1.Preliminary application of ultrasound real-time elastography in diagnosis of thyroid diffuse disease

Objective To explore the diagnosis value of ultrasound real-time elastography in thyroid diffuse diseases.Methods Eleven elastic quantitative parameters of 123 patients with thyroid diffuse diseases and 42 healthy volunteers obtained from the ultrasound real-time elastography were analyzed with principal components analysis to attain a comprehensive evaluation value which was named stiffness index(SI),reflecting the organization elastic information of different thyroid diffuse diseases.SI among each group was compared by ANOVA.Results Chronic lymphocytic thyroiditis SI was 86.37±20.45,subacute thyroiditis SI was 86.14±19.95,primary hyperthyroidism SI was 54.68±21.69,normal control group SI was 41.34±15.59,respectively.The SI between chronic lymphocytic thyroiditis,subacute thyroiditis,primary hyperthyroidism and normal controls had significant statistical differences (P<0.05),the SI between primary hyperthyroidism and chronic lymphocytic thyroiditis,subacute thyroiditis had significant statistical differences (P<0.05),the SI between chronic lymphocytic thyroiditis and subacute thyroiditis had no significant statistical difference (P>0.05).Conclusions Ultrasound real-time elastography could indirectly reflect the stiffness information of thyroid lension tissue and have a certain degree of diagnosis value in thyroid diffuse disease.

The Department of Obstetrics and Gynecology has performed series of teaching reforms for the seven-year medical students.For the theory classes,all the teachers are the senior ones who can enhance the bilingual teaching and combine the clinical cases with the latest development in the subject.We let the interns follow the diagnosis and treatment of the patients just as the patients'relatives.We also let a tutor to guide each intern to help him master the basic clinical art.During the ward inspection for teaching,we let the students be the main speaker,hoping that we can build a new student-centered teaching model.
4.Polymorphisms of the high-affinity receptor for IgE alpha-chain (FcεRIα) gene in patients with atopic dermatitis

Objective To assess the relationship between single nucleotide polymorphisms of FcεRIαgene and atopic dermatitis. Methods Genomic DNA samples were extracted from peripheral blood of 97 patients with atopic dermatitis and 283 normal human controls. The polymorphism at the distal promoter region of FcεRIα gene was determined by PCR-ligase detection reaction assay followed by gene sequencing. Results A G/T polymorphism was observed at position rs61828219 in the promoter region of FcεRIα gene, while all the tested individuals were homozygous for T/T at position rs12135235 and A/A at position rs36233780 in the promoter region of FcεRIα gene. The mutation frequency at position rs61828219 was 1.04% and 2.17% in patients with AD and normal human controls, respectively (both P > 0.05). Conclusions In the Chinese Han population, there is a G/T polymorphism at position rs61828219 in FcεRIα gene promoter region, which is unlikely related to the development of AD; however, no polymorphism is detected at position rs12135235 or rs36233780.

6.Effect and mechanism of JianNao mixture on function of learning and memory in mice

AIM: To observe the effect and mechanism of Chinese traditional medicine JianNao mixture on function of learning and memory in memory disorder mice. METHODS: Step down test, step through test and "Y" maze test were used to examine the function of learning and memory in memory disorder mice induced by scopolamine, sodium nitrite and alcohol, and in dementia model mice induced by D galactose and aluminum chloride. Cholinesterase activity in brain, LPO and SOD in blood were measured. RESULTS: JianNao mixture could promote both the learning and memory function in impaired acquisition, consolidation and retrieval of memory in memory disorder mice induced by scopolamine, sodium nitrite and alcohol, respectively. It also could reduce wrong times and prolong latency in mice's step through test, depress cholinesterase activity of brain in aluminum chloride model mice, reduce wrong times in mice's "Y" maze test, enhance SOD activity of whole blood and decrease LPO content of brain in D galactose model mice. CONCLUSION: JianNao mixture can improve the function of learning and memory in memory disorder mice. The mechanism may be due to improve the cholinergic function and the balance of free radical in vivo.
7.Gross Motor Function and Manual Ability Classification in 4~12 Year Old Children with Cerebral Palsy of Different Types

Objective To explore the distribution and correlation of gross motor function and manual ability in 4~12 year old children with cerebral palsy of different types. Methods 143 children with cerebral palsy (CP) were recruited according to the inclusion and exclusion criterions. They were assessed with Chinese version of Gross Motor Function Classification System (GMFCS) and Manual Ability Classification System (MACS). Results The classification of GMFCS and MACS was Ⅰ~Ⅱ in 33 (60.0%) children with spastic diplegia; Ⅲ~Ⅴ in 28 (75.7%) children with spastic quadriplegia; Ⅰ in 21 (55.3%) children with spastic hemiplegia, and GMFCS Ⅱ in 2 children, MACS Ⅱ in 14 children. There was moderate correlation between GMFCS and MACS (r=0.67,P<0.05). Conclusion GMFCS and MACS can be used to determine the gross motor function and manual ability in CP children of different types.
8.Correlation between antithrombotic therapy and ischemic stroke in elderly patients with nonvalvular atrial fibrillation aged over eighty years

