1.Study on Quality Standard of Hovenia dulcis Thunb Extract

Objective To establish a quality standard of Hovenia dulics Thunb. Method The presence of quercetin was identified and assayed by TLC and HPLC, respectively. Result Linearity of marker was obtained over the range of 1.32 ~ 5.28 ?g (r =0.9999). The average recovery rate was 98.78% (RSD=1.08%). Conclusion TLC is specific. The method of quality is accurate, reappearance, simple, rapid, and suitable for the quality control of Hovenia dulcis Thunb.
2.Clinical characteristics and antibiotics resistance of pseudomonas aeruginosa infection in pediatric intensive care unit

Objective To explore the clinical characteristics of critically ill children infected with pseudomonas aeruginosa(PA) and PA antibiotics resistance in pediatric intensive care unit (PICU).Methods Case records of children with PA infection admitted to PICU in children′s hospital affiliated to Shanghai Jiaotong University from Jan 2007 to Dec 2009 were reviewed for clinical characteristics,case fatality rate,prognosis and drug resistance.Results (1) Clinical features:12 cases were community-acquired infection and 46 cases were hospital-acquired infections in 58 cases.On the same period,hospital-wide surveillance obtained PA 232 strains,PICU obtained PA 112,the ratio was 48.3%.Twelve cases died and total mortality was 20.7%.The mortality was significantly difference between community-acquired infections (5 cases,41.6%)and hospital-acquired infections (7 cases,15.2%)(P<0.05).The main symptom of children with community-acquired infections were intestinal infection (5 cases) and sepsis (5 cases).The children had acute onset and developed to shock and multiple organ dysfunction syndrome rapidly.Laboratory examination revealed the white blood cell normal (7/12) and decreased in 5 cases (5/12).The value of C-reactive protein was increased significantly,and the concentration of blood endotoxin were also increased.In the hospital-acquired PA infection cases,the main symptom was respiratory abnormal (38 cases),worsen primary disease,extended staying days in PICU.(2)Drug resistance analysis:112 PA,69.8% of ceftazidime-resistant,72.8% of the imipenem-resistant.Conclusion There is significant difference of the clinical features between PA community-acquired infection and hospital-acquired infection.The former is mostly primary infections with high fatality rate.PA hospital-acquired infection has become an important pathogen of nosocomial infection in PICU.And it is important to prevent PA infection caused by a long term broad-spectrum antibiotics application and invasive medical procedures.

Epithelial-mesenchymal transition (EMT) refers to tne transition during which epithelial cells undergo the loss of apical-basal polarity, acquisition of migration capability and transformation into mesenchymal cells. EMT induces breast cancer in situ to developing into metastasis and associates with the drug resistence. The multiple elements including signal pathways, transcriptional factors and downstream genes orchestrate the transition. Among them, the transforming growth factor β (TGF-β) signaling pathway plays a key role in the regulation of EMT in breast cancer. And this paper reviews the development of TGF-β signaling pathway induced EMT in breast cancer.

4.Clinical study ofShaoyao-Gancao decoction combined withSiwu decoction for the treatment of diabetic peripheral neuropathy

