1.Research of the examination method of ET-CO2 based on high speed operation chip ADμC7026

Objective To develop one kind of mainstream ET-CO2 (carbon dioxide of the expiration tip)detector which provides accurate and real-time testing results. Methods Applying ADμC7026 as central processing unit for data pre-processing, temperature compensation algorithm and non-linear correction, the ETCO2 concentration, respiration waveform and respiration rate were obtained. Results The analyzing results of the experimental data shows that the system meets the design requirements with fine stability, rapid response, good repeatability and high accuracy. Conclusion ET-CO2 detector based on ADμC7026 makes the detection of CO2 more accurate and reliable, which provides more accurate real-time data for critical patient monitoring, anesthetic care, pediatric respiratory care and so on.
2.Perioperative nursing of modified maze procedure using bipolar radiofrequency ablation accompanied with valve replacement for surgical treatment of permanent atrial fibrillation

Objective To summarize the perioperative nursing of modified maze procedure using bipolar radiofrequency ablation accompanied with valve replacement for the surgical treatment of heart valve diseases complicated with permanent atrial fibrillation (AF).Methods A total of 55 patients with permanent AF and heart valves diseases were undergone surgical treatment from June 2010 to February 2013 in the Second Affiliated Hospital of Anhui Medical University.Preoperative psychological care and health education were given to patients.The Medtronic Cardioblate 68000 flush bipolar radiofrequency ablation system was applied to all patients.After valve replacement surgery in addition to routine care,rhythm and heart rate monitoring were paid attention to,maintenance of cardiac function,observation of drainage,maintenance of electrolyte balance,observation of pacemaker,observation of amiodarone medication and nursing,as well as health guidance and psychological care were implemented.Results The surgery of 55 patients was successfully completed.The day after surgery,atrial fibrillation in 48 cases turned into sinus rhythm.In all patients,2 of them had suffered Ⅲ degree atrioventricular block after surgery and permanent pacemakers were applicated.There were no cardiac perforation and non-hospital mortality.The average length of stay was(12.5±2.1) d.The mean follow-up was(8.6±3.0) months.87.3% of patients maintained sinus rhythm,9.1% in AF rhythm,3.6% in paced rhythm.Conclusions Modified maze procedure using bipolar radiofrequency ablation is a simple,safe and effective surgical procedure for the treatment of permanent atrial fibrillation.High quality of perioperative nursing is to improve the success rate of surgery and the key to reduce mortality.

4.Efficacy of Acupuncture Point Injection for Fine Motor Functions of Children with Cerebral Palsy

ObjectiveTo investigate the efficacy of acupuncture point injection for fine motor functions in children with cerebral palsy.Methods57 children with cerebral palsy were treated by acupuncture point injection combined with occupational therapy. The efficacy was evaluated by the Peabody Developmental Motor Scale-Fine Motor (PDMS-FM).ResultsThe fine motor quotient (FMQ) of 34 children (59.7%) elevated. The raw scores of grasping subtest (Gr) elevated in 45 children (78.9%) and the raw scores of visual-motor integration (VI) elevated in 53 children (93.0%).ConclusionThe efficacy of acupuncture point injection combined with occupational therapy is significant in children with cerebral palsy.
5.Surgical treatment of cervical esophageal carcinoma

Objective To explore the application of gastric(enteric)-pharyngeal anastomosis for cervical esophageal carcinoma. Methods The clinical data of 12 cases with surgical management of cervical esophageal carcinoma were retrospectively analyzed. Results The resectability of cervical esophageal carcinoma was 100%,no case complicated with pharyngeal fistula.Swallowing function of all cases was in a good state.The overall follow-up was 1 to 7 years,among them 9 surviving,3 dead.The surviving 5 cases are over 3 years,the ongest beyond 7 years. Conclusions Gastric(enteric)-pharyngeal anastomosis is a good primarily rehabilitating method of the cervical esophageal defect after surgical treatment of cervical esophageal carcinoma.
6.Characteristics and prognosis of child hemophagocytic syndrome in different age groups

