2.Clinical reevaluation of the echocardiographic value in diagnosing the arrhythmogenic right ventricular cardiomyopathy

Objective To investigate the diagnostic value of echocardiography in arrhythmogenic right ventricular cardiomyopathy (ARVC) by summarizing and comparing the electrophysiological and the imaging features.Methods The echocardiography and MRI were performed in the 65 cases of ARVC to measure the right ventricle and the free wall,noted as TTE-RV,MRI-RV and TTE-RVFW.The velocity of tricuspid valve regurgitation (TRmax) and left ventricular ejection fraction (LVEF) were measured.The three-dimensional electric anatomical model of right ventricular was get by the Carto system,and the right ventricular area (Area-RV),the scar area (Area-Scar) was calculated.Results Twenty-seven cases (41.5 %) was confirmed by the echocardiography,21 cases (32.3 %) was suspiciously diagnosed,14 cases (32.3%) was miss diagnosed,and 3 cases (4.6%) was misdiagnosed.Statistically significant difference could be detected among the echocardiography confirmed groups and the other two groups for the parameters TTE-RV,MRI-RV,Area-RV,Area-Scar,and TTE-RVFW (P < 0.05).Also there was a statistically significant difference of the parameters of Area-RV and Area-Scar between the suspiciously and miss diagnosed groups (P <0.05).Different echocardiographic findings was found in ARVC with different stages,but myocardial fibrosis and low voltage scar could be detected in all patients on the MRI imaging and and electrophysiological mapping.Conclusions The diagnosis of typical ARVC can be confirmed by echocardiography,but for the patients with early and middle stages,comprehensive evaluation should be refered to the clinical data.
3.Anterior cervical intervertebral space decompression under microscope for cervical spondylotic myelopathy in elderly patients.

OBJECTIVETo investigate the clinical effects of anterior cervical intervertebral space decompression under microscope in treating cervical spondylotic myelopathy in elderly patients.
METHODSFrom June 2009 to March 2012, 43 patients with cervical spondylotic myelopathy were treated with anterior cervical intervertebral space decompression and intervertebral fusion under microscope. There were 26 males and 17 females, aged from 60 to 72 years old with an average of (64.9±3.7) years. Japanese Orthopaedic Association System (JOA) score was from 7 to 12 points with an average of (9.5±1.8) points before operation. The function of nerves was assessed before and after operation according to JOA.
RESULTSAll patients were followed up from 10 to 18 months with an average of (14.7±1.6) months. Postoperative JOA score was (13.81±1.44) points (ranged, 10 to 16), had significantly higher than preoperative (P<0.01). According to the rate of the improved JOA score, 9 cases got excellent results, 26 good, 7 fair, 1 poor.
CONCLUSIONAnterior cervical intervertebral space decompression under microscope for cervical spondylotic myelopathy in elderly patients is safe and effective.

4.Effect of curcumin on growth and function of fibroblast in human hyperplastic scar.

OBJECTIVETo seek an effective drug for treatment of human hyperplastic scar through studying the effects of curcumin on fibroblast growth and collagen synthesis.
METHODSFibroblasts derived from scar tissue and from normal epidermal tissue were isolated and cultured separately with tissue-block method, their morphology were observed under invert phase contrast microscope, their growth curve was drawn respectively to determine the speed of growth. Then, fibroblasts from scar were stimulated with curcumin in different concentrations (0, 12.5, 25, 50 and 100 micromol/L) for detecting the inhibitory effect of curcumin on growth of fibroblasts using MTT methods and that on activity of procollagen alpha-1 gene transcription in fibroblast was detected by RT-PCR.
RESULTSThe cell growth curve showed that double-multiplying time was 5 days in fibroblasts from scar and 4 days in those from normal dermis, showing significant difference between them (P < 0.05). MTT showed that curcumin in 12.5 micromol/L showed a cell proliferation enhancing trend, and its absorbance value was significantly higher than that in the normal group, but the effect turned to inhibition when concentration increased to over 25-100 micromol/L, and became significant inhibition at concentration of 50 and 100 micromol/L. Besides, curcumin also showed markedly inhibition on collagen type I synthesis in fibroblasts (P < 0.01).
CONCLUSIONHigh concentration curcumin can inhibit effectively the fibroblast proliferation and collagen I synthesis in hyperplastic scar, therefore, may has therapeutic effect on the disease in human being.