ObjectiveTo explore clinical effect and safety ofShaoyao-Gancao decoction combined withSiwu decoction for the patients with diabetic peripheral neuropathy (DPN).MethodsA total of 76 patients with DPN in our hospital from 2013 July to October 2015 were divided into control group and observation group by the random number method, 38 patients in each group. The control group were treated with conventional treatment combined with oryzanol injection, and the observation group were treated with Shaoyao-Gancaodecoction combined withSiwu decoction on the basis of control group. Both groups were treated for one month. The level of FPG, 2 hPG, HbAlc, TC, TG, LDL-C and HDL-C was detected respectively before and after treatment. The whole blood viscosity and plasma viscosity was detected with LBY-N6A self-cleaning rotary viscosimeter. The red blood cells deposited and platelet aggregation rate were tested with Winchester points vascular method and turbidimetric method, respectively.The leg nerve and sural nerve of nerve conduction velocity (nerve conduction velocity, NCV) were determined with electromyography, at the same time the clinical curative effect was evaluted.ResultsTotal effect rate of observation group was 89.5% (34/38), which was significantly higher than the control group 71.1% (27/38), and the difference was statistically significant (χ2=4.070,P=0.044); After treatment, the levels of FPG (6.12 ± 0.38 mmol/Lvs.6.58 ± 0.52 mmol/L, t=4.403), 2 hPG (7.83 ± 0.82 mmol/Lvs. 8.41 ± 0.93 mmol/L,t=2.884), HbAlc (6.27 ± 0.52%vs. 6.82 ± 0.64%, t=4.112) in the observation group were lower than those in the control group (P<0.05). The levels of TC (4.73 ± 0.83 mmol/L vs. 5.11 ± 0.64 mmol/L,t=52.235), TG (1.83 ± 0.35 mmol/Lvs. 2.03 ± 0.41 mmol/L, t=2.287) and LDL-C (2.91 ± 0.54 mmol/Lvs. 3.25 ± 0.58 mmol/L,t=2.645) in the observation group were lower than those in the control group (P<0.05), and the HDL-C (1.47 ± 0.33 mmol/Lvs. 1.26 ± 0.31 mmol/L,t=-2.859) in the observation group was higher than than that in the control group(P<0.05). The level of whole blood specific viscosity (10.16 ± 2.12 mPa?svs. 11.33 ± 2.51 mPa?s,t=2.195), plasma viscosity (1.24 ± 0.25 mPa?svs. 1.62 ± 0.37 mPa?s,t=5.246), red blood cell volume (0.31 ± 0.16L/Lvs. 0.42 ± 0.08 L/L,t=3.791) and platelet aggregation rate (50.21% ± 8.03%vs. 54.16% ± 7.82%,t=2.172) in the observation group were significantly lower than those in control group (P<0.05). The tibial nerve velocity (45.22 ± 3.20 m/svs. 38.26 ± 5.19 m/s,t=-7.037) and the common nerve velocity (43.22 ± 6.34 m/svs. 36.23 ± 4.81,t=-5.415) in the observation group were as significantly higher than those in control group (P<0.05).ConclusionsThe Shaoyao-Gancaodecoction combined withSiwu decoction could improve the state of blood sugar, blood lipid and blood rheology of patients with DPN, and improve nerve transfer speed of leg nerve and sural nerve, the curative effect is better than conventional western medicine therapy alone.
5.Bioactivity of scFv antibody against human NH-lipopolysaccharide binding protein in vitro and in vivo

Objective To observe the bioactivity of recombinant human scFv antibody against NH-lipopolysaccharide binding protein in vitro and in vivo. Methods KM mice, weighing 20-25 g, were burnt on the back to the third degree covering 20% total body surface area. The mice in only burn group were intraperitoneally injected with 1 ?g/g LPS and those in scFv-treated group with 1 ?g/g LPS and 0.5 ?g/g scFv after burn. The mice without any treatment served as controls. All mice were sacrificed in 1, 3, 6, 12, 24 h after all treatment and the whole blood, the tissues of liver, lung and kidney were collected. The serum concentration of TNF-?, IL-6 in burnt mice with endotoxemia was detected by ELISA and the pathological changes of liver, lung and kidney tissues were observed by light microscope. The inhibition of scFv antibody of different concentrations against NH-lipopolysaccharide binding protein on FITC-LPS binding with U937 cells was assayed by FCM. Results The histopathological changes of liver, lung and kidney in mice of scFv-treated group revealed that less inflammatory cell infiltration, less degenerated cells, weaker congestion in tissue as compared with control and only burnt mice. 6.The Short Term Effect of Insulin on Proinsulin Gene Expression of HIT-T15 Insulinnoma Cells

