1.Clinical observation of scalp acupuncture plus speech rehabilitation for Broca's aphasia after cerebral stroke

Objective:To explore the clinical efficacy of scalp acupuncture plus speech rehabilitation in treating Broca's aphasia after cerebral stroke, for providing novel evidences for the treatment. Methods:Ninety-one eligible patients with Broca's aphasia after cerebral stroke were randomized into an observation group and a control group. Forty-six cases in the observation group were intervened by scalp acupuncture plus speech rehabilitation, while 45 cases in the control group were treated by speech rehabilitation alone. The aphasia battery of Chinese (ABC) and Boston diagnostic aphasia examination (BDAE) were adopted to evaluate the clinical efficacy. Results:After the treatment, the scores of oral expression, reading and writing and global score in the observation group were significantly higher than those in the control group (allP<0.05). There was a significant difference in comparing the BDAE grading between the two groups after the treatment (P<0.05). After intervention, the basically-recovered plus markedly-effective rate was 45.7% in the observation group versus 24.4% in the control group, and the between-group difference was statistically significant (P<0.05). Conclusion:Scalp acupuncture plus speech rehabilitation is effective in treating Broca's aphasia after cerebral stroke, and worth promoting.
2.The expression and clinical significance of 12 kinds of microRNAs in ovary cancer

Objective To analyse the expression and clinical significance of 12 kinds of microRNAs (miR) in patients with ovarian cancer using public gene expression databases. Methods The microRNA expression data were screened in dataset GSE14407 and TCGA database, then 12 kinds of microRNAs were obtained including miR-10B, miR-1244, miR-622, miR-21, miR-503, Let-7D, miR-155, miR-30C, miR-17, miR-101-1, miR-186 and miR-770. The expression data of these 12 kinds of microRNAs were compared and identified to find the differential ones between normal tissue and tumors. Data of 505 ovary cancer patients were divided into two groups by age, tumor grade, clinical stage, disease location, tumor residual and microRNA expression. Kaplan-Meier survival analysis and Cox multivariate analysis were used to compare the overall survival of ovary cancer patients between two groups. Results Compared with ovary cancer, the expression levels of Let-7D and miR-101-1 were higher, but the expression levels of miR-155 and miR-770 were decreased, in adjacent tissue of ovary tumor (P<0.05). The Kaplan-Meier survival analysis result showed that lower survival rates were found in patients with age≥59 years, clinical stage (Ⅲ+Ⅳ) and lower Let-7D expression (P<0.05). The multivariate Cox regression analysis showed that the decreased expression level of Let-7D was the independent risk factor for the prognosis of ovarian cancer. Conclusion The expression of Let-7D is correlated with the prognosis of ovarian cancer, which is the independent biomarker to predict prognosis of ovarian cancer.

4.Effects of salmeterol fluticasone plus tiotropium bromide on right cardiac and thyroid functions of ;patients of chronic obstructive pulmonary disease with chronic cor pulmonale

