1.Design and validation of a QOL scale for the Chinese patients with uterocervical cancer

Objective To develop and validate a qual it y of life(QOL) scale for the Chinese patients with uterocervical cancer (QOL-UC C). Methods Based on the internationally accepted de finition of QOL and the clinical characteristics of uterocervical cancer as wel l as the existing QOL scales, we designed the QOL-UCC. Then 113 patients were r ecruited and the reliability, validity and feasibility of the scale were tested. Results It was shown that the QOL-UCC had good reli ability, validity and feasibility. Conclusion QOL-U CC is a reliable and valid instrument for evaluation of the QOL in Chinese pa tients with uterocervical cancer.
2.Analysis of Quality of Life in patients with uterus cervical cancer

Objective To investigate the quality of life (QOL) of cervical cancer patients and its influential factors as well as to explore the method for improving the QOL in this population of patients. MethodsA QOL questionnaire for uterus cervical cancer (QOL-UCC) was designed and used in a survey of 81 UCC patients before they got any treatment and 124 UCC patients after they were treated for a period of time. SPSS 9.0 software was used to analyze data. ResultsThe patients got poor score with psychological status items(2.11? 0.34 ) before treatment, while those who had been treated for a period of time got poor score with social characteristics items(2.01? 0.33 ), especially with the work(1.87)and sexuality (1.00) items. Economic status of the patients, unsuccessful control of progress of the disease and recurrence were the main influential factors of the QOL in the UCC patients. ConclusionPsychosocial factors play very important roles in QOL of the UCC patients. As a result, psychosocial interventions should be stressed in the rehabilitation of this population of patients.
3.Relationship between cervical curvature index and cervical spine angle

BACKGROUND: How to evaluate the cervical curvature correctly has been a greatly concern of spine surgeons. In recent studies, cervical curvature index(CCI) and cervical spine angle(C2-7) were often used by foreign researchers.OBJECTIVE: Ishihara method and spine angle(C2-7) method were applied to measure the cervical curvature, and the correlation between the two methods was compared as well.DESIGN: An observational controlled trial based on the patients.SETTING: Department of Orthopaedics of Peking Union Medical College Hospital.PARTICIPANIS: Totally 63 cases including 34 male and 29 female patients with cervical spondylopathy were involved in the Department of Orthopaedics of Peking Union Medical Clooefe Hospital from July 2002 to July 2003.METHODS: The 63 cervical spondylopathy patients' cervical spine lateral X-ray films were evaluated.MAIN OUTCOME MEASURES: Main outcomes: ① The patients' CCI and the cervical spine angle; ② Relationship between their CCI and the cervical spine angle. Secondary outcomes: ① Regressive equation derived from their CCI and the cervical spine angle; ② Comparison of CCI and the cervical spine angle between male and female. RESULTS: The CCI(Ishihara method) was averaged 10. 1 (SD, 11.6), and the cervical spine angle(C2 -7) 17. 5 °(SD, 13.5). A highly significant correlation was found between the two indexes( P<0.01). Sex and age factors were found to be irrelevant to them.CONCLUSION: The CCI (Ishihara method) and the cervical spine angle (C2-7) are significantly correlated with each other.
4.Study on the prevention and therapeutic effects of Faecalibacterium prausnitzii on colitis of experimental rats