Objective To investigate the short term effect of insulin on proinsulin gene expression of HIT-T15 insu-linnoma cells(pancreatic isletβ-cell). Methods The HIT-T15 cells were randomly divided into four groups.Blank Con-trol Group (LG):complete medium contain 1.4 mmol/L glucose. Control group (LGC):co-cultured nifedipine with medium in order to restain endogenous insulin release. Experimental group (LINS or HINS) add 0.5 U/L insulin or 5 U/L insulin on top of LGC. After being stimulated for 0, 30, 60, 90, 120 mins, proinsulin (PI) mRNA level were assessed by semi-quantitative RT-PCR. Insulin receptor substrate1 (IRS1) tyrosine phosphorylation was detected by immunocytochemistry. Results (1) Expression of PI was up regulated by both LINS and HINS, and peak at 60 mins. (2) After stimulation for 30 mins, the level of IRS1 tyrosine phosphorylation in the experimental group was significantly higher than control group, and the peak time be-tween LINS and HINS was different. (3) Between group of LG and LGC, the expression of PI mRNA and IRS1 tyrosine phos-phorylation show no difference. Conclusion Short term exogenous insulin stimulation can promote expression of proinsulin genes,which is concentration dependent. The expression and regulation of PI were related with IRS1 signal transduction.
7.Brachial-ankle pulse wave velocity correlates to urinaryprotein in patients with type 2 diabetes mellitus

Objective Urinary protein is closely related to the development and progression of major vascular lesionsin diabetic patients.This study was to investigate the correlation between brachial-ankle pulse wave velocity(baPWV) and urinary protein in patients with type 2 diabetes mellitus ( T2DM) . Methods According to the level of 24-hour urinary albumin ( UAL) , 142 T2DM patients were divided into three groups:control (UA<30 mg, n=84), microalbuminuria(UAL<300 mg, n=26), and clinical albuminuria group ( UAL≥300 mg, n=33) .All the patients underwent examination offasting blood glucose, blood lipid, renal function, HbA1c, islet func-tion, C-reactive protein, blood pressure, and BMI.Their baPWV was determined with an Omron atherosclerosis detector and the correla-tion of baPWV with other factors evaluated by Pearson analysis and multivariate linear regression analysis. Results The baPWVwas significantly higher in themicroalbuminuria([1649.19±229.36]cm/s) and clinical albuminuria ([1759.21±291.05]cm/s) than in the control group ([1522.33±248.37]cm/s) (P<0.05).Pearson analysis showed a positive correlation between 24-hour UAL and baPWV ( r=0.347, P<0.01) and multivariate linear regression analysis manifested the same correlation ( P<0.05) . Conclusion The level of urinary protein is closely correlated with baPWV in T2DM patients.
8.Clinical analysis of multiply antimicrobial-resistant bacteria and monitoring of antimicrobial resistance

Objective To provide an effective basis for clinical control methods of multi-drug-resistant bacterial (MDRB) infections by analyzing the distribution and antimicrobial resistance of MDRB.Methods The French Merieux ATB Expression Automated Analysis System was used for bacterial identification,whereas a drug susceptibility testing was performed by K-B methods.Drug-resistance rate was calculated,and the predisposing factors were analyzed.Results Altogether 811(8.1%) strains were isolated from 9 954 specimens,and the majority of multiply antimircobial-resistant bacteria were Escherichia coli,Coagulase-negative staphylococci,Klebsiella Pneumoniae Staphylococcus aureus,Acinetobacter Baumannii,Pseudommonas aeruginosa,whereas the last two appeared pan resistant strains.Specimen source was mainly from respiratory specimens,accounted for 47.8%,and was mainly distributed in the ICU unit,atout41.8% ;MDRB enterobacter was highly sensitive to Carbapenems with resistance rates less than 1.5% and to Amikacin and other inhibitor drugs that rate was less than 30.0%.The resistance rates of MDRB nonfermentative bacteria was > 77.0% to Carbapenem antibacterial drugs whereas to non-resistance was found to polymyxin and only 20.0% resistance rate to Cefoperazone/sulbactam.MDRB staphylococcus was 100.0% sensitive to Vancomyci,Teicoplanin and Linezolid and less sensitive (< 30.0%) to chloramphenicol and rifampicin.MDRB showed high resistance rate to other antibacterial drugs.The predisposing factors included age,other disease,hospitalization over two weeks,the usage of multiply antimicrobial especially cephalosporins overtoppinh 7 days,and invasive operations.Conclusion The major MDRBs are resistant to common-used antimicrobial drugs.It is nesessary to pay attention to the differences.
9.Impact of nursing intervention mode of parent-child interactive on the adaption of postpartum family roles