Objective To explore the influence of salmeterol fluticasone plus tiotropium bromide on right cardiac and thyroid functions of patients of chronic obstructive pulmonary disease ( COPD) with chronic cor pulmonale .Methods A total of 92 cases of COPD patients with chronic cor pulmonale at our hospital from July to December 2012 were recruited as subjects and divided randomly into 2 groups of treatment ( n=47) and control (n=45).The control group received diuretics and vasodilative drugs while the treatment group had additional therapies of salmeterol fluticasone ( 50/500 μg ) twice daily plus tiotropium bromide once per day once daily for 6 months.Measure and compare two groups of patients′FEV1%, forced expiratory volume in one second(FEV1), status of vigor, brain natriuretic peptide(BNP), left ventricular ejection fraction ( LVEF ) , ultrasonic cardiogram ( right ventricular outflow tract , right ventricular internal diameter, right pulmonary artery trunk ) and value of thyroid function before and after treatment .Results Before treatment, the treatment group′s FEV1 (%) (36 ±10)%, FEV1 (1 036 ±410) ml, BNP (357 ± 101) ng/L, LVEF (46.2 ±10.4)%, internal diameter of right ventricular outflow tract (36.0 ±5.4) mm, right ventricular internal diameter (38.1 ±6.7) mm, right pulmonary artery trunk (24.0 ±3.6) mm, TT3 (1.47 ±0.38) nmol/L, TT4 (114.56 ±21.39) nmol/L, FT3 (4.47 ±1.42) pmol/L, FT4 (15.6 ± 2.5) pmol/L and TSH (2.50 ±0.91) mU/L showed no marked difference from those of control group (P>0.05).After treatment, the treatment group′s FEV1(%)(58 ±1)%, FEV1(1 706 ±456)ml, BNP(225 ± 52) ng/L, LVEF (57.1 ±11.5)%, internal diameter of right ventricular outflow tract (30.1 ±4.2) mm, right ventricular internal diameter ( 32.0 ±3.9 ) mm and right pulmonary artery trunk ( 20.2 ±2.3 ) mm greatly improved ( P <0.01 ) while its TT3 ( 1.46 ±0.42 ) nmol/L, TT4 ( 113 ±22 ) nmol/L, FT3 (4.46 ±1.38) pmol/L, FT4 (15.4 ±2.5) pmol/L and TSH (2.51 ±0.82) mU/L showed no marked difference from those of control group ( P>0.05 ) .However , no obvious difference existed in the control group′s BNP, LVEF, internal diameter of right ventricular outflow tract , right ventricular internal diameter , right pulmonary artery trunk and value of thyroid function before and after treatment ( P >0.05 ) . Conclusion Salmeterol fluticasone plus tiotropium bromide can effectively improve right cardiac functions of patients of COPD with chronic cor pulmonale , without marked suppression of thyroid function .
5.Clinical apply of middle and small skin defect in finger injury with lateral arm free perforator flap

Objective To introduce the clinical apply of middle and small skin defect in finger injury with lateral arm free perforator flap.Methods (1) Ten cadavers were injected with a modified lead oxide-gelatin mixture.Later,CT scan,three-dimensional reconstruction of the cutaneous perforator vessels on the later arm.Then simulate the flap design.All cadavers were dissected before CT scanning.(2) Eighteen cases of middle and small skin defects in finger injury were treated with lateral arm free flap.The defect area were from 3 cm × 4 cm to 6 cm × 9 cm.Results Our research showed that the average caliber diameter of perforators of profunda brachial artery(PBA) was (0.71 ± 0.15) mm,and Posterior radial collateral artery(PRCA) was (0.94±0.22)mm.The results of pedicle length of perforator of PBA was (2.74 ±0.42) cm:(2.96 ±0.37) cm,and PRCA was (4.78 ±0.63) cm:(4.86 ±0.51) cm.3D reconstructive results showed that the perforators of PBA and PRCA dominated the lateral upper arm area.The flap of 18 cases survived after the operation.The wound of providing area was directly sutured or skin grafting and got healing.All cases were followed up for 6 months to 3 years,and the average follow-up time was 11 months.There was a little swelling on the outlooks of the flap,but the texture and sensation of the flap was good.Conclusion The lateral arm free perforator flap has a stable vessel pedicle,good texture and sensation,so it was a good method to repair middle and small skin defect in finger injury.
6.Video-assisted Thoracoscopic Lobectomy with Small Auxiliary Incision:Report of 63 Cases

