1.Hepatocyte growth factor in combination with interleukin-6 to induce the in vitro differentiation of mesenchymal stem cells isolated from human umbilical cord blood into hepatocyte-like cells

AIM: Mesenchymal stem cells (MSCs) isolated from human umbilical cord blood can differentiate into hepatocyte-like cells under a suitable induction condition. The optimal condition is still unclear. This study investigated the feasibility of differentiation of MSCs isolated from human umbilical cord blood into hepatocyte-like cells with hepatocyte growth factor (HGF) and interleukin-6 (IL-6) in vitro to find a new cell source for live tissue engineering. METHODS: The experiment was performed from March to September 2007 at the Central Laboratory of First Hospital of Lanzhou University. ①Three cases of umbilical cord blood were collected from full-term pregnant women. Pregnant women had signed an informed consent. The experiment was approved by Hospital’s Ethical Committee. ②MSCs from umbilical cord blood were separated by density gradient centrifugation and adherent method. Cell surface molecule was measured by flow cytometry. Third passage of cells were divided into four groups and cultured in DMEM medium with HGF (HGF group), IL-6 (IL-6 group), HGF+IL-6 (HGF+IL-6 group) or no growth factor (control group). ③The characteristics of proliferation and growth of MSCs were studied by microscopy and methyl thiazolyl tetrazolium (MTT). The phenotypes of MSCs were identified by flow cytometry and immunohistochemical method. Albumin levels in culture supernatants were determined with enzyme-labeled immunosorbent assay (ELISA). RESULTS: ①Growth and division of adherent cells obtained from human umbilical cord blood were good. The shape of MSCs changed into triangle, polygonal or round on days 21-28 in induction groups. ②Positive staining reaction for alpha fetoprotein (AFP) on day 7, for CK18 on days 21, 28 after induction. 4.Clinical Study on Functional Dyspepsia by Auricular Point Sticking Therapy

Objective:To observe the therapeutic effects of auricular point sticking in managing functional dyspepsia.Methods:Seventy patients with functional dyspepsia were randomly divided into two groups,with 35 patients in the treatment group receiving auricular point sticking therapy,and the other 35 patients in the control group managed by taking Domperidone orally.The therapeutic effects were evaluated 4 weeks later.Result:The total effective rate was 85.7% in the treatment group and82.9% in the control group.Although the difference of the total effective rate between the two groups was not significant,the treatment group was superior to the control group in the improvement of several symptoms(P<0.05).Conclusion:Auricular point sticking therapy is effective in treatment of functional dyspepsia,and compared to Domperidone,it is better in managing gastric pain,abdominal bloating,and belching.
5.Effects of high glucose on number and activity of endothelial progenitor cells from peripheral blood

Objective To investigate the effect of high glucose on the number and proliferation, migration and adhesion of peripheral endothelial progenitor cells (EPCs). Methods Total mononuclear cells (MNCs) were isolated from peripheral blood by Ficoll density gradient centrifugation, and then the cells were plated on fibronectin coated culture dishes. After 7 days of culture, several groups of attached cells were incubated with glucose in a series of concentrations (15, 25, 35, 45mmol/L) for different durations (6, 12, 24 and 48h). EPCs were characterized as adherent cells which were double positive for DiLDL uptake and lectin binding by direct fluorescent staining demonstrated under a laser scanning confocal microscope. EPCs were further documented by demonstrating the expression of KDR, VEGFR 2 and AC133 with flow cytometry. EPCs proliferation, migration and in vitro vasculogenesis activity were assayed with MTT assay, modified Boyden chamber assay and in vitro vasculogenesis kit, respectively. EPCs adhesion assay was performed by replating MNCs on fibronectin coated dishes, and then the adherent cells were counted. Objective To investigate the principles of combined traditional Chinese and western medicine treatment of early postoperation inflammatory small bowel obstruction(EPISBO).Methods 93 cases of EPISBO were randomly divided into two groups including group A,47 cases received combined traditional Chinese and west- ern medicine treatment,and group B 46 received only western medicine treatment.Results 93 patients were all cured through nonsurgical treatment.The combined traditional Chinese and western medicine treatment was better in symtom improvement and reduce of expense.Conclusion The principle treatment of EPISBO is mainly nonsurgical. The traditional Chinese medicine and somatostatin are very important.

Objective:To observe and compare the clinical curative effect and safety of topiramate,carbamazepine and sodium valproate on the epilepsy secondary to encephalitis.Methods:80 cases of patients with epilepsy secondary to encephalitis who were treated in our hospital from January 2010 to September 2015 were selected and divided into the topiramate group,carbamazepine group and sodium valproate group,which were treated with topiramate,carbamazepine and sodium valproate respectively.The treatment effect,congnitive scores (including executive function and visual space,name,language,abstract,attention,delayed memory,directional),adverse reaction rate of three groups were compared.Results:The effective rate oftopiramate group was the highest(80.65%,25/31),carbamazepine was the lowest (70.00%,21/30),but there was no significant difference between the three groups (P>0.05).After treatment,the scores of executive ability and visual spatial,naming,abstraction,attention,orientation,language in topiramate group were significantly higher than those of carbamazepine group and sodium valproate group (P<0.05);the incidence of adverse reactions of topiramate group was 12.90%,which was significantly lower than that of carbamazepine group (36.67%) and sodium valproate group (29.62%)(P < 0.05).Conclusions:Topiramate,carbamazepine and sodium valproate have equal therapeutic effect on epilepsy secondary to encephalitis,but topiramate had less adverse reactions and best safety.
9.Assessment of left ventricular global and regional systolic function after transcatheter aortic valve implantation by three-dimensional speckle tracking imaging

