Objective To evaluate the efficacy and safety of linezolid in the treatment of gram-positive coccus in-fection in the elderly. Methods Clinical data of patients (>60 years old)infected with gram-positive coccus and treated with linezolid for 10 days between January 2013 and December 2014 were collected, the therapeutic efficacy of linezolid were analyzed, laboratory indexes before and 14 days after linezolid treatment were compared, possible adverse effects were analyzed. Results A total of 70 old patients were enrolled, the majority of patients were infec-ted in lower respiratory tract (62.86%)and were infected with Staphylococcus aureus (42.86%,of which 19 were MRSA),more than 80% of the patients were >70 years old,had length of stay > 30 days, and admitted in ICU, more than 70% of the patients were with deep venous catheterization and indwelling urinary catheterization.Platelet count (PLT)after 14 days of linezolid treatment was significantly lower than before treatment([132.00±45.00]× 109/L vs [156.00±78.00]×109/L,P =0.009);the total therapeutic efficacy of linezolid was 81 .43%(57/70), while the rate of adverse effects was 17.14% (12/70).Conclusion Linezolid is effective for treatment of gram-posi-tive coccus infection in the elderly, and may be a good choice of empirical treatment.PLT should be intensively mo-nitored during the process of linezolid therapy.

Objective To evaluate the clinical application of contrast-enhanced digital subtraction MRI in vertebral metastases. Methods 50 patients with vertebral metastases were scanned by contrast-enhanced T_1WI with an injection of Gd-DTPA(0.1 mmol/kg).Digital subtraction were performed between post-contrast-enhanced images and pre-contrast-enhanced images.Vertebral metastases were evaluated by means of a contrast to noise ratio(CNR) and signal to noise ratio(SNR),and boundaries were compared between subtraction MRI images and conventional contrast T_1WI MR images. Results Contrast-enhanced digital subtraction MRI images were better than conventional contrast T_1WI MR images in showing the lesions. All lesions had a higher CNR and SNR on subtraction MR imaging than that on conventional contrast T_1WI MR images.CNR was 155.05 ? 24.73(mean ? standard error) for subtraction MR imaging and11.11?14.26 for conventional contrast T_1WI MR images,(t=35.65,P

Objective To observe the effects of hyperbaric oxygen combined with rehabilitation training on the motor ability of ischemic stroke patients. Methods Eighty ischemic stroke patients were randomly divided into a treatment group and a control group, with 40 cases in each group. The control group was treated with Bobath' s approach, while the treatment group was treated with Bobath's methods supplemented by hyperbaric oxygen therapy. The United States National Institutes of Health Stroke Scale (NIHSS),the Fugl-Meyer motor assessment scale (FMA) (lower part) and each patient's maximum walking speed (MWS) were used to evaluate the patients before, and after 8 weeks of treatment. Results After treatment the average NIHSS score,FMA score and MWS were 4.17 ± 1.4%,31.2 ± 3.3 and 54,.3 ± 16.2 m/min,respectively,in the treatment group. The control group' s results were 6.81 ± 1.2%,26.2 ± 2.2 and 45.6 ± 18.3 m/min.The intra-group differences in evaluation results before and after treatment were statistically significant in both groups. An inter-group comparison showed that the treatment group performed significantly better after treatment than the control group in terms of FMA and MWS.After treatment, the treatment group showed significantly better walking performance in terms of cadence, stride length, step length on the affected side, gait cycle and double support duration. Conclusion Hyperbaric oxygen can make rehabilitation training more effective in improving the neurologic deficits, motor function and walking ability of hemiplegic stroke survivors.

Chinese Journal of Practical Internal Medicine 2006;0(S1):-
Objective To compare the effects of every-other-day dose simvastatin administration with that of daily therapy of same dose. Methods This was a randomized, prospective, nonblinded clinical trial. The 186 patients with high low-density lipoproteim cholesterol(LDL-C) and/or high total cholesterol (TC),triglycerides (TG),low high-density lipoprotein cholesterol(HDL-C)was studied. All patients were randomized into two groups. The every-other-day do- sing group recerived 20mg of simvastatin in alternate-day and daily dosing group received 20mg of simvastatin every day. Before and after 6 weeks,12 weeks of treatment, serum lipoprotein,Live function tests and ereatine kinase con centra- tion and so on were drawn and bad-side effect were studies. Results The two groups significantly reduce LDL-C,TC, TG and a little increased HDL-C compared with baseline. No stalistically significant differences existed between the two groups in percentage in decrease in lipoprotain at 6 weeks,12 weeks compared with baseline. The bad-side effect in the two groups also had not a singnificant different. Conclusion The every-other-day dose of simvastatin have similar effi- cacious and safe to daily dosing in patients with hyperlipidemia and some cost savings. It can take a primary prevention to coronary heart disease in patients with relatively low-risk hyperlipidemia.

Objective To improve the diagnosis and management of parapharyngeal space(PPS)neoplasms. Methods Records of 85 patients with PPS neoplasms operated in 11 years were reviewed with regard to presenting signs and symptoms, histological characteristics, imaging technique, surgical approach and postoperative complica- tions. Results Benign rumors were found in 77 cases and malignant tumors were founded in 8 cases.Forty-one tu- mors originated from salivary glands;neurogenic tumors were found in 36 patients and 8 patients with tumors in oth- er organs. All were treated by surgical operation:the trans-eervical approach was used in 76 cases,tans-oral in 6 cases and the combined transoral-cervical approach in 3 cases. The complications after operation were facial nerve palsy(4 cases),hypoglossal nerve paralysis(2 cases)Horner syndrome(2 cases),vocal cord paralysis(2 cases),thyroid gland fistula(1 case),infects(1 case)and difficulty in breathing(1 case). Conclusion Tumors originated from salivary glands were the most common neoplasms in the primary parapharyngeal space neoplasms, and neurogenic tumors fol- lowed.High-resolution computed tomography or magnetic resonance imaging was essential for diagnosis and presurgi- cal planning. The transcervical approach was the preferred procedure for PPS tumors.
