1.A Clinical Research of Self-made Jian-pi-hua-zhuo-xiao-yu Tang in the Treatment of Non-alcoholic Fatty Liver Disease

Objection]To study the clinical effect of self-prescribed jianpihuazhuoxiaoyu decoction in treating the patients with non-alcoholic liver of fat. [Methods]45 patients were treated with self-prescribed Jianpihuazhuoxiaoyu decoction in treatment group and other 45 patients with Polyene Phosphatidylcholine Capsules and Ursodeoxycholic Acid. Observe the patients'body mass index(BMI),radiological changes,liver function(AST/ALT) and lipids(TG/TC/HDL/LDL). [Results] The total effective rates were 91.3%,82.6% respectively, compared with the control group,the treatment group significantly improved liver function,lipid content decreased,obesity weight was effectively controlled and reduced,sonogram also showed improvement in the texture of the liver,compared with control group there were significant differences(P<0.05),difference was statistically significant.
2.Clinical Observation of Qianggu Capsule Plus Alendronate on Postmenopausal Osteoporosis

[Objective] To investigate the effects of Qianggu capsule plus alendronate on postmenopausal osteoporosis.[Methods]By random number generating method,120 patients were randomly divided into a treatment group with Qianggu capsule plus alendronate,control group A only with Qianggu capsule,and control group B only with alendronate.The pain of the patient before treatment,3 m and 6 m after treatment was assessed.At the same time,the bone mineral density (BMD) before and 6 months after treatment was recorded.[Results] There was significant difference before and after 3 m in the score of pain in three groups,but the difference in treatment group was higher than other two control groups.After 6 m,all the scores of pain in 3 groups had changed significantly compared to the ones before treatment,but the scores of pain in treatment group were lower than 2 control groups.The BMD of both vertebral and ward in treatment group after 6 months treatment was higher than before.[Conclusions]Qianggu capsule plus alendronate has definite therapeutic effects on postmenopausal osteoporosis.
3.The comparison of clinical efficacy of specific immune therapy for one year and two years of allergic rhinitis

Objective In recent years , SIT has gradually become the main way for the treatment of allergic rhinitis .To con-trast the clinical efficacy of specific immune therapy for one year and two years of allergic rhinitis and discuss the appropriate mainte -nance treatment time of subcutaneous immunotherapy with Dermatophagoides pteronyssinus . Methods Ninety-eight patients with al-lergic rhinitis to Dermatophagoides pteronyssinus were allocated to receive either treatment for one year (A group, n=46) or treatment for two years(B group, n=52).Nasal symptoms and medication scores of two groups were assessed to evaluate the clinical efficacy af -ter treatment respectively , and evaluate the quality of life of patients . Results There was no statistical significance in nasal symptom and medication scores between the two groups after one year treatment (7.00 ±1.08 vs 6.63 ±0.79, 1.50 ±0.51 vs 0.41 ±0.42, P>0.05).The quality of life in patients with decreased , but lack of statistical significance (345.00 ±31.89 vs 344.42 ±32.26, P>0.05).It presented statistical significance in nasal symptom and medication scores between the two groups after two years treatment (6.20 ±0.78 vs 4.29 ±0.64, 0.53 ±0.43 vs 0.21 ±0.34, P<0.05).The quality of life between patients also presented statistical significance (P<0.05). Conclusion Compared with A group, B group can significantly alleviate the symptoms of allergic rhinitis , so we think the optimal maintenance treatment time of specific immunotherapy for allergic rhinitis should be at least 2 years.
4.Characteristics of wedge factor of an enhanced dynamic wedge on Varian 600C accelerator

ObjectiveTo study the characteristics of wedge factor of enhanced dynamic wedge (EDW) on Varian 600C linear accelerator . MethodsEnhanced dynamic wedge factors (EDWFs) of different jaw settings were measured on a Varian 600C linear accelerator with FARMER 2570 dosimeter and 2571 0.6 ml Thimble ion chamber. At the same time,equations were used to compute EDWFs of symmetric and asymmetric fields and off axis points. The same measurements were carried out on 30 degree physical wedge in contrast to EDW. The accelerator's jaw position was described according to IEC radiotherapy equipment coordinate standard. Results The EDWFs of symmetric field decreased smoothly while the field size increased. Under the same field,EDWFs decreased as the wedge angle increased and the increasing extent became apparent when the field became larger. In off axis fields formed by the upper independent jaws,only the EDWFs of 60 degree had the tendency to increase when the field center moved toward the toe of the wedge,the rest gave the opposite results. The EDWFs of fixed points varied only by 0.5 percent when the fixed jaw remained at the same place and the moving jaw and lower jaws stayed at different positions. But the WF of 30 degree physical wedge increased by 2.9 percent when the lower jaw settings increased from 4 cm to 40 cm. The EDWFs derived from analytic equations for symmetric and asymmetric fields coincided well with measured results. But at off-axis points,the differences between calculated values and measured data were apparent. ConclusionsThe curve of EDWF versus field size is very smooth. The EDWF of fixed point only depends on the position of the stationary upper jaw,and is affected little by the moving upper jaw and lower jaws. The EDWFs of off axis fields are much more complex than those of physical wedge; it shouldn't be confused in clinical application. The conversion method of rectangular field to square field is of no use when EDW field is confronted. Analytic methods are highly conformal with the calculation of EDWFs of symmetric and asymmetric fields.
5.Clinical use of interventional therapy for occlusive lesion in iliac arteries and femoral arteries of 42 cases

