Background The filtration surgery is the main method of treating glaucoma, which usually fails due to postoperative scarring. The study about application of anti-scarring agents in filtration surgery is a hotspot. Objective This study was to investigate whether topical administration of hydroxycamptothecin (HCPT) could be used to prevent postoperative scarring in after experimental glaucoma filtration surgery, and explore its optimal dose.Metbods Trabeculectomy was performed on the right eyes of 40 New Zealand white rabbits to establish the trabeculectomy animal models. The rabbits were then randomized into normal saline solution group,0.3 g/L mitomycin C(MMC) group,0.3 g/L HCPT group and 1.0 g/L HCPT group based on the intraoperative topical drugs using randomized number table method. The different drugs above-mentioned were put beneath the conjunctival flap and scleral flap for 5 minutes during the surgery. The intraocular pressure (IOP) was measured before and day 1,4,7,14,21 and 28 after the surgery with Icare tonometer, and the filtering area and height were measured under the slit lamp microscope to assess the efficacy of various drugs. The adverse effects were evaluated by examining the responses of the ocular anterior segment and retinal change. The specimens at operative zone were obtained in 7,14 and 28 days after surgery with the size 5 mm×5 mm for the hematoxylin-eosin staining and Masson trichrome staining to estimate the anti-fibrosis effect of various drugs, and to evaluate the survival time of functional bleb.Kaplan-Meier analysis was used to compare the survival time of functional bleb of different groups. The use and care of the animals complied with the Regulation for the Administration of Affair Concerning Experimental Animals by State Science and Technology Committee. Results The IOP was significantly different in the rabbits from different groups among various time points (Fgroup =20.79,P =0.00 ; Ftime =85.34,P =0.00 ; Fiion =2.13,P =0.01).From 1 day through 28 days after operation, the IOPs in MMC group and 1.0 g/L HCPT group were significantly lower than those before operation (all at P<0.05).The survival time of functional bleb of different groups was (11.3 ±2.8),(19.5 ±2.4),(13.3 ±2.2) and (20.2 ± 4.5) days, respectively, showing a significant difference (log rank =11.92,P < 0.01),with a considerable prolong in the 1.0 g/L HCPT group. No significant change was found in the bleb area and height among the four groups within 7 days after operation, but postoperative 7,14,28 days, the area and height values of bleb were significantly smaller in the normal saline solution group and 0.3 g/L HCPT group compared with the MMC group and 1.0 g/L HCPT group (all at P < 0.05).Histopathological examination showed loosen subconjunctival tissue, less inflammatory cells and weaker collagenous fibrillary staining in the MMC group and 1.0 g/L HCPT group in comparison with the normal saline solution group and 0.3 g/L HCPT group. Conclusions The topical administration of 1.0 g/L HCPT inhibit the inflammatory response and collagen fibrosis and therefore prolong the survival time of functional bleb after glaucomatous filtering surgery.

This study is to explore the interventional effects of fluvastatin on anti-beta2GPI/beta2GPI-induced activation in THP-1 mononuclear cells. In vitro, human mononuclear cells THP-1 were treated with fluvastatin, LPS and anti-beta2GPI/beta2GPI, then the TF expression on THP-1 cells was detected by real-time quantitative PCR (RT-qPCR) or TF activity was detected by kit. TNF-alpha mRNA and its protein expression were investigated by RT-PCR and ELISA kit. The expression of phospho-NF-kappaB p65 and inhibitory protein of NF-kappaB (IkappaB-alpha) were measured by Western blotting. The results suggested that the expression of TF and TNF-alpha on THP-1 cells was significantly up-regulated with treatment of anti-beta2GPI/beta2GPI complex (100 mg x L(-1)), compared with that of untreated cells (P < 0.05). Fluvastatin (50 mg x L(-1)) could decrease TF (mRNA and activity) expression and the level of TNF-alpha (mRNA and protein) in THP-1 cells with anti-beta2GPI/beta2GPI complex. The expression of TF and TNF-alpha was shown in a concentration-dependent manner. Moreover, anti-beta2GPI/beta2GPI complex could downregulate IkappaB-alpha levels and increase the levels of phospho-NF-kappaB p65. And these effects of anti-beta2GPI/beta2GPI complex could be blocked by fluvastatin. In conclusion, fluvastatin may interfere the expression and regulation of NF-kappaB signal transduction pathway, thereby inhibit the effects of anti-beta2GPI/beta2GPI on activation of THP-1 cells, by decreasing the expression of TF and TNF-alpha.

Objective:Iris-fixed IOL implantation is a new and effective therapy for phakic eyes. Because the patients are young, they know very few about the operation, which mandated delicate pre-and postoperative nursing. This article discussed how to nurse the patients receiving iris-fixed IOL implantation operation in phakic eyes for super high myopia. Methods:Fifty eight eyes of 30 patients with super-high myopia received iris-fixed IOL (Verisyse IOL) implantation. Pre-and postoperative nursing course were summarized. Results:Focused education through preoperative visits, active cooperation in the operation and meticulous postoperative nursery were beneficial for the operation. Significant vision improvement was found after surgery. Conclusion:The planned, step by step, focused nursing course can improve the effects of operation and reduce the postoperative complication.

Objective To analyze the screening markers of minimal residual disease (MRD) in children with acute lymphoblastic leukemia( ALL), and to evaluate its clinical application and express characteristic. Methods Thirty-five patients with childhood acute lymphoblastic leukemia treated in our hospital from 2008.11-2009.12 were enrolled into this study. Bone marrow samples were taken to isolate sing-nucleus cells. Four-color fluorochrome labeled monoclonal antibodies were applied to analyze the cell immunophenotypes when meet the demand like these: low expression of CD38, CD45; high expression of CD58,CD21, CD22; co-expression of CD34 and Cu, and the expression of abnormal chromosome related antigen CD66c. After staining of surface and intracellular antigen, sample should be detected for leukemia-associated immunophenotypes by four-color FCM with double parameters. The leukemic cell populations located in flow cytometry dot plot different from those of normal were considered to be the effective screening markers. Results Total of cases of 31 patients at least had one MRD marker, coverage is 88.6%. 21/35 (60%)had 2 or more than 2 selection markers; TdT/CD10/CD34/CD19 is the most common four-color combination, followed by CD38/CD10/CD34/CD19, CD45/CD10/CD34/CD19, other index is rare. Conclusion TdT/CD10/CD34/CD19 was the most common four-color selection markers should be as regular and preferred detection ones. Study showed the immune phenotype Pro-B is lack of effective selection markers. Two or more interest markers should be improved the accuracy of MRD significantly.