Objective To know the impact of nursing intervention mode of parent-child interactive on the adaption of postpartum family roles.Methods One hundred families (maternal,husbands,baby at birth) were divided into the intervention group and the control group.The intervention group was cared by the responsibility nurses who were trained to master the parent-child interactive nursing intervention.The control group was cared by conventional nursing methods.The role adaptation of the fathers and the mothers of the two groups,and self-confidence of the parents to take care of newborns were observed.Results In the intervention group,the role adaptation of the fathers and the mothers was better than the control group; self-confidence of taking care of the newborn was better than the control group.The sleep time of the newborns was prolonged and the crying times were less than the control group.Conclusions The parent-child interactive nursing intervention model can help the family to adapt to the father and the mother role as soon as possible; teach the parents to comprehensively understand newborns,give stimulation of touch,sight,hearing,smell,so as to achieve positive response of newborns and increase the confidence of rearing newborns of the parents.
10.Application of intraoperative cell salvage combined with leukocyte depletion filter on radical nephrectomy for renal carcinoma with inferior vena cava tumor thrombus:2 case reports

Intraoperative cell salvage (IOCS) has been widely used to reduce allogeneic blood transfusion and prevent blood transfusion related complications during surgery.However, due to the risk of transfusion related reaction, contamination, and immunological reaction, its use for tumor patients has been controversial and limited.To explore the feasibility of the application of IOCS in cancer patients, we reported 2 cases of renal cell carcinoma (RCC) with tumor embolism in IVC that underwent radical nephrectomy,and inferior caval venous thrombectomy receiving IOCS combined with leukocyte depletion filter (LDF) from August 2016 to November 2016 in our hospital.The cell saver blood salvage (Haemone-tics, 5+) was used for these 2 cases.The salvaged blood was filtered through the LDF before infusion.For case 1 (male 45-year-old) the total operation time was 505 min, and the estimated blood loss was 4 500 ml.A total of 1 000 mL autologous blood, 12 u allogeneic packed red blood cells (PRBC), 1 200 mL fresh frozen plasma (FFP) were infused during the procedure.The patient was discharged from hospital after 75 days without complications.The postoperative follow-up for 3 months showed no tumor recurrence or metastasis.For case 2 (a male patient, aged 51 years), the total operation time was 490 min, and the estimated (blood loss was 7 000 mL.the patient received 2 700 mL autologous blood transfusion, 12 u allogeneic packed red blood cells (PRBC), and 2 400 mL fresh frozen plasma (FFP).The patient was discharged from hospital after 86 days without severe complications.the postoperative follow-up for 6 months showed no tumor recurrence or metastasis for this patient.Other relevant retrospective studies with this technique showed that cell salvage could be used safely in many kinds of cancer patients, for example, bladder cancer, liver cancer and kidney cancer.Furthermore,numerous researches have proved the safety and efficacy of the combination of these two techniques, the intraoperative cell salvage technique and leucocyte depletion filter in cancer patients.Here we only reported two cases using IOCS and LDF.Further work is needed to determine whether the use of intraoperative cell salvage combined with leukocyte depletion filter can be used safely for patients with HCC.
