1.Investigation of Residents’Perceptions for Pharmaceutical Care When Purchasing OTC Drugs in Yinchuan

OBJECTIVE:To explore consumers’perceptions for pharmaceutical care by community pharmacist when purchas-ing OTC drugs. METHODS:Semi-structured interview was conducted for 100 consumers who were recruited in Xingqing District, Jinfeng District and Xixia District of Yinchuan,Ningxia Hui Autonomous Region,their expection for pharmaceutical care by com-munity pharmacists was summarized. RESULTS:Totally 100 residents were interriewed,and 97 completed a full interview with ef-fective interview rate of 97%. Consumers still did not clearly on distinguishing the OTC drugs. The majority of consumers chosen OTC drugs were based on experience and brand,a handful of consumers were under the guidance of community pharmacists. Con-sumers were most concerned about the efficacy,adverse reactions and drug quality in using OTC drugs. Most consumers were not satisfied with the pharmaceutical care in purchasing OTC drugs. Nevertheless,due to a lack of medicine knowledge,consumer had a great demand for pharmaceutical care and willingness to accept it. Low professional level of community pharmacist and staff in current community pharmacy and lack of effective communication between pharmacist and consumer were the obstacles of develop-ing pharmaceutical care. CONCLUSIONS:Because of their distrust on competence of community pharmacist,consumers seek less pharmaceutical care of OTC drugs at community pharmacy though they had expectations on pharmaceutical care of OTC drugs by community pharmacist. Consumers’rely on self-choice of drugs make them exposed to more drug risks.
4.Orthotopic heart transplantations for end-stage heart diseases

Objective To summarize the outcomes and clinical experience of orthotopic heart transplantations in Shanghai Zhongshan Hospital.Methods From May 2000 through October 2005,141 patients,101 males and 40 females,diagnosed as having dilated cardiomyopathy in 118,hypertrophic cardiomyopathy in 2,restrictive cardiomyopathy in 2,end-stage ischemic heart disease in 9, primary malignant cardiac tumor in 4,valvular heart disease in 3 and others in 3,underwent orthotopic heart(transplantations) at our center.The operative procedures included 120 bicaval anastomotic cardiac transplantations,19 conventional Stanford orthotopic cardiac transplantations and 2 total heart transplantations.The immunosuppressive therapy strategies included Cyclosporine A or tacrolimus,corticosteroids and mycophenolate mofetil.For the latest 28 patients,induction therapy with Daclizumab was applied.Results There were 3 operative deaths with an operative survival of 97.9 %.During the follow-up from 1 month to 65 months,the actuarial survival rate was 90.8 % after 1 year,84.6 % after 3 year and 81.4 % after 5 year.Acute rejection,infections,graft failure and metastasis of primary cardiac tumor were the main causes of death in the 1st postoperative year,while acute rejection and graft coronary vasculopathy were the leading causes of death thereafter.Postoperative complications included acute rejection,infections,renal dysfunction and graft failure.Conclusions Orthotopic heart transplantation was proved to be a reliable choice for endstage heart disease with excellent outcomes.More attention should be paid to surveillance and management of acute rejection, infections and graft coronary vasculopathy in the long-term follow-up.
5.Clinical and prognostic analysis of 52 patients with gastric neuroendocrine carcinoma

Objective To investigate the clinical features,diagnosis,treatment and prognosis of gastric neuroendocrine carcinoma (G-NEC).Methods Clinical data of 52 G-NEC cases were analyzed.Follow-up was conducted by telephone.The survival curves were drawn using Kaplan-Meier method.Univariate analysis was performed by the Log-rank test and multivariate analysis was performed by COX proportional hazards model.Results The median overall survival rate was 19 months (range 6 to61 months),and the overall 1,3,5-year survival rates were 74%,16% and 5% respectively.Tumor stage surgery,vascular nerve involvement and Ⅲ and Ⅳ phase chemotherapy were related to prognosis (x2 =24.254,10.005,7.261,8.790,all P < 0.05).Multivariate analysis showed tumor stage and vascular nerve involvement were independent prognostic factors (x2 =17.170,5.810,all P < 0.05).Conclusion G-NEC is a highly malignant tumor with poor prognosis.Preoperative diagnosis rate is low.Surgery is the treatment of first choice.Definite diagnosis depends on postoperative pathology and immunohistochemical examination.
6.Evaluation of dose delivery stability of a new linear accelerator TrueBeam

Objective:To study the long-term stability of 6MV photon beam on TrueBeam, which is a new linear accelerator introduced by Varian Medical System. Methods:QA BeamChecker Plus (QABC+, Standard Imaging Inc., Middleton, USA) was used to measure 6MV X-ray on TrueBeam linac as daily quality assurance (Daily QA). Dosimetric parameter, including output Constancy, flatness and symmetry, were employed to evaluate the dose delivery stability of TrueBeam linac. Results: Daily QA results of 18 months showed the dose output delta was (0.0%±0.7%), only once out of tolerance of 2%. The constancy of Flatness was excellent with the delta of (0.1%±0.1%). The symmetric of axial and transverse varied within 1%. Conclusion:The output constancy of TrueBeam with 6MV is very stable, as well as flatness and symmetry. It’s stable and reliable to implement Truebeam linear accelerator in clinical.
7.A study of the mechanisms of recovery from aphasia by speech-activated SPECT brain imaging

