Objective To explore the effect of enteral nutrition combined with parenteral nutrition support on the prognosis of severe acute pancreatitis(SAP).Methods Sixty-one patients diagnosed with SAP in our hospital from July 2011 to June 2012 were chosen and randomly divided into EN and PN group(30 cases)and PN group(31 cases).At the early stage of treatment,the former group was provided parenteral nutrition(PN)alone while the latter group was given enteral nutrition together with parenter-al nutrition.Then we measured the patients′body mass index on the 1st,2nd,3rd,5th,7th,10th,14th day respectively after admis-sion,and recorded the following indexes:whether complications occurred or not,the time for the first passage of gas by anus and defecation,the total time and cost of hospitalization,the drug proportion and the time the serum and urine amylase returned to nor-mal.Results The two groups of patients′body mass index showed no significant differences in the first 3 days,but there were sig-nificant differences in statistics on the 5th,7th,10th and 14th day(P <0.05).In addition,the two groups of patients formed a sharp contrast in such aspects as the bowel sound recovery time,the anal exhaust time,the total cost of hospitalization and the incidence rate of the complications(P <0.05)whereas there were no remarkable differences in the serum and urine amylase recovery time and the total time of hospitalization(P >0.05).Conclusion Compared with PN,the combined therapy of EN and PN could improve the patients′nutrition,reduce the weight loss caused by consumption,promote intestinal peristalsis,effectively reduce infectious compli-cations caused by floraectopic and ease the patients financial burden.

Background and Purpose:Recent studies have shown that survivin is an anti-apoptosis gene,which is involved in carcinogenesis and drug resistance of leukemia.Antisense oligodeoxynucleotide(ASODN) can be used to inhibit the expression of survivin,inducing apoptosis and enhancing the chemosensitivity of leukemic cells.This study was designed to explore the effect of survivin ASODN on the growth,apoptosis,and caspase-3 activity of leukemia cell line K562/A02 with the phenotype of drug resistance.Methods:Survivin ASOND was transfected to K562/A02 cells by liposomal reagent,The rate of inhibition,expression of survivin mRNA,apoptosis,and activity of Caspase-3 were detected by colormetric MTT,RT-PCR,flow cytometry and fluorometer,respectively.Results:Survivin ASODN could inhibite the cell proliferation in a dose and time dependent manner.Compared with controls,expression of survivin mRNA decreased by 36.2%(P

Objective To compare the effects of combination of maternal hydration and hands and knees posture with simple hands and knees posture on correction of breech pregnancy. Methods One hundred patients who were diagnosed breech pregnancy from November 2015 to November 2016 were assigned to experimental group and control group with 50 cases each according to individual willingness. The patients in experimental group received the combination of maternal hydration and hands and knees posture;while the patients in control group received simple hands and knees posture. Results Amniotic fluid index was (18.94±2.44)cm in experimental group and (15.97±2.33)cm in control group, the difference had statistic significance (t=6.239, P<0.05). The effective transfer rate and successfully used time was 92%(46/50) , (2.10 ± 0.75) weeks in experimental group and 64%(32/50), (4.27 ± 0.98) weeks in control group, the difference had statistic significance (χ2=11.422, P<0.05; t=-12.463, P<0.05). For primipara and multipara, the effective transfer rate was 90.91%(30/33), 16/17 in experimental group, and 68.75%(22/32), 10/18 in control group, the difference had statistic significance (χ2=4.986, 6.806, all P<0.05). The rate of vaginal and cesarean section deliveries was 86%(43/50), 14%(7/50) in experimental group, and 56%(28/50), 44%(22/50) in control group, the difference had statistic significance(χ2=10.928, P=0.001). Patients who had fetal movement or who didn′t have time to do correction or missed was 17.10%(46/269), 1.49%(4/269) in experimental group, and 12.55% (22/255), 7.06% (18/255) in control group. Conclusions @Therapic effects of combination of maternal hydration and hands and knees posture is much better than that of the simple hands and knees posture therapy on correction of breech pregnancy, which is worthy of popularization and application in clinic.

