Objective: To explore the efifcacy and safety of sildenaifl for treating the patients of congenital heart disease (CHD) with severe pulmonary hypertension in plateau area.
Methods: A total of 50 CHD patients combining severe pulmonary hypertension treated in our hospital from 2010-01 to 2013-10 were studied. The patients were randomly divided into 2 groups, n=25 in each group. Control group, the patients received conventional treatment and Sildenaifl group, based on conventional treatment, the patients received additional sildenaifl medication. The hemodynamic, blood gas, routine and biochemistry were recorded and compared between 2 groups.
Results: Compared with Control group, Sildenaifl group had more reduction of pulmonary artery pressure, increased arterial pressure of oxygen, left ventricular output, cardiac index and oxygenation index, all P<0.05. The patients’ arterial pressure, blood routine and biochemistry were similar between 2 groups, P>0.05. There was no obvious adverse reaction observed in Sildenaifl group.
Conclusion: Based on conventional treatment, Sildenafil may effectively reduce the pulmonary artery pressure in CHD patients combining pulmonary hypertension in plateau area, it improving the cardiac function without adverse reaction.

Objective To identify the advantages and disadvantages of the rapid plasma reagin test (RPR), chemiluminescence immunoassay (CLIA) and Treponema pallidum particle agglutination test (TPPA) for detecting syphilis. Methods A total of 1,808 serum specimens were detected by RPR and CLIA; the negative specimens as detected by CLIA and by RPR were redetected by TPPA. Results There were 170 syphilis patients among the 1,808 patients. The sensitivity and positive predictive value of RPR were both significantly lower than those of CLIA and TPPA (P<0. 05). The specificities were not significantly different between CLIA and RPR(P>0. 05). However, the negative predictive value of RPR was significantly lower than that of CLIA (P<0. 05). The sensitivities were not significantly different between CLIA and TPPA(P>0. 05), however, the positive predictive value of CLIA was significantly lower than that of TPPA (P<0. 05). Biologically false positive results could be found for all the 3 methods and false negative results could be found for RPR and TPPA (P<0. 05). Conclusion CLIA and TPPA are superior to RPR in diagnosing syphilis. The procedure of diagnosing syphilis may need to be adjusted, and syphilis should be diagnosed by combining the medical history, symptom and lab results.

Objective: To optimize the extraction process of Yangjing Zhongyu Decoction and investigate the content change of loganin, paeoniflorin, and ferulic acid in the mixed and seperated decoctions. Methods: Orthogonal design was used to optimize the extraction process. The contents of the three components were determined under the same chromatographic conditions by HPLC. The detective wavelength of loganin and paeoniflorin was 230 nm, and the detective wavelength of ferulic acid was 316 nm. Results: The optimum extraction condition was as follows: six times water, extracting three times, every time for an hour. Conclusion: The optimum process is stable and feasible. The contents of the three components are more improved in the mixed decoction in varying degrees than single decoction.

Objective To analyze the changes and characteristics of cancer incidence and mortality among Jiaxing residents in the past 6 years,and to provide evidence for the prevention and control of cancer incidence and mortality.Methods Registered malignant tumor data from the Zhejiang Chronic Disease Information Monitoring and Management System were collected and analyzed based on the International Statistical Classification of Diseases (ICD-10).The statistical indexes used for analyses included crude incidence,standardized incidence,crude ortality,standardized mortality,potential years of life lost (PYLL),average years of life lost (AYLL),rate of potential years of life (PYLL‰) and so on.Results From 2010 to 2015,the crude incidence,the standardized incidence,the crude mortality,and the standardized mortality were 342.52/105,240.89/105,194.71/105 and 126.27/105,respectively.The crude incidence and mortality for males were both higher than those for females(378.44/105 vs.307.49/105,x2=763.06,P＜0.01;255.31/105vs.135.62/105,x2-3 814.11,P＜0.01).The PYLL,AYLL and PYLL‰ of residents in Jiaxing were 248,307 years,6.16 years and 12.00‰,respectively.The PYLL‰for males was significantly higher than that for females(x2=37073.85,P＜0.01).The AYLL for females was 6.50 years,higher than that for males(5.98 years)(t =125.15,P＜0.01).The incidence of malignant tumors increased with age,especially in the older than 40 age group,with the older than 80 age group showing the highest rate at 1 779.01/105 (x2 =38 840 907,P ＜ 0.01).Rates for Lung cancer (70.59/105),colorectal cancer (39.86/105),liver cancer (29.14/105),breast cancer(26.73/105) and stomach cancer (26.00/105) were the most common malignant tumors in Jiaxing.Conclusions The morbidity and mortality of malignant tumors are showing signs of rising and the incidences of lung cancer and gastrointestinal cancer are particularly pronounced in Jiaxing.In consequence,measures for the prevention and control of malignant tumors should be developed with clearly defined objectives and research on the pathogenesis of malignant tumors should be stepped upto reduce the incidence and mortality of malignant tumors.

