1.Impacts on the lower limb motor function in children with spastic cerebral palsy treated by Jin three-needle therapy combined with MOTOmed intelligent motor training.

OBJECTIVETo compare the differences in the efficacy on lower limb motor function in children with spastic cerebral palsy between the combined therapeutic program of Jin three-needle therapy and MOTOmed intelligent motor training and the simple MOTOmed.
METHODSEighty children with spastic cerebral palsy were randomized into an observation group and a control group, 40 cases in each group. In the observation group, Jin three-needle therapy was used at four mind points, three brain points, three intelligent points, three temporal points, three knee points and three foot points, as well as Chengjin (BL 56), Chengshen (BL 57) and the other. Additionally, MOTOmed was given in assistance with conventional rehabilitation training. In the control group, MOTOmed and the conventional rehabilitation training were adopted. In the two groups, the treatment was given once a day, 20 treatments made one session and there were 3 to 5 days at interval between the two sessions. The efficacy evaluation was performed after continuous 3 sessions of treatment. The gross motor function measure (GMFM) and modified Ashworth scale (MAS) were applied for the evaluation before and after treatment in the two groups.
RESULTSAfter 3 sessions of treatment, GMFM score was increased apparently as compared with that before treatment in the two groups (P < 0.01, P < 0.05). After treatment, GMFM score in the observation group was increased more remarkably than that in the control group (55.32 +/- 17.10 vs 47.48 +/- 17.18, P < 0.05). After treatment, the total effective rate of gastrocnemius Ashworth spasm grade [90.0% (36/40)] in the observation group was better apparently than [72.5% (29/40)] in the control group (P < 0.05).
CONCLUSIONThe combined therapeutic program of Jin three-needle therapy and MOTOmet intelligent motor training improves the lower limb motor function of the children with spastic cerebral palsy and its efficacy is better than simple MOTOmed.

2.Development of Computer Real Time Monitoring System for Rational Clinical Use of Drugs

OBJECTIVE:To promote rational clinical use of drugs by the aid of computer.METHODS:The databases for basic data of drugs,irrational use of commonly-used drugs,compatibility of agents for injection and consultation for drug use in periods of prequancy and lactation were established.Modules for inquiry,addition,revision,deletion and maintaince were designed.The computer real time monitoring system for rational clinical use of drugs was thus developed.RESULTS&CONCLUSION:This system is simple,flexible and quick,and can check the prescription and provide consultation for compatibility between drugs.It is suitable for extensive use in clinical practice.
3.Evaluation of Microcirculation of Pancreatic Carcinoma Using Whole Organ CT Perfusion Imaging

Purpose To analyze the perfusion differences of different pancreatic diseases using the low-dose whole organ dynamic volume CT perfusion imaging, and to provide the evidence for the clinical application. Materials and Methods Twenty-eight patients suspected as pancreatic disease were applied by 640 layer volume CT perfusion imaging for the pancreas. Data were collected at the same time of bolus injection of contrast agent, then were analyzed by spatial alignment and perfusion calculation using the perfusion software. The time-density curve, blood perfusion flow diagram and tissue artery blood flow were obtained using the maximum slope method. Results Normal pancreatic tissue (n=9) blood flow was (117.04±12.05) ml/(min?100 ml), pancreatitis organizations (6 cases with acute pancreatitis and 3 cases with chronic pancreatitis) (118.67±37.18) ml/(min?100 ml), pancreatic carcinoma tissue (n=10) was (67.16±18.94) ml/(min?100 ml). There was significant difference among three groups (F=8.59, P<0.001);the difference was demonstrated in pancreatic cancer vs. normal pancreas and pancreatic cancer vs. pancreatitis group (q=3.70, P<0.05), which could be clearly demonstrated by blood perfusion flow diagram. The difference was not statistically revealed pancreatitis and normal pancreas group (q=2.91, P>0.05). The total dose of X-rays in the whole scanning process was 21.5-23.9 mSv. Conclusion Low-dose whole pancreas organ CT perfusion scan can quantitatively analyze the hemodynamic changes in pancreatic disease, which be of great value for evaluating changes in microcirculation during the treatment of pancreatic cancer.
4.Correlation between low tube voltage in dual source CT coronary artery imaging with image quality and radiation dose.

The influence of low tube voltage in dual source CT (DSCT) coronary artery imaging on image quality and radiation dose and its application value in clinical practice were investigated. Totally, 300 cases of chest pain with low body mass index (BMI <18.5 kg/m(2)) subjected to DSCT coronary artery imaging were prospectively enrolled. The heart rate in all patients were greater than 65/min. The retrospective ECG gated scanning mode and simple random sampling method were used to assign the patients into groups A, B and C (n=100 each). The patients in groups A, B and C experienced 120-, 100-, and 80-kV tube voltage imaging respectively, and the image quality was evaluated. The CT volume dose index (CTDIvol) and dose length product (DLP) were recorded, and the effective dose (ED) was calculated in each group. The image quality scores and radiation doses in groups were compared, and the influence of tube voltage on image quality and radiation dose was analyzed. The results showed that the excellent rate of image quality in groups A, B and C was 95.69%, 94.72% and 96.33% respectively with the difference being not statistically significant among the three groups (P>0.05). The CTDIvol values in groups A, B and C were 51.35±12.21, 21.28±7.13 and 6.34±3.34 mGy, respectively, with the difference being statistically significant (P<0.05). The ED values in groups A, B and C were 9.27±1.63, 4.56±2.29 and 2.29±1.69 mSv, respectively, with the difference being statistically significant (P<0.05). It was suggested that for the patients with low BMI, the application of DSCT coronary artery imaging with low tube voltage can obtain satisfactory image quality, and simultaneously, significantly reduce the radiation dose.
6.Operative treatment and curative effects of the deltoid ligament injuries of the ankle joint.