Objective To explore the therapeutic effects and the mechanisms of Faecalibacterium prausnitzii (Fp) on trinitro-benzene-sulfonic acid-induced colitis.Methods Sixty Sprague-Dawley rats were divided into healthy control group, colitis model control group, Fp pretreated group,Fp supernatant pretreated group,Fp treated group and Fp supernatant treated group.Disease activity index (DAI),histological injury of colonic tissue,the content of butyrate in feces,forkhead box protein 3 (Foxp3) regulatory T cells (Treg) in peripheral blood and spleen and the level of interlenkin (IL)-17 and IL-6 in serum were evaluated.All the data were statistical analyzed by single factor analysis of variance. Results Compared with colitis model control group, DAI significantly lowered and histological injury obviously improved in Fp pretreated group, Fp supernatant pretreated group,Fp treated group and Fp supernatant treated group.The effects of Fp pretreated group were better than those of Fp treated group and Fp supernatant pretreated group were better than Fp supernatant treated group.The concentration of butyrate in Fp pretreated group,Fp supernatant pretreated group,Fp treated group and Fp supernatant treated group was (3091.08 ±485.50) × 106 mol/L,(1714.64 ± 351.25) × 10(-8) mol/L,(2064.75 ± 295.04) × 10-6 mol/L and (1089.13±321.23) × 10-6 mol/L respectively,there was significant difference between Fp pretreated group and other groups (F=49.796,P<0.01).The peripheral blood level of Foxp3+ Treg in Fp supernatant pretreated group was highest.The spleen level of Foxp3+ Treg in Fp pretreated group and Fp supernatant pretreated group were significantly higher than that of other groups.The serum level of IL-17 and IL-6 in Fp pretreated group,Fp supernatant pretreated group,Fp treated group and Fp supernatant treated group was significantly lower than that of colitis model group.Conclustons Fp plays a role in promoting the repair of intestinal inflammatory reaction in colitis model rats.The mechanism may be related with butyrate producing,the peripheral blood and spleen level of Foxp3+ Treg up-regulating,suppressing the secretion of proinflammatory cytokine IL-17 and IL-6.Rebuilding the balance of Treg/Th17 to reduce local intestinal inflammation.
5.Protective effects and mechanisms of Faecalibacterium prausnitzii in experimental colitis in rats

Objective To investigate the protective effects and mechanism of Faecalibacterium prausnitzii (Fp) and its products in 2,4,6-trinitrobenzene sulfonic acid (TNBS) induced colitis rats.Methods A total of 40 Sprague-Dawley rats were divided into healthy control group,colitis model group,Fp supernatant group,Fp bacteria group and Bifidobacterium longum (B.longum) group.The rats of the later four groups were enemaed with TNBS to establish the model.At five days before and one day after modeling,the rats were gavaged with phosphate buffer saline (PBS),the supernatant of Fp,live Fp bacteria and live B.longum respectively.Rats were executed at 48 hour after modeling.The colon tissues were taken for pathology examination.The content of short-chain fatty acids (SCFA) in fecal was tested by gas chromatography.The plasma level of interleukin-10 (IL- 10),interleukin-12 ( IL-12),interleukin-17 (IL-17) and interleukin-23 (IL-23) were detected by enzyme-linked immunosorbnent assay (ELISA) and the expression of IL-17 in intestinal mucosal tissues was measured by immunohistochemistry.The data were analyzed by one way ANOVA.Results Compared with the healthy control group,the rats of colitis group suffered serious weight loss and their intestinal pathology score increased [(193.57±14) g vs (170.25±19.18) g,(1.00±0.99) vs (3.34±0.38),t=2.83 and 7.55,all P value<0.05].The Fp supernatant group showed protective effects in terms of weight and intestinal pathology score [(187.00± 14.67) g,(2.50±0.44),t=2.1 and 2.9,all P<0.05].Compared with healthy control group,the plasma and colon tissue IL-17 concentration of colitis model group increased (16.61 pg/ml±2.45 pg/ml vs 20.47 pg/ml± 1.45 pg/ml,0.83±0.98 vs 5.14±0.90) (all P<0.05).Compared with the colitis model group,the plasma and colon tissue IL-17 concentration of Fp supernatant group decreased ( 17.54 pg/ml± 1.51 pg/ml and 2.86±0.69).Conclusion Fp can regulate immune response and suppress rat colonic inflammation,which may be related with the expression of IL-17.