Objective To investigate the alternation of left ventricular systolic function in patients underwent transcatheter aortic valve implantation operation(TAVI) by three-dimensional speckle tracking technology (3D-STI).Methods Totally 20 patients with severe aortic stenosis were enrolled.All the subjects underwent successful TAVI operation.The real-time 3D full volume datasets on apical four-chamber view were acquired on before,7 days and 1 month after TAVI.Left ventricular global longitudinal strain(GLS),regional peak systolic longitudinal strain(LS),regional peak systolic circumferential strain(CS) and regional peak systolic radial strain(RS),were analysed using off-line TomTec software,the differences among the three groups were compared.Results Compared with the preoperation,aortic valve blood flow velocity (AV),mean aortic valve pressure gradient(mPAG) of 7 days after operation decreased significantly.Threedimensional left ventricular ejecation fraction(3D-LVEF) among the patients whose 3D-LVEF under 50%had a remarkable increase and whose 3D-LVEF exceed 50% before operation had no significant change,while 1 month after operation the 3D-LVEF had a significant improvement compared with the preoperational data regardless of 3D-LVEF under 50% or not.The GLS and LS of all segments of 7 days after TAVI were higher than pre-operation(all P <0.05),and it had a further improvement 1 month after TAVI.Conclusions LV systolic function had improvement early after TAVI.3D-STI is a new,convenient way to detect the global and regional left ventricular systolic function of TAVI patients.
10.Clinical features and long-term follow-ups of juxtapapillary retinal capillary hemangioma

Objective:To evaluate the clinical features and follow-up effect of treatment for retinal capillary hemangioma (RCH) of myopic disc.Methods:Retrospective case series studies. A total of 14 patients (15 eyes) diagnosed with juxtapapillary RCH (JRCH) at Peking Union Medical College Hospital and followed up for more than 12 months from January 2007 to December 2019 were included. Among these patients, there were 7 males (7 eyes) and 7 females (8 eyes) with the average age of 29.8±12.7 years. There were 6 isolated JRCH cases, all of which were monocular patients, including 1 case with a single peripheral RCH; 8 cases with von Hippel-Lindau disease, including 5 cases of binocular RCH. Out of 15 eyes, 6 eyes were merged with peripheral RCH. Three eyes were simply observed, and 12 eyes were treated with photodynamic therapy (PDT) alone or PDT combined with other treatments. The average follow-up time was 55.2 months. Its clinical characteristics and long-term treatment effects were analyzed.Results:At the first visit, among the 15 eyes, 3 eyes had visual acuity <0.1, 5 eyes were 0.1-0.4, and 7 eyes were> 0.5. The endogenous and fixed exogenous JRCH were 12 (80%, 12/15) and 3 (20%, 3/15) eyes, respectively. The size of the tumor was 1/4 to 4 optic disc diameters (DD); the combined macular edema, epiretinal membrane, and macular hole were 11, 5, and 1 eyes, respectively. There were 3 eyes with tumor diameter less than 1 DD. The visual acuity at the first diagnosis was 0.6-1.0, and no treatment was given. The follow-up time was 12 months in 2 eyes and 120 months in 1 eye. At the last follow-up, his vision remained stable and the tumor did not enlarge. Six eyes with tumor diameter ranging from 1.0 to 2.5 DD, visual acuity was 0.06 to 0.6 at first diagnosis, accompanied by retinal exudation. 5 eyes were treated with PDT alone, and 1 eye was treated with PDT combined with anti-vascular endothelial growth factor drugs. The follow-up time was 12 to 84 months. At the last follow-up, the visual acuity improved or stabilized in 5 eyes and decreased in 1 eye. There were 6 eyes with peripheral RCH, including 4 eyes with retinal detachment, 2 eyes with epiretinal membrane, and 1 eye with vitreous hemorrhage. The visual acuity at the first visit was 0.02-0.6. All patients were treated with PDT combined with transpupillary thermotherapy, extrascleral drainage, and vitrectomy. The follow-up time was 12 to 132 months. At the last follow-up, all eyesight decreased.Conclusions:Visual decrease cause by JRCH often occurs at young adults. Bilateral JRCH are more common in patients with von Hippel-Lindau's disease. The tumor can cause retinal exudation, macular edema, epiretinal membrane and macular hole, resulting in gradual vision loss. PDT and/or combined anti-vascular endothelial growth factor drugs for medium sized JRCH with retinal exudation can maintain long-term vision stability. Patients with large JRCH or severe complications such as retinal detachment tend to have poor prognosis.