Objective To explore the mechanisms of recovery from aphasia by using speech-activated single photon emission computed tomography(SPECT)brain imaging.Methods The SPECT brain imaging of 7 aphasic patients caused by various brain disorders were performed while they were at rest and performing oral reading,respec- tively,with an one-day interval.A semi-quantitative analysis of regional cerebral blood flow(rCBF)was conducted using region of interesting(ROI).The change of rCBF before and after reading was calculated and compared to ana- lyze the role of both hemispheres in the recovery from aphasia.Results It was found that the activation pattern of brain region was associated with the speech performance of the patients.The activated brain regions were mainly loca- ted at the left hemisphere in 5 patients whose reading capacity was relatively better,and mainly at the right hemi- sphere in the other 2 patients who had poor performance in reading.Conclusion After a focal lesion of the left hemisphere,the recovery of speech function might be mainly attributed to the repair of the damaged left hemisphere language network.When the left-hemispheric centers were permanently impaired,the brain would recruit some right- hemisphere regions for speech processing,but this strategy was less effective than the repair of the original speech-rel- evant network.
8.The expression and significance of NEK2 mRNA in oral squamous cell carcinoma

42 cases of OSCC and 20 of healthy tissues were studied to detect the expression of NIMA-related Kinase 2(NEK2)mRNA by Real-time PCR.Over expression of NEK2 mRNA was observed in OSCC (z =-6.328,P <0.05),the expression was correlated with the differentiation (z =-5.012,P <0.05)and clinical stage (z =-4.950,P <0.05)of the lesion,but not with the lymph node metastasis (z=-0.051,P >0.05),age (z =-0.365,P >0.05)and gender (z =-3.450,P >0.05).NEK2 gene may be involved in the develop-ment of OSCC.
9.Effect of polysaccharides-2b from Mudan cortex on type 2 diabetes mellitus rats and its mechanism

AIM To study the effect of polysaccharides 2b from Mudan cortex (PSM 2b) on type 2 diabetes mellitus (T2DM) rats and explore its mechanism. METHODS T2DM rat model was established by low dosage streptozotocin and high sucrose fat diet. The drugs were administrated by ig for 5 weeks. The serum glucose was assayed with GOD POD method. Insulin was determined by radioimmunoassay. Insulin receptors of the plasma membrane from rat liver were determined with radioligand binding assay of receptors (RBAR). RESULTS PSM 2b could significantly lower fasting blood glucose (FBG), improve impaired glucose telarance (IGT) and dyslipidemia. It could also remarkably raise receptor total (RT2) of the low affinity insulin receptor (InsR) and insulin sensitivity index (ISI) in T2DM rats. CONCLUSION PSM 2b has an obvious therapeatic effect on T2DM in rats. Its mechamsm is relatively improve insulin resistance (IR) in T2DM rats by raising the number of InsR.
10.Protective Effect of Catalpol on Vascular Endothelial Cell Injured by High Glucose

This study was aimed to investigate the protective effect of catalpol against high glucose induced injury on vascular endothelial cells (ECV-304). ECV-304 cells were culturedin vitro. MTT method was applied to detect cell viability. NO and LDH level were measured by spectrophotometer method. The effect of catalpol against high glucose induced injury on ECV-304 was observed. The results showed that there were no obvious changes in cell viabilities of ECV-304 cultured in normal concentration of glucose medium among groups with different concentrations of catalpol. Compared with the control group, cell viability was significantly decreased (P< 0.01), the NO secretion was significantly reduced (P< 0.01), the LDH level was increased obviously (P< 0.01) in the high glucose (30 mmol·L-1 glucose) injury model group. The high osmotic (24.5 mmol·L-1 mannitol) group was set up as a positive control. Comparison between the low dose (1 ng·μL-1 catalpol + 30 mmol·L-1 glucose), middle dose (10 ng·μL-1 catalpol+ 30 mmol·L-1 glucose), and high glucose (30 mmol·L-1 glucose) injury model group showed no changes on cell viability and NO (P> 0.05); while the LDH level was reduced (P< 0.01). Comparison between the high dose catalpol (100 ng·μL-1 catalpol + 30 mmol·L-1 glucose) group and the high glucose model group showed that the cell viability was obviously increased (P< 0.01); NO secretion was increased (P< 0.05); and the LDH level was obviously reduced (P< 0.01). It was concluded that catalpol had protective effect against high glucose induced ECV-304 injury through the regulation of NO and LDH level. The effect of high dose was obvious.
