2.Operation Coordination for the da Vinci Robot Surgical System

[Summary] This paper reported the surgical cooperation in 72 cases of da Vinci robot operations between September 2014 and December 2014 in this hospital .All the 72 cases of total laparoscopic operation by using the da Vinci robot assistance were successfully completed, without robot faults caused by mismatch and nursing complications caused by improper nursing .We believe that nurse training with robot surgical system and qualification certification , preoperative patient visits , reasonable arrangement of operation room , careful observation with skilled coordination with the robot , proper postoperative placement of the robot , and maintenance and disinfection of the machine are crucial to a successful surgery .
6.Effect of Naoxintong combined with edaravone on efficacy and vascular endothelial function of patients with type-2 diabetes complicated with cerebral infarction

Objective To explore influence of Naoxintong capsule combined with edaravone on clinical efficacy and vascular endothelial function of patients with type-2 diabetes complicated with cerebral infarction.Methods 80 cases of patients with type-2 diabetes complicated with cerebral infarction treated in Shengsi People's Hospital in Zhejiang from January 2014 to January 2016 were selected as the objects, and were divided into control group and study group randomly, forty-one cases in each group.The control group were given edaravone treatment, and the study group were added Naoxintong capsule on the basis of the control group, treatment for two weeks in the two groups.Clinical therapeutic effect of the two groups and changes of TCM syndrome score before and after treatment were compared, and scores of nerve function defect and daily life ability were evaluated, and changes of blood glucose and glycosylated hemoglobin levels were detected, and endothelin ( ET-1 ) , nitric oxide ( NO ) , thromboxane B2 ( TXB2 ) and 6-keto prostaglandin F1(6-keto-PGF1) were tested before and after treatment in the two groups.Adverse reactions were observed in the two groups in treatment. Results The total effective rate of the study group was significantly higher than that of the control group(P<0.05), and TCM syndrome scores of the two groups were significantly decreased after treatment in the two groups(P<0.05), which of the study group was significantly lower than that of the control group(P<0.05).After treatment, NIHSS score of the two groups significantly decreased(P<0.05), and ADL score significantly increased(P<0.05), and changes of the study group were significantly higher than those of the control group(P <0.05).Fasting blood glucose and glycosylated hemoglobin levels significantly decreased in the two groups(P<0.05), which of the study group were significantly lower than those of the control group ( P <0.05 ).Levels of ET-1 and TXB2 significantly decreased ( P <0.05 ) , levels of NO and 6-keto-PGF1 significantly increased ( P <0.05 ) , and changes in the study group were significantly higher than those in the control group(P<0.05).There was no significant difference in incidence of adverse reactions between the two groups.Conclusion The effect is obvious of Naoxintong capsule combined with edaravone in treratment of patients with type-2 diabetes complicated with cerebral infarction, and can improve vascular endothelial function of patients.It can be widely used in clinical.
7.Efficacy of transcutaneous electrical acupoint stimulation (TEAS) for menopausal insomnia and its effect on serum hormone levels

Objective: To explore the therapeutic efficacy and mechanism of transcutaneous electrical acupoint stimulation (TEAS) for menopausal insomnia. Methods: A total of 80 patients with menopausal insomnia were randomly divided into a control group and an observation group, with 40 cases in each group. The patients in the control group received conventional Western medication treatment, and the patients in the observation group received TEAS on the basis of conventional Western medication treatment. The treatment for both groups lasted for 4 weeks. Before and after treatment, Pittsburgh sleep quality index (PSQI) and modified Kupperman scale were evaluated, and the serum levels of estradiol (E2) and follicular stimulating hormone (FSH) were measured. The therapeutic efficacy was evaluated after treatment. Results: After treatment, the total effective rate of the observation group was significantly higher than that of the control group (P<0.05); in the control group, the improvement of PSQI score was significant (P<0.05), while the change of modified Kupperman score was insignificant (P>0.05); the PSQI and Kupperman scores in the observation group were significantly improved after treatment (both P<0.05), and there were significant differences between the observation group and the control group in PSQI and Kupperman scores (both P<0.05). After treatment, the serum E2 and FSH levels in the control group were not statistically different from those before treatment (both P>0.05); the serum E2 level was significantly increased (P<0.05), and the FSH level was decreased (P<0.05) in the observation group after treatment, and the between- group differences in serum levels of E2 and FSH were significant (both P<0.05).Conclusion: TEAS plus conventional Western medication in treating menopausal insomnia is effective, and can significantly improve the symptoms of insomnia and menopause, which may be related to the regulation of serum E2 and FSH levels.
8.Relationship between secondary ankylosis of temporomandibular joint and childhood longitudinal fracture of mandibular condyle