Objective To evaluate the efficacy of interventional therapy of occlusive lesion in iliaco- femoral arteries. Methods During Feb 2001 to Feb 2006, 42 patients (30 male and 12 female) with arterial occlusive lesions in iliac artery and/or femoral artery accepted interventional therapies. The interventional therapeutic process included local thrombolytic therapy through endoarterial catheter, artery recanalization by guide wire, pereutaneous transluminal angioplasty (PTA) and stent. After interventional operation, patients were administrated anticoagulant and antibiotics one week. Follow-up interval ranged 2 years. Wilcoxon test and t test were used for statistics. Results Immediate technical successful rate was 97.6% (41/42),the secondary technical success rate was 100.0% (42/42). The arterial occlusions were resolved successfully (local thrombolytic therapy in 26 cases, recanalization by guide wire in 31 cases, PTA in 33 cases and 60 stents in 31 cases). There were no severe complications (such as angiorrhexis, perforation) during interventional procedure. The symptoms of lower limb ischemia were palliated or vanished in all patients after interventional therapy. The ankle-brachnial index(ABI) of diseased extremities increased from pre-operation 0.34±0.14 to past-operation 0.65±0.10 (t=25.924, P <0.01 ). During the follow-up, 11/42 (26.2%) iliac or femoral artery (treated with local intra-arterial thrombolysis, PTA and stent) occurred restenosis. After PTA and stenting, the restenosis arteries became fluent successfully. The primary patency rate was 92.8% (39/42)and the secondary patency rate was 100.0% (42/42)1 year after the procedure. Two years after the procedure, the primary patency rate was 71.4% (30/42) and the secondary patency rate was 97.6% (41/42). Conclusions Interventional therapy (including local thrombolysis, recanalization, PTA and stent) is an effective and safe therapy for occlusive lesion in iliaco- femoral arteries.
7.Automatic Classification of Epileptic Electroencephalogram Signal Based on Improved Multivariate Multiscale Entropy.

Traditional sample entropy fails to quantify inherent long-range dependencies among real data. Multiscale sample entropy (MSE) can detect intrinsic correlations in data, but it is usually used in univariate data. To generalize this method for multichannel data, we introduced multivariate multiscale entropy into multiscale signals as a reflection of the nonlinear dynamic correlation. But traditional multivariate multiscale entropy has a large quantity of computation and costs a large period of time and space for more channel system, so that it can not reflect the correlation between variables timely and accurately. In this paper, therefore, an improved multivariate multiscale entropy embeds on all variables at the same time, instead of embedding on a single variable as in the traditional methods, to solve the memory overflow while the number of channels rise, and it is more suitable for the actual multivariate signal analysis. The method was tested in simulation data and Bonn epilepsy dataset. The simulation results showed that the proposed method had a good performance to distinguish correlation data. Bonn epilepsy dataset experiment also showed that the method had a better classification accuracy among the five data set, especially with an accuracy of 100% for data collection of Z and S.

9.Influence to early postoperative quadriceps strength of minimally invasive and conventional approaches in total knee arthroplasty: a prospective randomized controlled

Objective To compare the quadriceps strength and short-term outcome in patients who had undergone primary total knee arthroplasty (TKA) with minimally invasive (MIS) or conventional surgical technique.Methods Sixty-eight knees undergoing TKA were evaluated and they were randomly divided into two groups.One group included 34 knees which were undergone conventional approach,and the other group included 34 knees undergone MIS approach.The age,BMI,quadriceps strength,extension lag,American Knee Society Score (KSS),Western Ontario and McMaster Universities (WOMAC) osteoarthritis index and visual analogue scale (VAS) of pain of all patients were evaluated at the day before surgery and the 1st,2nd,3rd,6th,and 12th months after surgery.Results At the 1st,2nd,3rd,6th,and 12th months after surgery,the quadriceps strength in MIS group was 0.59±0.12,0.74±0.18,0.86±0.16,1.02±0.17,1.05±0.04 N·m/kg respectively,while in conventional group was 0.46± 0.56,0.56±0.12,0.70±0.16,0.94±0.19,1.04±0.03 N· m/kg respectively.The extension lag in MIS group was 13.11°±6.56°,8.53°± 5.12°,4.79°±3.62°,1.53°±2.64°,1.62°±1.76° respectively,while in conventional group was 22.47°±8.41°,16.23°±6.26°,10.58°± 4.4°,2.58°±2.85°,1.62°±1.76° respectively.There were significantly differences in the two groups.Meanwhile,compared with conventional patients,MIS patients demonstrated significantly higher KSS scores at the 1st and 2nd months postoperatively.WOMAC scores were significantly lower in MIS group than in conventional group at the 1st month after surgery.During the first postoperative week,MIS technique significantly decreased VAS scores,showing significance.On the other hand,no significant difference was observed with respect to deep infection incidence and radiological outcome between two groups.Conclusion MIS TKA offers a significant improvement in extensor muscle strength over conventional surgery.These results suggest that the MIS approach results in better outcomes with regard to maintaining extensor strength than the conventional surgical approach.
10.Clinical study on gasless video-assisted endoscopic thyroidectomy by infraclavicular wall

Objective To evaluate the clinical value of gasless video-assisted endoscopic thyroidectomy by infraclavicular wall.Methods 15 cases received gasless video-assisted endoscopic thyroidectomy by infraclavicular wall.13 cases underwent total thyroidectomy,2 cases underwent subtotal thyroidectomy.All 15 cases were protected recurrent laryngeal nerve and parathyroid by inspecting during the operation.Results All 15 patients were cured without hoarse and tetany,and the flaps without hematomas and extravasated blood.Conclusion Gasless video-assisted endoscopic thyroidectomy by infraclavicular also can reach the principle demand by the tradional opening way.As the supremacy way of aesthetic and hidden,it can be used to the patients who want high request on faces.
