Objective To explore the role of plasminogen activator inhibitor-1(PAI-1)in development of progressive fibrosis via the inhibition of extracellular matrix degradation, and to reveal the contributive role of PAI-1 in muscular dystrophy(MD).Methods Expression and cellular localization of PAI-1 protein were examined in frozen muscle specimens obtained via biopsy from 5 patients with duchenne muscular dystrophy(DMD),3 patients with becker muscular dystrophy(BMD),9 patients with congenital muscular dystrophy(CMD) and 4 cases with normal muscle by immunohistochemistry, double immunofluorescence and Western-blot analysis. Results PAI-1 was positive only in vascular endothelial cells of normal muscle. Both immunohistochemistry and Western-blot analysis showed that PAI-1 expression distinctly increased in most dystrophic muscles of MD than that in normal muscles.Double immunolabeling revealed that PAI-1 strongly expressed in cytoplasm and nuclei of regenerating muscle fibers, macrophages, macrophage infiltrating necrotic fibers. Some activated fibroblasts in endomysium and perimysium of DMD and CMD muscles were positive for PAI-1.Conclusions The functional consequence of overexpression of PAI-1 in dystrophic muscles is unknown but the elevated local expression of PAI-1 in diseased muscles of MD and their distinct distribution pattern provide evidence that PAI-1 participate in pathogenesis of MD.

Objective To evaluate the effects of postconditioning with enalapril on myocardial injury induced by hind limb ischemia-reperfusion (I/R) in rats. Methods Thirty-six healthy male Sprague-Dawley rats, weighing 200-250 g,were randomly divided into 3 groups (n =12 each):group sham operation (group S),group I/R and group enalapril postconditioning (group EP).Limb ischemia was induced by occlusion of bilateral hind limbs for 3 h followed by 3 h reperfusion in groups I/R and EP.At 30 min before reperfusion, enalapril 0.04 mg/kg was injected via the internal jugular vein in group EP,while the equal volume of normal saline was injected in groups S and I/R.The rats were sacrificed at 3 h of repeffusion and myocardial specimens were obtained for microscopic examination of pathologic changes and for determination of cell apoptosis,Bcl-2 and Bax protein expression, superoxide dismutase (SOD) activity and malondialdehyde (MDA) content in myocardial tissues.Apoptosis index was calculated. Results Compared with group S,apoptosis index and MDA content were significantly increased,Bax protein expression was up-regulated,Bcl-2 protein expression was down-regulated,and SOD activity was decreased in groups I/R and EP (P < 0.05).Compared with group I/R,apoptosis index and MDA content were significantly decreased,Bax protein expression was down-regulated,Bcl-2 protein expression was up-regulated,and SOD activity was increased in group EP (P < 0.05).The pathologic changes were significantly attenuated in group EP as compared with group I/R.Conclusion Enalapril postconditioning can attenuate myocardial injury induced by hind limb I/R in rats, and the mechanism may be related to reduction of apoptosis in cardiomyocytes and lipid peroxidation.

OBJECTIVE To analyze the incidence of anatomic variations of the temporal bone to provide valuable reference for clinical otology. METHODS Sev-enty six healthy adults (152 ears) and 66 patients(96 ears) with chronic suppurative otitis media confirmed by clinical examination underwent axial and coronal High-resolution Computed Tomography(HRCT) scan. The incidence of high jugular bulb, dehiscent jugu-lar bulb, anterior location of sigmoid sinus, low-lying middle cranial fossa, dehiscent tegmen tympani, mas-toid antrum dysplasia, aberrant internal carotid artery, and large sinus tympani were recorded. RESULTS In normal, abnormal, well and poorly pneumatized groups, the incidence of highly positioned jugular bulb on the right was higher than that on the left. The total inci-dence of high jugular bulb on the left and right was 26.2 % and 46.0 % respectively. The incidence of ante-rior location of sigmoid sinus in the abnormal group (32.3 %) was higher than that in normal group(15.8 %), which was higher in poorly pneumatized group(43.5 %) than that in well pneumatized group(11.3 %). The inci-dence of dehiscent tegmen tympani in well pneuma-tized group(18.0 %) was higher than that in poor pneu-matized group(8.7 %). There was no significant differ-ence in dehiscent jugular bulb, low-lying middle cranial fossa, mastoid antrum dysplasia, aberrant internal ca-rotid artery, and large sinus tympani between the left and right within and among groups. The incidence of these varia-tions was 2.0 %,21.8 %,1.2 %,0.4 %,0.8 %respectively. CONCLUSION HRCT can show ana-tomic variances of temporal bone clearly before opera-tion and has great clinical value for reducing the inci-dence of complications caused by middle ear operation.

· AIM: To investigate the expression and the possible implication of CD40/CD40L costimulatory molecules in erythema nodosum of patients with Beh(c)et's disease.· METHODS: Sampling was done from erythema nodosum of 5 patients with Beh(c)et's disease and normal skin of 2 healthy individuals. Immunohistochemical staining was performed to examine the expression of CD4, CD8, CD19, CD68, HLA-DR,CD40 and CD40L molecules in the obtained tissues.· RESULTS: Approximately 90% of epidermic cells in erythema nodosum expressed CD40 molecule. In the dermis and subcutaneous tissue, a significantly increased number of CD4+Tcells, CD8+Tcells, CD19+cells, CD68+cells, HLA-DR+cells,CD40L+cells, and CD40+cells were observed in the erythema nodosum as compared with that in normal skin. Double staining showed that CD40L molecules were expressed on 45% of CD4+T cells. CD40 molecules were expressed on 100% CD68+ cells and 59.2% of HLA-DR+cells respectively.· CONCLUSION: A number of CD40/CD40L costimulatory molecules are upreguiated in the erythema nodosum of patients with Behcet's disease.