BACKGROUND: Much attention has been focused on mandibular condyle fracture, which has long been considered as a severe injury to the temporomandibular joint(TMJ). Restricted by diagnostic methods, many studies have been conducted on transverse fracture whereas little attention has been paid to longitudinal fracture of the mandibular condyle. Even less is known about longitudinal fracture in young children.OBJECTIVE: To evaluate the effects of childhood longitudinal fracture of the mandibular condyle on secondary ankylosis of TMJ.DESIGN: A randomized controlled study.SETTING: Department of Stomatology, General Hospital of Chinese PLA.MATERIALS: The experiment was completed in the Center for Experimental Animals, General Hospital of Chinese PLA. Twelve young Chinese experimental miniature pigs bred by the Institute of Experimental Animals, Beijing Agriculture University, aged 2 -3 months and weighing(5 -5.5) kg, were used and raised with mixed feed, and then divided into 3 groups randomly.METHODS: Food and water was forbidden for the miniature pigs 12 hours before operation. Under anesthesia with(5.0 - 15.0) mg/kg ketamine/xylazine abdominally injected, conventional disinfection was performed in the right lateral decubitus position and layer-by-layer preauricular incisions were made on the left side of the miniature pigs. The joint cyst was incised transversely and the lower cavity of TMJ was exposed and dragged downwards. The mandibular condyle was cleaved into two vertically along the inner 1/3 part of it with a 5.0 mm-wide bone chisel to cause inferior and posterior sagittal fractures. The miniature pigs were executed 3 months and 6 months after operation, and their condyles were cut for observation.MAIN OUTCOME MEASURES:①The morphological changes of mandibular condyle specimens in the three groups observed with the naked eyes.②The pathological changes of mandibular condyle specimens under light microscope.RESULTS: The articular disc was adhered to the mandibular condyle after 3months. Temporal fossa adhered to the articular disc and mandibular condyle could still be observed in one specimen. Bifid condylar deformity was found in two specimens and no disc perforation was found. The adhesions became more severe after 6 months and bifid condylar deformity was formed in all the 3 specimens, with distinctly thickened discs. Light microscope observation:Three months after fracture, the adhesion between disc and condyle was obvious. There was no clear boundary between the surrounding tissues. The adhesion was composed of fibro-connective tissues, with a large number of chondrocytes in it. Fibro-connective tissues tightly adhered to the trabeculae were found in some adhesions. The fiber of disc became deranged and blood vessels and fatty cells were seen. Six months after fracture, blood vessels, a large number of fibroblasts and sparse chondrocytes were seen in the adhesions. The collagen fibers of the discs were arranged in whirlpool shape. There were a large number of fatty cells and blood vessels within it. The fiber-cartilage of the condylar surface was found thickened and the fiber deranged in the unadhered tissues. Proliferative changes were seen on disc surfaces.CONCLUSION: Childhood longitudinal fracture of the mandibular condyle causes severe secondary injury to TMJ, suggesting a close correlation between longitudinal fracture of the mandibular condyle and TMJ ankylosis.
9.Clinical and pathological features of non-Hodgkin′s lymphoma in children

