OBJECTIVE:To explore the role of clinical pharmacist in the therapy for patient with community-acquired Pseudo-monas aeruginosa lung abscess. METHODS:Clinical pharmacist participated in the therapy for a patient with community-acquired P. aeruginosa lung abscess,analyzed anti-infective therapy plan,and adjusted drug use timely to correct agranulocytosis induced by anti-infective drugs. RESULTS:The physicians adopted the suggestions of clinical pharmacists,controlled the infection successful-ly and corrected agranulocytosis. CONCLUSIONS:Community-acquired P. aeruginosa lung abscess is rare but has high mortality. Clinical pharmacists participate in drug therapy,assist physician to improve and optimize therapy plan and formulate individual medication plan so as to promote promote care rate of patients.

In order to make sure whether Panax notoginseng is sensitive to chloridion and guide fertilization in planting of P. notoginseng, the effects of the different proportion of potassium chloride (KCl) and potassium sulfate (K2SO4) on the yield, quality of P. notoginseng were studied. The results showed that K fertilizer significantly improved the growth of P. notoginseng and increased the biomass per plant or per pot and the content of N, P, K and the content of saponin. In cases of conditions such as potassium, and the effects of K2SO4 on increasing the petiole length, leaf size, rhizome length, root length, and content and accumulation of Ginsenoside Rg1 were better than those of KCl. While compared with K2SO4, KCl was more conducive to augmenting height, root width, the biomass of shoot, rhizome, root and the content of Ginsenoside Rb1 and Rd. There was not remarkable difference in agronomic characters, biomass and the content of N, P, K among KCl, K2SO4 and the combination of KCl and K2SO4. However, the content of saponin of the treatment with combination of KCl and K2SO4 was significant higher than that of single KCl or K2SO4 treatments. K fertilizer significantly increased yield and the content of saponins. And P. notoginseng was not sensitive to chloridion. KCl increased the yield and the content of saponins of P. notoginseng as well as K2SO4, and the combination treatment was superior to single treatment. It is recommended that the KCl should be adopted in production, to reduce the cost of potash fertilizer.

AlM:To explore the effective method for the treatment of complicated ocular trauma accompanied with cyclodialysis.METHODS: Sixty - eight cases of complexity ocular trauma with cyclodialysis in different degrees were undergone vitrectomy and ( or ) combined with ciliary body reduction.RESULTS:All patients were followed up for 10 ~36mo (mean 17. 0±5. 7mo). The postoperative visual acuity was better than that of preoperation (P<0. 05). Compared with preoperative, intraocular pressure was significantly increased ( P < 0. 05 ). Successful rate of ciliary body restoration was 91%. CONCLUSlON: For the complicated ocular trauma accompaniedwith cyclodialysis, vitrectomy and ( or ) ciliary body reduction is an effective treatment method.

Objective:To explore the spatial distribution and clustering in birth defects from 2010 to 2013 in Shaanxi Province.Methods:Spatial distribution was used to describe the birth defects,while ordinary Kriging method was used to predict the status of birth defects in Shaanxi province.The spatial characteristics for the birth defects at the county/district level were analyzed by spatial autocorrelation.Results:The overall incidence of birth defects was 219.196/10 000;Birth defect did not appear to be a random distribution but show a significant spatial aggregation.Spatial interpolation predicted the geographic distribution for occurrence of birth defects in Shaanxi Province.Local autocorrelation analysis showed nine hot spot areas for birth defects,such as Qian County,Liquan County,Yongshou County,Bin County,Fufeng County,Jingyang County,Chunhua County,Wugong County and Xingping City,and seven cold spot areas including Jia County,Yuyang District,Mizhi County,Suide County,Wubu County,Qingjian County and Zizhou District.Conclusion:There are spatial clustering in birth defects from 2010 to 2013 in Shaanxi Province.Spatial interpolation and spatial autocorrelation can be used to predict the spatial features of birthdefects in the whole province and provide evidence for the further intervention.

