1.Advances of tumor necrosis factor-α inhibitor in the treatment of junior idiopathic arthritis associated uveitis
Chinese Journal of Ocular Fundus Diseases 2021;37(5):399-403
Uveitis is the most common extra-articular manifestation of juvenile idiopathic arthritis, typically as chronic anterior uveitis with insidious onset. Delayed and inadequate treatment may result in loss of patients' vision and even blindness. For refractory or severe uveitis related to juvenile idiopathic arthritis, systemic immunosuppressive agents should be used as early as possible. With the advantage of controlling ocular inflammation, avoiding ocular complications and reducing the use of traditional immunosuppressant drugs and glucocorticoid, tumor necrosis factor-α inhibitors have been new therapeutic options for uveitis associated with juvenile idiopathic arthritis, although methotrexate is known as the first-line approach. However, there are no internationally unified guidelines for clinical issues regarding the timing of application, reduction and withdrawal of tumor necrosis factor-α inhibitors, and no agreement on the application of tumor necrosis factor-α inhibitors in the management of ocular complications either. An in-depth understanding of the application status and progress of tumor necrosis factor alpha inhibitors in the treatment of juvenile idiopathic arthritis-associated uveitis has important clinical significance.

Chinese Journal of Experimental Ophthalmology 2013;(3):243-247
Background Perineural invasion is an important biological character for adenoid cystic carcinoma (ACC) of lacrimal gland, which is different from those of other lacrimal gland tumors. As the important part of neurotrophic factors,glial-derived neurotrophic factor (GDNF) plays an important role in perineural invasion for ACC of salivary gland.GDNF regulation in the ACC cell biology function needs to be further explored. Objective This study was to investigate the effect of GDNF on proliferation and migration of ACC cells, and to explore the mechanism of neural invasion in ACC of lacrimal gland. Methods ACC-2 cell line was cultured and passaged in RPMI 1640 medium with 10% fetal bovine serum,100 U/ml penicillin and 0.1 g/L streptomycin.Single-cell suspension was prepared with the density of 2×104/ml using logarithmic phase of cells and then incubated to 96-well plate.GDNF with the final concentration of 20,60,80,100 and 120 μg/L was added into the medium respectively in the experimental groups, and the cells were cultured in the medium without GDNF as the control group. The expression of GDNF in ACC-2 cells was detected by immunohistochemistry.MTT assay was employed to assay the absorbance value at the wavelength of 570 nm (A570) for the evaluation of proliferation of ACC-2 cells after cultured by different concentrations of GDNF for different time points.Meanwhile, transwell chamber was used to examine the cell migrated number. Results Immunochemistry assay exhibited that ACC-2 cells showed the positive response for GDNF with the brown staining in the cytoplasm. In 48 hours after culture, the A570 value was elevated with the increase of GDNF concentration, showing a significant difference among various groups (F =3.336,P =0.026),and the A570 value in various concentrations of GDNF groups was higher than that of 0 μg/L GDNF group (all P<0.05).After action of 80 μg/L GDNF,the A570 value of the cells was gradually increased with the prolong of culture time (Ftime =39.979,P=0.000).In 30 minutes after GDNF cultured, the number of migrated cells increased with the increase of GDNF concentration (F=144.886,P=0.000).ACC-2 cells were cultured by 100 μg/L GDNF for 24,30 and 40 hours, the number of migrated cells were more as the time lapse, and more migrated cells were seen in GDNF group at various time points (Ftime =46.747,P =0.000 ; Fgroup =63.786,P =0.000).Conclusions GDNF can stimulate the proliferation and migration of ACC-2 cells in a dose-and time-dependent manner.

Objectives: To investigate the treatment effect of enteral nutrition rich in diet fiber in diarrhea because of bacteria disorder after severe brain injuries.Methods:20 cases with severe brain injuries were supported with enteral nutrition rich in diet fiber after the occurrance of diarrhea because of bacteria disorder as clinical treatment group.20 cases with the same conditions were treated with traditional methods as control. Results: Diarrhea of all patients in clinical treatment group had been controlled during 7-10 days, and the feeding quantities reached total calories. The nutrition state was improved, and the body internalenvironment was stable during the 20 days. Conclusion: Enteral nutrition rich in fiber can treat diarrhea caused by bacteria disorder after severe brain injuries.

AIM: To evaluate posterior capsular opacification (PCO) with hydrophobic acrylic intraocular lens (IOL, Sensar AR40e) and silicone IOLs after cataract surgery, to use a software program developed to provide an objective assessment of the amount of PCO in the digital images of the posterior capsule to quantify PCO. METHODS: Ninety-eight eyes underwent standardized phacoemulsification and "in the bag" IOL placement, were randomized to receive a three piece lens of hydrophobic acrylic or silicone, but lens materials were different in one case. In year 1 and 2, digitized retro-illumination images were taken from the posterior capsule. Images were analyzed by POCO software program, removing the Purkinje light reflexes, contrast enhancement, filtering to enhance low-density PCO. RESULTS: The percentage of PCO were 0.32±0.13 of hydrophobic acrylic IOLs in year 1, compared with 0.39±0.17 of silicone (P=0.37). In year 2, the percentage of PCO were 0.42±0.20 with hydrophobic acrylic IOLs and 0.34±0.18 with silicone IOLs (P=0.50). Of those patients with PCO in year 1 and 2, severity grades were 0.50±0.30 and 0.82±0.58 of hydrophobic acrylic cases, compared with 0.63±0.35 and 0.55±0.35 of patients with silicone IOLs (P=0.52,P=0.69) with no statistical significance.CONCLUSION: The POCO system is capable of producing an objective and repeatable measure of PCO that is relevant to assessing techniques of PCO prevention.

0.05).Conclusions The serum concentration of C_3 rises and the titer of AChRAb decreases in MG patients after treatment with glucocorticoid. There is no correlation between C_3 concentration and titer of AChRAb. The complement possibly cooperates with AChRAb in pathogenesis of MG.
