1.The influence of early mechanical ventilation and perfluorocarbons gasification inhalation on inflammatory factor levels in patients with severe pancreatitis

Objective To investigate the clinical effect by early mechanical ventilation and perfluorocarbons gasification inhala-tion on patients with severe acute pancreatitis (SAP) complicated with acute respiratory distress syndrome (ARDS) .Methods 34 cases of SAP and ARDS patients accepted early mechanical ventilation and vaporization of perfluorocarbon inhalation therapy (ob-servation group) ,and 34 patients received mechanical ventilation for early treatment alone (control group) ,The efficacy ,oxygenation index(PaO2/FiO2 ) and inflammatory cytokines IL-1 ,6 ,8 ,and TNF-a level of two groups were compared .Results The total effi-ciency of the observation group was 94 .12% ,which was significantly higher than the control group that was 76 .47% (P<0 .05) . After treatment ,the arterial oxygen saturation (SaO2 ) ,arterial oxygen pressure (PaO2 ) ,PaO2/FiO2 of the observation group in-creased significantly ,and higher than the control group(P<0 .05) .After treatment ,IL-1 ,6 ,8 ,TNF-αof the observation group were significantly lower than the control group(P<0 .05) .Conclusion The treatment of early mechanical ventilation with perfluorocar-bon inhalation gasification could control the inflammation in patients with SAP complicated with AROS ,which could improve the cure rate .
2.Investigations on nutritional status and nutritional risk in hospitalized pediatric patients

Objectives To validate the Screening Tool for the Assessment of Malnutrition in Pediatrics (STAMP) in hospitalized pediatric patients, and to assess the nutritional status and nutritional risk using STAMP. Methods the nutritional status of hospitalized pediatric patients was investigated by adopting STAMP. The STAMP score≥4 was deifned as a high standard of nutritional risk. Results Among 1 506 hospitalized children, children with high nutritional risk accounted for 26.56%. The overall prevalence of malnutrition and stunting was 19.12%and 10.16%, respectively. There was signiifcant diffe-rence in stunting among each department (P<0.05), which was the highest in department of internal pediatrics. The proportion of patients receiving nutritional support was 10.09%. Among them, the rate of parenteral and enteral nutrition support was 7.84%and 2.92%, respectively. The ratio of parenteral nutrition to enteral nutrition was 2.68:1. After intervention to hospitalized pa-tients with high nutritional risk, the overall prevalence of high nutritional risk decreased from 26.56%at admission to 21.71%at discharge (P<0.05). Conclusions STAMP can objectively relfect the possible malnutritional risk in hospitalized pediatric patients. The nutrition condition of the pediatric patients can be improved through nutritional risk management.

4.Correlation Study on Personality and Self-rated Health of Medical Students in Tianjin

Objective To understand the personality and self-rated health status and the relationship between them in medical students. Methods By multi-stage cluster random sampling, 878 medical students were selected to investigate the personality by Eysenck Personality Questionnaire-Revised (Short Scale For Chinese) and the health status by self-rated health measurement scale (SRHMS1.0). Results The score of L dimension among male and female medical students was significantly higher than that of nation- wide young population. The scores of E, N and P dimensions had no significant difference between them. The score of P dimension of male students was significantly higher than that of female students. The score of students' SRHMS was 339.28?41.19, which was higher than that of general population in China. The score of physical health subscale among male and female students was 144.90?11.41 and 142.73?11.36, respectively (t=2.829, P=0.005). The scores of SRHMS and its three subscales were significantly different in medical students with different personality traits except the score of physical health subscale in students with different grades of P dimension. The scores of E and L dimensions had significantly positive correlation with SRHMS, and the scores of N and P dimensions had significantly negative correlation with SRHMS. Conclusion The personality traits of medical students were remarkably influenced on their self-rated health. To encourage medical students to reform the wholesome personality will be helpful to their health.
5.Effect of ketamine on the cGMP level in different brain areas in a rat model of CFA-induced arthritis