Objective To explore the relationship between first trimester pregnancy infection and congenital malformation in the cardiovascular system.Methods A hospital-based study was conducted from June 2014 to June 2016.Totally 1618 mothers of infants with cardiovascular system malformation and normal infants were interviewed through a face-to-face questionnaire survey.A propensity score-matched study was conducted to investigate the association between first trimester pregnancy infection and congenital malformation in the cardiovascular system.Results All important covariates were balanced after matching.First trimester pregnancy infection increased the risk of congenital malformation in the cardiovascular system,single congenital malformation in the cardiovascular system and multi-congenital malformation in the cardiovascular system.After matched,the OR values were 1.65 (95％ CI:1.21-2.24;P=0.001),1.50 (95％ CI:1.02 2.20;P=0.037),and 1.90 (95％ CI:1.18-3.06;P=0.008),respectively.Conclusion First trimester pregnancy infection increases the risk of congenital malformation in the cardiovascular system.Avoiding infectious diseases during the first trimester in pregnancy is important in decreasing the incidence of congenital malformation in the cardiovascular system.

Objective To explore the relationship of four kinds of lifestyle factors,namely,prenatal passive smoking,hair dyeing or perming,exposure to noise,living in a house renovated in the past one year,with congenital heart disease.Methods A hospital-based case-control study was conducted.Totally 270 mothers in the case group and 1633 mothers in the control group were interviewed through a face-to-face questionnaire survey.A generalized linear model was employed to investigate the association between congenital heart disease and prenatal lifestyle factors.Results Both in Model 1 and Model 2,prenatal hair dyeing or perming had no association with congenital heart disease.On the other hand,prenatal passive smoking,exposure to noise and living in a house renovated in the past one year all increased the risk of congenital heart disease.After adjustment for all confounders,the OR values were 1.771 (95％ CI:1.079-2.909),2.079 (95％ CI:1.310-3.298),and 2.494 (95％ CI:1.511-4.116),respectively.Conclusion Prenatal passive smoking,exposure to noise and living in a house renovated in the past one year are the risk factors for congenital heart disease.It is very important to avoid such factors during pre-pregnancy and pregnancy.

The suspended magnetic moxibustion cupping therapy,as a specific moxibustion method which collects moxibustion,cupping,infrared and magnetic therapy into one,and integrates with the warming and heating effect of moxibustion,sucking effect of cups,and effect of infrared light energy and magnetic energy,is a particularly unique method for weight loss.This article introduces the basic situation and mechanism of suspended magnetic cupping therapy for weight loss.For pathogenesis of deficiency,phlegm,stagnation and cold in obesity,Shuang Long Xi Zhu (two dragons playing a ball) moxibustion,abdominal Jiu Gong (nine palaces) moxibustion,San Yang Kai Tai (auspicious beginning) moxibustion and Chang Long (long dragon) moxibustion were given respectively.This therapy has inspired the clinical idea of Chinese medicine for obesity.

OBJECTIVETo study the changes of the fatty components of Pollen Typhae before and after being carbonized.

METHODPollen Typhae and Pollen Typhae carbonisatus were extracted with petroleum ether (60-90 degrees C) respectively. The two kinds of extracts were analyzed by GC-MS after saponificated and methanolized, and their constituents were searched through NIST. The contents of the constituents were determined by method of normalization.

RESULTEither in Pollen Typhae or in Pollen Typhae carbonisatus, 32 components were identified, among which 20 components were the same and 6 were different respectively. Among the same components, the relative contents of 3-methyl-2-butenoic acid-2-phenylethyl ester, hexanedioic acid-dimethyl ester, dimethyl phthalate, diethyl phthalate, diphenylamine, sebacic acid dimethyl ester, 1,2-benzenedicarboxylic acid, ethyl methyl ester, methyl-2-ethylhexyl phthalate and diisooctyl phthalate etc. increased obviously, and the relative contents of nonanedioic acid-dimethyl ester, diisobutyl phthalate and stigmastan-3,5-dien etc. decreased greatly. Among the different components, 8-hydroxy-octanoic acid-methyl ester, 9-hydroxy-nonanoic acid-methyl ester, 10-octadecenoic acid-methyl ester, m-hydroxycinnamic acid-methyl ester,3-[4-( acetyloxy)-3-methoxyphenyl]2-propenoic acid-methyl ester and 11-octadecenoic acid-methyl ester were detected in Pollen Typhae, 3-hydroxyspirost-8-en-11-one, benzenepropanoic acid-methyl ester, 2,4-dimethylhexanedioic acid; 2,4-bis (1,1-dimethylethyl)-phenol, undecanedioic acid-dimethyl ester and 9,10-dihydroxy-octadecanoic acid-methyl ester were detected in Pollen Typhae carbonistatus.