7.Construction of Helicobacter pylori Lpp20 DNA vaccine and primary study of its immunocompetence in mice

Objective To construct an eukaryotic expression plasmid PeDNA3.1 (+)/Lpp20 and to detect its expression in HeLa cells, and to observe the humoral and cellular immune responses in C57BL/6 mice induced by the Helicobacter pylori Lpp20 DNA vaccine injected intramuscularly. Methods The Lpp20 gene was amplified by PCR. PCR product was subcloned into the eukaryotic expression vector pcDNA3.1 (+)/ Lpp20, and the recombinant plasmid was transfected into HeLa cells using Liposome. After verifying that the Lpp20 antigen gene could be expressed in HeLa cells. Six weeks old C57BL/6 mice were immunized with pcDNA3.1 (+)/Lpp20 or pcDNA3.1 (+) or PBS buffer intramuscularly at 2-week interval for four times. ELISA was used for the quantitative detection of the specific IgG antibody in the sera of C57BL/6 mice and the cytokine IFN-γ in mice spleen lymphocyte culture medium after stimulating by Lpp20. The proliferation response of spleen cells was detected by MTT assay. The Lpp20 gene in muscle was identified by PCR. Results The significant specific antibody titers were detected by ELISA in DNA vaccine groups and the highest titer was 1:1024 after 6 weeks. The cytnkine IFN-γ in mice inoculated with pcDNA3.1 (+)/Lpp20 was increased and reached (410.36±56.23) pg/ml. A significant difference was tested between the experiment group and the control group[(25.26±10.85)pg/ml] ,P <0.01. The proliferation response of spleen cells of DNA vaccine group(SI: 2.37±0.22) was significantly higher than those of mice injected with pcDNA3.1 (+) (SI:1.53+0.47) ,P<0.01. Lpp20 gene could exist constantly in musculature cells of mice. Conclusion The eukaryotic expression recombinant pcDNA3.1 (+)/Lpp20 was successfully constructed. Strong humoral and cellular im-munity can be induced by DNA vaccine of pcDNA3.1(+)/Lpp20 in C57BL/6 mice, which might be helpful for further investigation concerning the immunoprotection of DNA vaccine.

9.Studies of the expression of CD40~+、CD40L~+ and CD8~+/CD28~+ on peripheral blood cells in patients with chronic B hepatitis

0.05).There was a positive corrlation on CD40~+ and CD40L~+ in chronic B hepatitis patients.There was a positive corrlation on CD8~+/CD28~+ and CD40~+、CD40L~+ in chronic B hepatitis patients.There were no remarkable corrlation on CD8~+/CD28~- and CD40~+、CD40L~+ in chronic B hepatitis patients.Conclusion:The costimulation molecules CD40~+、CD40L~+ and CD8~+/CD28~+ are lower,while CD8~+/CD28~- are higher in chronic B hepatitis patients than in the health.To test the expression of CD40~+、CD40L~+ and CD8~+/CD28~+ on peripheral blood of the chronic B hepatitis could help to evaluate patients's celluar immunity and guide clinical treatment.
10.Combination of gemcitabine and cisplatin in the treatment of patients with non-small-cell lung cancer

Purpose:To evaluate the efficacy, toxicity and side effects of the combination of gemcitabine and cisplatin in the treatment of patients with non small cell lung cancer(NSCLC).Methods:35 patients with NSCLC diagnosed by pathology or cytology were enrolled into the study. The patients received gemcitabine 1 000 mg/m 2 on d 1,8 and 15 and cisplatin 80 mg/m 2 on d 1 of 28 day cycle (4 week regiment), or received gemcitabine 1 200 mg/m 2 on d 1,8 and cisplain 80 mg/m 2 on d 8 of the 21 day cycle (3 week regiment). Results:28 of all the cases could be evaluated. The total response rate was 53.6% (all of them were partial response). Response rate of 4 week regiment and 3 week regiment were 58.3% and 50.0%, there was no significant difference between the two groups. The major toxicity and side effects included leucopenia and thromasthenia, but they were acceptable. Conclusions:The combination of gemcitabine and cisplatin is an effective and tolerable regiment in the treatment of NSCLC. Further study on the efficacy, toxicity and side effects in the treatment of NSCLC with different regimens should be carried out in the future.