Objective To explore the indications and safety of video-assisted thoracoscopic lobectomy with small auxiliary incision for benign pulmonary diseases and non-small cell lung cancer.Methods Between May 1996 and April 2007,63 cases of lobectomy were performed by video-assisted thoracoscopy.Before the operation,18 of the patients were diagnosed with benign pulmonary diseases,and 45 had pulmonary mass or malignancies.Lobectomy of the right lung was carried out on 47 patients,including the right upper lobe resection in 12,the middle lobe in 8,and the lower in 27.The other 16 patients received resection of the left upper lobe(5)or left lower lobe(11).Results Thoracoscopic lobectomy was completed in all the 63 patients,in 6 of them the incision was lengthened during the operation.No perioperative death occurred.The chest tube was withdrawn 4 to 7 days(mean,4.5 days)after the operation.Postoperative pathological examination showed NSCLC in 37 patients(58.7%),metastatic lung cancer in 1(1.6%),carcinoid tumor in 1(1.6%),and benign conditions in 24(38.1%).Among the 37 patients with NSCLC,8 were at stage Ⅰa,13 at stage Ⅰb,14 at stage Ⅱa,3 at stage Ⅱb,6 at stage Ⅲa,1 at stage Ⅲb,and 2 at stage Ⅳ.Of the patients with malignancies,34 were followed up for 2 to 60 months(2 to 12 months in 7,13 to 24 months in 10,25 to 36 months in 6,37 to 48 months in 6,and 49 to 60 months in 3),2 of them survived 11 years,5 of the patients(14.7%)died of tumor recurrence or metastasis,1(2.9%)survived with tumor,and 28(82.4%)lived free of tumor.The 1-and 3-year survival rate of the patients with stage Ⅰ or Ⅱ cancer were 100%(19/19)and 90%(9/10)respectively.In the patients with stage Ⅲ cancer,5 received follow-up,3 of them died of recurrence of metastasis.In the 2 patients who had stage Ⅳ cancer,1 died of metastasis.Conclusion Video-assisted thoracoscopic lobectomy is a safe and effective procedure and can be a regular surgical treatment for benign pulmonary disease,and peripheral type non-small cell lung cancer at stage Ⅰ or Ⅱ.
7.Insulin and glucose levels in maternal serum,umbilical venous serum and umbilical artery serum:Comparison between normal pregnancy and intrauterine growth retardation

Objective:To determine levels of insulin and glucose of maternal serum (MS),umbilical venous serum (UVS) and umbilical artery serum (UAS) in normal term pregnancy and intrauterine growth retardation(IUGR).Methods:One hundred and seven cases of normal gestational weeks on induction of labors and spontaneous deliveries and 32 cases of pregnancies complicated with IUGR.Levels of insulin were measured with sensitive radioimmunoassay and levels of glucose were measured by the glucose oxidase-peroxidase method.Results:Level of insulin of maternal serum was not significantly different before 28 gastational weeks and peaked on 28 th gastational week,then it decreased with gestational weeks and peaked again at last term.Levels of insulin increased in UVS and UAS.Levels of glucose in MS,umbilical venous serum and umbilical artery serum were not significantly different.Level of insulin and glucose of MS in IUGR had no change,but it was significantly different in IUGR of UVS and UAS (P<0.01).Conclusion:Our study suggested a decrease of insulin and glucose in UVS and UAS with IUGR.

9.A study on the mechanism of iodine-induced thyroid epithelial cell injury in the induction of autoimmune thyroiditis

Objective To investigate the effect of iodine excess on thyroid follicle epithelial ultrstructure and the relationship between thyroid iniury and autoimmune thyroiditis. Methods NOD.H-2h4 mice and Kunming mice were randomly divided into four groups receiving plain water,5 fold,10 fold,and 100 fold excessive iodine water.4,8 and 24 weeks after receiving iodine water,the mice were killed.After fixation with osmic acid and dual staining with uranyl chloride and citrate lead,thyroid gland ultrstructure was examined with electron microscopy.Resuits Iodine treated NOD.H-2h4 mice exhibited marked accumulation of peroxisome and secondary lysosomes,apoptosis and necrosis of thyroid epithelial cell.damage of thyroid follicles and lymphocvtic infiltration.The observed changes induced by iodine were in a dose dependent way.Conclusion The oxidative iniury on the thyroid epithelial cells induced by iodine excess might be the prerequisite for the creation of autoimmune thyroiditis.
