1.Clinical Study on 48 Cases of Diabetic Gastroparesis Treated with Weichangshu

Objective:To observe efficacy of Weichangshu in treatment of diabetic gastroparesis(DGP).Methods:96 cases of DGP conformed with the criteria were randomly divided into two groups.They were treated by oral administration of Weichangshu and Mosapride respectively for 4 weeks in addition to original treatment program for controlling blood sugar.Changes of clinical symptoms,FBG and P2hBG,gastric antrum-duodenum interdigestive period migrating motion compound wave(MMC)and electrogastrogram(EGG)before and after treatment were investigated.Results:Clinical symptoms were significantly improved in both groups.The markedly effective rate was 62.50% and the total effective rate was 91.67% in the Weichangshu group,and the corresponding index was 60.42% and 89.58% in the Mosapride group.After treatment,MMC significantly improved in both groups,with significant differences in lasting time of phase Ⅱ,and phase Ⅲ,and the contraction amplitude as compared with those before treatment(P
3.The Relationship between the Expression of RARα,RARβ,ER, PR and the Endometrial Carcinoma

Objective:To evaluate the role of the retinoic acid raceptor α (RARα) , retinoic acid receptor β(RARp) , estrogen receptor(ER) and progesterone receptor(PR) in the clinical pathological stage, histologcal classification and the muscular invasive depth of the endometrial carcinoma through detecting the expression of these receptors.Methods:48 paraffin sections with endometrial carcinoma by pathological conformation ware selected.Immunohistochemistry was used to detect the expression of RARα, RARβ, ER and PR.The relationship between these raceptors and the clinical pathological parameters was evaluated.Results:①The positive expression ratio of the RARα and RARβ were 47.92% (23/48) and 25.00% (12/48) respectively.The level of RARα was higher than that of RARβ (P<0.05).②The expression of RARα in the endometrial carcinoma increased with the increasing of the pathological stage and the decreasing of the histological differentiation (P<0.05).But the expression of these two receptors had no significant relation with the histological classification and the muscular invasive depth (P >0.05).③The level of the RARβ was consistent with that of the ER and PR, and it decreased with the increasing of the malignant extent of the endometrial adenocarcinoma (P < 0.05) ,and had no significant relation with the pathological stage, the histological classification and the muscular invasive depth (P >0.05).④The expression of RARβ was positively correla ted to the expression of ER and PR, but the expression of RARα was negatively correlated to the expression ER and PR.Conclusions :There is expression of RARα、RARβ in endometrial carcinoma, and it has close relationship between the expression of RARα and the development of endometrial carcinoma.The absence of the RARβ expression and the high expression of the RARα are related to the histological classification of endometrial carcinoma.

7.Clinical observation on jiangni hewei decoction in treatment of 45 patients with reflux esophagitis.

OBJECTIVETo observe the therapeutic efficacy of Jiangni Hewei Decoction (JHD) in treating reflux esophagitis (RE).
METHODSSeventy-five RE patients conformed with the inclusive criteria were randomly divided into the treated group (45 patients) and the control group (30 patients). Patients in the treated group received JHD orally, one dose per day, while those in the control group received Omeprazole, 20 mg once per day. The therapeutic course was 8 weeks. The efficacy on clinical symptoms, RE grades and scores determined by gastroscope were evaluated.
RESULTSPatients' symptoms were obviously improved after treatment of JHD and Omeprazole. The cured rate and the total effective rate in the treatedgroup were 37.8% (17 cases) and 84.5%, respectively, while in the control group were 36.7% (11 cases) and 83.3% respectively. The symptomatic score and the total score in both groups after treatment were obviously lowered (P < 0.01). Gastroscopic examination showed that the patients in the two groups with RE of 0 grade were significantly increased and no patients with RE of III grade. The healing rate and the total effective rate were 48.9% (22 cases) and 91.1% respectively in the treated group, and the corresponding numbers in the control group were 46.7% (14 cases) and 93.3% respectively. Significant difference was found in comparison of the gastroscopic figures in the same group before and after treatment (P < 0.01). The therapeutic effect of JHD on RE was similar to that of Omeprazole in aspects of improving clinical symptoms such as heartburn, acid regurgitation, chest pain, etc., and in curing oesophageal mucous inflammation (under gastroscope) and esophagitis. But the recurrence in the JHD group was obviously lower than that in the control group in 12 weeks after withdrawal of treatment (P < 0.05). Besides, no adverse reaction happened in the JHD group.
CONCLUSIONJHD showed favorable therapeutic efficacy in treating RE.

8.Clinical observation on effect of weichangshu in treating diabetic gastroparesis.