Objective To retrospectively analyze the treatments of nonvalvular atrial fibrillation (nvAF) in elderly patients aged 80 years and over,and to investigate the influencing factors for occurrence of stroke and transient ischemic attack(TIA)and relationships between antithrombotic therapy and stroke or TIA.Methods 101 elderly patients with nvAF were enrolled and grouped according to the occurrence of stroke/TIA and antithrombotic-correlated bleeding.The influencing factors were retrospectively analyzed and antithrombotic schemes were compared.Results Incidence rate of stroke/TIA was 28.7% (29/101).Among all patients,70 cases were treated with antiplatelet therapy,19 cases were treated with anticoagulation therapy,while 12 cases received no antithrombotic (antiplatelet or anticoagulation) therapy before stroke.Both the nvAF time course and the antithrombotic strategy were significantly different between post-AF stroke/TIA group and non-postAF stroke/TIA group(both P<0.05).The difference was reflected in ratios of antiplatelet therapy/anticoagulation therapy.The proportion of anticoagulation therapy was higher in non stroke/TIA group(x2 =5.778,P =0.016).Different antiplatelet therapy scheme significantly affected occurrence of stroke/TIA(P<0.05).There was no significant effect of antithrombotic schemes on hemorrhagic events(x2=0.708,P =0.702).Multiple logistic regression analysis showed that hypertension,coronary heart disease,cancer,diabetes and previous stroke history,as well as nvAF duration were the independent risk factors for post-AF stroke/TIA(OR=1.351,95 %CI:1.129-1.617).Conclusions Currently,the proportion using anticoagulation therapy is low,and single antiplatelet therapy is the main regimen in the elderly patients with nvAF.For elderly patients with nvAF,anticoagulation therapy has a protective effect against the occurrence of post-nvAF stroke/TIA,meanwhile there is no significantly increased risk of bleeding,which makes anticoagulation therapy advisable in the elderly.The nvAF time course is one of the risk factors,which is worth experts' attention in risk evaluation of thrombus in elderly patients.
9.Clinical Characteristics of 100 Children with Cerebral Palsy

ObjectiveTo analysis the clinical characteristics and the high risk factors of children with cerebral palsy.MethodsClinical data of 100 children with cerebral palsy were retrospectively analyzed.Results51% of children were born with asphyxia, 42% were born prematurely, 39% were low birth body weight. 85.7% of children involved had mental retardation, 12% had hearing loss and 7% had visual impairment. 88% of children involved had abnormal cranial CT results and 46.1% had abnormal EEG. 50.0% of the children had abnormal brain stem auditory evoked potentials.ConclusionAsphyxia, prematurely birth and low birth body weight are high risk factors of cerebral palsy. Most of the children with cerebral palsy in this group are mental retarded.
10.Calculation of underwater decompression schedule for the simulated 100 m Trimix conventional diving and verification of the schedule with animal experiment.

OBJECTIVETo explore the underwater decompression schedule for 100 m Trimix conventional diving operations and evaluate its safety through a simulated rabbits Trimix conventional diving.
METHODSAccording to the Haldane theory, the assumed time units, the classification of tissue compartments, the nitrogen super-saturation safety coefficient and the selection of methods used for the calculation of the simulated 100 m Trimix conventional diving schedule were properly selected, and the calculating method for the dive decompression schedule was thus firmly established. In our experiments, five tissue compartments were selected during the calculation of decompression schedule: 5 min, 10 min, 20 min, 40 min and 75 min, and the nitrogen super-saturation safety coefficient was calculated by 1.6. Eight New Zealand rabbits were performed a simulated 100 m Trimix dive program which was established according to the Haldane theory, and eight rabbits for intact group. The tissues wet/dry ratio and ethology were detected and observed before and after the simulated diving to evaluate the safety of decompression schedule.
RESULTSBy using the developed underwater decompression schedule, abnormal ethology changes in rabbits could not be observed after compression and decompression to the surface; and the tissues wet/dry ratio of simulated diving rabbits had no significant changes compared with the intact group (P > 0.05).
CONCLUSIONThe decompression schedule calculated by Haldane theory seemed to be safe and reliable, the diving breathing gas concentration did not cause oxygen toxicity and nitrogen narcosis among the dive rabbits, and dive efficiency was greatly improved by using enriched oxygen gas in UPTD safety range during decompression.