CONCLUSIONThe species and contents of the fatty components in Pollen Typhae changed before and after being carbonized, but their chemical types didn't change too much.

Objective To investigate the diagnostic value of dual-energy CT pulmonary angiography (DE-CTPA) and dual-energy CT lung perfusion (DE-CTLP) in the assessment of pulmonary embolism comparing with pulmonary ventilation-perfusion scintigraphy. Methods Fifty patients suspected of PE (26 males, 24 females) underwent both DE-CTPA, DE-CTLP and pulmonary ventilation-perfusion scintigraphy. The results were compared and the correlation between the intravascular clots in DE-CTPA and the perfusion defects in DE-CTLP was analyzed. The sensitivity, specificity and accuracy of DE-CTLP for PE were assessed according to scintigraphic results which was considered as a "gold" standard. Result ( 1 ) Of 50 cases,920 segments in 46 cases were diagnostable, 4 cases were not included because of the poor image quality. lntravascular clots were found in 262 segments on DE-CTPA and perfusion defects in 266 segments were identified on DE-CTLP, while 268 segments were positive on scintigraphy. (2) The perfusion defects on DE-CTLP were correlated well with the clots on CTPA ( r = 0. 883, P < 0. 01 ). The Dual energy CT Lung perfusion imaging had a good consistence with scintigraphy (Kappa = 0. 940, P <0. 01 ). The sensitivity, specificity, positive and negative predictive value were 96. 2% (279/290), 98.0%(641/654), 95. 5% (279/292) and 98. 3% (641/652), respectively. (3)The radiation dose of DE-CTPA and CE-CTLP scan was(4. 37 ± 0. 47) mSv by using CareDose 4D technique. Conclusion DE-CTPA and CE-CTLP can provide pulmonary vascular morphology and parenchyma perfusion information which are useful and valuable for the diagnosis of PE.

Objective To investigate the dynamic epidemiological characteristics of perinatal birth defects in Xi'an so as to provide scientific evidence for the priority of future birth defect prevention.Methods We made a statistical description of the data of perinatal birth defects from 2010 to 2015 reported by all the maternity hospitals in Xi'an.Results The incidence of birth defects in Xi'an from 2010 to 2015 was 89.62/104 (87.60/104,91.74/ 104) and had an annual ascending tendency (x2-157.13,P＜0.01).The top five main birth defects in a descending order were congenital heart disease (26.53/104),polydactyly (13.10/104),total cleft lip (11.42/104),neural tube defects (6.06 /104),and congenital hydrocephalus (5.08/104),accounting for 69.39％ of the total number of birth defects.And the incidence of congenital heart diseases showed a year-by-year ascending trend (x2=837.65,P＜ 0.01).The incidence of birth defects was higher in urban areas than in rural areas (OR 1.108,95％ CI:1.056-1.162).The birth defects were more frequently seen in males than in females (OB=1.151,95％ CI:1.097-1.208).Mother's age ＜20 (OR=1.764,95％ CI..1.532-2.032) and mother'sage≥35 (OR=1.283,95％ CI:1.179-1.395) also were risk factors for birth defects.Up to 72.54％ of the defects were live births.There were 52.83％ cases diagnosed by ultrasound and 46.24％ ones by clinical examination.The prenatal diagnosis rate was 33.20％.Conclusion The incidence of perinatal birth defects in Xi'an from 2010 to 2015 showed an annual ascending trend.We should improve the prenatal diagnosis level of all monitored hospitals in order to reduce incidence of birth defects and increase the quality of population.
