Objective To explore the early diagnosis of Alport′s syndrome(AS).Methods Renal and skin biopsy was carried out in 5 patients who manifested with isolated hematuria and nephritic syndrome(NS).By using indirect immunofluorescence method,the expression of type Ⅳ collagen ? chains was detected on epidermal basement membrane(EBM) and glomerular basement membrane(GBM).Results ?_1 chains on EBM and GBM were expression in all patients,but ?_5 chains on EBM and ?_3,?_5 chains on GBM form 2 female patients were segmental expression.Thus the goal for early diagnosis was achieved.Conclusions ? chains for EBM type Ⅳ collagen and GBM type Ⅳ collegan should be investigated if condition permits for those patients with isolated hematuria,NS(steroid-resistant) and thinned GBM in electron microscopy.It can be useful for diagnosis and differenial diagnosis of AS.

Objective To investigate the expression of homeobox gene MSX-2 during cranial suture fusion of SD rats and discuss its significance. Methods SD rats aged 1, 2, 5, 8, 12, 15, 18, 22, 30 and 45 days were selected, and immunohistochemistry and Real-time PCR were employed to localize and quantify the expression of MSX-2 in different regions of cranial sutures. Results MSX-2 expressed in calvarial suture tissues including the extreme ends of the osteogenic fronts and the underlying dura mater. The expression of MSX-2 was low in posterior frontal suture (PF) and sagittal suture (SAG) from postnatal day 1 to day 8 before the initiation of suture fusion, while it was higher in PF than in SAG from postnatal day 12 to day 22 after the initiation of PF suture fusion. The expression of MSX-2 significantly declined in PF and was moderately higher than that in SAG from postnatal day 30 to day 45 after the initiation of suture fusion. Conclusion There is different expression of MSX-2 in PF and SAG during different suture fusion periods, which suggests the expression of MSX-2 may participate in the regulation of cranial bone development and the fusion of cranial sutures.

Objective:To examine the expression of nitric oxide synthase(NOS),the content of nitric oxide and blood flow in nasal mucosa of allergic rhinitis guinea pigs,so as to further investigate the mechanism of allergic rhinitis.Methods:One hundred and twenty guinea pigs were randomly divided into control group(injected with normal saline)and allergic group(nasal challenge with egg albumin).The guinea pigs were executed before and immediately,24,48,72 h after the last nasal challenge;the expression of endothelial nitric oxide synthase(eNOS)and inducible nitric oxide synthase(iNOS)and the content of nitric oxide were examined in mucosa tissues.The blood flow in the nasal mucosa was determined in animals before execution.Linear regression correlation was used to analyze the relationship between the nitric oxide content and blood flow in nasal mucosa.Results:The immunostaining for iNOS in surface epithelium of allergic rhinitis guinea pigs was markedly stronger than that of normal guinea pigs at all time points(P

Objective To explore the operative treatment of severe acromioclavicular joint dislocation.Methods 15 cases of severe acromioclavicular joint dislocation were diagnosed as type Ⅲ injury according to Allman classification system.11 cases had the fibrocartilage removed,reconstruction of the coracoacromial ligament was done in 7 cases,in 3 cases a cancellous screw was inserted between the clavicle and aromion process and complete reconstruction of the acromio-clavicular ligament was performed and fixation was done with 2 K wires.Results The follow-up ranged from 10 months to 6 years,reviewuation of the results was done according to Karlsson.12 cases were graded as A,3B,all patients resumed their preoperative activities.Soft tissue calcification in the coracoclavicular gap in 10 cases,the acromial joint movement was not affected.Conclusion The patients with severe acromiocavicular joint dislocation should be treated with surgery as early as possible.Cross internal fixation with K wires is a simple and effective method,the final outcome is not affected by the resection of fibrocartilage and restoration of coracoacromial ligament.

Objective To study the relationship of clinical manifestation and pathological changes and prognosis in Henoch-Schonlein purpura nephritis(HSPN)in children.Methods Clinical and pathological characteristics of 42 children with HSPN were analysed.Among them,40 children were detected of angiotensin-convertion enzyme(ACE)gene and had been followed up.Results Among them,there were 9 cases of level Ⅰof pathological types,21 cases of level Ⅱ,12 cases of level Ⅲ,but no cases of level Ⅳ.Ⅰand Ⅱ level were found in those cases of clinical manifestation with solitary hematuria and albuminuria.Pathological grades were Ⅰ,Ⅱ and Ⅲ levels in the cases of hematuria and albuminuria.Pathological types of nephrotic syndrome(NS)were Ⅱ and Ⅲ level,which were of more gross hematuria than those of other grades.ACE gene DD had serious pathological damnification.Conclusions Change of pathology cannot only be anticipated by clinical manifestation of HSPN.But if pathological damnification gets more serious,the albuminuria gets more serious.Gross hematuria and albuminuria can serve as indicators of biopsy.NS of ACE DD type have serious pathological damnification.Children with HSPN has favourable prognosis in the future.

OBJECTIVETo determine the concentration of schisandrin in Shengmaiyin with HPLC.

METHODThe sample was extracted with ethyl acetate through supersonic wave. The solution was filtrated and evaporated. The residue was resolved with methanol and determined by HPLC using PHENOMENEX C18 (4.6 mm x 250 mm, 5 microns) chromatographic column, methanol-acetonitrile-water (15:15:10) as mobile phase. The wavelength for detection was 254 nm.