ObjectiveTo evaluate biomechanical properties of transoralpharyngeal atlantoaxial reduction plate (TARP) prepared from magnesium alloy and titanium alloy for the atlantoaxial dislocation by using three-dimensional finite element analysis and to exam the feasibility of using magnesium alloy for preparation of TARP system so as to provide a theoretical basis for clinical surgery.MethodsA patient with typical atlantoaxial fracture dislocation was involved in the study,and received thin CT scan with clinically used titanium alloy TARP system for obtaining DICOM image data.Three-dimensional finite element analysis software was imported to simulate magnesium alloy and titanium alloy TARP systems for reduction and fixation.Then,stress changes of the atlas,axis,internal fixators and C2/3 zygapophysial joints were determined with three-dimensional finite element analysis and analyzed statistically.Results ( 1 ) The finite element model of atlantoaxial dislocation reduction and fixation had lifelike outline and good geometric similarity.There were 53 586 nodes and 180 784 units.(2) During the simulation of head in neutral position,the stress concentration region was C2/3 zygapophysial joints followed by the anterior arch,posterior arch and lateral mass of atlas respectively,and C2 vertebral arch again.( 3 )Magnesium alloy and titanium alloy TARP systems showed significant difference in stress distribution (P ＜0.05).Conclusions(1)The atlantoaxial model established according to its structure information on CT can be used for biomechanical experiments.(2) For the treatment of atlantoaxial dislocation using the existing titanium TARP system,maintaining the integrity of anterior and posterior arch of atlas and confirming the bone fusion in lateral mass can better keep the stability of the atlantoaxis.After atlantoaxial fusion,the increased stress of the zygapophysial joints of the adjacent segments accelerates structural degeneration,which should be closely followed up.( 3 ) Magnesium alloy TARP system for fixation and reduction shows the fall in peak value of the stress concentration region,and improvement of the uniformity of stress distribution as compared with titanium alloy TARP.

OBJECTIVE To select the superior one from two skin-cleaning disinfection methods so as to reduce the possibility of hospital infection.METHODS The water plus soaps(control group) and Daniel disinfectants(test group) were used separately to clean and disinfect the skin of patients in intensive care unit(ICU).The degree of skin-cleaning of patients and hand pollution of nurses were observed and analyzed.RESULTS The number of bacteria on the skin of patients of the test group was remarkably lower than that of the control group with a statistically significant difference(t=7.94,P

Objective:To observe the efficacy and safety of dexzopiclone plus auricular acupressure in intervening primary insomnia.Methods:A total of 72 participants who met the inclusion criteria were enrolled in a randomized controlled trial,with 36 cases allocated to a treatment group and 36 cases allocated to a control group.Both groups were given dexzopiclone as the routine treatment.Patients in the treatment group were given auricular acupressure with Wang Bu Liu Xing (Semen Vaccariae) seeds at the auricular acupoints related to sleep and emotion based on meridian theory,whereas for patients in the control group,the medical plasters with Wang Bu Liu Xing (Semen Vaccariae) seeds were only gently stuck to acupoints unrelated to sleep without stimulation.Patients in both groups were required to visit the hospital once a week for replacing the seeds and plasters.The course of intervention lasted for 8 weeks and the patients were followed up for another 4 weeks.Pittsburgh sleep quality index (PSQI) and Karolinska sleep diary (KSD) were used to evaluate the outcomes.Meanwhile,adverse effects were monitored and recorded.Results:In the enrolled 72 cases,4 patients (one in the treatment group and three in the control group) reported thirst and a bitter taste,and one case in the control group reported nausea and vomiting.At last,3 cases in the control group dropped out for adverse reactions,and 69 cases completed the clinical trial.After 8 weeks of treatment,the global scores of PSQI in both treatment and control groups decreased significantly compared with the baseline (both P＜0.001).Furthermore,the global score of PSQI in the treatment group was lower than that in the control group (P＜0.01).The global scores of PSQI in both groups at the follow-up were significantly different from the baseline (both P＜0.001),but insignificantly different compared with the post-treatment results (both P＞0.05).According to KSD,both treatment protocols could prolong the total sleep time,shorten sleep-onset latency,improve sleep efficacy and sleep quality significantly,and the changes in the treatment group were more significant.The total effective rate was 88.9％ in the treatment group,higher than 81.8％ in the control group,though the difference was statistically insignificant (P＞0.05).Conclusion:Dexzopiclone plus auricular acupressure is effective and safe for patients with primary insomnia both in short and long terms,and it is more effective than monotherapy of dexzopiclone.
