Objective To explore the main inhalational and alimentary allergens which triggers allergic asthma in children in Suzhou area.Methods The serum specific aspiration allergens were detected with enzyme-linked immunosorbent assay(ELISA) in 119 children with bronchial asthma,including 7 kinds of common inhalational allergens.The 6 kinds of alimentary allergens test was conducted in 65 children.Results The total inhalational allergens positive rate of the subject was 75.63%(90/119).The highest rate was dust mite 49.58%(59/119),then was house dust 21.02%(25/119),animal hair 19.33%(23/119).The positive rate of aspiration allergens increased with the patient′s age and showed a positive correlation.Positive rate of food allergens was 49.23%(32/65).Of them,the highest rate was fish 20.0%(13 cases),then was lobster and claw 16.92%(11 cases),egg 12.31%(8 cases).Conclusions Inhalational allergens are the main allergens in children with asthma in Suzhou district.The positive rate of aspiration allergens increases with the patient′s age.The most common allergen is dust mite.The allergen test plays a clinically significant and advisable role in treatment and prognosis in bronchial asthma in children.

This article has shown the experiences to achieve more effective teaching results by integration of multiple teaching patterns,including case-based,problem-based learning,in the teaching of water-electrolyte metabolism and acid-base balance disorders.

Objective To detect the serum growth hormone(GH),insulin-like growth factor-I(IGF-Ⅰ),insulin-like growth factor binging protein-3(IGFBP-3) levels in children with hypothyroidism and explore the change regulation of GH-IGF axis and thyroxine.Methods Fifty-six hypothyroidism children were divided into congonital hypothyroidism(CH)(14 cases) and Hashimoto's thyroiditis(32 cases).Serum GH,IGF-Ⅰ,IGFBP-3 levels of 56 hypothyroidism children before and after luothyroxine sodium L-T4 treatment and 50 normal children were detected with radioimmunoassay or enzyme-linked immunosorbent assay methods.Results Serum IGF-I,IGFBP-3 levels in 9 CH newborn group were lower,and they were normal after thyroxine substitute therapy.But those of 5 children with CH and 32 cases with Hashimoto's thyroiditis were not significantly changed.Conclusions The GH-IGF axis is inordinate in the CH children,which is an important reason of stature short.Early thyroxine substitute therapy is very helpful to maintain normal growth and development of children with CH.

Objective To explore influencing factors for complete resection and operation time of endoscopic submucosal dissection( ESD) for colorectal tumors. Methods This retrospective study included 95 consecutive colorectal tumors in 88 patients whose pathological diagnosis was adenoma and carcinoma, treated with ESD at the Department of Endoscopy of the First Hospital of Jilin University from January 2013 to December 2014. Multiple logistic regression analysis was conducted on the factors related to complete resection and operation time. Results Average tumor size was 28. 7±14. 1 mm(range,8?80 mm), and the average procedure time was 80. 72±63. 90 min. The rate of complete resection was 92. 6%(88/95),and the rate of incomplete resection was 7. 4%(7/95). Multivariate logistic regression analysis revealed that fibrosis (P=0. 012,OR=52. 473, 95%CI:2. 571?1140. 438) contributed to incomplete resection. Conclusion The severer fibrosis can induce higher rate of incomplete resection. The more severe fibrosis is, the larger tumor size is, and the longer operation time is.

Bilateral lung lavage was performed under general anesthesia with muscle relaxation in 3 patients with pulmonary alveolar proteinosis(PAP)aged 43-57 yrs weighing 48-68 kg.Left lung lavage was performed first and the right lung was lavaged one week later.Left radial artery and right internal jugular vein were cannulated for MAP,HR and CVP monitoring.SpO_2 and P_(ET) CO_2 were monitored during lavage.Left-sided Robertshaw double-lumen tube was used for both right and left lung lavage.After proper placement of the double- lumen tube was verified both lungs were ventilated with 100% O_2 for 10 min to wash out N_2.Hyperoxygenated lavage fluid was made by aerating the lactated Ringer's solution with 100% O_2 at 1 L?min~(-1) flow rate for 30 seconds.The tidal aliquot of lavage was 700 ml which was kept in the lung for 50 seconds and then drained.Each lung was lavaged with hyperoxygenated and plain lactated Ringer's solution 10 times each alternatively.While one lung was being lavaged the contralateral lung was mechanically ventilated with 100% O_2.SpO_2,MAP,HR,CVP and P_(ET)CO_2 were recorded immediately before lavage and at 10,20,30,40,50,60,70 and 80 seconds after the lung was filled with lavage fluid.SpO_2 was significantly higher during lavage with hyperoxygenated fluid than with ordinary lactated Ringer's solution,but there was no significant difference in MAP,HR,CVP and P_(ET)CO_2.The PaO_2 of the 3 patients breathing room air was 46-52 mm Hg before lung lavage and increased to(72.3?2.1) mm Hg one week after left lung lavage and to(83.5?4.8)mm Hg 3d after right lung lavage.During the lung lavage,hyperoxygenic significantly improve oxygen supply in patients with pulmonary alveolar proteinosis compared with lactated Ringer's solution.

Objective:To explore the effect of paclitaxel (PTX) and cisplatin (DDP) on the expression of NKG2D ligands of hu-man esophagus carcinoma cell EC9706 and on the cytotoxicity of cytokine-induced killer (CIK) cells, as well as to discuss its molecu-lar mechanisms. Methods: The half maximal inhibitory concentration (IC50) values of PTX and DDP against EC9706 cells for 24 h were measured by MTT assay. The expression levels of NKG2D ligands (MICA, MICB, ULBP1, ULBP2, and ULBP3) on the EC9706 cell surface before and after 24 h culture with 1/2 IC50 of PTX or DDP were assayed by flow cytometry. Cytotoxicity of CIK cells against EC9706 cells before and after 24 h culture with 1/2 IC50 PTX or DDP was analyzed by lactate dehydrogenase release assay at an effector to target cell ratio (E:T) of 20:1 and 30:1, respectively. The expression levels of DNA damage repair genes (ATM, ATR, CHK1, CHK2, and p53) of EC9706 cells before and after 24 h incubation with 1/2 IC50 PTX or DDP were detected by quantitative fluorescent PCR. Results:The IC50 values of PTX and DDP were 10 and 5μg/mL, respectively. MICB, ULBP2, and ULBP3 on EC9706 cells were upregulated after 24 h culture with 1/2 IC50 PTX (P<0.05), and the expression levels of MICA, MICB, ULBP2, and ULBP3 were higher after 24 h culture with 1/2 IC50 DDP (P<0.05). Conclusion:PTX or DDP can enhance the susceptibility of EC9706 cells to CIK cell-mediated lysis by upregulating the expression of NKG2D ligands through activating DNA damage repair genes.
