1.Roles of TRPV1 receptor in electroacupuncture regulating the jejunal motility of mice: an experimental study.

OBJECTIVETo observe the intestinal movement of transient receptor potential vanilloid 1 or vanilloid receptor 1 (TRPV1) knockout mice after stimulated by electroacupuncture (EA), and to primarily explore the roles of TRPV1 receptor in the jejunal motility regulated by acupuncture.
METHODSNormal wild-type CL57BL/6 and TRPV1 gene knockout mice were recruited in two groups, the B6 group and the TRPV1 group, 15 in each group. The thermal threshold and the mechanical pain threshold were respectively detected using JL-F digital photo thermal analyzer instrument and ALMEMO2450 machine. The difference between the two thresholds were compared. Meanwhile, a self-made pressure head was placed in the jejunum. The internal pressure was monitored. When the pressure was stable, 2 mA 2/15 Hz EA at Quchi (LI11), Tianshu (ST25), Shangjuxu (ST37), and Dachangshu (BL25) to observe the changes of intestinal pressure value between before and after EA. The curve of internal pressures was recorded.
RESULTS(1) The stimulation of light/heat and mechanical stimulation were obviously slowed in the TRPV1 group than in the B6 group (P < 0.01). (2) In the intestinal pressure observation experiment, the bowel movement was not obviously seen in the two groups when acupunctured at Quchi (LI11) and Shangjuxu (ST37) (P > 0.05). (3) Acupuncture at Tianshu (ST25) and Dachangshu (BL25) of TRPV1 knockout mice could lead to intestinal movement (P < 0.05), mainly inhibited bowel movement. The changing degree was equivalent to that of B6 mice.
CONCLUSIONSUnder the physiological condition, TRPV1 might be mediated by thermal and mechanical stimulation. But TRPV1 mediated acupuncture effect was quite complex, indicating TRPV1 mice might be one of intestinal movement mediating factors.

2.Analysis on Learning Curve of Laparoscopic Pancreaticojejunostomy in Pigs

Objective To evaluate the surgical effects of laparoscopic pancreaticojejunostomy in pigs and investigate the learning curve. Methods A total of 40 porcine cases of laparoscopic pancreaticojejunostomy were performed by a single surgical team from September 2012 to February 2013.According to the operation sequence, pigs were divided into 4 groups with 10 in each group.For analyzing the efficacy of surgery in different stages, the operation time and efficacy of anastomoses were compared between these groups. Results The operation was finished successfully in all the 40 cases.There were statistical significances in operation time among the four groups.The group Ⅳ had significant shorter time (112.7 ±8.3) min than those in the group Ⅰ [(145.1 ± 16.6) min, q=9.154, P<0.05], the groupⅡ[(133.0 ±8.7) min, q=5.735, P<0.05], and the group Ⅲ[(137.2 ±9.0) min, q=6.922, P<0.05], respectively.There was no difference between the group Ⅱ and the group Ⅰ in operation time (q=3.419, P>0.05), so as to other groups.The results of anastomoses among the four groups were not significantly different (χ2 =0.586,P=0.900). Conclusions Laparoscopic pancreaticojejunostomy in pigs is feasible.The learning curve of the operation is about 30 cases.
3.Effect of methazolamide in patients with refractory uveitic macular edema

AIM:To evaluate the efficacy and safety of methazolamide in treating refractory uveitic macular edema.METHODS: Retrospective self-controlled study was designed.A total of 15 patients (20 eyes) with refractory uveitic macular edema which used methazolamide as adjuvant therapy were enrolled in Shanghai First People`s Hospital from January 2015 to June 2016.The changes of central macular thickness (CMT) and best corrected visual acuity (BCVA) were observed at baseline and 2, 4, 8wk after treatment.We also focused on the incidence of complications and relapse.RESULTS: The CMT was 445.95±154.10μm, 338.83±138.34μm, 251.50±40.20μm, 244.90±35.68μm at baseline, 2, 4 and 8wk after treatment, respectively.The differences among them were statistically significant (F=15.467, P<0.05).The BCVA (log MAR) were 0.40±0.17, 0.28±0.21, 0.19±0.20, 0.18±0.21 at baseline, 2, 4 and 8wk respectively, with a significant difference among them (F=5.208, P<0.05).When the cumulative dose reached to 700mg and 1400mg, no one had methazolamide-related complications;and when it came to 2800mg, 5 patients (33%) had methazolamide-related complication.After the withdrawal of methazolamide 1wk, 1 and 3mo, 3 patients (20%), 5 patients (33%) and 8 patients (53%) relapsed, respectively.CONCLUSION: Methazolamide is beneficial in improving macular edema and vision in 4wk.When the cumulative dose is more than 1400mg, we need pay attention to the complications.After discontinuing methazolamide for 1wk, macular edema relapsed in some patients, and more than half of patients recurred after 3mo.So the patients should be followed closely in 3mo after withdrawal of methazolamide.
5.Clinical study on human lamellar keratoplasty for fungal corneal ulcers with porcine acellular corneal stroma

