Seven guaiane-type sesquiterpenoids, a new compound 6-formyl-5-isopropyl-3-hydroxymethyl-7-methyl-1H-indene (1), a new natural product 5-isopropyl-3, 7-dimethyl-1H-indene-1-one (2), along with five known compounds: guaiazulene (3), 4-formyl-7-isopropyl-10-methylazulene (4), sesquiterpene ketolactone (5), alismoxide (6) and guaia-1 (5), 6-diene (7), were isolated from gorgonian Muriceides collaris collected in South China Sea. Their structures were elucidated on the basis of extensive spectroscopic analysis [MS, IR, 1H NMR, 13C NMR (DEPT), HMQC, HMBC, NOESY] and by comparison of the spectral data with those of the literatures.

Tolvaptan is a novel oral selective arginine vasopressin V2 receptor antagonist.Tolvaptan improves heart failure signs and symptoms without serious adverse events.Tolvaptan has no effect on all-cause mortality and cardiovascular death or admission rate for heart failure. But in heart failure patients with hyponatremia, tolvaptan can decrease cardiovascular death and admission rate for heart failure.

Objective To study the effect of traditional Chinese medicine on survival time in children solid tumors with chemotherapy. Methods 76 children with solid tumors, which were collected from January 2005 to March 2006, were randomly divided into treatment group (35 cases), control group (41 cases). The treatment group was treated with chemotherapy + self-dispensing Fuzheng Jianpi Chinese medicine. The control group was treated with chemotherapy alone. Results The survival rate of the treatment group was higher than that of the control group, the difference was statistically significant (P = 0.0024). The average survival time after chemotherapy in the treatment group was (31± 2) months (95 % CI 27-34 months), the control group was (21±2) months (95 % CI 16-25 months), which was shorter than the treatment group. Conclusion Chinese medicine and chemotherapy can prolong the survival time of children with solid tumors.

Objective To explore the clinical characteristics of IgG4-related disease (IgG4-RD) in Chinese by detailed clinicopathological and laboratory assessments. Methods The baseline features of 36 patients with biopsy-proven disease were reviewed. The diagnosis was confirmed by pathology review according to consensus diagnostic criteria and clinicopathologic correlation.Disease activity and damage were assessed by the IgG4-RD responder index (RI).Results Thirty (83.3%) of the patients were male, while six were female, and the average age of onset was 65.1 years. All of the 36 patients had active disease,in which submandibular gland, lymph nodes, retroperitoneal tissue were the most common affected organs in this group of patients.Among 36 patients,77.7% had elevated serum IgG4 concentrations and 44.4% had hypocomplementemia.Patients with elevated serum IgG4 had a higher RI,a greater number of organs involved (P < 0.01 for all comparisons). The correlation between serum IgG4 level and RI (r=0.737,P < 0.01) was stronger than IgG,ESR,CRP and serum complement levels. The incidence of hypocomplementemia in IgG4-RD patients with renal involvement was higher than that in IgG4-RD patients with other organs involvement (P < 0.01).Twenty-eight patients received glucocorticoids therapy, and had lower RI and serum IgG4 concentration after therapy (P < 0.05).Conclusions Both IgG4-RD RI and IgG4 concentration may be regarded as assessment markers of disease activity and therapeutic effect of IgG4-RD.The diagnosis of IgG4-RD should be supported by histopathology and clinical features.

Chinese Journal of Neurology 2000;0(04):-
Objective To inquire and compare the value of 4 proteins in the cerebrospinal fluid (CSF) for early diagnosis of Creutzfeldt-Jakob disease (CJD).Methods The CSF samples were obtained from 14 CJD patients in the early stage,t-tan,t-tan,t-tau/t-tau,S-100?,14-3-3 and 14-3-2 protein had been examined and compared with other dementia (OD) and none dementia (ND) as controls by capture assay and ELISA.Results (1)t-tan protein: the concentration in CJD ,OD and ND is 8295,300 and 161 pg/ml respectively,t-tau/t-tau ratio is 0.0092,0.2258 and 0.2471 respectively.(2) S-100? protein: the concentration in CJD,OD and ND is 1.576,0.639 and 0.239 ng/ml respectively.(3) 14-3-3 protein:the concentration in CJD,OD and ND is 40.00,2.65 and 3.10 ng/ml respectively.(4) 14-3-2 protein: the concentration in CJD,OD and ND is 48.43,14.00 and 20.50 ng/ml respectively.(5) At the cut-off point 500 pg/ml for t-tan,sensitivity for diagnosis of CJD is 84.6% and specificity is 87.6% ,t-tan has no diagnostic value to CJD.(6) At 1.626 ng/ml for S-100?,the sensitivity is 92.3% and specificity is 83.8%.(7) At 9 ng/ml for 14-3-3 protein, the sensitivity is 86.7% and specificity is 86.4%.(8) At a level of24 ng/ml for 14-3-2 protein, the sensitivity for diagnosis of CJD is 78.6% and specificity is 77.3%. Conclusion It is the CSF detection of neuroproteins that have great valuble significance in diagnosis for sporadic CJD in the early stage.
