.Chemical constituents of Rauvolfia verticillata.

The study on the Rauvolfia verticillata (Lour.) Baill., which belongs to Apocynaceae, was carried out to look for its chemical constituents and pharmacological activity. The isolation and purification were performed by chromatography on silica gel, Sephadex LH-20 and ODS (octadecyl silane) open column. The structures of obtained compounds were elucidated on the basis of physicochemical properties and spectral analysis. Three indole alkaloids and one acridone alkaloid were isolated from chloroform layer extract and identified as ajmalicine B (1), sandwicine (2), raunescine (3) and 7-hydroxynoracronycine (4) separately. Ajmalicine B (1) is a new compound belonging to indole alkaloid. Compound 4 as an acridone alkaloid was a new type compound isolated from Rauvolfia genus for the first time. We also did some biological activity research on the new type compound (4) to explore other pharmacological activities in addition to antihypertensive activity.
3.Utilization of IQM data of GEM3000 blood gas analyzer for evaluating instrument performance

Objective To apply the IQM quality data of the GEM 3000 blood gas analyzer to evaluate the instrument performance and to compared it with the traditional evaluation method .Methods The precision and accuracy were calculated by using the IQM data ,the external quality control substance was adopted to verify the main performance indexes of the blood gas analyzer .Results The precision of PH ,PCO2 and PO2 calculated by using the IQM data was 0 .02% ,2 .16% and 0 .63% ,which by the traditional e-valuation method was 0 .07% ,1 .68% and 1 .28% respectively ,the accuracy of PH ,PCO2 and PO2 calculated by using the IQM data was 0 .00% ,1 .15% and 0 .49% ,which by the traditional evaluation method was 0 .11% ,2 .91% and 1 .07% respectively .Conclu-sion The intelligent quality management can provide objective and reliable indicators for the performance evaluation of the blood gas analyzer ,and compared with the traditional performance evaluation scheme which is more simple and convenient to operate .
4.Studies on Pharmacokinetics and Bioequivalence of Valaciclovir Hydrochloride Tablets

OBJECTIVE: To study pharmacokinetics of valaciclovir hydrochloride tablets and to evaluate the bioequivalence of test and reference tablets. METHODS: In a two-period cross-over design test, 18 healthy volunteers were given a single oral dose of 600 mg test valaciclovir hydrochloride and 600 mg reference formulations respectively. Plasma concentrations of acyclovir were determined by HPLC method. The pharmacokinetic parameters of the two formulations were calculated and analyzed statistically. RESULTS: The main pharmacokinetic parameters of test vs. reference valacyclovir hydrochloride tablets were as follows:tmax(1.69?0.25)h vs. (1.72?0.26)h, Cmax(3.34?0.58) ?g?mL-1 vs. (3.40?0.49)?g?mL-1, t1/2(2.73?0.31)h vs. (2.97?0.33)h, AUC0~14(11.22?2.21) ?g?h?mL-1 vs. (11.12?1.90)?g?h?mL-1, AUC0~∞(11.76?2.15) ?g?h?mL-1 vs. (11.61?1.86)?g?h?mL-1, respectively. As compared with reference formulation, the relative bioavailability of test formulation was (101.06?11.72)%. CONCLUSION: The two preparations are bioequivalent.
5.Clinicopathological study of deep angiomyxoma

Objective To study the clinicopathologic features and differential diagnosis of deep angiomyxoma(DAM). Methods Seven cases of DAM were collected from 2000 to 2008. All the patients were examined by microscopy and immunohistochemistry. Results 6 patients with DAM were females and 1 was male, with median age of 48.5 years. Median maximum dimension was 5.9cm, with invasive growth pattern. The tumor cells of DAM were infantile, spindle or stellate,diffuse and nodular arrangement. A distinctive histologic feature of DAM was its vascularity. Non-arborizing, thin-wall, ectastic capillaries or more commonly, small thick-wall vessels were dispersed throughout the tumor. Mast cells and extravasated red blood cells were frequently found in the stroma, immunohistochemical study showed that 7 cases were positive for vimentin, desmin, ER and PR, 5 for CD34 and SMA, and negative for S-100 and CK. Conclusion DAM is a rare soft tumor that occurs principally in the vulval and vagina region of woman. Misdiagnosis has happened frequently. Immunohistochemical staining are helpful to diagnosis for DAM, but no significance to distinguish it.
6.Analysis and study on the factors of degenerative lumbar spondylolisthesis by multi-slice spiral CT

Objective To explore the influencing factors of degenerative lumbar spondylolisthesis (DLS).Methods Sixty-one DLS patients (DLS group) were enrolled in this study.The control group was formed by 61 patients with same gender and age matched.The lesion position was L4 in two groups.The correlation of intervertebral joint angle,degeneration degree,lumbosacral angle and lumbar spondylolisthesis were analyzed.Results The intervertebral joint and sagittal angle in DLS group was less than that in control group[(37.11 ± 7.52)° vs.(42.44 ± 7.61)°] (P < 0.01).The lumbosacral angle between two groups had no significant difference (P > 0.05).The degeneration degree between two groups had significant difference (P < 0.01).Conclusion Intervertebral joint and sagittal angle and intervertebral joint degeneration degree have important roles in DLS.
7.Analysis of prognostic factors and index model in patients with brain metastases after whole-brain radiotherapy