AIM:To observe the transplantation of acellular porcine corneal stroma on the treatment of superficial keratitis by drug-resistant fungal.METHODS:We performed a retrospective analysis of 16 cases of fungal keratitis received the transplantation of acellular porcine corneal matrix from June 2015 to March 2016 with a follow-up of 6mo.We analyzed on items as postoperative visual acuity, corneal graft status, postoperative recurrence and postoperative complications.RESULTS:We observed a healing time of corneal epithelium in 7 to 10d postoperatively generally and the absence of corneal edema in 1mo, while the cornea gradually returned transparent in the 16 cases.Two cases required medication for an epithelial recovery and 3 cases received intervention for decreasing intraocular pressure to a certain level.During the follow-up we observed no cases of cornea degeneration, recurrence of infection or rejection.The vision acuity showed 1.27±0.22, 1.11±0.13, 0.79±0.22 in 1, 3 and 6mo after operation respectively.There was no statistical difference between vision in 1mo and the vision before surgery (P=0.06);while we found a statistical difference when comparing the vision of 3 and 6mo with vision before surgery (P=0.01,0.001).The vision in 6mo increased with a statistic difference to the vision at 1 and 3mo (P<0.001) while no statistic difference was observed between 1 and 3mo(P=0.11).CONCLUSION:Transplantation of acellular porcine corneal matrix is a safe and efficient treatment for fungal keratitis.
6.Meta-analysis of phacoemulsification versus phacotrabeculectomy for primary angle closure glaucoma with cataract

Background Increase of lens thickness at incipient cataract is a key factor of onset of primary angle-closure glaucoma (PACG).Phacoemulsification (Phaco) or phacotrabeculectomy (Phacotrabe) have been documented to be effective for the patients of PACG associated with cataract.However,which surgery is more effective and safe is lack of evidence.Objective This study was to assess and compare the clinical effectiveness of Phaco versus Phacotrabe for PACG with cataract.Methods The relevant literature was searched electronically from the PubMed (1966 to June 2011),EMB Reviews (1966 to June 2011) and Cochrane Library (Issue 1,2011).The manually searching of relevant conference proceedings was used as the supplement.The articles of randomized controlled trial (RCT) about the clinical effectiveness of Phaco versus Phacotrabe for PACG with cataract were included.The methodology quality of included literature was graded.The analysis indexes included intraocular pressure (IOP)-lowing range,postoperative administration of glaucoma drugs,incidence of positive complication,postoperative best corrective visual acuity (BCVA) and perimetry damage.The RevMan4.2 software from Cochrane Collaboration was used for the Meta analyses.Results Three RCTs about phaco versus Phacotrabe for PACG with cataract were selected in this study with the 164 eyes of 164 cases.Meta analysis showed that the IOP-lowing range was larger in the Phacotrabe group compared to only Phaco group with the WMD of 1.17 and 95% CI of 0.06-2.27 (P =0.040),and the drug dosage of anti-glaucoma was less in the Phacotrabe group in comparison with the Phaco group with the WMD of 0.5 and 95% CI of 0.24-0.77 (P =0.000).However,the incidence of postoperative complication was higher in the Phacotrabe group than that of the Phaco group with the RR of 0.08 and 95% CI of 0.02-0.33 (P =0.000).No significant difference was found in the BCVA (WMD =0,95% CI:-0.13-0.13,P=1.00) andperimetry (WMD =1.01,95%CI:0.56-1.82,P=0.98).Conclusions Compared with Phaco,Phacotrab has a better IOP-lowing effectiveness and slightly worse safety.Phaco and Phacotrab have a fairly influencc in the postoperative BCVA and perimetry.As the sample sizes of the included trials are relatively small,more welldesigned large-scale RCTs are needed.
7.Construction of recombinant eukaryotic expression plasmid of human thyrotropin receptor extracellular domain encapsulated with cationic liposomes