Objective To explore the clinical characteristics and prognosis factors of child hemophagocytic syndrome(HPS)in different age groups.Methods Eighty-five children cases of HPS were divided into 3 age groups,0-2 years old (group A),>2-8 years old (group B) and >8-14 years old (group C).The etiology,pathology,clinical symptoms,laboratory detection and prognosis were retrospectively analyzed.Results In the etiology,the etiology in the group A,B and C was dominated by EB virus infection,the infection positive rate in the group C was significantly higher than that in the group B and C;in the bone marrow examination:the occurrence rate of granulocytes to erythrocytes ratio <1.0 in the group A was higher than that in the group C;in the clinical symptoms,cervical lymph node enlargement mostly occurred in the group B,hepatosplenomegaly was most rare in the group C,and the serous cavity effusion commonly occurred in the group B;in the laboratory detection,the LDH level in the group B was higher than that in the group A,the above differences were statistically significant(P<0.05).There were no statistically significant differences in other clinical symptoms and laboratory detection results among the three groups (P>0.05);In the comparison between the survival group and death group,the serous cavity effusion,triglyceride(TG),CD4/CD8 ratio and granulocytes to erythrocytes ratio in bone marrow had statistical difference(P<0.05).The above factors were performed the Logistic analysis,the results showed that TG>3.5 mmol/L,CD4/CD8 ratio <1.24 and bone marrow granulocytes to erythrocytes ratio <1.02 were the unfavorable risk factors affecting the prognosis of child HPS (P<0.05).Conclusion Partial clinical characteristics of child HPS are related with the onset age,TG level,CD4/CD8 ratio and bone marrow granulocytes to erythrocytes ratio are the risk factors affecting prognosis.
7.Clinical Features and Treatment of Eosinophilic Lymphoid Granuloma in Children

Objective To explore the clinical features and treatment of eosinophilic lymphoid granuloma(ELG) in children,in order to increase understanding of the disease.Methods Clinical features,laboratory examination,pathological examination,treatment and the-(rapeutic) effect of 5 patients with ELG were retrospectively studied,and the related literature were reviewed.Results Five cases were all boys.They all presented many subcutaneous swellings and lymphadenopathy.A case among them was associated with nephrotic syndrome,and 2 cases had the complication of eczema repeatedly,and 1 case ever had 2 times episodes of asthmatiform bronchitis.All of them had no hepatomegaly and splenomegaly,physically was well developed.Only chemotherapy was supplied to all the patients.During the course of therapy,4 cases among them recurred.After that,they were successfully treated with irradiation of lesion or cyclosporine(CsA).Conclusions ELG in children is as same as adult in clinicopathological features.It tends recur only with chemotherapy.We can get better curative effect with the synthetic treatment of chemotherapy,radiotherapy,operation or oral CsA and so on.

9.Gross Motor Function and Manual Ability Classification in 4~12 Year Old Children with Cerebral Palsy of Different Types

Objective To explore the distribution and correlation of gross motor function and manual ability in 4~12 year old children with cerebral palsy of different types. Methods 143 children with cerebral palsy (CP) were recruited according to the inclusion and exclusion criterions. They were assessed with Chinese version of Gross Motor Function Classification System (GMFCS) and Manual Ability Classification System (MACS). Results The classification of GMFCS and MACS was Ⅰ~Ⅱ in 33 (60.0%) children with spastic diplegia; Ⅲ~Ⅴ in 28 (75.7%) children with spastic quadriplegia; Ⅰ in 21 (55.3%) children with spastic hemiplegia, and GMFCS Ⅱ in 2 children, MACS Ⅱ in 14 children. There was moderate correlation between GMFCS and MACS (r=0.67,P<0.05). Conclusion GMFCS and MACS can be used to determine the gross motor function and manual ability in CP children of different types.
10.Development of Cerebral Palsy-Gross Motor Function Questionnaire

Objective To work out the Cerebral Palsy-Gross Motor Function Questionnaire(CP-GMFQ) and evaluate the reliability,validity and responsiveness of CP-GMFQ.Methods 160 children with cerebral palsy(CP)(97 males and 63 females;average age: 47.7±20.1 months,range: 18~90 months) were involved.The first 27 children received second assessment to assess test-retest reliability with a interval of 7 to 18 days.Criterion-related validity was evaluated by comparing CP-GMFQ scores with Gross Motor Function Classification System(GMFCS) levels and scores of Gross Motor Function Measure(GMFM).21 children were randomly selected for responsiveness analysis by effect size.After a interval of 3 to 7 months the re-assessment GMFQ and GMFM were tested.30 children were randomly selected for comparative testing time of GMFQ and GMFM.Results GMFQ had excellent test-retest reliability(ICC=0.9940);criterion-related validity was excellent between GMFQ scores and GMFCS levels and GMFM scores(r=0.77~0.92).Effect size of GMFQ(0.34) between GMFM88 and GMFM66,testing time of GMFQ(average 4.7±1.6 min) were significantly less than testing time of GMFM(average 17.7±4.6 min)(P<0.001).Conclusion The reliability,validity and responsiveness of GMFQ are satisfying.The gross motor functions of children with cerebral palsy could be easy and reasonably measured by GMFQ.