Objective:To retrospectively analyze the clinical data of 73 children with non-Hodgkin′s lymphoma to determine its clinical features, pathological types and relationship with efficacy.Methods:In Shanghai Children′s Hospital from October 2014 to October 2018, 73 children with newly diagnosed non-Hodgkin′s lymphoma were enrolled for a retrospective study, with 52 males and 21 females, aged between 2 and 13 years old, so as to comprehensively assess cumulative organs, pathological diagnosis, stage of disease, surgery and efficacy.Results:Among the 73 children with non-Hodgkin′s lymphoma, 66 cases had a clear pathological diagnosis, including 38 cases of surgical pathology and 20 cases of biopsy pathology.All of them were confirmed by immunohistochemisty, and 8 cases were determined by flow cytometry.Meanwhile, 6 cases of the rest 7 cases obtained approximate pathological classification.Only 1 case was unknown.Among the 66 cases, Burkitt lymphoma was the most common in 31 cases, followed by 14 cases of T-lymphocyte type, 7 cases of enlarged lymphoma, 6 cases of B-lymphocyte type, 5 cases of diffuse large B lymphoma, 1 case of extranodal natural killer/T cell lymphoma-nasal type, 1 case of blastic plasmacytoid dendritic cell neoplasm, and 1 case of panniculitis-like T lymphoma.The initial site or cumulative organ involved multiple organs, with most common in neck(20 cases), abdominal cavity(15 cases), and mediastinum(6 cases). In view of outcomes and efficacy, the complete remission rates of stage Ⅱ, Ⅲ, and Ⅳ in clinical were 100.0%, 85.1%, and 72.2%, respectively.The event-free survival rates in stage Ⅲ and Ⅳ were lower than those in stage Ⅱ(80.9% vs. 66.7% vs. 100.0%). From the perspective of the main pathological types grouping, the results revealed that lymphoblastic lymphoma had a lower event-free survival rate than Burkitt′s lymphoma (73.7% vs.90.3%), while there was no statistical significance(all P>0.05). Conclusions:Non-Hodgkin′s lymphoma in children is more common in men, and less common in infants and young children.Pathological type is simpler than adults, Burkitt lymphoma is the most common type, and clinical stage and pathological type may be related to prognosis.
10.Expression and significance of EGFR and ADAM9 in pancreatic cancer stem cells and differentiated cells

Objective To enrich pancreatic cancer stem cells through culturing mammospheres, and to detect the expressions of epidermal growth factor receptor(EGFR) and a disintegrin and metalloprotease 9 (ADAM9) and investigate their significance. Methods PANC1 cells were cultured in serum-free conditioned medium to continuously generate mammospheres, and parts of mammospheres were cultured on a collagen substratum to induce differentiation. Mammospheres cells and differentiated cells were collected, flow cytometry was used to detect the proportion of side population (SP) cells, and the expressions of EGFR, ADAM9 mRNA and protein were detected by real-time PCR and Western blotting. Results PANC1 cells mammospheres were successfully generated and could be passed continuously. After differentiation, mammospheres cells could regain the ability of adherent growth. The proportion SP cells in mammospheres cells and differentiated cells were ( 5.40 ± 0.38 ) % and (2.80 ± 0.42 ) %, and the difference was statistically significant ( P < 0. 05 ).Compared with differentiated cells, the expression of EGFR and ADAM9 mRNA of mammospheres cells upregulated 2.5 and 3.0 folds ( P < 0. 05 ). The expressions of EGFR and ADAM9 protein of mammospheres cells were 0.90 ± 0. 09 and 0.64 ± 0.07, which were significantly higher than those in differentiated cells (0.62 ±0.11 and 0.48 ±0.09, P <0.05). Conclusions Mammosphere cells contained higher proportion of pancreatic cancer stem cells. ADAM 9 may play an important role in the occurrence and development of pancreatic cancer through the EGFR signaling pathway.