AlM:To construct recombination eukaryotic expression plasmid of human thyrotropin receptor extracellular domain encapsulated with cationic liposomes.
METHODS:We amplified the target gene of shuttle vector PHMCMVTSHR289, conjugated the target gene and eukaryotic expression plasmid pcDNA3. 1 +, and accredited whether pcDNA3. 1+/TSHR289 was connected or not by enzymatic digestion and sequencing. Cationic liposomes encapsulated the recombination plasmid pcDNA3. 1+/TSHR289.
RESULTS: Recombination plasmid pcDNA3. 1+/TSHR289 digested with enzyme Hindlll and the fragment through 0. 8% gel electrophoresis showed 512bp strip. Recombination plasmid pcDNA3. 1+/TSHR289 were found synonymous mutation through forward ( AAC to AAT ) and reverse sequencing ( GCG to GCT) . The volume ratio of cationic liposomes and recombinant plasmid was 3:1.
CONCLUSlON: lt is successful to construct the recombination plasmid pcDNA3. 1+/TSHR289 by accredit it through enzymatic digestion and sequencing.
8.Role of TRPV1 in the Regulation of Gastric Motility by Electroacupuncture

ObjectiveTo discuss the role of transient receptor potential vanilloid 1 (TRPV 1) in the regulation effect of electroacupuncture on gastric motility.MethodTRPV1 gene knock-out mice (KO mice) and wild-type C57BL/6 mice (WT mice) were selected to receive acupuncture at Zusanli (ST36),Quchi (LI11), Zhongwan (CV12), and Weishu (BL21), and the intragastric pressure was observed before and after acupuncture.ResultElectroacupuncture at Zusanli caused both excitation and inhibition in WT mice, predominated by mild excitation, while electroacupuncture at Weishu, Quchi and Zhongwan all caused inhibition effect; in the KO mice, electroacupuncture at Zusanli, Quchi, Zhongwan, and Weishu all inhibited gastric motility.Conclusion TRPV1 bears certain regulating effect on gastric motility, andacupuncture can inhibit the gastric motility in TRPV gene KO mice.
9.Clinical Application of 64-slice Spiral CT Colonography

Objective To evaluate the techniques and application of 64-slice helical CT colonography in colonic lesions. Methods Thirty-eight patients (including 12 colonic carcinomas,10 polypi, 9 colon multiple scrobiculus, 2 multiple diverticula, 2 negative, 1 congenital bowel malrotaion,1 ulcerative colitis and tunica mucosa glandular organ hyperplasy,1 colocolic anastomosis of terminal ileum and sigmoid colon) underwent volume scanning using 64-slice helical CT after cleaning colon. Six types of reconstruction including CT virtual colonoscopy(CTVC), volume rendering(VR), multiple planar reconstruction(MPR), 360?sectional view, RaySum,and navigation were gained.CTVC appearances were compared with that of conventional colonoscopy(CC).Results In 12 colonic carcinomas and 10 polypi,the lesions’ morphology,number,size, were satisfactorily shown by CTVC. The lesions’ location,range,and 1 intestinal canal obviously stenosis were also exactly shown by RaySum.The relationship between tumor and environment, 3 liver or retroperitoneal lymph node metastasis were satisfactorily shown by MPR and 2D transection image. On 360?sectional view,the distance between tumor and anus was exactly measured and the result was in conformity to that of CC. Of the CTVC appearances,1 sigmoid carcinoma and 1 transverse colon polyp was in unconformity to that of CC;1 ulcerative colitis and descending colon carcinoma was diagnosed as ulcerative colitis and tunica mucosa glandular organ hyperplasy by pathology.Conclusion 64-slice helical CT is a valuable imaging technique for detecting colon diseases. CTVC can obtain more clinical information than CC combining MPR,RaySum and VR.