OBJECTIVETo observe the effect of Weichangshu (WCS) in treating diabetic gastroparesis (DGP).
METHODSNinety-six patients with DGP were randomly divided into two groups. Besides the same conventional blood glucose controlling regimen was given to both groups, WSC was given to the treated group and Mosapride Citrate tablet to the control group additionally. The treatment course for them was 4 weeks. Therapeutic effect on clinical syndromes was assessed, the fasting and 2-hr postprandial blood glucose, antro-duodenal interdigestive migrating motor complex (MMC) and electrogastrogram (EGG) were measured before and after treatment.
RESULTSThe therapeutic effect in the two groups were similar with no statistical significant difference. The time of MMC phase II was shortened, time of MMC phase III prolonged, and the constriction amplitude of which increased after treatment in both groups, showing significant difference as compared to those before treatment (P < 0.05). EGG were significantly changed after treatment mainly manifested as increase of proportional dominant frequency (PDF) and strengthening of proportional dominant amplitude (PDP ) (P <0.05 or P < 0.01). Besides, WSC also promoted the decrease of post-prandial blood glucose.
CONCLUSIONWSC has dual effect in promoting gastric motility and decreasing blood glucose, with less adoerst reaction.

9.Varieties textual research and usage investigation of white flos gentianae, a Tibetan medicine.

OBJECTIVETo clarity the original plants and the main application varieties of White Flos Gentianae.
METHODHerbal textual research, wild specimen collection, investigation and collection of the samples from Tibetan hospital, Tibetan pharmaceutical factory and medical material market were carried out simultaneously to identify the original plants of White Flos Gentianae.
RESULTThe results of varieties textual research and specimen identification showed that Gentiana szechenyii, G. purdomii and G. algida were in accord with the record of Tibetan herbal textual The three species above were the original plants of White Flos Gentianae. The identification of 20 batches samples showed that G. szechenyii was the main application variety. The other varieties were only used in Tibetan hospitals. All the samples above were flowering branches.
CONCLUSIONIt was necessary to strengthen the research on variety systematization of White Flos Gentianae make a further discussion on the taxonomy position of G. purdomii, G. algida and the white flos population. Its was also nessary to establish and improve the quality standard of different variety based on the principle of "one species, one name". The quality specification of White Flos Gentianae should be established and improved to standard clinical utilization and produce feeding. More study of resources investigation and cultivation of G. szechenyii should be carried on to meet the demand of produce and clinic.

10.Analysis of corticosteroids combined with sequential therapy of Chinese Guishao Dihuang pill and Yupingfeng granules on 50 cases with yin deficiency of liver and kidney and nephrotic syndrome in children

Objective To explore the clinical effect of corticosteroids combined with Chinese medicine sequential therapy of Guishao Dihuang pill and Yupingfeng granules on children nephrotic syndrome (NS) with yin deficiency of liver and kidney.Methods Fifty patients (the loss of children during the course of treatment were not included in the statistical analysis) with definitely diagnosed NS accompanied by yin deficiency of the liver and kidney admitted to the First Affiliated Hospital of Gannan Medical University from January 2012 to June 2016 were enrolled,and they were divided into control group and observation group by random number table,25 cases in each group.Conventional treatment of western medicine and long-term treatment of corticosteroids were given to the two groups.In addition,sequential therapy of Guishao Dihuang pills and Yupingfeng granules was given to the observation group on the bases of conventional treatment.The changes of platelet aggregation rate (PAT),cholesterol,cortisol,T cell subsets CD3,CD4,CD4/CD8 were observed before and after treatment for 8 weeks and 6 months,then the clinical efficacy and infection,etc.complications were compared between the two groups.Results There were no statistical significant differences in cholesterol,cortisol and curative effect compared between the two groups before treatment and after treatment for 8 weeks and 6 months (all P > 0.05).Mter treatment for 8 weeks and 6 months,the PAT,cholesterol,cortisol of two groups were significantly decreased compared with those before treatment,while CD3,CD4 were obviously higher than those before treatment,and the degrees of changes of PAT,CD3,CD4 in the observation group were more remarkable than those of the control group [PAT:treatment after 8 weeks was (63.01 ± 6.99)% vs.(66.62 ± 4.65)%,treatment after 6 months was (51.42 ± 7.74)% vs.(57.01 ± 6.77)%,CD3:treatment after 8 weeks was 0.56 ± 0.06 vs.0.52± 0.06,treatment after 6 months was 0.61 ± 0.05 vs.0.58 ± 0.03,CD4:treatment after 8 weeks was 0.33 ± 0.06 vs.0.30 ± 0.05,treatment after 6 months was 0.39 ± 0.05 vs.0.33 ± 0.06,all P < 0.05].CD4/CD8 ratio was significantly higher in observation group after treatment for 8 weeks and 6 months than those before therapy (1.68 ± 0.76,1.82 ± 0.95 vs.1.16 ± 0.67,both P < 0.05);the number of patients with infection in observation group was obviously lower than that of the control group (86 cases vs.175 cases,P < 0.05).Conclusions The therapeutic effect of corticosteroids combined with Chinese medicine sequential therapy of Guishao Dihuang pills and Yupingfeng granules for treatment of children NS with yin deficiency of the liver and kidney is better than that of using western medicine therapy alone,as the combined sequential therapy has good effects on reduction of infection,anti-thrombosis and elevation of immunity.