OBJECTIVETo explore the operative methods and curative effects of the deltoid ligament injuries.
METHODSFrom 2002 to 2008, all 61 patients with ankle fractures complicated with deltoid ligament injuries were treated with open reduction and firm internal fixation. Among the patients, 39 patients were male and 22 patients were female, ranging in age from 14 to 71 years, with an average of 41 years. During the operation, the deltoid ligament was reconstructed to restore the medial and lateral stability of ankle joint.
RESULTSAll the patients were followed up ranged from 5 to 30 months, with an average of 17 months. Fifty-nine patients had incision healed at the first stage; 2 patients had superficial infections at lateral malleolus, and healed at the 3rd week after changing dressings. The incisions at the internal medial malleolus were all healed at the first stage. According to Qi evaluation criteria, 35 patients got an excellent result, 13 good and 13 fair.
CONCLUSIONThe deltoid ligament should be treated properly in the treatment of ankle joint fractures when open reduction and firm internal fixation were emphasized.

7.Cost-effectiveness analysis for integrated prevention of mother-to-child transmission of HIV.

OBJECTIVETo evaluate the cost-effectiveness and economic efficiency of integrated prevention of mother-to-child transmission (PMTCT) of HIV in four high-incidence counties.
METHODSData of local resource investment and total cost for PMTCT in 4 counties in China from 2003 to 2006 were collected. Cost analysis and cost-effectiveness analysis were conducted. Average costs of a confirmed HIV case, a prevented case and a disability-adjusted life-year (DALY) saving were calculated.
RESULTSAverage cost of identifying one HIV-infected mother was yen5512. Costs of a pediatric HIV case prevention and per DALY saving were yen46 747 and yen1870 ($231), respectively, based on the total cost perspective.
CONCLUSIONThe cost of integrated prevention of mother-to-child transmission of HIV was low. The PMTCT program was economical efficiency.

8.The accuracy of real-time continuous monitoring system at different stages and its association with glucose excursion

Objective To study the accuracy of real-time continuous monitoring system (RT-CGMS) at different stages and its association with glucose excursion. Methods Totally 33 patients with type 1 diabetes or type 2diabetes were under surveillance of RT-CGMS for 5 d. Capillary glucose values were measured 7 times daily.Correlation coefficient, error grid analysis (EGA), and Bland-Altman analysis methods were used to assess the correlation, accuracy and agreement of RT-CGMS at different stages and in general level; The mean amplitude of glucose excursion (MAGE) and the frequency of glucose excursion ( FGE ) were also calculated. Results ( 1 ) The correlation coefficient of RT-CGMS with capillary glucose values at fasting, postprandial stages, and in general level were 0.94,0.92, and 0.93 respectively( P<0.01 ). (2) EGA showed that 98.82%, 98.39%, and 98.64% of the results fell in the A and B zones and 1. 18%, 1.61%, and 1.36% fell in the D zone respectively at fasting,postprandial stages, and in general level. There is no result fell in C and E zones. ( 3 ) The agreement analysis showed that RT-CGMS readings were in close agreement with capillary glucose values at fasting, postprandial periods, and in general level. (4)The MAGE at fasting, postprandial periods, and in general level were (3.57±2.66), (4.07±3.09), and (4. 02 ±3.04) mmol/L (P>0. 05), (0±0. 5), (3± 1), and( 1 ±3) d for FGE (P<0. 01 ).Conclusion RT-CGMS at fasting stage has higher accuracy than postprandial stage and general level, FGE at fasting stage is higher than postprandial stage and general level.
10.2-dimensional gel electrophoresis on protein patterns from suspension cultures of Taxus cuspidate induced by SA.

The permeability of cell membrane was enhanced by exogenous SA in the culture of Taxus cuspidata. Nuclei condense and fragments were observed in some cells by using fluorescent microscope, and the degraded chromosomal DNA was observed by using agarose gel electrophoresis. The changes in soluble proteins of the suspension cultures of Taxus cuspidata induced by salicylic acids were analyzed by using two-dimensional polyacrylamide gel electrophoresis. Comparing with the control, seven new protein spots were found and six protein spots were not found in the cultures grown with SA at 48h. The results obtained showed that SA could induce the expression of some special proteins that might be related with the action of SA in the suspension cultures of Taxus cuspidata.