Objective To investigate prospectively the diagnostic significance of ulnar/median nerve amplitude ratio in motor neuron disorders.Methods Patients referral to our department between May 2009 and February 2012,due to muscle weakness,inflexible,cramps and atrophy,were consecutively enrolled.Conventional nerve conduction studies of 4 extremities (using surface electrodes) and needle electromyography were performed in all patients with fixed examiner.The compound muscle action potentials (CMAP) were recorded from abductor digiti minimi (ADM) and abductor pollicis brevis (APB) with stimulation of ulnar and median nerve at wrist,respectively.Moreover,the ratio of CMAP amplitude between ADM and APB (ADM/APB) was calculated in the patients who met the diagnostic criteria for definite amyotrophic lateral sclerosis (ALS) and Hirayama disease (HD).The patients with Guillain-Barré syndrome (GBS) and other popyneuropathies (PN) were served as case-controls,and 34 healthy volunteers (aged (45.7 ± 16.3) years) as normal-controls.Results (1) There were 78 cases with ALS,25 HD,51 GBS and 140 other PN,with the mean age(years) of 54.7 ± 11.6,17.6 ± 2.2,41.3 ± 18.4 and 57.1 ± 14.3,respectively.(2) ADM/APB in the ALS subgroup was 2.28 ±2.87 (0.12-22.38),HD0.66±0.36 (0.05-1.34),GBS 1.42 ± 1.33 (0.25-9.85),other PN 1.36 ± 1.48 (0.08-14.44) and normal-controls 1.07 ± 0.28 (0.61-1.64,F =6.872,P =0.000),respectively.(3) The areas under receiver operator characteristic curve in patients with ALS was 0.830 (s-x =0.039) and HD 0.691 (sx =0.039,P =0.000) ; the diagnostic sensitivity and specificity for ALS patients were 36.7％ and 93.3％,respectively,with cutoff value of ADM/APB =2; and the diagnostic sensitivity and specificity for HD patients were 53.6％ and 89.0％,respectively,with cutoff value of ADM/APB =0.7.Conclusions The ulnar/median CMAP amplitude ratio increases in ALS,but decreases in HD,which may be served as a relatively specific electrophysiological index.ADM/APB amplitude ratio ＞ 2.0 is suggested to be a diagnostic parameter for ALS and ＜ 0.7 for HD.

Objective To discuss the correlation between cervical intraepithelial neoplasia(CIN) and uterine cervix cancer and the combination of TCT and colposcope for the investigation of cervical lesion.Methods 5545 patients were preliminarily screened by cervical fluid basement cell folium smear,in which 307 patients are masculine and 219 are negative,the 526 patients are suspected cervical lesions in clinical symptoms and they further underwent colposcopy and biopsy,the results were analyzed by pathohistology which is golden standard.Results CIN coherent dangerous factors analyzed by single factor Logistic regression analysis are:cervical HPV pollution,age of sexual activity,sexual partners numbers,protection of sexual life, STD history,age and frequency of miscarriage ( P < 0.05 ).The coincidence between TCT,colposeopy and pathologic diagnosis are 49.4%, 83.8% ;the omission diagnostic rate of LSIL,HSIL and CC from TCT are 63.63% ,74.44% ,100% ;the omission diagnostic rate of LSIL,HSIL and CC from colposcopy are 33.37%, 12.22% ,0;the sensitivity of TCT,colposcopy and TCT combine colposcopy for CIN are 61.2% ,84.6% ,94.5% and specificity are 85.4% ,88.2.% ,90.8%.Conclusions (1)The generation of CIN and cervical cancer are correlated with cervical HPV pollution,sexual behavior and various kinds of lower genital tract infection.(2) Fluid basement cell folium smear can promote the property of diagnosis,meet the demands of early stage uterus neck cancer and precancer lesion investigation,are suitable to be routine cheek methods in primary hospital.