Objective To investigate the effect of ketamine on the cGMP content in cerebral cortex, hypothalamus and brain stem in a rat model of CFA-induced arthritis. Methods Twenty-four SD rats weighing 200-220 g were randomly divided into three groups with 8 animals in each group, group Ⅰ: normal rat + intraperitoneal normal saline (ip NS);group Ⅱ : arthritic rat+ ip NS;group Ⅲ: arthritic rat + ip ketamine 100mg?kg-1 . Arthritis was produced by injecting CFA 0.05 ml into left ankle joint which developed inflammatory response within 24h and became hyperalgesic. The hyperalgesia persisted for more than 6 weeks. Three days after the arthritis model was established, paw withdrawal latency (PWL) to radiant heat was measured. 15 min after ip ketamine the animals were decapitated and cerebral cortex, hypothalamus and brain stem were separately isolated for determination of cGMP content using radioimmunoassay. ObjectiveTo investigate the effect of arsenic trioxide (ATO) on anti-dsDNA antibody and the expression of DNA methyltransferase 1-mRNA and CD11a-mRNA in lupus MRL/lpr mice.Methods MRL/lpr mice were divided into three groups:the ATO group,the sodium chloride(NS) group,and the cyclophosphamide(CTX) group.The control group consisted of 20 syngeneic normal C57/BL mice,which were subdivided into the ATO group and the NS group.After two-month treatment,all mice were sacrificed.Blood routine test was conducted by SYSMEX KX21.The anti-dsDNA antibody in the serum were detected by ELISA.The expression of DNMT1 and CD11a was determined by RT-PCR.ANOVA and paired t test were used for statistical analysis.Results① The serum level of anti-dsDNA antibody(0.89±0.07) and the gray scale value of CD11a-mRNA(0.43±0.25) in the ATO group were much lower than those in the NS group of MRL/lpr mice(1.77±0.28,P<0.01; 0.99±0.31,P<0.05),while the gray scale value of DNMTI-mRNA (0.32±0.30) was significantly higher than that in the NS group(0.16±0.26,P<0.05 ).② The serum levels of anti-dsDNA antibody was low in both the ATO group and the CTX group (0.90±0.07,0.66±0.22),and it was higher in the ATO group than that in the CTX group (P<0.05).The gray scale value of DNMT1 mRNA in the ATO group (0.32±0.30) was higher than that in the CTX group (0.16±0.18,P<0.05) in MRL/lpr mice,and the gray scale value of CD11a mRNA in the ATO group(0.43±0.25) was lower than that in the CTX group (0.86±0.31,P<0.05) in MRL/lpr mice.③ There was no difference in above parameters between the ATO-group and NS group in C57/BL mice(P>0.05).ConclusionArsenic trioxide can reduce the serum level of auto-antibody and reverse low methylation.But it has no effect on normal mice.
7.Relationship between HBsAg level and HBV DNA level as well as illness severity in patients with HBV-related acute-on-chronk liver failure

Objective To analyze the relationship between HBsAg level and HBV DNA level as well as illness severity in patients with HBV-related acute-on-chronic liver failure (ACLF).Methods One hundred and nineteen patients with chronic hepatitis B (CHB group) and 98 patients with ACLF(ACLF group) were enrolled.HBsAg and HBeAg were assayed with Roche electrochemical luminescence method.HBV DNA was quantified using a real-time polymerase chain reaction (PCR) assay.HBsAg and HBV DNA levels were compared between two groups and between HBeAg-positive and HBeAg-negative patients in ACLF group respectively,also the correlationbetween HBsAg and HBV DNA was studied.Results The proportions of HBeAg-negative patients were 68.4%(67/98) and 42.9%(51/119) in ACLF group and CHB group respectively,and there was significant difference between two groups (P <0.01).There was no significant difference in HBV DNA between two groups (P > 0.05).HBV DNA in HBeAg-positive patients was higher than that in HBeAg-negative patients in two groups(P < 0.05).There was no significant difference in HBsAg between HBeAg-positive patients and HBeAg-negative patients in ACLF group (P > 0.05 ),but they were higher than that in HBeAg-positive patients in CHB group (P< 0.05).HBsAg was correlated to ALT,AST in HBeAg-positive patients (P < 0.05).No significant correlation was found among HBsAg and HBV DNA as well as biochemical changes (P> 0.05).There was no significant difference in the ratio of different HBsAg levels among the patients of different HBV DNA in ACLF group (P> 0.05).Conclusion The level of HBsAg does not directly correlate with serum HBV DNA level,and has no directly correlation with the severity of the disease in patients with HBV-related ACLF.
8.Study on the Doses of Oxaliplatin in Patients with Hepatocellular Carcinoma after Transcatheter Arterial Che-moembolization