8.Comparative study of high frequency ultrasound and MRI in the diagnosis of rotator cuff tears

Objective To comparative evaluation the application value of high frequency ultrasound (HFU)and magnetic reso-nance imaging (MRI)in the diagnosis of rotator cuff tears(RCT).Methods 86 patients of unilateral RCT confirmed by shoulder ar-throscopy were chosen.The detection rate by HFU and MRI before the surgery was compared.Shoulder arthroscopic finding was as the evaluation standard,the accuracy of HFU and MRI in the diagnosis of RCT was calculated which included:total (full and partial) RCT,full RCT,partial RCT.Chi-square test was used to compare the accuracy rate.Results Among 86 patients,30 patients with full RCT and 56 patients with partial RCT were detected by shoulder arthroscopy,in which 28 patients with full RCT and 43 patients with partial RCT were found by HFU,and 28 patients with full RCT and 5 1 patients with partial RCT were found by MRI respec-tively.The accuracy of HFU and MRI in the diagnosis of total,full,partial tear RCT were 82.6%,93.3%,76.8% and 91.9%,93.3%, 91.6% respectively.There was no significant difference between HFU and MRI in diagnosing total and full RCT(P >0.05),the ac-curacy on HFU in diagnosing partial RCT was slightly lower than that on MRI (P <0.05).Conclusion Both HFU and MRI have relatively high accuracy in diagnosing full RCT,HFU is slightly lower than MRI in diagnosing partial RCT.
9.Registry and Management for Cerebral Palsy in Minhang District of Shanghai

Objective To report the preliminary results of registry and management for cerebral palsy in Minhang District of Shanghai. MethodsThe registry and management for cerebral palsy in Minhang District of Shanghai were reviewed. Results and ConclusionFrom February, 2009, the children with cerebral palsy (born from Jan.1, 2000 to Dec.31, 2008) and their families who resided in Minhang District participated in the registration adopting Scpchild Manual of Registry and Systematic Management for Cerebral Palsy. At the end of 2009, 41 children with cerebral palsy entered in the Registry System. Comparing with epidemiological investigation, registry and management for cerebral palsy can afford more information. The acceptance of the parents for registry need to be improved to extend the registry to more children.
10.Distribution and susceptibility of pathogens isolated from postoperative infections in liver transplantation recipients

Objective To investigate the epidemiology of bacterial infections after liver transplantation and anaIyze the antimi- crobial susceptibility of major pathogens to provide reference for clinical therapy.Methods A retrospective survey was conduc ted in 174 patients who underwent liver transplantation during 2001 and 2004.Identification and susceptibility of pathogens were assayed by Microscan Walkaway 40 Automatic System.Results Infection was identified in 59.8% of the 174 patients after liver transplantation.A total of 218 non-duplicate strains were isolated.Most infections were caused by single pathogen.The infection was frequently identified in respiratory tract,biliary tract,blood stream or intra-abdominal cavity.The top 5 patho- gens were Acinetobacter baumannii,Staphylococcus aureus,Pseudomonas aeruginosa,Stenotrophomonas maltophilia and Escherichia coli.Gram-negative bacilli were usually resistant to multiple antimicrobial agents,but less resistant to piperacillin- tazobactam or imipenem.Most of S.aureus isolates were methicillin-resistant,which were susceptible to vancomyein.Conclu- sions Pathogens of postoperative infections in liver transplantation recipients are mostly multi-drug resistant.The microbiologi- cal surveillance is important for guiding clinical therapy.
