Objective To explore the dead reasons and related factors of acute leakemia(AL)in children.Methods The clinical data of 42 dead cases of children with AL,who were admitted to Guangzhou Children's Hospital between Jan.1999 and Mar.2007 were reviewed.Nineteen cases were boys,23 cases were girls;the aging from 4 months to 12 years old and 5 months.Of 42 children,21 cases were acute lymphoblastic leukemia(ALL),18 cases were acute myeloid leukemia(AML),3 cases were unclassified AL.Results Ten out of 12 early-dead cases died of bleeding.Among 30 cases who received chemotherapy,4 cases died of bleeding,others of myelosuppression combined with infection,14 of whom died from sepsis,3 cases with fungal infection,1 case with virus infection,4 cases with unknown pathogens.Sixteen children relapsed among those who got remission,half of them relapsed within 1 year since diagnosed.Treatment-related mortality(TRM)was 90%(27 out of 30 cases).The main causes of TRM were infection and hemorrhage.Hyperleukocytosis and bleeding were 2 risk factors related to early death in new-diagnosed children.Conclusions More emphasizes should be put on bleeding tendency during early treatment in children with AL and treatment of infection complications during induction chemotherapy to reduce TRM.

Objective To study wether dezocine (DZ) can attenuate neuropathic pain in rats and to investigate its mechanism.Methods Neuropathic pain was induced by tight ligation of the left L5 spinal nerve of the male SD rats. 24 Spinal nerve ligation (SNL) model rats were randomly divided into three groups:the first group (saline group, n=8), rats were given intraperitoneal injection of 1ml of saline 14 days after SNL surgery; the second group (dezocine group, n=8), rats were given intraperitoneal injection of 5mg/kg of dezocine in 1ml saline 14 days after SNL surgery;the third group (morphine group,n=8),rats were given intraperitoneal injection of 5 mg/kg of morphine 14 days after SNL surgery. Results Intraperitoneal injection dezocine and morphine had significant analgesic effect in neuropathic pain rats. This analgesic effect lasted for 4-6 hours.Continuous injection of morphine for 3 days produced significant tolerance, while continuous injection of dezocine for 7 days still had significant analgesic effect on neuropathic pain. Conclusion Intraperitoneal injection of dezocine and morphine can attenuate SNL neuropathic pain, continuous injection of morphine for 3 days can produce significant tolerance, while continuous injection of dezocine for 7 days still has significant analgesic effect on neuropathic pain.

Objective To investigate visuospatial-visuoconstructional ability in patients with mild cognitive impairment(MCI).Methods The subjects were divided into three groups:122(51 males and 71 females)in the normal control group; 205(95 males and 110 females)in the MCI group,including 133 amnestic MCI(aMCI)and 72 non-amnestic MCI(naMCI),and 75(36 males and 39 females)in the Alzheimer' s disease(AD)group.The subjects were assessed for visuospatial-visuoconstructional ability by three tests:Rey-Osterrieth Complement Fixation Test(CFT),Clock Drawing Task(CDT),and Stick Test.Mini-mental State Examination(MMSE)was also performed in all groups.SPSS 11.5 statistical software was used for statistic analysis; Chi-square test was used to analyze enumeration data; One-way analysis of variance was used in pairwise comparison between the groups,and Bonferroni and LSD methods were used for multiple comparison.Results(1)The average MMSE score in the normal control group,MCI group and AD group was 28.24 ± 1.74,27.39 ± 1.83 and 19.98 ±3.23,respectively.(2)In the normal control group,either CFT imitating score or total CDT score had no obvious correlation with age or education,while Stick Test-Rotating part(STR)was correlated with age(r =-0.179,P ＜ 0.05),but had no significant correlation with education.(3)There were correlations between CFT imitating score and total CDT score (r =0.337),CFT imitating score and STR(r =0.232),and total CDT score and STR(r =0.235).The correlations were statistically significant(P ＜ 0.01).(4)CFT imitating and total CDT showed the greatest correlation with Trail Making Test B and Stroop Color Word Test Card C time consumption,an indicator of executive function.STR had greatest correlation with Auditory Verbal Learning Test,an index of memory.There were significant differences in CFT imitating,total CDT score and STR among the normal,MCI and AD groups(P ＜ 0.01).(5)STR is a more informative test to be used in diagnosis of MCI than CFT imitating and total CDT score,as shown by comparison of these tests in three subjects groups.Conclusion Visuospatial-visuoconstructional ability impairment is one of the symptoms of MCI.In detection of MCI,the assessment of space rotating ability is better than CFT imitating and total CDT score.