OBJECTIVE:To investigate the effect of different doses of oxaliplatin on the efficacy and safety and related index-es of patients with hepatocellular carcinoma(HCC)after transcatheter arterial chemoembolization(TACE). METHODS:100 HCC patients were randomly divided into control group (50 cases) and observation group (50 cases). After TACE,control group re-ceived arterial infusion chemotherapy of 40 mg/m2 oxaliplatin,once a day+20 mg Epirubicin hydrochloride for injection,once a day,with little lipiodol. Observation group received arterial infusion chemotherapy of 80 mg/m2 oxaliplatin (the same usage with control group)+Epirubicin hydrochloride for injection(the same dosage with control group),with little lipiodol. Alanine aminotrans-ferase (ALT),aspartate aminotransferase (AST),total bilirubin (TBIL),white blood cell count (WBC) and alpha-fetoprotein (AFP) in 2 groups after 3 d treatment were observed,and the total overall survival (OS) and the incidence of adverse reactions were followed-up. RESULTS:Abdominal pain,incidences of nausea and vomiting,ALT,AST and TBIL in observation group were significantly higher than control group,WBC was significantly lower than control group,the differences were statistically sig-nificant (P<0.05). There were no significant differences in fever,hair loss,incidence of peripheral neurotoxicity,OS in 1 year and 3 years,and AFP in 2 groups(P>0.05). CONCLUSIONS:Compared with 40 mg/m2 oxaliplatin,80 mg/m2 can reduce the in-cidence of abdominal pain,nausea,vomiting,but other effects are poor than 40 mg/m2,and it can not prolong the survival time of patients.
9.Effect Observation of Entecavir Combined with Intervention in Liver Cancer Patients with HBV DNA-nega-tive Hepatitis B

OBJECTIVE:To observe the clinical efficacy of entecavir combined with intervention in the treatment of liver cancer patients with HBV DNA-negative hepatitis B. METHODS:100 liver cancer patients with HBV DNA-negative hepatitis B were random-ly divided into observation group and control group,50 cases in each group. Control group received intervention,percutaneous punc-ture of femoral artery,then injected Pirarubicin hydrochloride for injection + lipiodol in parent artery until blood stagnation,and also conventional liver protection therapy was conducted;observation group additionally received 1 mg Entecavir tablet,qd,for continuous 6 months. Clinical efficacy,HBV DNA quantification,Child-Pugh score and liver function indexes in 2 groups were compared. RE-SULTS:The total effective rate in observation group was 44.0%,which was significantly higher than control group(26.0%),the dif-ference was statistically significant(P<0.05). There were no significant differences in HBV DNA quantification,Child-Pugh score and liver function indexes between 2 groups(P>0.05). After treatment,HBV DNA quantification,Child-Pugh score,fetoprotein,alanine aminotransferase, total bilirubin and aspartate aminotransferase significantly decreased in observation group,the differences were statis-tically significant compared with control group (P<0.05). CONCLUSIONS:Entecavir combined with intervention can obviously im-prove the clinical efficacy and liver function indexes of liver cancer patients with HBV DNA-negative hepatitis B.
10.Comparisons of four equations to estimate glomerular filtration rate and their impacts on chronic kidney disease prevalence in community-dwelling elderly people in Beijing

Objective To investigate the differences of four equations to estimate glomerular filtration rate (GFR) and their impacts on chronic kidney disease (CKD) prevalence in communitydwelling elderly people in Beijing.Methods A total of 489 participants aged above 70 years were enrolled.The GFR was estimated using the Cockcroft Gault (CG) equation,Chronic Kidney DiseaseEpidemiology Collaboration (CKD-EPI) equation,Modification of Diet in Renal Disease (MDRD) equation and Berlin Initiative Study (BIS) equation respectively.The internal-consistency check was made on the four equations for the GFR (ml min 1 1.73m 2) estimates.Results The mean age of participants was (81.8±7.6) years.The mean GFR estimated using the CG,CKD-EPI,MDRD and BIS equation was 58.4± 17.2,71.9± 15.3,76.7± 19.1 and 62.7± 12.7,respectively.And the prevalence of CKD was 56.2%,22.7%,17.8% and 41.7%,respectively.The greatest differences of equations to estimate GFR were seen in elderly people aged 90 and above,and those with body mass index< 20.0 kg/m2 or serum creatinine concentration< 88.4 μmol/L.Conclusions The GFR estimated using different equations has a large difference which has a significant effect on CKD classification in elderly people.The equation to estimate GFR for the elderly is urgently needed.Until then,the eGFR and CKD classification estimated using different equations should be regarded with caution.