Background and purpose: Radiation therapy is still the most primary treatment of brain metastases, and prognosis is affected by many factors. The aim of this study was to identify the prognostic factors and to establish a prognostic index model in patients with brain metastases after whole-brain radiotherapy (WBRT). Methods: We reviewed the clinical date of 140 patients with brain metastases radiotherapy in our hospital from Jan. 2008 to Jul. 2011. The signiifcance of prognostic variables in the survival was resulted from both univariate analysis and multivariate analysis. The prognostic index (PI) was established based on Cox regression analysis and subgrouping values. It was assessed whether recursive partitioning analysis classes (RPA), basic score for brain metastases (BS-BM) and the graded prognostic assessment index (GPA) were related to prognosis. Results:The median survival time was 222 days. The univariate analysis showed that the independent prognostic factors were KPS performance status, number of brain metastases, presence of extracranial metastases, primary tumor status, radiation dose, hemoglobin. The multivariate analysis showed that KPS performance status (P=0.002, Wald=9.700), presence of extracranial metastases (P=0.018, Wald=5.604) and primary tumor status (P=0.001, Wald=10.212) were signiifcantly correlated with overall survival. RPA, BS-BM and GPA were closely related to their prognosis by Log-rank test. In predicting 3 months and 6 months of survival for patients, PI was better than other modes. Conclusion:Our data suggest that the 3 indexes RPA,BS-BM and GPA are valid prognostic index models, but PI model is better.
9.Identification and differentiation of breast cancer stem cells under tumor microenvironment

BACKGROUND:Breast cancer stem cel s have a greater impact on the occurrence and metastasis of breast cancer. Under simulated tumor microenvironment, we can better analyze the proliferation and differentiation of breast cancer stem cel s. OBJECTIVE:To explore the tumor microenvironment effect on the differentiation of breast cancer stem cel s. METHODS:Breast cancer cel s and MCF-7 cel s were primarily cultured in fibroblast supernatant and serum-free PCM-2 medium, and formation of breast cancer cel s microspheres was observed. Proliferative ability of breast cancer cel s was detected using MTT colorimetry, and the surface markers of breast cancer stem cel s and epithelial-mesenchymal transition markers were measured using immunocytochemistry and RT-PCR methods. RESULTS AND CONCLUSION:The diameter of primary cel microspheres was larger in the serum-free PCM-2 medium than in the fibroblast supernatant, but the culture speed was faster in the fibroblast supernatant than the serum-free PCM-2 medium. At 3 days of primary culture, the expression of ALDH1 in primary cel s was greatly higher in the serum-free PCM-2 medium than in the fibroblast supernatant. However, the expressions of E-cadherin and vimentin were up-regulated in the fibroblast supernatant than in the serum-free PCM-2 medium. In addition, the expressions of E-cadherin and vimentin in MCF-7 cel s cultured in the fibroblast supernatant were up-regulated, while the expressions of ALDH1 and Oct-4 were downregulated. These findings indicate that the tumor environment has some certain effects on the growth and differentiation of breast cancer stem cel s, and some cytokines secreted from fibroblast supernatant can promote the proliferation and differentiation of breast cancer stem cel microspheres to some extent.
10.Outcomes and clinical prognostic factors for elderly patients with esophageal squamous cell carcinoma treated after definitive chemoradiotherapy

Background and purpose: With the increase of aging population, elderly patients (age ≥70 years) with esophageal squamous cell carcinoma occurred more and more. However, few studies have focused on elderly esophageal squamous cell carcinoma patients. This study aimed to assess the outcomes and prognostic factors for elderly patients with esophageal squamous cell carcinoma treated after deifnitive chemoradiotherapy. Methods:We retrospectively analyzed 53 patients (age≥70 years) with esophageal squamous cell carcinoma and treated with deifnitive radiotherapy with or without chemotherapy from Fudan University Shanghai Cancer Center from Mar. 2009 to Dec. 2011. Results:Median age was 74 years. Twenty-nine patients underwent radiotherapy, 24 patients underwent radiochemotherapy. 1-, 2-, 3-, and 5- year survival was 62%, 44%, 33% and 19% respectively. Grade 2 and above acute radiation-induced esophageal toxicity and radiation pneumonitis occur rate was 6% and 9% respectively. No treatment-related deaths occurred and no patients experienced any grade 4 and above toxicities. Multivariate analysis identiifed treatment modality, tumor site and smoking history as independent prognostic factors for overall survival. Conclusion:Radiotherapy may be an acceptable treatment option for elderly patients with esophageal squamous cell carcinoma. In appropriately selected patients, concurrent chemotherapy could bring a better overall survival.