Objective To present the development of the Naming test tool ( Huashan Naming Test, HNT)for Chinese cultural and by amnestic mild cognitive impairment ( aMCI ) and mild Alzheimer' s disease (AD) between the detection of memory decline,and to analyze the validity of its trial.Methods 100 normal elders from communities in Shanghai, 100 patients with amnestic mild cognitive impairment (aMCI), and 95 patients with mild Alzheimer's disease (AD) who received an education of junior high school or above and were evaluated by neuropsychological tests including mini mental state examination ( MMSE), auditory verbal memory test, Huashan Naming tests etc.8 cognitive tests.The groups of MCI and AD patients finished cranial MRI.100 items with HNT including 20 animals,10 vegetables,10 fruits ,20 tools ,20 household , 10 vehicles, 10 stationery.Results 1.HNT items to determine: 22 items were excluded due to the completion of the three groups were not significantly different; 8 items were excluded due to the completion of the normal elderly group was lower than 75%; 10 items were excluded from the analysis of variance Fvalue of the minimum value.The remaining 60 items,according to the size of the arrangement and completion rates were divided into two versions of odd and even, respectively HNT-Ⅰ and HNT-Ⅱ.2.HNT characteristics:in normal elderly group age, sex were found to had no significant factors affecting overall scores of HNT-Ⅰ and HNT-Ⅱ but level of education, MMSE score was significantly correlated (P ＜0.05).As cut-off score ≤ 26 for spontaneous naming of HNT,the sensitivities of HNT-Ⅰ for aMCI ,mild AD were 44%, 84% respectively, specificities were all around 84%; the sensitivities of HNT-Ⅱ for aMCI , mild AD were 56% ,83% respectively,specificities were all around 80%.Conclusion HNT is a Chinese cultural background,time-consuming short and good name validity test,and it is worth further promoting the application.

OBJECTIVE:To study the preventive and therapeutic effects of Xinmaitong tablets on model rats with atherosclero-sis (AS) and corresponding mechanism. METHODS:48 rats were randomized into normal group,model group,positive group (simvastatin tablets,2 mg/kg)and the groups of high,medium and low-dose(500,250,125 mg/kg)Xingmaitong tablets,with 8 rats in each group. The rats in all groups except for the normal group were fed high fat diets and given vitamin D3 ip for the estab-lishment of AS model. Meanwhile,the rats in the drug administration groups were given corresponding drugs ig,while those in the normal group and the model group were given normal saline ig,once a day,for 10 consecutive weeks. The levels of cholesterol (TC),triacylglycerol(TG),nitric oxide(NO)and endothelin 1(ET-1)in serum were determined. The pathological change of the aorta tissue was observed under the light microscope,and the size of the aortic atherosclerotic plaque and intima thickness were measured. RESULTS:Compared to the normal group,the rats in the model group had significantly higher levels of TC,TG and ET-1 in serum,a markedly larger aortic atherosclerotic plaque,remarkably thicker intima and much lower level of NO(P<0.01);and obvious aortic atherosclerotic plaque formation was found. Compared to the model group,the above-mentioned indexes of the rats in all drug groups all improved obviously (P<0.05 or P<0.01),except that the decrease in TC in serum of the rats in the group of low-dose Xinmaitong tablets was not significant,which were positively correlated with dose;and the aortic lesion in drug groups was found to be improved. CONCLUSIONS:Xinmaitong tablets have certain preventive and therapeutic effect on the model rats with AS by a mechanism which may be related to the reduction in lipid deposition and the size of an atherosclerotic plaque,the improvement in NO/ET balance and the alleviation of an endothelial injury.