RESULTThe peak of schizandrin appears on about 7.10 minutes. The standard curves of schizandrin were linear in the concentration range of 0.2-2.0 micrograms, r = 0.9996. The average recovery of schizandrin were 100.5% (RSD 2.84%).

CONCLUSIONThis method was found to be sensitive, quick and accurate for the measurement of schizandrin concentrations in Shengmaiyin.

AIM: To investigate the efficacy and safety of needle revision with 5-fluorouracil (5-FU) on the dysfunctional filtering blebs after trabeculectomy and to assess the factors that may impact the success.
METHODS:Eighty- three eyes in 76 patients underwent the needle revision and 5-FU subconjunctive injection for the dysfunctional blebs after trabeculectomy and were followed up for 12mo. The intraocular pressure ( IOP), the number of drugs, corneal endothclium, bleb morphology and complications were observed and recorded.
RESULTS: IOP decreased significantly from 35. 3 ±5. 8mmHg(1kPa = 7. 5mmHg) of pre - needling to 17. 0 ±4. 3mmHg of post - needling ( P < 0. 01 ); the average numbers of medications decreased significantly from 1. 7±0. 9 of pre-needing to 0. 4±0. 7 of post-needing (P<0. 01). At 12mo after needling, the success rate of filtering blebs was 89. 2% and the complete success rate was 69. 9%. The Kaplan - Meier survival analysis estimated mean survival period was 11. 0mo (95% CI: 10. 3 - 11. 6). Statistically, there were no significant difference on needling effect with reference to the types of glaucoma, the use of mitomycin C ( MMC ) during the previous filtration surgery, the ages of patients, the intervals of needling operation from previous trabeculectomy, while there were significant difference on needling effect with reference to bleb appearance before needling, and the mean number of needling in patients that had surgery within 3mo were less than those who had surgery for more than 3mo.
CONCLUSION: The needle revision combined with 5-FU is a safe, effective and simple method. Dysfunctional blebs should be treated early after trabeculectomy.

Objective To investigate the clinical features and treatment outcomes of patients with neuropathic pain.Methods We analyzed the relationship between treatment outcomes and clinical characteristics,such as clinical type,duration and age,in 74 patients with neuropathic pain,admitted to our hospital from February 2012 to December 2012.The efficacy and incidence of adverse effects of patients used gabapentin and pregabalin were evaluated three weeks after treatment.Results Among the 74 patients with neuropathic pain,patients with peripheral neuropathic pain (n=60,81.1％)was much more than those of central neuropathic pain (n=12,16.2％),and the cure rate was significantly higher in the patients with peripheral neuropathic pain (31.7％ vs.0％; x2=5.162,P=0.028).The visual analogue scale (VAS) scores in pain (2.21±1.55 points) after 3 weeks treatment was significantly decreased as compared with those in the baseline period (6.38±1.14 points,t=18.644,P=0.000).Sleep and mood disorders after treatment were significantly improved as compared with those of baseline (P＜0.05).The efficacy of pregabalin-treated patients was more pronounced than that of gabapentin-treated group with significant difference (P＜0.05).Only nine patients (12.1％) presented with transient dizziness,fatigue,peripheral edema and other adverse reactions.Conclusion As compared with the central neuropathic pain,peripheral neuropathic pain is more common and its cure rate is relatively high; early standard treatments not only contribute to the patient' s recovery but also improve quality of life.

OBJECTIVETo study the incidences of comorbidities and behavioral problems in children with functional articulation disorders.

METHODSOne hundred and twelve children with functional articulation disorders (aged 4-11 years) were enrolled. Their comorbidities were identified based on clinical investigations and the DSM-IV diagnosis criteria of attention deficit hyperactivity disorder (ADHD), stuttering, tic disorders and enuresis. Behavioral problems were evaluated by the Conners Parent Symptom Questionnaire and the Child Behavior Checklist.

RESULTSSixty-nine patients (61.6%) had one or more comorbidities. The incidence of comorbidity in children with middle-severe functional articulation disorders was higher than in those with mild disorders. The most common comorbidity was language impairment (30.4%), followed by stuttering (16.1%), enuresis (13.4%), and tic disorders (6.3%). In school age children, ADHD (47.5%) was the most common comorbidity. The incidence of behavioral problems was 40.2% by the Child Behavior Checklist and 57.1% by the Parent Symptom Questionnaire.

CONCLUSIONSThe children with functional articulation disorders have high incidence of comorbidity and many behavioral problems.

Objective To determine the diagnostic value of plain CT in tuberous goiter and thyroid papillary carcinoma.Methods The CT images including 63 lesions from 58 patients with thyroid papillary carcinoma and 103 lesions from 86 patients with tuberous goiter proven by pathology were reviewed retrospectively.The analyses were mainly focus on the internal density,the presence of calcification, andthe relation of the lesion with the normal thyroid and adjacent tissue.Results There were statistically significant differences in the internal density,the morphology of calcification and the relation of the lesion with normal thyroid and adjacent tissue between the papillary carcinoma and tuberous goiter lesions.The common CT appearances of papillary carcinoma included uniform density,crab pattern change,micro calcifications and unclear boundaries of the lesion.Focal necrosis and cystic changes were also occasionally found in papillary carcinoma and usually occured at the peripheral area.Tuberous goiters usually manifested as mixed density with macro or eggshell calcifications as well as clear boundaries both with normal thyroid tissue and adjacent tissue.Conclusion The plain CT helps for the preliminary distinction of tuberous goiter and thyroid papillary carcinoma.
