Objective To determine the incidence of clinically common acute avulsions of the popliteus tendon (PT) and the lateral collateral ligament (LCL) of the femur (peel-off lesion) in knees with multiple injured ligaments. Methods A retrospective review of 48 acute grade 3 posterolateral corner (PLC) knee surgeries was conducted. All office evaluations were reviewed to determine the demographic data, tear description, preoperative MRI examination, physical examination under anesthesia, intraoperative arthroscopic findings and surgical techniques. Results In total,19 patients were surgically verified as having femoral peel-off lesions as a portion of a series of multiple pathologic findings.Tear patterns were typically categorized into 3 patterns:isolated PT tear (pattern 1,4 patients),combined PT and LCL tears (pattern 2,8 patients) and complex tears (associated intrasubstance and/or fibular-based injuries, pattern 3,7 patients).Seventeen of the 19 patients underwent preoperative MRI examination, which demonstrated conclusive signs of femoral insertion separation and discontinuity in 13 patients (76%) and inconclusive signs of femoral insertion separation.Examination under anesthesia (EUA) revealed that for the 4 patients with pattern 1 injuries, the tibial external rotation increases were all greater than 10°.For the 15 patients with pattern 2 and 3 injuries, the yarus instability was as follows:grade 3 in 8 patients, grade 2 in 2,grade 1 in 1,and negative in 4.External rotation increases were greater than 10° in 12 patients and less than 10° in 3.Seventeen of the 19 patients were inspected with an arthroscope prior to PLC surgery:of these patients,94% had acute avulsion findings and positive lateral gutter drive-through sign;8 patients were repaired with recess or reattachment procedures;6 patients were primarily reconstructed;4 patients were treated with combined repair and reconstruction;and 1 patient was left untreated for secondary reconstruction due to priority repair for concurrent patellar tendon rupture. Conclusion For adequate clinical identification,we recommend a comprehensive diagnostic protocol, including MRI,physical examination and arthroscopic inspection,to avoid underdiagnosis of the lesion in multiple-ligament injured knees. The lateral gutter arthroscopic evaluation is particularly sensitive for detecting the lesion.

Objective To establish embryonic stem cells (ESC) that can express green fluorescent protein (CFP) and differentiate them into neurons. It would provide tagging neurons for clinical transplantation to cure neural system diseases. Methods ESC (R1) was transfeeted with a plasmid containing the GFP by electroporation. A transgeuic cell line was obtained after selection with G418. The ESCs were characterized by AKP staining. Monolayer differentiation method was used to induce neural differentiation derived from GFP-ESC and immunofluorescence method was used to identify Tuj1 positive cells. Results There was no significant difference(X2=3.14,P0.05) in transfect rates between liposome and electroporation (65% vs 79%). The AKP staining of GFP-ESC was positive. GFP-ESC could be differentiated into neural cells. Conclusions These results show that ESC expressing GFP has been estabhshed, which can be differemiated into neurons.

AIMTo study the mechanisms of sodium ferulate (SF) on inhibition of collagen synthesis in hepatic stellate cells.
METHODSCollagen synthesis was analyzed by measuring 3H-proline incorporation. ELISA method was used to study the effect of SF on transforming growth factor beta1 (TGFbeta1) level in cultured HSC-T6 cell. The effect of SFon the TGFbeta1 activity in the supernatant of culture was analyzed by mink lung epithelial cell (Mv1Lu) proliferation inhibition by MTT assay.
RESULTSSF inhibited collagen synthesis in hepatic stellate cells stimulated with TGFbeta1. SF was shown to decrease TGFbeta1 level in the supernatant of HSC-T6 increased by oxidative stress. TGFbeta1 activity was intervened by SF.
CONCLUSIONSF could decrease collagen synthesis, with mechanism may be associated with that SF intervened TGFbeta1 activity, and reduced the level of TGFbeta1 increased by oxidative stress.

Chinese Journal of Tissue Engineering Research 2008;12(39):7759-7763
BACKGROUND:Routine posterior cruciate ligament(PCL)reconstruction can improve posterior stability of the knee joint. But the wear between graft and outlet of tibial tunnel always reduces graft mechanical strength or enlarges tunnel, resulting residual laxity.OBJECTIVE:To explore the clinical application of fluoroscopy-based navigation technique for assisting tibiai tunnel placement in arthroscopic PCL reconstruction and to investigate the efficacy and feasibility of the technique.DESIGN,TIME AND SETTING:Case analysis was performed at Beijing Jishuitan Hospital between August 2006 and March 2007.PARTICIPANTS:of 15 eases with compound ligament injury,14 underwent allograft, and 1 underwent autograft bone-1/3 of patellar tendon-bone.METHODS:The navigation system consisted of tibia tracker fixed into the proximal tibia and tool tracker attachcd with PCL tibial drill guide. With registration and calibration, the navigation system identi fied and captured the infrared signals actively from the two trackers. The computer calculated the 3D-position of the knee joint relative to the PCL tibial drill guide and then the virtue tibial tunnel was imposed into the interactive images formed by the intraoperative C-aim images. The virtue tunnel was precisely adjusted according to the intraoperative planning protocol until the ideal position achieved. In the standard anteroposterior view, the centre of the outlet of tunnel should be in the midpoint between the medial and lateral eminence and 1.5 cm from the articular surface. In sagittal plane, with the intact posterior wall, the tibial tunnel should be as close as to the posterior proximal tibial cortex to get a maximum angulation.MAIN OUTCOME MEASURES:Fluoroscopy time and times, postoperative standard anterior and lateral X-ray,outlet of tibial tunnel, angle between graft and tunnel, attachment of tunnel to posterior cortex, and integrity of posterior wall.RESULTS:Among 15 patients,14 succeeded without navigation related complications and 1 failed. The frequency of intraoperative fluoroscopy reduced from 2-10 times to 2-4 times and the exposure time for fluoroscopy shortened 10-30minutes.The tibial outlet in all success cases was located within the tibialinsertion area of posterior cruciate ligament. The average angulation between the tibial tunnel and the graft was 123.3°;all cases showed close spatial relationship between tunnel and posterior proximal tibial cortex within 2 mm,with intact posterior wall in 10 cases and slightly broken in 4 cases.CONCLUSION:With high accuracy and time-saving features,fluoroscopy-based navigation system is a feasible technique in assisting tibial tunnel placement in arthroscopic PCL reconstruction surgery.