Objective To explore the expression and significance of galectin-3 in COPD and lung cancer.Methods Sixty female SD rats were randomly divided into three groups:COPD group,lung cancer group and the control group with 20 rats in each group.COPD group was given tracheal injection of lipopolysaccharide and exposed to daily cigarette to establish the model.Lung cancer group was injected with 3,4-benzopyrene-corn mixture.The control group was fed normally.After 20 weeks,plasma galectin-3 concentration was assessed in each group using ELISA method.Results The galectin-3 average concentrations of COPD group,lung cancer group and the control group were (8.63 ± 0.46) ng/mL,(9.15 ± 0.69) ng/mL and (4.47 ± 0.51) ng/mL.The average concentrations in COPD and lung cancer groups were higher than that of the control group with a statistically significant difference (P＜0.05).Conclusion As expressed in COPD and lung cancer,galectin-3 may own a great significance to the occurrence,development and prognosis of COPD and lung cancer.

Objective:To explore and compare the clinical efficacy of lyophilized recombinant human brain natriuretic peptide (Lrh-BNP) and dobutamine (Dob) in the treatment of patients with acute heart failure (AHF) and impacts on the plasma galectin (Gal)-3,Cystatin C (CysC) and endothelin (ET-)-1 levels.Methods:114 cases of patients with AHF in our hospital from February 2015 to February 2017 were selected as the research objectives and randomly divided into two groups.Dob group was treated by Dob,while Lrh-BNP group was treated by Lrh-BNP.The cardiac function parameters,plasma Gal-3,CysC,ET-1 levels before and after treatment,clinical comprehensive efficacy and incidence of adverse reactions were compared between two groups.Results:The FS,LVEF levels of both groups at 72 hours after treatment were significantly higher than those before treatment (P＜0.01),but the LVEDD,plasma Gal-3,CysC,ET-1 levels were obviously decreased (P＜0.01),the index mentioned above of Lrh-BNP group improved more significantly than those of the Dob group(P＜0.01).The overall effective rate of Lrh-BNP group was 89.5 ％,which was significantly higher than that of the Dob group (73.7％,P＜0.05).No significant difference was found in the incidence of adverse reaction between two groups(P＞0.05).Conclusion:Lyophilized recombinant human brain natriuretic peptide was more effective in the treatment of AHF than Dobutamine with equal safety,which might be related to the decrease of plasma Gal-3,CysC,ET-1 levels.

OBJECTIVE:To develop an HPLC method for the determination of 5-ISMNconcerntrations in human plasma and study the pharmacokinetics of 5-ISMN orally disintegrating tablets and the reference tablets.METHODS:A single oral dose test capsule(orally disintegrating tablets)or reference capsule of 5-ISMN(60 mg)were administered by randomized crossover way in 20 healthy male volunteers with plasma 5-ISMN concentrations determined by HPLC.The pharmacokinetic parameters were calculated with 3p87 pharmacokinetic program and the bioavailability of the two preparations was evaluated.RESULTS:The main pharmacokinetic parameters of the test preparation vs.the reference preparation were as follows:Cmax:(613.42?73.83)vs.(368.64?38.66)?g?mL-1;tmax:(26.52?2.00)vs.(95.73?4.16)min;AUC0～∞:(73 872.24?543.89)vs.(77 978.47?646.37)ng?min?mL-1,respectively.CONCLUSION:The assay was proved to be sensitive,accurate and suitable for pharmacokinetic study of 5-ISMN.The results of pharmacokinetic study after oral administration of test and reference preparations of 5-ISMN showed that the two preparations were bioequivalent while the test preparation had a higher peak concentration and could reach peak level in less time,and the test preparation showed a rapid drug releasing behavior.
